本发明公开了一种治疗慢性胃炎、胃溃疡的中药制剂,它由姜半夏、黄芩、干姜、党参、炙甘草、黄连、大枣七味中药材制成,制备方法是:将以上七味用水或含醇水溶液提取,收集提取液,滤过,将滤液减压浓缩后上大孔吸附树脂柱,用水或含醇水溶液洗脱,收集洗脱液,浓缩成浸膏后加入辅料制成适合临床上使用的剂型。本发明由于富集了有效成分,减少无效成分,因此提高了药物疗效,降低了副作用,减少了服用剂量,提高了患者服药顺应性,对满足临床用药具有重要意义。

技术领域
本发明属于中药领域,具体涉及一种治疗慢性胃炎、胃溃疡的中药。
背景技术
我国人口接近14亿,慢性胃炎、胃溃疡发病率较高,统计数据表明,发病率可达60%以上,为临床上常见多发病、迁延难愈,严重影响了人们的生活质量。中医认为,胃居心下与脾互为表里,同居中洲,胃气以下降为顺,脾气以上升为畅,脾升胃降,中焦气机顺畅,腐熟运化有常,故能行使机体后天之本。若因伤寒误治,损伤脾胃、邪热内陷,脾胃升降失职,病邪结于中焦,而成寒热互结。或因素体中阳虚亏,脾胃虚弱,中阳不振,升降失职,加之饮食不节,情志不舒,劳逸失度,痰湿积滞,湿热之邪乘虚而入,导致脾胃功能紊乱,阴阳失调,上下不通,气机阻滞,寒热互结,气血运行不畅,胃之脉络不通,而发生胃脘疼痛之症。
现代医学治疗胃炎、胃溃疡多对症处理,药物多为吗丁啉和维酶素等,或针对幽门螺杆菌采用多种抗生素联合治疗,如采用质子泵抑制剂加克拉霉素加阿莫西林或甲硝唑三联疗法。虽然取得一定疗效,但愈后复发率高,且病程又长,服药时间长,同时还带来很多副作用,如:胃肠道不适、失眠、头昏、头痛等。
半夏泻心汤出自汉代著名医家张仲景的《伤寒论》,由姜半夏、干姜、黄芩、黄连、党参、灸甘草、大枣七味药材组成,为调和肠胃的代表方。方中姜半夏、干姜、辛温散结消痞,祛脾寒之气以和阴,黄芩、黄连、苦寒降泄,清胃气之热以和阳,四药互用,辛开苦降,阴阳得和,脾胃得调,为后世医家辛开苦降法之始作。又佐以党参、甘草、大枣甘平入脾胃,调和诸药,补气健脾,以固后天之本。即合《内经》“正气存内,邪不可干”之旨。全方配伍严谨,祛邪补虚,融为一炉,为治疗脾胃病的经典之作。大量临床研究报道,此方具有和胃降逆,平调寒热之功效,对治疗脾虚胃热证之胃脘痛,脘腹痞满、呃逆嗳气、泛酸嘈杂、即见有上症之慢性胃炎、消化性溃疡有很好的疗效,对人体内的幽门螺杆菌也有很好地抑灭效果。因此将其开发成新的治疗慢性胃炎、胃溃疡的中成药对满足临床用药显得尤为重要。
另外,半夏泻心汤属于传统汤药,存在不易贮存、服用剂量大,患者用药顺应性差等缺点,而且是一种工艺简单粗放的中药制剂,质量不易控制,疗效和安全性得不到保障。
发明内容
为了更好地满足临床用药需求,本发明提供一种疗效确切,副作用少,质量稳定可控,患者用药顺应性好的治疗慢性胃炎、胃溃疡的中药。
为实现上述目的,本发明采用了以下技术方案:
一种治疗咳嗽的中药制剂,它由下述重量配比的中药材原料制成:
姜半夏3~5份、黄芩2~4份、干姜2~4份、党参2~4份、炙甘草2~4份、黄连1~3份、大枣1~3份。
制备方法是:按比例取所述中药材原料,加水或用体积含量不超过60%的含醇水溶液提取,收集提取液,滤过,将滤液减压浓缩至相对密度为1.05~1.15的浓缩液,上大孔吸附树脂柱,用水或体积含量不超过60%的含醇水溶液洗脱,收集洗脱液并浓缩成浸膏,再加入辅料制成适合临床上使用的剂型。
优选的,所述制备方法是:按比例取所述中药材原料,用体积含量为40~60%的含醇水溶液加热回流提取,收集提取液,滤过,将滤液减压浓缩至相对密度为1.05~1.15的浓缩液,上大孔吸附树脂柱,用体积含量为40~60%的含醇水溶液洗脱,收集洗脱液并浓缩成浸膏,再加入辅料制成适合临床上使用的剂型。
进一步优选地,所述加热回流提取的次数为1~3次,每次提取的时间为0.5~2小时,所述提取中含醇水溶液与中药材的重量比为5~15∶1。最佳的加热回流提取工艺是,提取的次数为2次,每次提取的时间为2小时,所述提取中含醇水溶液与中药材的重量比为10∶1。
