Abstract
本发明公开了一种治疗鼻咽癌及配合鼻咽癌放化疗的中药组合物,属于中药技术领域。该中药组合物,其有效成分提取自重楼、白花蛇舌草,石上柏,半枝莲,猫爪草、栀子、白芍、夏枯草、五指毛桃,太子参,薏苡仁,甘草,菟丝子,蜈蚣,守宫。本发明是将现代医学的技术和中国传统的中医学知识结合起来,首先从从经络角度对鼻咽癌患者以及健康人员进行辨识和统计,提出以攻补兼施的方法,清肝解毒并补脾益肾进行鼻咽癌治疗。配方中,以重楼、猫爪草、白花蛇舌草清热解毒,五指毛桃、太子参、薏苡仁、菟丝子健脾益气补肾共为君,以蜈蚣、守宫攻毒散结为臣;以白芍、石上柏、半枝莲、栀子、夏枯草滋肝养阴清热散结为佐,以甘草调和诸药为使。

技术领域
本发明属于中药技术领域,具体涉及一种治疗鼻咽癌及配合鼻咽癌放化疗的中药组合物。
背景技术
鼻咽癌为常见之恶性肿瘤,多见于广东、广西等南方诸省。具有一定的地域和种族特点,有家族倾向性,30-50岁为高发病年龄,男性比女性多见。主要临床表现为血涕、颈部淋巴结肿大以及癌侵犯颅神经后的症状(神经痛、头痛、眼球麻痹、耳鸣耳聋等),鼻咽镜可查见鼻咽腔有菜花样肿物,呈色灰白、表面粗糙或有溃疡、触之易出血。其所处鼻咽位置隐蔽,检查不易。此外,鼻咽癌的早期症状比较复杂,缺乏特征,故容易被人忽视,延误诊断和治疗,所以必须提高警惕性。常见症状有:①出血:主要是吸鼻后韧中带血,或鼻出带血鼻涕。开始常为少量血丝,容易被忽视,及至出血量较多时,往往病变已入中、晚期。②头痛:早期就可有头痛,而且多偏向一侧,呈间歇性;晚期则出现持续性剧烈头痛,容易误认为神经性偏头痛。③颈部淋巴结肿大:一侧或双侧颈部出现肿块,无能,质较硬,活动度差,常易误认为淋巴结核或淋巴结炎。④其他症状:除表现上述某一个或所有症状外,还可出现鼻塞、耳闭、耳鸣、耳部麻木、复视、上睑下垂等症状。晚期癌肿易向颅内侵犯及骨、肝、肺等远处转移。诊断:鼻咽镜检查是发现痛肿最有效、可靠的方法,最后确诊要靠活检切片。EB病毒血清学检查有利于早期发现。肝脾B超,胸部、颅底照片或CT扫描等检查对了解病变侵犯范围及有无远处转移有意义。
目前鼻咽癌的治疗方法是以放疗为主,同时结合化疗、生物治疗、及手术等的综合治疗,使鼻咽癌的临床治疗有效率明显提高。但新增鼻咽癌仍保持较稳定的发病率,说明鼻咽癌的治疗和/或预防仍有待进一步挖掘新的方法。此外,经放射线治疗鼻咽癌时,放射线杀灭肿瘤的同时对鼻咽周围组织也会造成杀伤,导致放射性咽炎、鼻炎、中耳炎及放射性脑病等后遗症,给患者带来较大的痛苦。再者鼻咽癌经常规放疗还不能杜绝其复发转移,故对于鼻咽癌的发病机理、治疗以及临床的研究均需要加强。
传统的中医学认为,鼻咽癌的发生于机体内外多种致病因素有关,病因包括先天禀赋不足、正气虚弱、情志不遂、饮食不洁、脏腑功能失调致邪毒乘虚而入,凝结成癌肿,中医病因病机包括热毒、痰凝、气滞血瘀和正虚。但这些结论均是从古文献研究及理论探讨中得出,中医文献及医案研究较少经过现代医学以及病理学的研究确认,因此佐证其结论的论据欠充分。但一般人根据这些认识,多采用清热解毒类中药治疗鼻咽癌,寒凉药过多。苦寒药败胃损伤人体正气,虽一时有效,久用此类药反而对鼻咽癌患者不利。
发明内容
本发明的目的在于提供一种治疗鼻咽癌及配合鼻咽癌放化疗的中药组合物。
本发明所采取的技术方案是:

优选的,所述中药组合物,其有效成分的原料药配比为:重楼5-35份、白花蛇舌草25-65份,石上柏5-20份,半枝莲5-20份,猫爪草5-35份、栀子5-20份、白芍5-20份、夏枯草5-20份、五指毛桃25-65份,太子参5-20份,薏苡仁25-65份,甘草3-15份,菟丝子10-35份,蜈蚣1-3份,守宫3-12份;所述份数为质量份。
