技术领域
本发明属于中药技术领域,涉及一种治疗小儿哮喘的中药组合物及其制备方法。
背景技术
小儿哮喘是一种多种因素共同参与的气道慢性炎症,是儿童时期的常见病,多见于婴幼儿和学龄期儿童。临床症状表现反复发作性咳嗽、喘鸣和呼吸困难,并伴有气道高反应性,是一种可逆性、梗阻性呼吸道疾病。诱发支气管哮喘的因素是多方面的,常见因素包括:1)过敏原,包括呼吸道病毒、尘螨、屋尘、多价花粉、羽毛等吸入性过敏原以及食物过敏等;2)非特异性刺激物质,如灰尘、烟、气味等;3)气候,儿童患者对气候变化很敏感,如气温突然变冷或气压降低,常可激发哮喘发作;4)精神因素,大哭大笑或激怒恐惧后可引起哮喘发作;5)遗传因素,家庭及个人具有过敏史的儿童,其患病率较一般群体高;6)剧烈持续的奔跑和一些药物也可诱发哮喘。
吸入糖皮质激素是目前西医治疗小儿哮喘最常用的有效办法,给药途径方便,药物靶点明确,可以减轻哮喘症状、降低气道高反应性及减少哮喘的急性加重,但对慢性哮喘的控制不理想,且长期大剂量使用会对患儿的生长发育造成影响,并有导致全身不良反应的可能性。其他应用于小儿哮喘的药物如拟交感胺类和共碱类等平喘药物,茶碱类和抗胆碱类药物等,药物副作用明显,长久使用可产生耐药性,只适用于小儿哮喘急性发作的治疗。
祖国医学关于支气管哮喘早有认识,俗称“吼喘”。《内经》中描述为“喘吁”、“喘鸣”。在《素问·通评虚实论》中有“乳子中风热,喘鸣肩息者”的记载,专门提到小儿“喘鸣”。《金匮要略》、《幼科全书》、《证治准绳》等著作中,也都有关于哮喘的症状描述和病机论述。中医认为,小儿哮喘的病机与肺、脾、肾有关,《保婴撮要》指出:“多因脾肺气虚,腠理不密,外邪所乘”,使肺失宣降所致。由于肺气根于肾,如哮喘延久,肾气虚衰,并可出现肾不纳气或上实下虚的症象。发作时治疗以平喘降逆、宣肺化痰为主,而缓解期的治疗,需抓住肺、脾、肾三脏不足和痰瘀内伏的疾病之本,根据患儿不同体质状况,采用益肺、健脾、补肾、活血之法进行治疗。其中补肺可固表,以御六淫邪气之侵袭,减少复发。现代药理研究也已证实,补肺气能提高机体免疫防御功能,增强皮肤黏膜的屏障作用。
中国专利申请201410251750.6公开了一种治疗小儿哮喘的中药组合物及其制备方法,其主要由以下原料制得:紫苏10-50 份,胖大海10-20 份,桑叶5-20 份,紫菀5-15份,栀子 5-10 份,茯苓5-15 份,款冬花5-15 份,党参 1-10 份,龙胆草5-15 份,黄芪 9-15份,黄连9-15 份,香薷5-15 份,麻黄1.5-9 份,白芍5.5-15 份,五味子6-9 份,甘草10-15份。该中药组合物具有免疫调节及活血化瘀的效果,但临床试验发现其存在轻度腹泻的不良反应。
中药治疗小儿哮喘具有作用温和,标本兼治,起效持久等特点,结合现代中医药发展理论,对中药成分进行筛选提取及科学的配伍,寻求一种药效稳定,副作用低,疗效显著的治疗小儿哮喘的中药组合物,具有十分重要的临床意义。
发明内容
本发明要解决的技术问题是提供一种治疗小儿哮喘的中药组合物,该中药组合物具有宣肺平喘,化痰止咳,润肺下气和生津利咽的功效,具备治疗效果显著、药效稳定、制备工艺简单、药物成本低和易于推广应用等优点。
此外,本发明还提供相应的制造工艺将该中药组合物在保证疗效的前提下制成方便易用的成品制剂。
本发明中药组合物包括以下重量份数的制备原料:
紫苏子27-36份、款冬花26-32份、麻黄19-25份、桔梗20-25份、川贝母21-27份、胖大海18-22份、百部16-21份、杏仁10-17份、乌梅14-18份、半夏8-13份、山楂11-16份、地龙8-12份、生姜7-13份和甘草5-12份。