进一步优选地,所述大孔吸附树脂柱中,大孔吸附树脂的用量为中药材重量的1~3倍。
进一步优选地,所述洗脱液的体积为大孔吸附树脂柱体积的5~20倍。
进一步优选地,所述醇为C1~4脂肪醇,包括甲醇、乙醇、丙二醇、正丁醇、异丁醇、丁二醇等,从降低成本和安全考虑,选择乙醇效果最佳。
所述中药材原料按照上述方法提取精制后,可以制成适合于临床使用的任何一种剂型,例如:散剂、片剂、颗粒剂、胶囊剂(软胶囊)、丸剂(滴丸)、膏剂、合剂、糖浆剂等。所述辅料可以是蔗糖、淀粉、糊精、硬脂酸镁、山梨醇、甘露醇、乳糖、微晶纤维素、微粉硅胶、羧甲基纤维素、水、矫味剂等这些药物制剂中常规辅料的一种或多种的组合,具体选择哪一种或哪几种辅料应根据所需要制成的剂型来确定,而根据剂型需要来选择适当的辅料和制剂方法是本领域技术人员都能掌握的常规技术。
本发明优选的剂型为颗粒剂、片剂、胶囊剂、糖浆或口服液。
本发明可以富集半夏泻心汤中的有效成分,减少无效成分,从而:1)有利于减少服用剂量,提高患者服药顺应性;2)有利于提高药物疗效,降低副作用;3)有利于制剂成型,便于贮藏、携带、运输;4)有利于控制产品质量,提高药品稳定性。本发明提供的是一种新的治疗慢性胃炎、胃溃疡的中成药,对满足临床用药具有重要意义。
具体实施方式
下面结合实施例对本发明进行详细说明。以下实施例中所涉及到的醇溶液含量均是指体积含量,如40%乙醇指的是每100ml乙醇水溶液中含有乙醇40ml。
实施例1
取姜半夏30Kg、黄芩20Kg、干姜20Kg、党参20Kg、炙甘草20Kg、黄连10Kg、大枣10Kg,用水煎煮提取两次,每次1小时,每次加水量为650Kg(为药材重量的5倍),合并提取液,滤过,将滤液减压浓缩至相对密度为1.14的浓缩液,浓缩液上D101型大孔吸附树脂柱(390KgD101树脂装柱,树脂重量为药材重量的3倍),用20%乙醇7000L(为大孔吸附树脂柱体积的20倍)洗脱树脂柱,收集洗脱液,50℃减压浓缩至相对密度为1.35的浸膏,加蔗糖粉、糊精适量,制软材,制粒,干燥,整粒,制得颗粒剂。
实施例2
取姜半夏40Kg、黄芩30Kg、干姜30Kg、党参30Kg、炙甘草30Kg、黄连20Kg、大枣20Kg,用40%乙醇加热回流两次,每次回流2小时,溶剂用量为2000Kg(为药材重量的10倍),合并提取液,过滤,将滤液减压浓缩至相对密度为1.10的浓缩液,浓缩液上AB-8型大孔吸附树脂柱(200KgAB-8树脂装柱,树脂重量为药材重量的1倍),用40%乙醇850L(为大孔吸附树脂柱体积的5倍)洗脱树脂柱,收集洗脱液,60℃减压浓缩至相对密度为1.32的浸膏,加入微晶纤维素、淀粉适量,混匀,制粒,干燥,整粒,再加入0.5%硬脂酸镁,混匀,压片,即制得片剂。
实施例3
取姜半夏50Kg、黄芩40Kg、干姜40Kg、党参30Kg、炙甘草20Kg、黄连10Kg、大枣10Kg,用60%甲醇溶液加热回流三次,每次0.5小时,每次的溶剂用量为1600Kg(为药材重量的8倍),合并提取液,过滤,将滤液减压浓缩至相对密度为1.05的浓缩液,将浓缩液上HPD-722型大孔吸附树脂柱(400KgHPD-722树脂装柱,树脂重量为药材重量的2倍),用60%甲醇3600L(为大孔吸附树脂柱体积的10倍)洗脱树脂柱,收集洗脱液,80℃减压浓缩至相对密度为1.30的浸膏,加糊精、淀粉适量,制粒,干燥,整粒,加入0.5%硬脂酸镁,混匀,装胶囊,制得胶囊剂。
实施例4
取姜半夏30Kg、黄芩20Kg、干姜20Kg、党参40Kg、炙甘草40Kg、黄连30Kg、大枣30Kg,用30%乙醇加热回流两次,每次1小时,每次的溶剂用量为3150Kg(为药材重量的15倍),过滤,将滤液减压浓缩至相对密度为1.15的浓缩液,将浓缩液上HPD-600型大孔吸附树脂柱(210KgHPD-600树脂装柱,树脂重量为药材重量的1倍),用70%乙醇850L(为大孔吸附树脂柱体积的5倍)洗脱树脂柱,收集洗脱液,60℃减压浓缩至相对密度为1.2的浸膏,加入蔗糖、苯甲酸钠适量,加水溶解,搅匀,制得口服液。

