背景技术
瘘是发病和死亡的主要诱因,因为每年有超过十万的病理瘘病例,其中死亡病例超过一万。每年耗费医疗体系数亿美元来进行治疗。
瘘是体腔与中空器官之间或此类体腔或器官与体表之间的组织线路连接。瘘管道包括从主瘘开口延伸至盲端或通向一个或多个副瘘开口的软组织中的空穴。瘘通常是由于感染形成或伴随着脓肿形成。虽然一些瘘是出于治疗目的而有意形成的,例如气管造口管道、胃进食管道或用于透析接入的动静脉瘘,但病理学瘘通常是先天性出现或手术、与手术相关的并发症或外伤之后形成的病态管道。它们最常见的是已上皮化、内皮化或黏液化的开口管道。

虽然瘘可形成在体内的许多位置,但它们几乎普遍使患者处于高度病态并且临床医生难以治疗。治疗选择可能包括外科手术,同时其他治疗选择可能包括设计成有助于瘘闭合的可植入式装置。然而,这些装置可引起对患者不利的免疫反应,可容许装置周围的液体泄漏,或者可能导致污染瘘管道。在使瘘闭合的可植入式装置领域中,所需的是降低不利免疫反应的机会、减少流体经由瘘管道泄漏以及在使用中减少污染瘘管道可能性的装置。
发明内容
本发明公开了一种专门用于治疗肛门直肠瘘的可植入式瘘闭合装置、相关套件和使用方法。在一些实施例中,肛门直肠瘘治疗装置可能包括由凹面和凸面组成的圆盘状远端锚定件、外缘区、从凹面突出的内部密封区以及用于附接连接部件的附接区,其中,内部密封区可能至少比附接区的一部分厚,且外缘区被调整到与内部密封区成锐角。用于治疗肛门直肠瘘的远端锚定件可能具有锥形边,外缘区和内部密封区可以构型形成单独密闭用以抵靠远端开口的组织。肛门直肠瘘治疗装置的远端锚定件的直径范围可在大约0.06"到0.09"之间。此外,远端锚定件的外缘区的平均厚度范围可在大约0.005"到0.03"之间,和/或低于远端锚定件外径的10%。在一些实施例中,外缘区的平均厚度可在锚定件外径的大约3%到5%之间。外缘区的宽度范围也可在大约0.01"到0.3"之间,和/或在远端锚定件外径的大约5%到45%之间。在一些实施例中,外缘区的宽度范围可在远端锚定件外径的大约15%到30%之间。肛门直肠瘘治疗装置内部密封区的平均厚度范围可在大约0.03"到0.09"之间,和/或低于远端锚定件外径的大约25%。在一些实施例中,内部密封区的平均厚度可在远端锚定件外径的大约5%到10%之间。肛门直肠瘘治疗装置内部密封区的宽度范围也可在大约0.05"到0.35"之间,和/或在远端锚定件外径的大约5%到45%之间。在一些实施例中,内部密封区的宽度可在远端锚定件外径的大约5%到25%之间。肛门直肠瘘治疗装置附接区的平均厚度范围可在大约0.08"到0.15"之间,和/或至少为远端锚定件外径的30%。附接区的宽度范围可在大约0.15"到0.35"之间,和/或在远端锚定件外径的大约15%到60%之间。在一些实施例中,肛门直肠瘘治疗装置附接区的宽度范围可在远端锚定件外径的大约25%到40%之间。此外,肛门直肠瘘治疗装置可包含由同心外部边缘及内部边缘组成的内部密封区,且远端锚定件还可包括在内部密封区的内部边缘径向测量的曲率半径,该曲率半径的范围在大约0.12"到0.75"之间,和/或在远端锚定件外径的大约20%到85%之间。在一些实施例中,在内部密封区内部的边缘之间径向测量的曲率半径可在远端锚定件外径的大约35%到55%范围内。
肛门直肠瘘治疗装置可包括附接到远端锚定件的附接区的连接部件和管形部件。在一些实施例中,连接部件可穿过管形部件。肛门直肠瘘治疗装置的远端锚定件可包括第一盘形材料,且附接区可包括不同于第一物质的第二材料。此外,附接区可包括至少一个层部件,其厚度小于附接区厚度。在一些实施例中,层部件可由网孔组成。远端锚定件可包括至少一个带有至少一个孔的层部件。在一些实施例中,层部件可粘附在远端锚定件上。在其他实施例中,层部件可嵌在远端锚定件内。肛门直肠瘘治疗装置的连接部件可连结至在部件间建立接触的层部件上。连接部件和层部件间形成的接触可嵌在远端锚定件内,或者可置于远端锚定件的外表面上。在一些实施例中,远端锚定件还可包括远端锚定件凸面上的触觉标记。触觉标记可包括一个或多个突出节结或一个或多个凹槽。