优选地,所述的中药组合物包括以下重量份数的制备原料:紫苏子32份、款冬花27份、麻黄22份、桔梗23份、川贝母23份、胖大海20份、百部18份、杏仁15份、乌梅16份、半夏10份、山楂13份、地龙11份、生姜9份和甘草9份。

本发明还提供了所述中药组合物的制备方法,包括以下步骤:
(1)取紫苏子、款冬花、麻黄、桔梗、川贝母和杏仁,碎成粗粉,按照粗粉总重量的4-9倍加入体积浓度为60%-95%的乙醇溶液,回流提取2-3次,每次回流时间为1.5-3h,合并提取液,回收乙醇,减压浓缩至60-65℃环境下相对密度为1.20-1.40的浸膏,减压干燥,粉碎,过80-100目筛,得浸膏细粉A;
(2)取胖大海、百部、乌梅、半夏、山楂、地龙、生姜和甘草,加入药材总重量6-8倍量的水煎煮2-3h,过滤;滤渣加入药材总重量4-6倍量的水煎煮2-2.5h,过滤;合并两次滤液,减压浓缩至60-65℃环境下相对密度为1.05-1.20的浸膏,加入乙醇至含醇量为60-90%(v/v),静置12-24小时,取上清液,回收乙醇,减压浓缩至60-65℃环境下相对密度为1.15-1.25的浸膏,减压干燥,粉碎,过80-100目筛,与上述浸膏细粉A混合,充分混匀后即得本发明中药组合物。
利用现代通用的中药制剂技术,本发明中药组合物可制成临床需要的成品制剂,如口服液、糖浆剂、颗粒剂或散剂等剂型。
本发明中药组合物所用中药材的来源、性味、归经及功效:
紫苏子:本品为唇形科植物紫苏的干燥成熟果实。味辛,性温;归肺经;降气消痰,平喘,润肠。
款冬花:本品为菊科植物款冬的干燥花蕾。味辛、微苦,性温;归肺经;润肺下气,止咳化痰。
桔梗:本品为桔梗科植物桔梗的干燥根。味苦、辛,性平;归肺经;开宣肺气,利咽,祛痰,排脓。
川贝母:本品为百合科植物川贝母、暗紫贝母、甘肃贝母或梭砂贝母的干燥鳞茎。味苦、甘,性微寒;归肺、心经;清热润肺,化痰止咳。
麻黄:本品为麻黄科植物草麻黄、中麻黄或木贼麻黄的干燥草质茎。味辛、微苦,性温;归肺、膀胱经;发汗散寒,宣肺平喘,利水消肿。
百部:本品为百部科植物直立百部、蔓生百部或对叶百部的干燥块根。味甘、苦,性微温;归肺经;温润肺气,止咳。
胖大海:本品为梧桐科植物胖大海的干燥成熟种子。味甘,性寒;归肺、大肠经;清热润肺,利咽解毒,润肠通便。
杏仁:本品为蔷薇科植物杏或山杏等味苦的干燥种子。味苦,性温;归肺、大肠经;祛痰止咳,平喘,润肠。
半夏:本品为天南星科植物半夏的干燥块茎。味辛,性温;归脾、胃、肺经;燥湿化痰,降逆止呕,消痞散结。
乌梅:本品为蔷薇科植物梅的干燥近成熟果实。味酸、涩,性平;归肝、脾、肺、大肠经;敛肺,涩肠,生津,安蛔。
山楂:本品为蔷薇科植物山里红或山楂的干燥成熟果实。味酸、甘,性微温;归脾、胃、肝经;消食健胃,行气散瘀。
地龙:本品为巨蚓科动物参环毛蚓、通俗环毛蚓、威廉环毛蚓或栉盲环毛蚓的干燥体。味咸,性寒;归肝、脾、膀胱经;清热定惊,通络,平喘,利尿。
生姜:本品为姜科植物姜的新鲜根茎。味辛,性微温;归肺、脾、胃经;解表散寒,温中止呕,化痰止咳。
甘草:本品为豆科植物甘草、胀果甘草或光果甘草的干燥根。味甘,性平;归心、肺、脾、胃经;补脾益气,清热解毒,祛痰止咳,缓急止痛,调和诸药。
本发明药物组方的组方分析:
本发明药物组方以紫苏子和款冬花为君药,平喘,润肺下气,止咳化痰;以麻黄、桔梗、百部、川贝母和胖大海为臣药,清热润肺,宣肺平喘,化痰止咳,利咽;以杏仁、乌梅、半夏、山楂和地龙为佐药,生津,行气,通络,敛肺,涩肠;以生姜和甘草为使药,温中,解毒,调和诸药,使诸药搭配,协同作用,共同达到宣肺平喘,化痰止咳,润肺下气,生津利咽的功效,对小儿哮喘具有十分显著的治疗效果。