取健康Wistar大鼠,随机分为正常对照组、雷尼替丁组、半夏泻心汤干浸膏组(原料处方与实施例1相同,采用常规的方法煎煮提取,提取液浓缩、干燥得到的干浸膏)、实施例1~4干浸膏组(将实施例1~4中减压浓缩后得到的浸膏在制剂前进行干燥)、每组均10只,雌雄各半。半夏泻心汤干浸膏组和实施例1~4干浸膏组的给药剂量相同,即将干膏折算成生药材的重量,按10g生药材/kg体重进行灌胃给药,正常对照组给予等容量蒸馏水。试验第一天上午各组分别灌胃一次,下午开始禁食不禁水,第二、三天上午再分别灌胃一次,第三天下午(即禁食48h后)开始手术。手术时,将大鼠用乙醚麻醉、沿腹中线切开腹壁,找出胃与十二指肠,用缝合线避开血管结扎胃幽门与十二指肠结合部,并由十二指肠再给药一次,随后缝合腹壁、碘酊消毒后放回笼中,同时禁食禁水。18h后,拉颈椎处死大鼠,剖腹取出胃并收集胃液,随即向胃内注入1%福尔马林溶液10ml,再将胃放入1%福尔马林溶液中固定10min以上。然后沿胃大弯剪开胃,将其平展于玻璃板上,仔细观察前胃部粘膜溃疡的发生情况,并按下述标准进行溃疡分级。0:无溃疡;1:溃疡点、出血点(<1mm);2:1-5个小溃疡(>1mm,<3mm);3:6个以上小溃疡或1个大溃疡(>3mm);4.2个以上大溃疡;5:穿孔性溃疡。实验结果用t值法进行统计分析,结果见表1。