在任何实施例中,肛门直肠瘘治疗装置可包括由凹凸两面组成的盘形远端锚定件;凹面由外缘区、内部密封区、构造成附接至连接部件的附接区及同心位于内部密封区和附接区之间的中间区。附接区的厚度可大于中间区的厚度;内部密封区的厚度可大于外缘区及中间区的厚度。内部密封区和附接区横截面积的形状可以是圆形、三角形、椭圆形、矩形等形状。在一些实施例中,内部密封区到外缘区和附接区的距离相等。在一些实施例中,内部密封区与附接区之间的距离可小于或大于内部密封区与外缘区之间的距离。在一些实施例中,远端锚定件的外缘区可包括近端环和远端环。远端环可配置于第一平面内。此外,内部密封区可包括同心外部及内部边缘和环,其由内部密封区边缘间最大厚度区表面形成,且可配置于第二平面内。第一及第二平面可相互平行,且第二平面可位于第一平面和附接区之间。在一些实施例中,外缘区可包括同心外部及内部边缘,且外部边缘可配置在第一平面内。此外,在这些情况下,第一平面和外缘区内表面形成的夹角可小于或等于45°。在不同实施例中,内部密封区可包括同心外部及内部边缘和位于内部密封区边缘间中点并可能突出于远端锚定件凹面的环。此外,外缘区可包括同心外部及内部边缘,且远端锚定件还可包括在外缘区的内部及外部边缘之间径向测量的曲率半径,可在大约0.03"和0.15"范围内。在一些实施例中,在外缘区的内部及外部边缘之间径向测量的曲率半径可在远端锚定件外直径的大约5%和15%范围内。在一些实施例中,外缘区可包括同心外部及内部边缘,且远端锚定件还可包括在外缘区的内部及外部边缘之间径向测量的第一曲率半径。此外,内部密封区可包括同心外部及内部边缘,且远端锚定件还可包括在内部密封区边缘之间径向测量的第二曲率半径。在一些实施例中,第一曲率半径可在第二曲率半径的大约10%和大约65%范围内。在一些变化中,第一曲率半径可在第二曲率半径的大约15%和大约35%范围内。

具体实施方式
瘘管道可以是非线性的或曲线的,且在管道内不同间隔处包含尺寸不同的腔体。瘘管也可以由多个互联的或分支通道构成。本文公开的可植入式瘘闭合装置可以利用有利的设计、构型技术和属性满足这些约束,并且例如说,可以用于治疗肛门直肠瘘。
见图1,描述了一种可植入式瘘闭合装置10及其输送装置12。如图所示,该瘘闭合装置10包括密封件或远端锚定件13以及分段式可扩张体部14,该体部由通过连接部件18或系绳或缝线联接在一起的多个独立的可扩张体部或部件16组成。部件16可以以紧邻抵靠方式或间隔开的方式联接在一起(如图所示),并且可以在两个构型之间滑动。一旦装置10在可扩张部件16处于收缩或压缩状态时插入瘘管道,那么该可扩张部件16一旦暴露于体温/体液中,能够变柔性和/或扩张以填满各可扩张部件16所在的瘘管道的部分。值得注意的是收缩或压缩状态有助于装置10方便地插入瘘管道。此外,装置10的体部14的分段性质,或更具体而言,装置的体部14由多个独立部件16形成的事实,容许体部14更容易弯曲并被放入,且使其在瘘管道内扩张时更容易顺应瘘管道的曲折且直径变化的构型。因此,一旦容许体部14在瘘管道内扩张,则该装置大致完全地填充瘘管道。应当理解为可扩张部件16可以根据瘘管的尺寸、形状和特征,设计成能够径向扩张以达到不同的最终直径。在一些实施例中,可扩张部件16几乎没有扩张,以至于可以忽略其最初直径和最终直径之间的差异。而在其它实施例中,可扩张部件16会显著增加。此外,如果需要的话,不同扩张特征的可扩张部件16可以用于一种瘘闭合装置10。设计出一种具有不同可扩张部件的瘘闭合装置10有助于更好地适应直径变化的瘘管。
如图1所示,输送装置12可包含装入瘘闭合装置10的带有内腔22的导管或护套20。使用时,装入的导管或护套20可以插入瘘管道(如肛门直肠瘘),并且密封部件或锚定件13可以放置在紧邻瘘管道远端开口的地方。接着,从装置体部14周围取出装入的导管或护套20,使装置体部14留在管道内。于是装置体部14扩张进而填充并闭塞管道。