优选的,所述中药组合物,其有效成分的原料药配比为:重楼10-30份、白花蛇舌草30-60份,石上柏10-15份,半枝莲10-15份,猫爪草10-30份、栀子10-15份、白芍10-15份、夏枯草10-15份、五指毛桃30-60份,太子参10-15份,薏苡仁30-60份,甘草5-10份,菟丝子15-30份,蜈蚣1-2份,守宫5-10份;所述份数为质量份。

本发明的有益效果是:
本发明是将现代医学的技术和中国传统的中医学知识结合起来,首先从从经络角度对鼻咽癌患者以及健康人员进行辨识和统计,分析得到鼻咽癌患者与健康人存在明显(统计学)差异的经络有3条(有实证也有虚证),实证表现为肝实,虚证表现为脾肾虚。根据上述研究理论,申请人及团队经过反复摸索和验证,提出以攻补兼施的方法,清肝解毒并补脾益肾进行鼻咽癌治疗。得到本发明的配方,该中药配方组成及含量为:重楼10-30克、白花蛇舌草30-60克,石上柏10-15克,半枝莲10-15克,猫爪草10-30克、栀子10-15克、白芍10-15克、夏枯草10-15克、五指毛桃30-60克,太子参10-15克,薏苡仁30-60克,甘草5-10克,菟丝子15-30,蜈蚣,微火焙干研粉,1-2克,守宫(壁虎),微火焙干研粉,5-10克。配方中,以重楼、猫爪草、白花蛇舌草清热解毒,五指毛桃、太子参、薏苡仁、菟丝子健脾益气补肾共为君,以蜈蚣、守宫攻毒散结为臣;以白芍、石上柏、半枝莲、栀子、夏枯草滋肝养阴清热散结为佐,以甘草调和诸药为使。
申请人及团队对本发明的中药组合物进行细胞和临床的研究。特别的是,本发明的中药组合物在临床上的研究主要以远期疗效为主,并结合统计学进行分析。因为随着放疗或化疗技术的进步,鼻咽癌患者进行放疗及化疗后大多数鼻咽肿块基本上都得到控制,所以仅以进行放化疗及放化疗合并中药治疗的近期疗效为标准来评价两者的疗效意义不大。所以本发明远期疗效为电话随访,以三年及五年存活率为指标,并结合统计学的分析,其效果的说服力优于现有技术中仅能提供少数近期效果的中药组合物或相关产品。此外,在临床研究中,本发明还观察了6例不接受放化疗仅进行中药治疗的患者情况,结果显示本发明中药组合物效果显著。这充分说明了本发明中药组合物的疗效。
本发明中药组合物可单独用于治疗临床各期(早、中及晚期)鼻咽癌,亦可配合用于鼻咽癌同时行放化疗者,临床证实其对早、中及晚期鼻咽癌都具有较好的治疗效果,改善癌症患者的鼻塞、涕血、咽干、咽疼、颈淋巴结肿大等症状,提高癌症患者的生存质量,延长癌症患者的生存期。

具体实施方式
初诊鼻咽癌经络虚证及实证研究
传统的中医学认为,鼻咽癌的发生于机体内外多种致病因素有关,病因包括先天禀赋不足、正气虚弱、情志不遂、饮食不洁、脏腑功能失调致邪毒乘虚而入,凝结成癌肿,其病因病机包括热毒、痰凝、气滞血瘀和正虚,但这些结论是从古文献研究中得出,由于古代没有病理技术,因此这些记录显得论据欠充分。
本发明中,申请人及团队依据中医原理,利用现代科学技术,在病理确诊的前提下对鼻咽癌患者的体质做探索。以57例初诊未进行治疗的鼻咽癌患者为研究组,217例健康体检人群为对照,采用经络检测仪(SMF-Ⅲ,芜湖圣美孚科技有限公司)检测人体十二经脉经络虚实特征,结果见表1、表2和表3。

而鼻咽癌与健康人其它经络的差异无显著性。