结果表明,本发明制备的提取物对大鼠幽门结扎型胃溃疡具有显著的抑制作用,与按传统水煎法制得的半夏泻心汤干浸膏相比效果区别不大,且实施例2的抑制效果略优于半夏泻心汤干浸膏。以上结果说明,采用大孔树脂精制工艺没有导致有效成分的损失,该工艺可行。
实验例2抗炎作用研究
取健康Wistar大鼠,随机分为正常对照组、阿司匹林组、半夏泻心汤干浸膏组、实施例1~4干浸膏组、每组均10只,雌雄各半。进行灌胃给药,正常对照组给予等容量蒸馏水。连续三天,每天灌胃一次,最后一次灌胃后0.5h,从大鼠左后肢足趾皮下注射新鲜蛋清0.1ml/鼠致炎。致炎前测量一次踝关节周长,并于致炎后2h分别测量一次踝关节周长,计算关节肿胀率,结果见表2。

结果表明,本发明制备的提取物具有较好的抗炎作用。
实验例3抗胃幽门螺杆菌(HP)作用研究
取健康大鼠,随机分为正常对照组、无治疗药物组、甲硝唑组、半夏泻心汤干浸膏组、实施例1~4干浸膏组、每组均10只,雌雄各半。除正常对照组外其余各组连续十天空腹灌胃食醋2.5ml/只;第十一天,采用空腹束缚水浴6h;从第十二天起,上述隔天空腹灌胃HP混悬液(1×109CFU/ml)1.5ml/只,共三次;第十六天除正常对照组和无治疗药物模型对照组外其余各组灌胃给药,连续给药十天。第二十五天下午所有组动物禁食过夜,次日将各组动物脱颈椎处死,在无菌的条件下,取胃粘膜快速进行尿素酶试验和HP接种培养,同时直接涂片观察HP阳性率。试验结果见表3。

结果表明,实施例1~4制备的提取物能较好的杀灭感染大鼠体内HP,具有较强的抗炎效果。

Claims (7)

1.一种治疗慢性胃炎、胃溃疡的中药,其特征在于:它是由以下重量份数的中药材原料制备而成:
姜半夏3~5份、黄芩2~4份、干姜2~4份、党参2~4份、炙甘草2~4份、黄连1~3份、大枣1~3份,
制备方法是:按比例取所述中药材原料,加水或用体积含量不超过60%的含醇水溶液提取,收集提取液,滤过,将滤液减压浓缩至相对密度为1.05~1.15的浓缩液,上大孔吸附树脂柱,用水或体积含量不超过60%的含醇水溶液洗脱,收集洗脱液并浓缩成浸膏,再加入辅料制成适合临床上使用的剂型。
2.根据权利要求1所述治疗慢性胃炎、胃溃疡的中药,其特征在于所述制备方法是:按比例取所述中药材原料,用体积含量为40~60%的含醇水溶液加热回流提取,收集提取液,滤过,将滤液减压浓缩至相对密度为1.05~1.15的浓缩液,上大孔吸附树脂柱,用体积含量为40~60%的含醇水溶液洗脱,收集洗脱液并浓缩成浸膏,再加入辅料制成适合临床上使用的剂型。
3.根据权利要求2所述治疗慢性胃炎、胃溃疡的中药,其特征在于:所述加热回流提取的次数为1~3次,每次提取的时间为0.5~2小时,所述提取中含醇水溶液与中药材的重量比为5~15∶1。
4.根据权利要求1~3任何一项所述治疗慢性胃炎、胃溃疡的中药,其特征在于:所述大孔吸附树脂柱中,大孔吸附树脂的用量为中药材重量的1~3倍。
5.根据权利要求1~3任何一项所述治疗慢性胃炎、胃溃疡的中药,其特征在于:所述洗脱液的体积为大孔吸附树脂柱体积的5~20倍。
6.根据权利要求1~3任何一项所述治疗慢性胃炎、胃溃疡的中药,其特征在于:所述醇为C1~4脂肪醇。
7.根据权利要求1所述治疗慢性胃炎、胃溃疡的中药,其特征在于:所述剂型为颗粒剂、片剂、胶囊剂、糖浆或口服液。