在一些实施例中,输送装置12包括两个或更多直径不等带有内腔的导管、套管或护套。例如,第一导管可以带有一个足够大内腔,以便适应连接部件18。第一导管也可以包含一个锥形近端,这样能够使在瘘管中移动更容易。第二导管可以包含一个能容纳可扩张部件16的足够大的内腔,并且能够容许在可扩张部件16放入瘘管道时,第二导管可以纵向滑过第一导管。在一些变型中,这两种导管可以用同一种材料制成。在其它的变型中,这两种导管可以用具有不同特性的材料制成,以便插入,例如,可以使用硬度不同的材料。此外,第一导管的近端可以包括阻力部件或闭锁装置来阻止或防止第二导管完全滑过第一导管。在一些实施例中,阻力部件或闭锁装置能够防止连接部件18由于疏忽与输送装置12分开。如图1所示,闭锁装置具有防止第一导管与第二导管分开的所有特征,包括但不限于,直径大于第二导管内腔的环、在第一导管近端径向延伸的扣环、或者是连接部件18环绕的孔。
在某些实施例中,当体部14扩张以填充瘘管道时,通常装置会阻止流体流经瘘管道。通过阻止或减少源于管道远端的体液通过瘘管道,此外,在一些实施例中,也通过减少源于管道本身的体液流经瘘管道的流量或速率来降低闭合的时间以及手术的必要性(例如,十分显著)。在某些实施例中,本文公开的瘘闭合装置能够减少或消除流体经过管道的通道,同时提供能够促使组织生长的基质。
尽管已经说明了分段式体部14,组织治疗的某种实施例可以包含一种非分段式体部(即,为连续的单件式体部,相对于多个体部16组成)。
图2A为锚定件13的顶视图,且图2B为沿线2B-2B的剖视图。如图2A和图2B所示(连同图1),锚定件13为一个凹面有弹性的肠锚定件。该锚定件形成双重密封构型,以抵靠一个瘘远端开口周围的组织表面,例如一个肛门直肠瘘。由锚定件13形成的密封可完全阻塞远端开口或入口,例如,锚定件13可完全在远端开口周界形成一个密封。在某些情况下,锚定件13可包括一个或多个实质上不渗透或无孔材料,可实质上不渗透或无孔。在某些实施例中,锚定件13的水蒸气传输率(WVTR)可为小于约6000g/m2/24hr(例如,小于约5000g/m2/24hr,小于约3000g/m2/24hr,小于约2000g/m2/24hr,小于约1000g/m2/24hr,小于约500g/m2/24hr,小于约250g/m2/24hr,小于约100g/m2/24hr,小于约50g/m2/24hr,小于约25g/m2/24hr,小于约15g/m2/24hr,小于约10g/m2/24hr)。此外,在一些实施例中可形成一个密封。
在一些实施例中,可在恰当位置固定锚定件,这可提高瘘开口周围的密封性。在某些情况下,装置10末端的一个近端夹具可被用来向连接部件和锚定件上施加张力,这可增强锚定件形成的密封,和/或进一步将装置10固定在管道中。在一些变型中,锚定件13可包括一个或多个固定孔口,孔口可使锚定件13附着在皮肤或真皮瘘开口周围的一个绷带上。这些固定孔口可在锚定件周缘附加隔开。可使用任何适当尺寸、合适数目的孔口。在这些变型中,锚定件可用合适药线缝到恰当位置,包括但不限于,可吸收线和涂有抗菌成分的线。在其他示例中,锚定件可包括粘合在它的近端,该粘合表面接触瘘附近的皮肤且阻止运动。在一些变型中,粘合剂可在制造过程中应用在锚定件上,在这种情况下,锚定件可进一步包括一个防粘衬里,防粘衬里可在使用前去除。在其他变型中,医务人员可在将装置植入病人之前应用粘合剂。在任何一个实施例中,锚定件可包括显微针、倒钩、钩子或任何其他合适的固定装置将锚定件固定在组织表面。固定装置可分布在整个邻面,沿邻面的周界,仅在内部密封区向内,或仅在内部密封区和外缘区之间。
锚定件13实际上自身不必进入瘘管道。而是,锚定件的密封通常发生在瘘开口周围。锚定件13的凹面或弯曲性质可导致锚定件13的中间部分偏移并远离锚定件13的边缘。此外,例如在肛门直肠瘘的情况下,凹面或弯曲的锚定件13可在使用过程中接触到一个相应的凹面或弯曲组织表面(即,远端瘘开口周围的组织表面)。应当鉴别,当描述的是一个凹面或向下弯曲的锚定件时,在一些实施例中可能使用的是一个平的或凸面(向上弯曲)的锚定件。针对瘘开口,锚定件13可提供多种密封。当大多数实施例描述双重或三重密封时,可通过在锚定件的密封面上附加额外密封面形成任何数量的密封。当需要双重密封时,锚定件13可如下安装,外缘区30和内部密封区32密封在瘘开口周围的锚定件,同时连接部件附加区38偏移组织表面。然而,锚定件可如下安装,外缘区30、内部密封区32和连接部件附加区38的任意组合可充当密封。在某些情况下,在瘘周围形成三重密封满足需要,在这种情况下,锚定件可在外缘区30、内部密封区32和连接部件附加区38形成一个密封。虽然图2A描述的密封为圆环,应当鉴别,密封可为螺旋形中断环或其他可提供密封或治疗效果的形状,例如,在伤口处密封更紧或增强透气性。此外,虽然锚定件和密封通常描述成圆形,锚定件本身、外缘区、内部密封区或连接部件附加区可能是矩形、椭圆形或其他任何合适的形状。

