Abstract

背景技术
流感可以包括以呼吸道急性炎症以及发热、寒冷、肌肉痛和虚脱为特征的急性接触传染性病毒感染。流感还可以包括家养动物的多种病毒感染中的任一种,其特征通常在于急性炎症、发热和呼吸道累及。尽管不象流感一样严重,普通感冒(其可以由超过200种不同的病毒造成)可以包括头和胸的充血。

概述
一个实施方案涉及用于治疗流感、普通感冒和急性、亚急性和慢性炎症的方法、制剂和试剂盒。所述方法和制剂包括、但不限于:用于给有需要的患者递送有效浓度的左西替利嗪和孟鲁司特的方法和制剂。所述方法和制剂可以包括常规的和/或改进的释放元件,用于给患者提供药物递送。

本发明的实施方案涉及作为用于治疗急性、亚急性和慢性炎症的药物的左西替利嗪和孟鲁司特的组合。几个实施方案涉及用于治疗非IgE介导的、IgE-介导的和/或混合的非IgE介导的和IgE介导的炎症的左西替利嗪和孟鲁司特的组合。传统的变应性鼻炎是一种IgE介导的疾病;高达70-80%的哮喘患者也具有变应性鼻炎(特应性哮喘)。左西替利嗪和孟鲁司特的联合施用会在流感、普通感冒、变应性鼻炎和急性、亚急性和慢性炎症的治疗中表现出协同效应和意外地优良的结果。此外,左西替利嗪和孟鲁司特的组合可以与许多现有的治疗方案相结合地安全地使用。

左西替利嗪会使体外人鼻病毒滴度下降log-2。不受特定理论的约束,提议的细胞作用机理是,细胞内蛋白复合物NF-kB(核因子κB)的活化的减少,这又会引起I-CAM-1的减少。I-CAM-1(一种跨膜蛋白)被视作人鼻病毒进入细胞中的入口。在约50%的急性哮喘病例中可以发现鼻病毒,并且鼻病毒可以引起30-50%的“普通感冒”病例。已经独立地确定了病毒滴度的1个对数下降(one-log reduction)与征状改善相关联。另外,已经证实左西替利嗪会减少嗜酸性粒细胞迁移和减少炎症介质IL-4、IL-6和IL-8。IL-6是一种信号传递蛋白,它部分地调节:发热、身体对创伤的应答和急性(即时)期的变应性反应。
孟鲁司特(一种白三烯受体拮抗剂)如下起作用:以高亲和力和选择性结合CysLT1受体,从而抑制白三烯LTD4的生理学作用。白三烯是脂肪信号传递分子,其效应包括:与炎症性过程有关的气道水肿、平滑肌收缩和改变的细胞活性。白三烯的过度产生是哮喘和变应性鼻炎中的炎症的主要原因。半胱氨酰白三烯(LTC4、LTD4、LDE4)是花生四烯酸代谢的产物。这些白三烯从包括肥大细胞和嗜酸性粒细胞在内的多种细胞释放出来。它们会结合在人气道中的受体和在其它促炎症细胞(包括嗜酸性粒细胞和某些骨髓干细胞)上的受体。已经将半胱氨酰白三烯与哮喘和变应性鼻炎的病理生理学相关联。
白三烯D4是能够最有效地收缩气道平滑肌的半胱氨酰白三烯。白三烯受体(诸如CysLT1)遍布于呼吸树细胞(包括气道平滑肌细胞和气道巨噬细胞)以及体内的其它促炎症细胞(特别是嗜酸性粒细胞和某些骨髓干细胞)。白三烯也起促进嗜酸性粒细胞、树突细胞和T细胞的募集的功能。有些作者将嗜酸性粒细胞浸润视作炎症的标志。

象第二代抗组胺剂左西替利嗪一样,孟鲁司特在小于2天内达到稳态水平。与其它目前可得到的白三烯调节剂齐留通和扎鲁司特不同的是,不需要肝功能的常规监测。与华法林、茶碱、地高辛、特非那定、口服避孕药或泼尼松没有药物相互作用。
所述2种分子是安全的,即,在美国被FDA批准用于低至6个月龄的变应性障碍。它们可以为主地使用,或与许多现有的、用于治疗炎症(包括、但不限于流感、急性哮喘和普通感冒)的治疗方案联合使用。两种药物都是妊娠B类(表II)。