根据表1、表2和表3的结果显示:鼻咽癌患者与健康人存在明显(统计学)差异的有实证也有虚证,实证表现为肝实,虚证表现为脾肾虚。申请人根据常年治疗鼻咽癌经验用药经验认为:治疗鼻咽癌不能一味清热解毒,防苦寒败胃,损伤人体正气;也不能一味温补,防毒留为患反而助邪,故立法为清肝解毒、补脾益气。申请人及团队经过反复摸索和验证,得到本发明的配方,该中药配方组成及含量为:重楼10-30克、白花蛇舌草30-60克,石上柏10-15克,半枝莲10-15克,猫爪草10-30克、栀子10-15克、白芍10-15克、夏枯草10-15克、五指毛桃30-60克,太子参10-15克,薏苡仁30-60克,甘草5-10克,菟丝子15-30,蜈蚣,微火焙干研粉,1-2克,守宫(壁虎),微火焙干研粉,5-10克。配方中,以重楼、猫爪草、白花蛇舌草清热解毒,五指毛桃、太子参、薏苡仁、菟丝子健脾益气补肾共为君,以蜈蚣、守宫攻毒散结为臣;以白芍、石上柏、半枝莲、栀子、夏枯草滋肝养阴清热散结为佐,以甘草调和诸药为使。

一种治疗鼻咽癌及配合鼻咽癌放化疗的中药组合物,其原料药配比如下:
重楼10克、白花蛇舌草30克,石上柏10克,半枝莲10克,猫爪草10克、栀子10克、白芍10克、夏枯草10克、五指毛桃30克,太子参10克,薏苡仁30克,甘草5克,菟丝子15克,蜈蚣1克,守宫(壁虎)5克。
制备工艺如下:
1)将重楼、白花蛇舌草,石上柏,半枝莲,猫爪草、栀子、白芍、夏枯草、五指毛桃,太子参,薏苡仁,甘草,菟丝子;采用常规水煎煮提取;
2)蜈蚣,微火焙干研粉,溶于中药水提液中;
3)守宫(壁虎),微火焙干研粉,溶于中药水提液中,制备得到鼻咽癌及配合鼻咽癌放化疗的中药组合物制剂。
实施例2
一种治疗鼻咽癌及配合鼻咽癌放化疗的中药组合物,其原料药配比如下:
重楼30克、白花蛇舌草60克,石上柏15克,半枝莲15克,猫爪草30克、栀子15克、白芍15克、夏枯草15克、五指毛桃60克,太子参15克,薏苡仁60克,甘草10克,菟丝子30克,蜈蚣2克,守宫(壁虎)10克。

利用实施例的中药组合物制剂进行下述实验,来验证本发明中药配方的效果。
实验1、血清细胞学研究
(一)实验材料
1.实验动物及细胞:新西兰大白兔6只,体重1.5±0.2kg,由中南大学湘雅医学院实验动物中心提供。动物合格证号:SCXK(沪)2007-0005。Raji细胞:为鼻咽癌细胞株,购自中南大学湘雅医学院细胞中心。
2.实验药物:实施例1的中药组合物制剂。
3.仪器与试剂:二甲基噻唑二甲基四唑(MTT)、1640培养基、小牛血清,北京华美生物工程公司生产;培养瓶、96孔细胞培养板、6孔细胞培养板由Greiner Bio-One Gmbh生产;病毒DNA小量抽提试剂盒、PCR引物合成、荧光探针,上海生物工程技术有限公司生产;5×定性缓冲液、5×定量缓冲液、Taq DNA聚合酶、dNTPs由广州达安基因股份有限公司生产。
(二)实验方法
1.动物分组与给药:将6只新西兰大白兔随机平均分为2组:A组:治疗鼻咽癌的中药配方组(实施例1),B组:生理盐水对照组,每组3只。A组以9.7g/kg/d剂量灌胃,B组以等体积的生理盐水灌胃。兔子用药常规用药的剂量,是参照人的用药量按照体表面积计算得出的给药剂量,计算公式参考徐淑云教授主编的《药理实验方法学》(第三版)。每天分2次灌胃,连续给药3天,末次于上午8时给药,90min后心脏采血20ml/只,将同组2只动物血混合,离心(1000r/min,5min),分离血清,即用于细胞培养。
2.细胞培养:将Raji细胞悬浮在10%小牛血清的1640培养液中,37℃、5%CO2孵箱中培养。