在一些实施例中,连接部件在使用之前可以附接到附接区38。附接区38可以由同一种材料作为锚定件13的剩余部分,或者,它可以由一种或多种不同的材料组成。如图2A和图8所示,附接区38可以进一步地由一个或多个层部件74构成,其可以作为一个整体任意选择具有小于附接区38的厚度。举例来说,附接区可以由三个层部件组成,包括材料相同的基本层部件和顶端层部件来作为锚定件13的剩余部分,以及由另一种不同材料制成的中间层部件,在另一个实施例中,中间层部件被夹在由锚定件13的上表面和下表面形成的两个层部件之间。可以用能够使中间层部件附接到连接部件上比附接到锚定件13上更容易的材料,举例来说,可以使用网状材料或由比锚定件13的强度和硬度更高的材料制得的固态盘。层部件可以以任何合适的方式与锚定件连接,包括但不限于,将层部件粘合到锚定件13上,或者将其嵌入锚定件13内。
连接部件可以以任何合适的方式与附接区连接,包括但不限于,粘合剂的使用,或者通过锚定件或层部件的孔编织连接部件。举例来说,在一个实施例中,连接部件18的各个端部可以通过锚定件13的基部的进入孔插入,在锚定件13的近端或下侧形成一个环。两个端部进而可以贯穿层部件的进入孔并且在层部件的远端平行聚集在一起。端部因而能够通过平行的出口孔被一起插入层部件和锚定件。出口孔可以位于两个进入孔之间(比如;中间)使得连接部件在层部件的远端形成两个较小的环。层部件上形成的环使得连接部件18和层部件之间有了接触。该触点可以在远端锚定件13的内表面上,比如,当层部件被嵌入远端锚定件13时,或者它可以在远端锚定件13的外表面上。装置体部14和输送装置12进而能够穿过连接部件18的端部。而且,很多连接部件均能够被连接到锚定件的附接区。在一些实施例中,不止一个连接部件被连接到附接区,比便使用锚定件治疗多个瘘管,举例来说,可以用来阻塞分支或互联的瘘管,或彼此接近的瘘管。
在另一个实施例中,连接部件包括两个连接在一起的元件,举例来说,一端一个结。层部件和锚定件13因而能够以同样的方式穿过两个元件的相反端,如上所说,这样会分别在锚定件和层部件的近端和远端处形成环。但是,利用层部件和锚定件13的基部上的两个平行孔时,方向相反的(即,层部件的远端和朝着近端的锚定件)层部件和锚定件也可以穿过连接部件元件。装置体部14和输送装置12进而可以穿过连接部件18的元件的两个自由端部,或者它们可以在被穿过之前连接在一起(也就是说,融合、粘合等)。也可以使用任何合适的粘合材料将连接部件18粘合到层部件或锚定件13的表面。
连接部件18可以包含任何适用的药线或缝线。在一些例子中,药线上可以包覆一层药物涂层复合物或抗菌物质。此外或者可以选择的是,药线也可以用含有治疗因子、抗生素或其它的治愈剂的治疗剂浸泡。
如图6所示,附接区38可包括锚定件凸面上的触觉标记。触觉标记可帮助内科医生在能见度很低的情况下,确定锚定件的适当位置,并为后续治疗起到标记的作用。触觉标记可采取通过触摸就能够确定锚定件13位置的任何形式或结构.图6是带有突出节结600的锚定件13的横截面。如图7A-7D所示触觉标记可包括,例如,突出或缩进节结700、十字纹702、凹槽704、突出螺旋706或任何其他突出于缩进图案的标记。此外,或者,通常锚定件或尤其附接区可包括射线不透性材料或标识物,这可进一步帮助内科医生通过可视化X射线、CT或其他成像方式,确定锚定件的位置。