现有的炎症治疗聚焦于表象的潜在病症和性质。常用众多的药剂,诸如:苯海拉明()、氧、肾上腺素、甾体、β-激动剂、非甾体抗炎药(NSAIDS)、解热药、抗生素、抗真菌药和抗病毒剂。自相矛盾地,常用的NSAIDS实际上会增加白三烯的生成。
甾体(其被广泛地用于治疗炎症)具有显著的短期和长期副作用(表III)。在治疗与鼻鼻窦炎有关的炎症方面,鼻甾体具有它们的限制,特别是在老年人中和在使用处方中的阿司匹林、氯吡格雷或华法林来减少中风和心脏病发作风险的那些患者中。甚至在没有使用这些传统的“血液稀释剂”的患者中,由鼻甾体喷雾剂引起的自发鼻出血的风险是4-22%。鼻出血的风险是药物依赖性的。在许多55岁或更老的患者中,鼻出血是一项重要的考虑因素。

就成年人而言,左西替利嗪的典型日剂量是5mg,左西替利嗪表现出下述有利性质:i)达到峰值血浆水平的时间较短-0.9小时;ii)达到稳态水平的时间较短-40小时;iii)低分布容量(直接到达靶受体);iv)高的24小时受体占有率-约75%;v)在发炎的组织中增加的受体亲和力(酸性pH;高达第一代分子的受体亲和力的5倍);vi)妊娠B类;vii)FDA批准用于低至6个月的其它疾病状态,即,常年性变应性鼻炎和慢性特发性荨麻疹;viii)抗炎性质;和ix)抗病毒性质。在人类中的研究已经证实,可以安全地施用最高30mg/天的左西替利嗪剂量。
孟鲁司特(一种白三烯受体拮抗剂)共同地起作用,以保护呼吸树以及阻断炎症级联中的介质。就成年人而言,孟鲁司特的典型日剂量是10mg,孟鲁司特表现出下述有利性质:i)孟鲁司特是一种选择性的受体拮抗剂,其抑制LTD4在CysLT1受体处的生理学作用;ii)孟鲁司特以高亲和力和选择性结合CysLT1受体,且不会产生任何激动剂活性;iii)孟鲁司特会被快速地吸收;iv)孟鲁司特在3-4小时内达到峰值血浆浓度;v)孟鲁司特的口服生物利用度和Cmax不受标准进餐的影响;vi)孟鲁司特具有至50mg的线性药代动力学;vii)在成年人中低至5mg的剂量会造成LTD4诱发的支气管收缩的基本阻断;viii)在安慰剂对照的交叉研究中,孟鲁司特使由抗原攻击导致的早期支气管收缩抑制了75%;ix)孟鲁司特被FDA批准用于低至6月龄;和x)孟鲁司特与华法林、茶碱、地高辛、特非那定、口服避孕药或泼尼松没有药物相互作用。孟鲁司特已经以高达200mg/天的剂量施用给成年患者持续22周,且在短期研究中,以高达900mg/天施用给患者持续大约1周,都没有临床上重要的不利经历。
因此,左西替利嗪和孟鲁司特在美国都是妊娠B类,且在美国被FDA批准用于低至6月龄幼儿的其它疾病过程。此外,两种药物只需每天1次用药,且就大多数临床情况而言,不需要常规的血液监测工作。此外,两种药物表现出与其它药物极微小的临床上有关的相互作用。如本文所述,左西替利嗪和孟鲁司特都在2天内达到稳态水平,以快速地产生协同效应和互补的抗炎效应。
孟鲁司特和第二代抗组胺剂非索芬定的施用,在变应性鼻炎的治疗中具有协同效应。变应性鼻炎(也称作花粉症或花粉热)是鼻气道的一种变应性炎症,其发生在具有遗传上敏感的免疫系统的个体(估测20%的人群)吸入变应原(诸如花粉或粉尘)时。所述变应原会触发抗体生成,所述抗体是一种血清特异性的免疫球蛋白E(IgE),其又可以结合含有组胺的肥大细胞和嗜碱性粒细胞。在再次暴露于侵犯的抗原以后,组胺会释放,造成瘙痒、肿胀和黏液生成,这些是季节性变应性患者熟知的。孟鲁司特和非索芬定的组合会主观地(使用患者日记和VAS评价)和客观地(使用鼻腔测压法和身体检查)减轻鼻塞,与单独的非索芬定相比,或与非索芬定与安慰剂相比,具有统计显著性。