与现有技术相比,本发明中药组合物具有如下技术优势:
(1)本发明中药组合物为纯天然中药制剂,毒副作用小,长期服用也能避免或减少药物的不良反应,适用于小儿哮喘的长期治疗。
(2)本发明中药组合物药材与药量配伍科学,具有宣肺平喘,化痰止咳,润肺下气,生津利咽等方面的功效,临床上用于治疗小儿哮喘具有起效快,疗效好等显著的治疗优势,可明显改善喘息、呼吸困难、胸闷、咳嗽等临床症状,有利于保护和维持患者正常的肺功能。
(3)本发明中药组合物还具有药效稳定、制备工艺简单、成本低和易于推广应用等优点。
具体实施方式
以下将通过具体实施例对本发明进行进一步描述,本领域技术人员应理解,本发明除以下实施例外还可做出其他类似的改变获得相似的结果而不脱离本发明的内容和精神范围。
实施例1
本发明实施例1中药组合物包括以下重量份数的制备原料:
紫苏子27份、款冬花26份、麻黄19份、桔梗20份、川贝母21份、胖大海18份、百部16份、杏仁10份、乌梅14份、半夏8份、山楂11份、地龙8份、生姜7份和甘草5份。
制备方法如下:
(1)取紫苏子、款冬花、麻黄、桔梗、川贝母和杏仁,碎成粗粉,按照粗粉总重量的7倍加入体积浓度为70%的乙醇溶液,回流提取2次,每次回流时间为2h,合并提取液,回收乙醇,减压浓缩至60℃环境下相对密度为1.25的浸膏,减压干燥,粉碎,过80目筛,得浸膏细粉A;
(2)取胖大海、百部、乌梅、半夏、山楂、地龙、生姜和甘草,加入药材总重量7倍量的水煎煮3h,过滤;滤渣加入药材总重量5倍量的水煎煮2h,过滤;合并两次滤液,减压浓缩至60℃环境下相对密度为1.10的浸膏,加入乙醇至含醇量为90%(v/v),静置24小时,取上清液,回收乙醇,减压浓缩至60℃环境下相对密度为1.25的浸膏,减压干燥,粉碎,过80目筛,与上述浸膏细粉A混合,充分混匀后即得本发明中药组合物细粉;
(3)取上述细粉1份,蔗糖粉3份、糊精1.25份及乙醇适量,制软材,过16目筛制颗粒,干燥,即得本发明中药组合物颗粒剂。
实施例2
本发明实施例2中药组合物包括以下重量份数的制备原料:
紫苏子36份、款冬花32份、麻黄25份、桔梗25份、川贝母27份、胖大海22份、百部21份、杏仁17份、乌梅18份、半夏13份、山楂16份、地龙12份、生姜13份和甘草12份。
制备方法同实施例1。

(1)取紫苏子、款冬花、麻黄、桔梗、川贝母和杏仁,碎成粗粉,按照粗粉总重量的9倍加入体积浓度为70%的乙醇溶液,回流提取2次,每次回流时间为2h,合并提取液,回收乙醇,减压浓缩至60℃环境下相对密度为1.20的浸膏,减压干燥,粉碎,过80目筛,得浸膏细粉A;
(2)取胖大海、百部、乌梅、半夏、山楂、地龙、生姜和甘草,加入药材总重量8倍量的水煎煮3h,过滤;滤渣加入药材总重量6倍量的水煎煮2h,过滤;合并两次滤液,减压浓缩至60℃环境下相对密度为1.05的浸膏,加入乙醇至含醇量为90%(v/v),静置24小时,取上清液,回收乙醇,减压浓缩至60℃环境下相对密度为1.15的浸膏,减压干燥,粉碎,过80目筛,与上述浸膏细粉A混合,充分混匀后即得本发明中药组合物细粉。
(3)取400g蔗糖加600mL水加热溶解制成糖浆,加入上述细粉25g和5%羟苯乙酯10mL,充分溶解和混匀,过滤后加水至1000mL,搅匀,灌装于10mL口服液瓶中,灭菌,包装,即得本发明中药组合物口服液。
实施例4