许多不同几何特征会影响远端锚定件13的密封特性。可确定远端锚定件13密封性的几何变量可包括,但不限于,外部及内部密封区30及32还有附接区38的宽度、内部密封区62的高度、外缘区的厚度、相对于外缘区30和附接区38的内部密封区32的位置、内部密封区32及附接区38的横截面形状、从病人的组织到外缘区30内表面测量的角度和锚定件13上不同位置的曲率半径。
图2B绘出了由提供第一层密封的外缘区30和提供第二层密封的内部密封区32构成的双封型锚定件13的一个实施例的横截面。此外,附接区38可穿其宽度(例如,直径)56或沿其周长,作为第三层密封面。外缘区30的直径50等于锚定件13的直径。外缘区的内径52也等于内部密封区32的第一边缘的直径。可以从外缘区50的外直径中减去内部密封区52的第一边缘直径,算得外缘区32的宽度。可以从内部密封区52的第一边缘直径中减去内部密封区54的第二边缘直径,算得内部密封区32的宽度。附接区56的宽度是锚定件上点与点之间的距离,这里锚定件的厚度为附接区38任一侧基础厚度60。
在一些实施例中,外缘区30的宽度是在大于0.01"和0.45"之间,和/或在远端锚定件50外直径的大于5%和大约60%范围内(例如,在大约15%和大约30%之间)。此外,在某些实施例中,内部密封区的宽度可在大于0.05"和0.35"之间,和/或在远端锚定件50外直径的大于5%和大约45%范围内(例如,在大约5%和大约25%之间)。在发明的任何实施例中,附接区56的宽度可在大约0.15"和大约0.35"(例如,0.247")之间,和/或在锚定件13外直径的大约15%和大约60%之间(例如,在大约25%和40%之间)。
内部密封区52和54各自第一和第二直径都是根据内部密封区62的高度测量的。内部密封区可包括突出于远端锚定件13凹面的高度62。内部密封区62的高度为内部密封区的最大厚度(例如,在环的中心)减去锚定件60的基础厚度而得。如图2C所示,为测量内部密封区的高度,如果高度在无内部密封区32造成任何额外投影的情况下形成,则绘出一条基线64,从而保存锚定件的基础厚度。同样地,绘出一条峰线66,与内部密封区32的顶点(最大投影点)成切线。基线与内部密封区在其第一边缘相交,形成第一边缘交叉68;在其第二边缘相交,形成第二边缘交叉70。内部密封区投影距离72为第一边缘交叉68和第二边缘交叉70之间的距离。通过第一边缘交叉68加上1/4内部密封区投影距离72,测得内部密封区52的第一边缘直径。通过第二边缘交叉70减去1/4内部密封区投影距离72,测得内部密封区的第二边缘直径。
在一些实施例中,内部密封区32的平均厚度在大约0.03"和大约0.09"之间,和/或小于远端锚定件50的外直径的25%(例如,约5%或10%的范围)。此外,内部密封区的最大厚度位于内部密封区34的第一边缘和内部密封区36的第二边缘之间的中点处的环上。在其它的实施例中,内部密封区34的最大厚度更接近于其中的一个边缘。
外缘区30具有同心外缘和内缘,处于两者之间的外缘区的厚度会有所变化。例如,在图2B所示的实施例中,外缘区为锥形,使得厚度沿外缘区的内缘到外缘区的外缘向外径向地减小。如图5所示,当连接部件18向附接区38施加向下力时,外缘区30的锥形能够增强其密封性能。外缘区的锥形可以提供沿锚定件周边的额外的弹性,使得当压力施加到锚定件的中心时,外缘不会羽化或向上翻转。在一些实施例中,外缘区30的厚度在大约0.005"到大约0.03"的范围之间,和/或小于远端锚定件50的外直径的10%(例如,约3%或5%的范围)。而且,在某些实施例中,当外缘区的第一边缘位于与包含内部密封区的顶点的第二平面平行的第一平面内时,该第二平面可以位于第一平面和附接地点之间,使得锚定件的横截面(不施加力)在不受外力时,呈现半圆形状。
改变由外缘区30的内表面和倾向于外缘区的第一边缘的平面形成的表面角61会影响锚定件的密封性能。此外,改变由外缘区的内缘和外缘之间径向测得的外缘区30的曲率外半径,也会极大地影响锚定件的密封能力。这些特征使外缘区30和内部密封区32之间产生了一个凹面,使得在瘘管中安装过程中,向附接区38施加向下力时,内部密封区32的顶角在密封组织表面的同时也能够使外缘区30接触组织表面。在外缘区的内缘和外缘会形成一个开放空间。根据表面角61和外缘区的曲率半径,该开放空隙的面积会发生变化。在某些情况下,为了获得最大的密封面,需要具有较大曲率半径的半圆形空隙。在这种情况下,可以使用具有较大表面角61和较小曲率外半径的锚定件。在一些实施例中,表面角61可以在0度到45度之间,并且外缘区的曲率外半径可以在大约0.03"和0.15"之间,和/或锚定件50的外直径的大约5%和15%范围之内。