3.药物浓度筛选:用细胞培养基1640将药物血清按1∶2,1∶4,1∶8,1∶16,1∶32,1∶64稀释,加样于2.0×104个/孔的96孔细胞培养板的细胞孔中,以相同稀释倍数的不含中药的血清作为对照。37℃5%CO2培养48h,加入MTT(5mg/mL),培养结束时,离心去上清,加入二甲基亚砜100-150μL/孔,振荡15min,在酶标仪上测定光吸收值,测定波长为490nm,并绘制药物浓度及时间的作用曲线。
4.非毒浓度含药血清与Raji细胞共培养:选取上述筛选的非毒药物最高浓度药物血清加至5.0×104Raji细胞接种的6孔板中,设置3个复孔。37℃5%CO2培养48h,收集细胞。按DNA试剂盒说明书操作方法提取EBV DNA。
5.PCR引物和探针:选择EB病毒DNA全序列20206~20426位为外扩增区,20279~20343位为内扩增区,荧光探针在内扩增区内20299~20318位。
(1)外扩增引物:
上游外扩增引物5’-TCAAAACTTTAGAGGCGAATGG-3’(SEQ ID NO:1);
下游外扩增引物5’-CTGAGAAGGTGGCCTAGCAAC-3’(SEQ ID NO:2);
扩增片段长度为221bp。
(2)内扩增引物:
上游内扩增引物5’-CCAGGAAGCGGGTCTATGG-3’(SEQ ID NO:3);
下游内扩增引物5’-GGCTTATTCCTCTTTTCCCCTCTA-3’(SEQ ID NO:4);
扩增片段长度为65bp。
(3)荧光探针:Taqman探针序列5’-FAM-TGGCTGCGCTGCTGCTATC-TAMRA-3’(SEQ IDNO:5)。
6.标准品的制备:取Raji细胞冻存液进行细胞计数。根据文献(Lin JC,CherngJM,Hung MS,et,al.Inhibitory effects of some derivatives of glycyrrhizic acidagainst Epstein-Barr virus infection:structure-activity relationships[J].Antiviral Res.2008,79(1):6-11.)报道每30个Raji细胞约含有1500个拷贝的EBV-DNA进行计算,得到Raji细胞冻存液中EBV-DNA的浓度为6.95×105copies/μL。
7.巢式荧光定量PCR检测
(1)标准品的外扩增反应体系及反应条件:将Raji细胞抽提的EBV-DNA稀释成一定浓度梯度(1×107、1×106、1×105、1×104、1×103/50μL反应体系)。50μL反应体系加入:5×定性缓冲液10μL,Taq DNA聚合酶1μL,dNTPs1μL,上游外扩增引物1μL,下游外扩增引物1μL,标准品nμL(n为根据浓度计算的体积),加水补至50μL。反应条件:95℃5min;95℃45s,55℃45s,72℃45s,30个循环;72℃7min。
(2)细胞样品的外扩增反应体系及反应条件:同标准品的外扩增反应体系及反应条件。
(3)内扩增反应体系及反应条件:标准品与细胞样品相同。50μL反应体系加入:5×定量缓冲液10μL,Taq DNA聚合酶1μL,dNTPs 1μL,上游内扩增引物1μL,下游内扩增引物1μL,Taqman探针1μL,外扩增产物5μL,加水补至50μL。反应条件:95℃5min;95℃45s,55℃45s,40个循环。
(三)结果
1.鼻咽癌的中药配方含药血清作用浓度筛选