在这里,我们描述了组合左西替利嗪和孟鲁司特的意外协同效应。不希望受特定理论的约束,在细胞水平对左西替利嗪的药代动力学的详细检查,阐释了延伸到超过IgE介导的组胺释放的独特炎症性质。左西替利嗪表现出低分布容量(0.4L/kg)、在酸性pH中延长的从H1受体分解的时间、增强的受体亲和力(作为西替利嗪的纯异构体)和任何目前可得到的抗组胺剂中最高的24小时受体占有率。这些参数会通过减量调节IL-4、IL-6、IL-8以及细胞黏附分子来赋予炎症效应。所述细胞黏附分子是在细胞与细胞黏附、细胞的募集、寻靶(homing)和愈合中涉及的可诱导的免疫球蛋白、整联蛋白和选择蛋白的同质组。另外,已经证实左西替利嗪会在体内减少ICAM-1、IL-6、IL-8、TLR3表达和NF-κB活化,从而导致人鼻病毒滴度降低log-2。许多鼻病毒血清型具有相同的细胞受体,从而将ICAM-1鉴别为进入细胞中的入口。左西替利嗪抑制气道上皮细胞中鼻病毒诱导的ICAM-1和细胞因子表达和病毒复制。病毒脱落下降1个对数会导致HRV感染的(人鼻病毒)患者的显著临床益处。
一个未满足的临床需要源自2009年的H1N1大范围流行。流感的主要候选药物奥塞米韦没有表现出减少流感相关的下呼吸道并发症。就神经氨酸酶抑制剂而言,疾病仅缩短半天至1天,这表明,神经氨酸酶抑制剂不会阻止感染或停止经鼻的病毒分泌,因此可能是中断病毒大范围流行传播的次优方式。此外,在该时间范围内,加利福尼亚州报告了关于孕妇和产后妇女中的H1N1流感严重性的惊人数据,即,从2009年4月23日至8月11日,22%的孕妇和产后妇女需要重症监护来治疗H1N1,且8%死亡。临床上已经证实,左西替利嗪+孟鲁司特的组合(加入后者来保护下气道;二者都是妊娠B类)可以安全地且有效地用于改善/缩短流感病程。
不希望受特定理论的约束,甾体模型提示,左西替利嗪在NF-kB水平以非IgE介导的能力起作用(参见图1),而孟鲁司特作用于CysLT1受体来抑制LTD4的生理学作用。已知两种分子会减少嗜酸性粒细胞的量或它们向炎症部位的迁移。另外,孟鲁司特还会减少树突细胞和T细胞的募集。
左西替利嗪+孟鲁司特的作用胜过各自单独的生理学机制,很好地超出变应性鼻炎和哮喘的治疗。至少部分地,左西替利的抗病毒和抗炎性质与核因子kB相对;左西替利嗪和孟鲁司特二者抑制嗜酸性粒细胞量/迁移的能力,会强化孟鲁司特对LTD4的作用的抑制,这会产生协同作用。该协同作用反映为,在众多急性和慢性炎性疾病状态中显著改善的临床结果。

几个实施方案涉及左西替利嗪和孟鲁司特的组合用于治疗伴渗出液中耳炎和有关的耳病诸如慢性乳突炎和咽鼓管(从鼻后连通至中耳的咽鼓管)功能障碍的用途。在有些实施方案中,左西替利嗪和孟鲁司特可以与抗生素一起施用,以治疗例如伴脓性中耳渗出液的急性中耳炎。在有些实施方案中,可以在没有抗生素的情况下施用左西替利嗪和孟鲁司特,以治疗慢性中耳渗出,例如,慢性中耳炎。在有些实施方案中,左西替利嗪和孟鲁司特可以与其它治疗方式(如,但不限于,甾体和/或抗病毒剂)一起施用。
几个实施方案涉及左西替利嗪和孟鲁司特的组合用于治疗变应性真菌性鼻鼻窦炎(AFRS)的用途。在有些实施方案中,左西替利嗪和孟鲁司特可以与其它治疗方式(如,但不限于,甾体和/或抗真菌剂)一起施用。