见图2B,在一些实施例中,直径50可以在大约0.6"到大约0.9"之间(比如:0.75"),直径52可以在大约0.45"到大约0.7"之间(比如0.57英寸),并且直径54可以在大约0.35"到大约0.65"之间(比如:0.45")。而且,连接部件附接区38的厚度58为,比如说在大约0.08"到大约0.15"之间(比如:0.095英寸)。此外,锚定件13的基本厚度60可以是,比如说,在大约0.03"到大约0.06"之间(比如:0.045英寸),而且内部密封区高度62可以为,比如,在大约0.001"到大约0.03"之间(比如:0.02")。
尽管连接部件附接区38通常被描述成圆形,但是其可以使用任何合适的形状和构型,包括但不限于半圆形、三角形和椭圆形。在某些情况下,连接部件附接区38可以相对厚一点或显著一些。这样能够,比如说,有助于锚定件13相对于瘘管开口居中。在某些情况下,连接部件附接区38的厚度至少为锚定件13的外直径50的30%(比如:至少大约40%,至少大约50%)。在某些实施例中,连接部件附接区38可以设计的不那么突出(比如:厚度为外直径50的大约5%到大约25%的范围之内)。这可能会,比如说,限制了产生压力区的可能性,这样反过来会导致坏死。
可以使用构型不同(比如:尺寸和/或形状)的锚定件或密封部件的其它实施例。举例来说,图3A-3M分别为锚定件或密封件300、302、304、306、308、310、312、314、316、318、320、322和324的不同实施例的剖视图。如图所示,锚定件300具有相对拱形构型,从其内部封闭区相对急剧地过渡到外缘区。锚定件302的过渡较为平缓,锚定件304的横截面相对地呈圆形。锚定件306较之锚定件300,拱形不明显,可以用于,比如说,抵靠一个相对较扁平或不太圆的表面。
当放置了锚定件时,通过允许外密封区和内密封区牢固接触瘘周围的组织,弓形锚定件可增强其密封力。在某些情况中,曲线形锚定件允许外密封区和内密封区与瘘周围的组织接触,无须迫使瘘开口与附接区相接触。锚定件的拱可允许附接区从瘘开口表面垂直地轻微转移,从而防止弓形部件干扰治疗过程。图3A-3M描述的弓形锚定件以锚定件曲率内半径为特征,正如径向测量第二内密封区部件内部边缘。在一些实施例中,锚定件中心部分曲率内半径以一般弓形结构为特征,介于0.12"和0.75"之间。附加或可选地,曲率内半径可介于锚定件13的外径50的大约20%和85%之间(例如,从大约35%到55%)。可选地,外部边缘区部件曲率外半径可介于曲率内半径的大约10%和65%之间(例如,介于大约15%和35%之间)。
图3A-3E描述了锚定件13的各种截面实施例。这些实施例的直径通常小于图3F-M所显示的实施例直径。这些实施例适合于小瘘或要求远端锚定件13且带有几何特征的瘘。图3A-E以及3F-M显示的实施例在几何构型上不同,例如,关于锚定件曲率半径以及所形成的表面角。图3A描述了与锚定件302相比,带有较小曲率内半径但较大表面角的小型锚定件300。锚定件302是与锚定件300和304相比形状较为扁平,且与锚定件300相比表面角较小的小型锚定件。锚定件304与图3A的曲率内半径相似,但表面角较小。锚定件300、302及306显示了构成带有极大最大厚度且大致相似截面形状的内密封区的实施例,而锚定件308则描述了一个带有不显著的内密封和附接区的较小实施例。
而图3A-3E描述了带有密封区的实施例,其特征为明显的椭圆截面,当设计内密封区和附接区时,可使用任何合适的截面形状,这应当是正确的。在一些实施例中,内密封区的截面可为,例如,圆形、三角形或矩形。此外,附接区的截面形状也可能不同,且可包括但不局限于三角形、圆形、椭圆形和矩形截面。