将含鼻咽癌中药配方的药物血清稀释为1∶2,1∶4,1∶8,1∶16,1∶32,1∶64,与Raji细胞共同培养48h,采用MTT法分析其对细胞生长的影响,结果发现含鼻咽癌中药配方的药物血清对Raji细胞生长无毒性最高浓度为1∶8(即鼻咽癌中药配方的药物血清在1:1,1:2,1:4高浓度时Raji细胞都会死,在1:8及1:16及再稀释时Raji细胞不死,所以确定1:8为Raji细胞生长无毒性最高浓度)。
2.鼻咽癌中药配方含药血清对鼻咽癌Raji细胞EBV DNA复制的影响
为消除因为药物毒性作用影响细胞生长而导致结果差异,根据上述实验结果,选取非毒性药物最高血清浓度(1:8)用于实验。结果见表4和图1。
表4鼻咽癌中药配方血清对Raji细胞DNA复制的影响
表4结果显示:在含鼻咽癌中药配方的兔血清作用后,Raji细胞EBV-DNA拷贝数明显降低,显示本发明中药有明显抑制鼻咽癌Raji细胞EB病毒DNA的复制作用。从细胞实验显示其治疗鼻咽癌的作用。
图1为鼻咽癌中药配方血清对Raji细胞DNA复制的影响。其中,横坐标表示循环数,纵坐标表示DNA荧光量。图中三条曲线表示三次实验结果,曲线相似说明重复性良好,实验稳定。
实验2、临床研究
(一)临床资料
1.一般资料:鼻咽癌及鼻咽癌放化疗后患者均来自我院门诊及住院病人,初诊鼻咽癌6人,其中2期(中期)病人3例,3期(中期)病人2例,4期(晚期)病人1例。只单纯用中药治疗。放化疗后鼻咽癌共60例,在常规放化疗后患者出现咽痛、鼻塞等放疗后遗症,服用中药治疗。60例放化疗后鼻咽癌临床资料:男35例,女25例;年龄最大69岁,最小26岁,平均(58.65±10.5)岁。鼻咽癌分期:Ⅰ期3例,Ⅱ期8例,Ⅲ期40例,Ⅳ期9例,病理:分化性非角性癌53例,未分化癌7例。
2.诊断及排除标准
鼻咽癌的诊断依据病理确诊。年龄小于18岁或大于70岁者,全身检查合并有严重肝病及其它肿瘤者为排弃对象。
3.治疗方法
处方立法原则:前期鼻咽癌调查显示:鼻咽癌存在着气虚兼热表现,前期经对57例鼻咽癌经络研究显示:鼻咽癌患者存在着肝经的实证及脾、肾的虚证,再结合常年治疗鼻咽癌经验,立法为清肝解毒、补脾益肾,处方:实施例1的中药配方,包括重楼10克、白花蛇舌草30克,石上柏10克,半枝莲10克,猫爪草10克、栀子10克、白芍10克、夏枯草10克、五指毛桃30克,太子参10克,薏苡仁30克,甘草5克,菟丝子15克,以上中药采用常规水煎煮;蜈蚣,微火焙干研粉,1克,守宫(壁虎),微火焙干研粉,5克,溶于中药煎取液中,服用,一天一剂。治疗1月后进行疗效评价。
4观察评价指标:
远期疗效:电话随访,以三年及五年存活率为指标。
近期疗效:常规以鼻咽局部肿块的控制率进行比较。评判指标如下。完全缓解:鼻咽肿块完全消失。部分缓解:鼻咽肿块缩小50%以上。无变化:鼻咽肿块缩小不及50%或增大未超过25%。进展:鼻咽肿块增大25%以上或出现新病变。
随着放疗或化疗技术的进步,鼻咽癌患者进行放疗及化疗后大多数鼻咽肿块基本上都得到控制,所以仅以进行放化疗及放化疗合并中药治疗的近期疗效为标准来评价两者的疗效意义不大。由于放射线以杀伤肿瘤组织的同时对正常组织也会造成电离损伤,以及化疗药的都存在着毒副作用,使鼻咽癌患者接受放化疗后基本上都会出现程度不一的后遗症如放射性咽炎、放射性鼻-鼻窦炎,所以我们以放疗后咽炎、鼻-鼻窦炎的发生率来评价中药的治疗效果。
放疗后咽炎的诊断:咽痛咽干严重,需不停饮水,查见咽粘膜充血明显,诊断为重度咽炎;咽痛咽干轻微,偶尔饮水,查见咽粘膜充血较轻,诊断为轻度咽炎;无咽痛无咽干,查咽粘膜无充血水肿,诊断为无咽炎。