左西替利嗪和孟鲁司特(后者目前在美国处于研究中)的静脉内治疗,会增强目前施用口服药物看到的单个的和联合的临床应答。静脉内孟鲁司特血浆浓度曲线型式(curveprofile)下面积(7mg)比得上批准的10mg口服孟鲁司特片剂。已经在急性哮喘中证实,与安慰剂相比,前者在10分钟时会显著提高FEV1(1秒钟用力呼气容积)。
因此,针对急性炎症的用药可以是如上所述在相同背景中每天进行,作为双层片剂,和/或作为供10天疗程的含有两种药物的泡罩包。就中等至严重的临床表现而言,在第一个24小时的一天(24hour day)期间,可以在时间零点(5mg)、12小时(5mg)和24小时(5mg)施用左西替利嗪组分,以便在小于40小时内达到该分子的稳态水平。已经在高达30mg/天进行了左西替利嗪人用药安全性研究。镇静是在更高的剂量发生的主要副作用。独立研究已经证实,可以以20mg/天单独给药左西替利嗪,以治疗特发性荨麻疹的严重病例。
左西替利嗪和孟鲁司特的组合的用途包括、但不限于:治疗、改善或预防下述征状。就流感而言,所述组合可以用于缩短季节性流感的病程,预防或最小化下呼吸道感染/并发症的发展,和/或在下一次大范围流行(例如,H5N1,它伴有50%的死亡率)之前建立改善的、安全的、全世界的流感方案。就上呼吸道感染(不限于鼻病毒)而言,所述组合可以用于限制感染本身,和/或预防或减少继发性鼻窦炎、支气管炎和肺炎的潜在发展。所述组合可以用于治疗Ebstein-Barr病毒,特别是、但不限于累及呼吸的那些患者。
就与现有方案相结合的急性哮喘(不限于由鼻病毒造成的恶化(约50%的病例))而言,所述组合可以用于缩短病程,减少住院治疗和死亡。所述组合可以用于预治疗对一类或更多类需要抗生素的抗微生物治疗过敏的患者。这些患者发展随后的ALE(变应性样事件)的风险是一般人群的4-10倍。就具有中等至严重的危及生命的疾病的、需要两种/三种抗生素的患者而言,所述组合可以用于减少从主要治疗药物发展出副作用的可能性。在辐射疗法过程中和以后,所述组合可以用于改善炎症应答。就需要甾体来治疗炎症的患者而言,所述组合可以是有用的,否则所述患者会处于发展甾体诱发的并发症的增加的风险中。实例包括、但不限于下述的:i)严重的胰岛素依赖性的糖尿病,伴有感染诸如面瘫,和ii)具有潜伏性结核病的患者。就使用抗病毒药物来治疗急性疾病的患者而言,所述组合可以用于预防与药物有关的并发症以及与疾病过程本身有关的并发症。所述组合可以用于治疗血清病,伴随使用或不使用甾体。就接受免疫疗法的患者的预治疗而言,所述组合可以用于预防或改善全身性反应的风险。具有发展危及生命的全身性事件的潜在高危患者的实例包括、但不限于:严重的哮喘患者,具有并发的呼吸道感染的那些患者,和具有先前的全身性反应史的那些患者。就接受化学疗法的那些患者的预治疗和治疗内(intra-treatment)而言,所述组合可以用于改善与化疗药物的施用有关的副作用。就表现出输血反应的患者而言,所述组合可以在最初反应期间和在为任何必要的后续输血做准备期间,用于限制副作用/危及生命的事件。
本领域技术人员会容易地明白,要施用的左西替利嗪和孟鲁司特的有用体内剂量和具体给药模式将随下述因素而变化:患者的年龄、体重、医学状况,要治疗的病症的严重性,给药途径,患者的肾和肝功能,和治疗的哺乳动物物种,采用的具体化合物,以及这些化合物的具体用途。本领域技术人员使用常规的药理学方法,可以确定有效剂量水平,即实现希望的结果所需的剂量水平。通常,产物的人临床应用从较低的剂量水平开始,增加剂量水平直到实现希望的效果。有利地,本发明的实施方案的化合物可以以例如单次日剂量施用,或者可以以分份剂量(每天2、3或4次)施用总日剂量。