图3F-3M更清楚地描述了附接区38及内密封区32的各种截面形状。锚定件310、318和320描述了带有显著矩形内密封区截面的实施例,而锚定件322则是带有显著三角形内密封区截面的实施例例子。此外,锚定件312、314、316和318为构成三角形附接区截面的实施例例子。相反,锚定件322和324描述了构成椭圆形附接区截面的实施例。图3A-3M中包含的截面差异并非详尽的,仅仅为示范性的。
如图3F所示,与附接区和内密封区之间的中间区相比,一个锚定件的附接区38具有显著较大平均厚度。然而,在其他实施例中,例如,锚定件322所描述的实施例,附接区的平均厚度可能与附接区和内密封区之间的中间区厚度相似。进一步地,外部边缘区的第一边缘和内密封区最大厚度处的环之间的距离可能不同。此外,内密封区最大厚度处的环和附接部件区之间的距离可能不同。例如,锚定件324描述了一个实施例,其中,与附接区相比,内密封区的最大厚度更接近于外部边缘区的第一边缘,而锚定件318描述了另一个实施例,其中,与外部边缘区相比,内密封区最大厚度处的环更接近于附接区。在一些实施例中,内密封区最大厚度处的环可能与附接区和外部边缘区第一边缘的距离相等。与图3A-3E相似,图3F-3M显示了带有不同曲率外半径和表面角的实施例。锚定件324是带有较小曲率外半径的实施例,例如,与锚定件310相比。此外,图3F-3M也描述了构成各种宽度外部边缘区的实施例。锚定件316和锚定件318描述了构成带有较大宽度外部边缘区的实施例,而锚定件320、322和324显示了构成带有较小宽度外部边缘区的实施例。
当然,锚定件大小各异。如文,例如,锚定件308就相对较小。举例来说,锚定件的大小由正经治疗的瘘管道的尺寸决定。相比之下,锚定件310相对较大并有一个更尖角的连接部件附接区,从其内密封区部件相对急剧地过渡到外缘区部件。锚定件312也相对较大,但是其内部及外部边缘区部件之间过渡更平缓。锚定件314和锚定件310构型相似;但是,锚定件314的外缘区部件比锚定件310的外缘区部件薄。锚定件316是图3A-3M中所示最大的锚定件,且其环形部件之间过渡相对平稳,其中外缘区具有锥形边。锚定件318和锚定件316的构型相似,但是却更小,并具有更薄的外缘区部件。锚定件320相对紧凑,其内部密封区部件及其连接部件附接区之间有一部分相对厚。锚定件322也相对紧凑,其内部密封区部件及其连接部件附接区之间有一部分甚至更厚。最后,锚定件324有相对平缓及平稳的曲线,具有一个圆形连接部件附接区和一个横截面相对较圆的内部密封区部件。
尽管本文己经示出和描述了锚定件或密封部件的某些实施例,但是具有不同形状及构型的其他实施例也可用于瘘闭合装置中。此外,锚定件或密封部件可由任何适当的材料制成,包括但不限于聚合物,比如聚硅氧烷或聚硅酮、聚乙烯、聚氨酯、含氟聚合物(例如,聚四氟乙烯(PTFE)、氟化乙烯丙烯(FEP)、全氟烷氧基(PFA0))、金属和(或)金属合金。在一些情况下,锚定件或密封部件可至少包括一种硅酮聚合物。在一些变化中,锚定件还可包括药物洗脱涂层或材料,或用可能含有愈合因子、抗生素或其他愈合剂的治疗药浸泡的材料。
再次参见图1,输送装置12还包括一个可能用于防止连接部件18因疏忽与输送装置12分开的阻力部件或闭锁装置24(例如,缝合闭锁装置)。如图所示,连接部件可能经由闭锁装置24的孔口26及28的路径,以致连接部件从输送装置12的内部穿到外部,并再次返至内部。尽管本文显示了两个孔口,但是也可采用任何适当数量的孔口及任何适当的连接部件路径构型。