放疗后鼻-鼻窦炎的诊断:持续鼻塞,流粘或黄涕,查见鼻粘膜充血,鼻道有粘脓涕,CT检查见鼻窦有炎性影像表现,诊断为重度鼻窦炎;间歇性鼻塞及流涕,查见鼻粘膜轻度充血,鼻道少量粘脓涕,CT检查见鼻窦有轻度炎性影像表现,诊断为轻度鼻窦炎;无鼻塞及流涕,查鼻粘膜无充血,鼻道无鼻涕,诊断为无鼻窦炎。
(二)结果
1.6例只用中药治疗的鼻咽癌患者治疗情况
到目前为止,鼻咽癌的治疗共识还是以放化疗为主的综合治疗,这6例患者均是在综合性医院肿瘤科就诊后,4例不愿进行放化疗治疗,2例为鼻咽癌晚期,已不能接受放疗的患者,均为分化型非角化性癌,年龄均在50-70岁,临床分期均为Ⅳ期。签知情同意书后经中药治疗后,其中不能进行放化疗的2例情况稳定,慢慢减少止痛药治疗,半年后失访(失访即失去联络,一般认为无效;即半年时有效,半年后为不知,可能为无效),4例鼻咽癌患者现还在服用中药,已存活3年,能正常干家务活。
近期疗效:6例,部分缓解1例,无变化5例。
远期疗效:6例,三年生存4例,生存率66.7%。
2.鼻咽癌放化疗后应用中药治疗的情况

表5结果显示:鼻咽癌放化疗后应用中药治疗,早中期患者5年生存率达90.9%,中晚期患者5年生存率达77.6%,比目前报道的中晚期5年生存率明显增高。

表6和表7的结果显示,中药治疗鼻咽癌放化疗后患者具有明显的效果,可延长5年生存期,同时减少鼻咽癌放化疗后合并咽炎及鼻-鼻窦炎等并发症的出现,效果显著。
上述实施例为本发明较佳的实施方式,但本发明的实施方式并不受上述实施例的限制,其他的任何未背离本发明的精神实质与原理下所作的改变、修饰、替代、组合、简化,均应为等效的置换方式,都包含在本发明的保护范围之内。

Claims (7)
1.一种治疗鼻咽癌及配合鼻咽癌放化疗的中药组合物,其特征在于,其有效成分提取自重楼、白花蛇舌草,石上柏,半枝莲,猫爪草、栀子、白芍、夏枯草、五指毛桃,太子参,薏苡仁,甘草,菟丝子,蜈蚣,守宫。
2.根据权利要求1所述的中药组合物,其特征在于,其有效成分的原料药配比为:重楼5-35份,白花蛇舌草25-65份,石上柏5-20份,半枝莲5-20份,猫爪草5-35份,栀子5-20份,白芍5-20份,夏枯草5-20份,五指毛桃25-65份,太子参5-20份,薏苡仁25-65份,甘草3-15份,菟丝子10-35份,蜈蚣1-3份,守宫3-12份;所述份数为质量份。
3.根据权利要求2所述的中药组合物,其特征在于,其有效成分的原料药配比为:重楼10-30份,白花蛇舌草30-60份,石上柏10-15份,半枝莲10-15份,猫爪草10-30份,栀子10-15份,白芍10-15份,夏枯草10-15份,五指毛桃30-60份,太子参10-15份,薏苡仁30-60份,甘草5-10份,菟丝子15-30份,蜈蚣1-2份,守宫5-10份;所述份数为质量份。
4.根据权利要求1-3任一项所述的中药组合物,其特征在于:重楼、白花蛇舌草、石上柏、半枝莲、猫爪草、栀子、白芍、夏枯草、五指毛桃、太子参、薏苡仁、甘草、菟丝子的有效成分为其水提物。
5.根据权利要求4所述的中药组合物,其特征在于:重楼、白花蛇舌草、石上柏、半枝莲、猫爪草、栀子、白芍、夏枯草、五指毛桃、太子参、薏苡仁、甘草、菟丝子的有效成分为其共煎物。
6.根据权利要求1-3任一项所述的中药组合物,其特征在于:蜈蚣的有效成分为其微火焙干并研磨得到的粉剂。
7.根据权利要求1-3任一项所述的中药组合物,其特征在于:守宫的有效成分为其微火焙干并研磨得到的粉剂。
