当前的临床的现有技术在天生的股骨头的或髋关节头的磨损的情况下规定由金属或陶瓷制成的表面代替假体。迄今的陶瓷的表面代替假体很大程度上依据已知的金属的设计方案并且原则上被粘固。内部的假体面设有粘固剂凹处和定心栓。基于放热反应(其在骨骼粘固剂的聚合的情况下出现)而存在如下危险,即热地损伤毗邻的骨骼组织。此外在陶瓷和骨骼粘固剂之间的结合不是长时间稳定的并且会在几年之后导致假体的松开(Lockerung)并且接着导致植入物的失效。
现有技术为EP 0 542 815 B1、EP1 268 364 A1和DE 10 2013 202 243 A1。
发明内容
根据本发明的无粘固剂的全陶瓷的带有集成的多孔的骨能够结合的内部的套面的表面代替假体呈现为有利的创新。
这样的无金属的表面代替假体将省去骨骼的手术的优点与生物兼容的材料的、滑动配对的小的磨损的和在骨骼中突出的和长时间稳定的锚固的优点统一。
根据本发明的用于天生的股骨头的表面代替假体(所述表面代替假体具有外部的球形的表面和带有内部的套面的内部的容纳空间)的特征在于设置有集成的多孔的骨能够结合的套面和设置有全陶瓷的结构。所述表面代替假体的优点进一步在上面描述。
根据本发明的用于制造根据权利要求1所述的表面代替假体的集成的多孔的骨能够结合的套面的方法的特征在于,所述表面代替假体由烧结模制体制成并且将多孔的骨能够结合的覆层施加到所述套面上。套面是指面向骨骼的侧面,所述侧面在所述植入之后贴靠在所述骨骼处。
在所述方法的一种根据本发明的实施方案中,所述多孔的套面通过在生坯状态中施加陶瓷的浆料以及占位件(Platzhaltern)(孔形成器(Porenbildner))以及在烧结时烧尽所述占位件来制成。优选地,所述多孔的覆层能够通过陶瓷的浆料在所述生坯状态中的起泡以及接下来的烧结产生。通过烧结过程实现结合。通过在烧结时烧尽所述占位件来产生具有裂缝的表面的开孔的结构,其有利地支持所述骨骼的生长和长入。为此还参见EP 1268 364 A1。
所述多孔的套面也能够通过在烧结的状态中将密实的模制体与多孔的陶瓷的模制体接合来产生。所述多孔的套面能够通过陶瓷的浆料的起泡以及烧结产生或者通过将多孔的基底材料渗入有浆料并且烧结产生。所述接合优选地通过焊接或粘结实施。
通过所述烧结过程实现结合并且产生具有裂缝的表面的开孔的结构,其有利地支持所述骨骼的生长和长入。这种结合陶瓷的基体的涂层的类型同样无金属地关于过敏反应是有利的。
所述多孔的骨能够结合的套面根据本发明还能够通过双色注塑(密实的阶段和多孔的或形成孔的阶段)以及烧结来制成。这种结合陶瓷的体的涂层的类型也是无金属的。
在另一根据本发明的设计方案中,所述集成的多孔的骨能够结合的套面通过在已经烧结的陶瓷基体上施加多孔的金属的生物兼容的覆层来产生(参见EP 1 052 949 B1)。
所述多孔的面向骨骼的套面能够优选地通过另外的涂层来获得附加的功能性、如更高的结合速度或抗菌的作用。结合速度被理解成以所述骨骼来长入的时间。优选地,这能够利用生物玻璃、羟基磷灰石、功能蛋白或水凝胶来实现。优选地,这能够利用生物玻璃、羟基磷灰石、功能蛋白或水凝胶来实现。
根据本发明的表面代替假体完全由陶瓷制成并且能够通过多孔的套面来直接植入。所述骨骼快速地长入到所述多孔的套面中,从而在所述手术之后不久已经保证有固定的配合(Sitz)。在所述植入时不使用骨骼粘固剂。
Claims (11)
1.用于天生的股骨头的表面代替假体,所述表面代替假体具有外部的球形的表面和带有内部的面向骨骼的套面的内部的容纳空间,其特征在于,设置有集成的多孔的骨能够结合的套面和设置有全陶瓷的表面代替假体。
2.用于制造根据权利要求1所述的表面代替假体的集成的多孔的骨能够结合的套面的方法,其特征在于,所述表面代替假体由烧结模制体制成并且将多孔的骨能够结合的覆层施加到所述套面上。
3.根据权利要求2所述的方法,其特征在于,所述多孔的套面通过在生坯状态中施加陶瓷的浆料以及占位件(孔形成器)以及在烧结时烧尽所述占位件来制成。
4.根据权利要求2所述的方法,其特征在于,所述多孔的套面通过陶瓷的浆料在生坯状态中起泡以及接下来的烧结制成。
5.根据权利要求2所述的方法,其特征在于,所述多孔的套面通过在烧结的状态中将密实的模制体与多孔的陶瓷的模制体接合产生。
6.根据权利要求5所述的方法,其特征在于,所述多孔的模制体通过陶瓷的浆料的起泡以及烧结制成或者通过将多孔的基底材料渗入有浆料并且烧结制成。
7.根据权利要求5或6所述的方法,其特征在于,所述接合通过焊接或粘结实施。
8.根据权利要求2所述的方法,其特征在于,所述多孔的套面通过双色注塑(密实的阶段和多孔的或形成孔的阶段)以及烧结来制成。
9.根据权利要求2所述的方法,其特征在于,所述多孔的套面通过在已经烧结的表面代替假体上施加多孔的金属的生物兼容的覆层来产生。
10.根据权利要求2至9中任一项所述的方法,其特征在于,所述多孔的套面通过另外的涂层来获得附加的功能性、如更高的结合速度或抗菌的作用。
11.根据权利要求10所述的方法,其特征在于,多孔的背侧面以生物玻璃、羟基磷灰石、功能蛋白或水凝胶来涂层。