任何适当方法均可用于输送或展开在此描述的瘘治疗装置。举例来说,图4A-4E绘出了将图1的瘘闭合装置10输送至肛门直肠瘘管道400的方法实施例。首先,图4A显示出瘘管道400,经由肛门402及齿状线404。在图4B中,导线406已穿过瘘管道400。接着,参考图4C,用输送装置12,将瘘闭合装置10输送至瘘管道400。更具体地说,举例来说,连接部件18可附接至导线406,且可用导线406,将装置12拉入瘘管道400。现在参考图4D,可撤回护套20,从而将装置体部14(包括部件16)留在瘘管道400内。图4E显示出,装置10被输送到至目标区域,装置体部14被放置在瘘管道400内,锚定件13被面朝瘘管道400远端开口408的相反方向放置,且可选近端锚定件410被面朝瘘管道400近端开口412的相反方向放置(且,例如,附接至连接部件18)。虽未在此显示,但是在某些情况下,装置10可另外适当缝合在瘘管道400近端开口412的位置上。在一些变化中,移植期间,可使用一个肛门镜或其他内窥镜。在一些进一步变化中,肛门镜或其他内窥镜可包括沿其长度的开槽或孔口,以便在移置或支持周围组织的同时允许直接进入瘘管开口,从而改善可视化。
有利地,瘘闭合装置10和输送装置12的设计可允许部件16(例如,胶原栓)输送至瘘管道400内,无需在进入瘘管道400之前,先接触到潜在的大量污染物。因此,可避免或至少限制瘘管道400的污染物。
为执行上述程序,可提供包含瘘闭合装置10及输送装置12在内的套件。在某些情况下,套件内也可包括多种不同瘘闭合装置(例如,具有带有不同构型的密封部件13)。瘘闭合装置10可在制作时或使用时联接到输送装置12上。因此,可预先附接或与输送装置12分离地设置在套件中。套件还可包括一个或多个其他物品,包括但不限于一根导线(例如,0.038"导线)、一个剥离护套(例如,7F、8F、9F、10F或12F护套)、一个或多个注射器(例如,0.5cc、1cc、5cc和/或10cc注射器)、盐水或生物相容的流体、造影剂、解剖刀、一个或多个自由针和非可吸收的缝线(例如,3-0或4-0尼龙缝线)。还可将瘘管道扩张器设置在套件内。工具箱套件目录可采用无菌包装。可在套件上或内,或者通过互联网或另一个间接方法,提供说明书,并指导如何使用套件(例如,如文中所述之一,概括展开方法)。
尽管本文己经示出和描述了本发明的优选实施例,但对本领域的技术人员来说清楚的是,这些示例仅以举例的方式。本领域的技术人员在未脱离本发明的情况下可进行许多改变、变型和置换。应当理解的是,本文所述的本发明的实施例的各种各选方案可用于实施本发明。以下权利要求限定出本发明的范围,且在这些权利要求范围内的方法和结构均由此覆盖。
Claims (10)
1.一种肛门直肠瘘治疗装置,包括:
盘形远端锚定件,其包括:
凹面;
凸面;
包括沿着远端锚定件的外缘的第一环的外缘区;
从凹面突出并包括同心地布置在第一环内的第二环的内部密封区;
由外缘区形成并从内部密封区朝向远端锚定件的外缘延伸的凹面过渡;
从远端锚定件的凹面的中心突出并且用于附接连接部件的附接区;以及
位于内部密封区和附接区之间的中间区,其中,附接区的平均厚度大于中间区;
其中,内部密封区至少比附接区的一部分厚。
2.根据权利要求1所述的肛门直肠瘘治疗装置,所述远端锚定件具有锥形边,所述外缘区和内部密封区构造成单独密封用以抵靠远端开口的组织。
3.根据权利要求2所述的肛门直肠瘘治疗装置,所述远端锚定件的直径在0.6"至0.9"的范围内。
4.根据权利要求1所述的肛门直肠瘘治疗装置,所述外缘区的平均厚度不到远端锚定件外径的10%。
5.根据权利要求1所述的肛门直肠瘘治疗装置,所述外缘区的平均厚度在远端锚定件外径的3%和5%范围内。
6.根据权利要求1所述的肛门直肠瘘治疗装置,所述外缘区的平均厚度在0.005"和0.03"范围内。
7.根据权利要求1所述的肛门直肠瘘治疗装置,所述外缘区的宽度在远端锚定件外径的5%和60%范围内。
8.根据权利要求1所述的肛门直肠瘘治疗装置,所述外缘区的宽度在远端锚定件外径的15%和30%范围内。
9.根据权利要求1所述的肛门直肠瘘治疗装置,所述外缘区的宽度在0.01"和0.45"范围内。
10.一种肛门直肠瘘治疗装置,包括:
盘形远端锚定件,包括:
凹面;
凸面;
包括沿着远端锚定件的外缘的第一环的外缘区;
从凹面突出并包括同心地布置在第一环内的第二环的内部密封区;
由外缘区形成并从内部密封区朝向远端锚定件的外缘延伸的凹面过渡;
从远端锚定件的凹面的中心突出并且构造成附接至连接部件的附接区;以及
同心位于内部密封区和附接区之间的中间区;
其特征在于,附接区的厚度大于中间区的厚度,内部密封区的厚度大于外缘区及中间区的厚度。