取决于急性过程的严重性,可以修改表VI中的剂量。例如,可以在时间零点(在提供时)施用年龄适当的剂量的左西替利嗪,在12小时时施用额外的年龄适当的剂量。为了保护下气道,特别是在支气管炎/肺炎的情况下,可以在时间零点(在提供时)施用一定剂量的孟鲁司特,在12小时时施用额外的年龄适当的剂量的孟鲁司特。以此方式,两种药物的稳态水平会接近24小时。象左西替利嗪一样,孟鲁司特被视作非常安全的分子。孟鲁司特已经以高达200mg/天的剂量(20x标准成年人日剂量)施用给成年患者持续22周,且在短期研究中,以高达900mg/天(90x标准成年人日剂量)施用给患者持续大约1周,都没有临床上重要的不利事件。用药持续时间可以与就他们各自的疾病状态而言普遍接受的方案并行。例如,急性传染性疾病过程的常规治疗通常施用5-14天。组合的左西替利嗪(每天1次)+孟鲁司特(每天1次)的疗程可以以相同的持续时间施用。对于慢性炎性疾病状态的治疗,还可以施用年龄适当的每天1次剂量的每种药物。

实施例7-与孟鲁司特结合施用左西替利嗪来治疗流感的协同效应
基于发明人的使用左西替利嗪和孟鲁司特的临床经验,在对照研究中预见到下述结果。鉴别出20个患者的组群,并分成4个治疗组,所述患者在15-30岁之间(且没有其它共病疾病过程),呈现典型的流感征状:头痛、发热、干咳和极度疲劳。第一组(CONTROL)每人接受2片安慰剂,每天同时一起施用,持续10天的时段。第二组(LEVO)接受左西替利嗪(5mg,口服)和1片安慰剂,每天同时一起施用,持续10天。第三组(MONT)接受孟鲁司特(10mg,口服)和1片安慰剂,每天同时一起施用,持续10天。第四组(COMBO)同时接受左西替利嗪(5mg,口服)和孟鲁司特(10mg,口服),持续10天。
CONTROL患者表现出“正常的”流感病程,征状持续7-10天。实验室数据应当由以下组成:全血计数、综合代谢检测组合、C-反应蛋白、T和B细胞淋巴细胞检测组合(panel)、胸部x-射线和气道的病毒和好氧菌培养物。在初次就诊时记录生命体征,在中间跟踪患者的发热特性。记录征状(头痛、发热、咳嗽、疲劳)的发作、持续时间和强度以及征状消退的时间(时间零点-第一次给药)。第二个终点数据包括恢复完全活动、工作/上学所需的时间。
LEVO患者表现出减轻的发热特性和总体疲劳感,疾病过程没有显著的缩短。
MONT患者表现出堵塞和呼吸短促的征状的改善。
COMBO患者表现出征状的改善,疾病过程共缩短了超过3天。与CONTROL、LEVO和MONT的7-10天相比,患者在3-5天以后表现出有限的/没有疾病病理学。
所述结果与所述组合的协同效应相一致。
实施例8-测试与孟鲁司特结合施用左西替利嗪来治疗鼻病毒的协同效应
鉴别出100位非特应性健康志愿者(经测试为人鼻病毒16(HRV-16)阴性)的组群,用于参与随机的、双盲的、安慰剂对照的试验,所述试验设计成测试使用左西替利嗪和孟鲁司特的治疗是否会减少与HRV-16的实验性接种有关的鼻炎征状的持续时间和/或严重性。在研究开始时,将最大流量计(peak flow meter)分发给参与者,并要求参与者每天填写最大流量和感冒征状日记,在随机化之前持续7天,在接种前持续7天,然后在接种后持续14天。在基线时、在急性感冒期内和在恢复后,从参与者收集鼻灌洗液、痰和血液样本。
使用Jackson量表(Jackson scale),以4-点量表(0,不存在;1,轻度;2,中等;3,严重)评估13种感冒征状(咳嗽、流涕、打喷嚏、鼻塞、咽喉痛、头痛、不适、寒冷、寒战、发热、喉炎、关节或肌肉痛和眼流泪或灼热)。在研究时间,每天2次对征状评分,并使用每种征状的最高值来计算总的每天最高征状评分(TDHSS;最大可能每天评分为39)。
将参与者随机化,并分成4个治疗组。第一组(CONTROL)每人接受2片安慰剂,每天同时一起施用,贯穿研究持续时间。第二组(LEVO)接受左西替利嗪(5mg,口服)和1片安慰剂,每天同时一起施用,贯穿研究持续时间。第三组(MONT)接受孟鲁司特(10mg,口服)和1片安慰剂,每天同时一起施用,贯穿研究持续时间。第四组(COMBO)同时接受左西替利嗪(5mg,口服)和孟鲁司特(10mg,口服),贯穿研究持续时间。
给所有参与者接种HRV-16,同时开始使用研究药物/安慰剂。通过多重聚合酶链式反应(PCR),分析接种当天采集的样品中其它呼吸道病毒的存在。
在给4个含有WI38细胞(人肺二倍体细胞;ViroMed,Minnetonka,Minnesota)的组织培养管接种每个样品的连续10倍稀释液(100-10-7)和在滚动的同时在33℃孵育10天后,计算来自鼻灌洗液的病毒滴度。在24小时,对试管进行读数,然后每隔一天进行读数,直到第10天。将TCID50计算为能够感染50%的试管的浓度。将病毒滴度表示为TCID50/毫升。从鼻灌洗液样品提取总RNA,并反转录,测量HRV RNA的量。
使用Wilcoxon秩和检验来比较结果,包括最大流量、痰和鼻细胞计数和百分比、感冒征状评分、病毒脱落和RNA。使用Fisher精确检验来比较分类结果,包括鼻嗜酸性粒细胞的检测。对于每天记录的数据(征状评分),计算4个研究周(基线、随机化、急性期、康复期)中的每一个的平均值和最大值,并在组之间进行对比。将连续结果总结为中位值(四分位数间距[IQR])。将两侧(2-sided)P<.05视作统计上显著的。
基于发明人的使用左西替利嗪和孟鲁司特的临床经验,在对照研究中预见到下述结果。
预期CONTROL患者表现出“正常的”普通感冒病程,征状持续7-10天。
预期LEVO患者表现出普通感冒征状(TDHSS)的轻度至中等减轻,总疾病过程的持续时间没有显著缩短。

实施例9-测试与孟鲁司特结合施用左西替利嗪来治疗伴有鼻息肉病的慢性鼻鼻
窦炎(CRSwNP)的协同效应
鉴别出诊断患有CRSwNP的志愿者的组群,用于参与随机的、双盲的、安慰剂对照的试验,所述试验设计成测试使用左西替利嗪和孟鲁司特的治疗是否会减少与慢性鼻鼻窦炎有关的鼻息肉和征状的数目和大小。在研究开始时,每个参与者接受:(1)鼻周窦的基线CT扫描,(2)具有照片的鼻内窥镜评价,和(3)标准化的疾病相关生活质量(QOL)问卷,诸如鼻结膜炎(rhinoconjunctivitis)生活质量问卷。
将参与者随机化,并分成4个治疗组。第一组(CONTROL)每人接受2片安慰剂,每天同时一起施用,贯穿研究持续时间。第二组(LEVO)接受左西替利嗪(5mg,口服)和1片安慰剂,每天同时一起施用,贯穿研究持续时间。第三组(MONT)接受孟鲁司特(10mg,口服)和1片安慰剂,每天同时一起施用,贯穿研究持续时间。第四组(COMBO)同时接受左西替利嗪(5mg,口服)和孟鲁司特(10mg,口服),贯穿研究持续时间。
在12个月的持续时间,每2个月评价参与者。采集参与者的鼻道的内窥镜照片,以记录鼻息肉的数目和大小以及炎症水平。重复生活质量问卷。在研究结束时,在每位参与者上获得以下:(1)鼻周窦的跟踪CT扫描,以与基线检查进行对比,(2)内窥镜照片,和(3)最终的QOL问卷。
基于发明人的使用左西替利嗪和孟鲁司特的临床经验,在对照研究中预见到下述结果。
预期CONTROL患者在研究期间表现出有限的或不表现出鼻息肉的数目和大小的改善,以及有限的或没有QOL改善。
预期LEVO患者在研究期间表现出鼻息肉的数目和大小的轻度至中等改善和/或QOL的轻度至中等改善。
预期MONT患者在研究期间表现出鼻息肉的数目和大小的轻度至中等改善和/或QOL的轻度至中等改善。
预期COMBO患者与CONTROL、LEVO和MONT相比表现出QOL的显著改善。进一步预期COMBO患者表现出鼻息肉的数目和大小的显著减少。
Claims (17)
1.左西替利嗪和孟鲁司特的组合在制备用于缩短选自流感和感冒的病症的持续时间的组合药物中的应用。
2.根据权利要求1所述的应用,其中所述病症是流感。
3.根据权利要求1所述的应用,其中所述病症是感冒。
4.根据权利要求1所述的应用,其中所述病症由病毒感染造成。
5.根据权利要求4所述的应用,其中所述病毒感染由选自由以下各项组成的组的病毒造成:鼻病毒、冠状病毒、流感、人副流感病毒、人呼吸道合胞体病毒、腺病毒、肠道病毒和间质肺病毒(metapneumovirus)或Epstein-Barr病毒。
6.根据权利要求1所述的应用,其中所述病症与细菌感染相关。
7.根据权利要求6所述的应用,其中所述细菌感染由选自由以下各项组成的组的细菌造成:肺炎链球菌(Streptococcus pneumoniae),流感嗜血菌(Haemophilus influenzae),黏膜炎莫拉氏菌(Moraxella catarrhalis),金黄色葡萄球菌(Staphylococcus aureus),厌氧菌和革兰氏阴性细菌。
8.根据权利要求1-7中任一项所述的应用,其中所述左西替利嗪和孟鲁司特以连续方式使用。
9.根据权利要求1-7中任一项所述的应用,其中所述左西替利嗪和孟鲁司特以基本上同时的方式使用。
10.根据权利要求1-7中任一项所述的应用,其中在征状发作时使用所述左西替利嗪和孟鲁司特,用于治疗流感和感冒。
11.根据权利要求1-7中任一项所述的应用,其中所述左西替利嗪和孟鲁司特被独立地配制成用于经由选自以下的递送途径来使用:口服的、肠内的、静脉内的、腹膜内的、吸入、肌肉内的和皮下的。
12.根据权利要求1-7中任一项所述的应用,其中所述左西替利嗪和孟鲁司特被配制成用于经由相同的递送途径来使用。
13.根据权利要求1-7中任一项所述的应用,所述组合药物还包含甾体。
14.根据权利要求1-7中任一项所述的应用,其中所述组合药物还包含抗生素。
15.根据权利要求1-7中任一项所述的应用,其中所述组合药物还包含额外的试剂,所述额外的试剂选自由苯海拉明,肾上腺素,β-激动剂,NSAID,解热药,抗真菌药,抗病毒剂及其组合组成的组。
16.根据权利要求1-7中任一项所述的应用,其中在暴露给病原体后1小时、6小时、12小时、1天、2天、3天、4天、5天、6天、7天、8天、9天、10天、11天、12天、13天、14天、15天、16天、17天、18天、19天、20天、21天、22天、23天、24天、25天、26天、27天、28天、29天和/或30天,所述左西替利嗪和孟鲁司特被立即施用。
17.根据权利要求1-7中任一项所述的应用,其中所述左西替利嗪和孟鲁司特被预防性使用以阻止病毒性呼吸道感染发展成急性的、经常是机会性的、继发的细菌性鼻窦炎、支气管炎和/或肺炎。