本发明实施例4中药组合物包括以下重量份数的制备原料:
紫苏子29份、款冬花30份、麻黄22份、桔梗21份、川贝母24份、胖大海20份、百部17份、杏仁10份、乌梅16份、半夏8份、山楂15份、地龙9份、生姜11份和甘草10份。
制备方法同实施例3。
本发明中药组合物药效学研究:
试验一:本发明中药组合物对过敏性哮喘豚鼠作用的的实验研究
1、试验动物选择与分组:健康清洁级豚鼠72只,体重280-320g,雌雄各半,随机分为9组,每组8只。
2、过敏性哮喘豚鼠模型的建立:
正常对照组以生理盐水致敏和引喘,其余各组豚鼠腹腔注射1mL10%卵蛋白(OA)生理盐水溶液致敏。2周后,给予1%OA 生理盐水溶液雾化吸入诱导哮喘发作,以豚鼠腹肌出现明显收缩、呛咳、喘鸣等呼吸道梗阻症状为阳性,连续诱喘3天后,对各组豚鼠进行灌胃给药。
3、试验药物及给药方案:
A组:将本发明实施例1制得的中药组合物颗粒剂,用水调配成浓度为0.6g/ml、0.4g/ml和0.2g/ml的溶液,备用。取3组过敏性模型豚鼠,标记为A组高剂量组(剂量1.2g/kg)、中剂量组(剂量0.8g/kg)和低剂量组(剂量0.4g/kg),高剂量组用浓度为0.6g/ml的药物溶液,中剂量组用浓度为0.4g/ml的药物溶液,高剂量组用浓度为0.2g/ml的药物溶液,按剂量每天灌胃1次,连续给药8天。
B组:试验药物为本发明实施例2制得的中药组合物颗粒剂,给药方案同A组。
模型对照组:等容积生理盐水每天灌胃1次,连续灌胃8天。
阳性对照组:阳性对照药为小儿咳喘灵颗粒(国药准字Z20053197,江西药都仁和制药有限公司),用水调配成0.6g/ml的溶液,按1.2g/kg的剂量每天灌胃给药1次,连续给药8天。
正常对照组:等容积生理盐水每天灌胃1次,连续灌胃8天。
4、试验方法:豚鼠末次给药124h后用1.5%戊巴比妥钠60mg/kg腹腔麻醉,腹主动脉取血后,立即进行肺泡灌洗术,每次抽取生理盐水5mL,缓慢注入气管后回收,共3次,收集所有支气管肺泡灌洗液(BALF),离心,取上清备用。测定血液和BALF中嗜酸粒细胞(EOS)、内皮素-1(ET-1)和一氧化氮(NO)的含量。
5、试验结果:
各组豚鼠血液中EOS、ET-1和NO的含量如表1所示,各组豚鼠BALF中EOS、ET-1和NO的含量如表2所示。

注:与正常对照组比较,模型对照组▲▲P>0.01;与模型对照组比较,药物试验组* P<0.05,** P<0.01;与阳性对照组比较,本发明中药组合物药物试验组# P<0.05,## P<0.01。

6、试验结论:
试验数据显示,与模型对照相比,药物试验组均能有效降低血液和BALF中EOS、ET-1和NO的含量。与阳性对照组相比,本发明中药组合物高剂量组对过敏性哮喘模型豚鼠的治疗效果具有显著性的差异,而中剂量组和低剂量组的治疗效果也优于或与阳性对照组的治疗效果相当。
本发明中药组合物对过敏性豚鼠模型的实验研究表明,本发明中药组合物具有显著降低嗜酸粒细胞、内皮素-1和一氧化氮的作用,说明其具有降低炎症反应和支气管高反应性的作用,同时也具有纠正ET-1/NO失衡现象,改善支气管平滑肌收缩和降低血管通透性的效果。试验结果说明本发明中药组合物对哮喘的治疗效果是多方位、综合作用的结果,治疗效果显著,具有很高的临床推广价值。
试验二:本发明中药组合物的临床试验
(一)受试对象
1、中医诊断标准
参照中华人民共和国中医药行业标准《中医病症诊断疗效标准·中医儿科病症(哮喘)诊断疗效标准》的内容拟定。诊断标准为:
1)发作前常有喷嚏、咳嗽等先兆症状,或夜间突然发作。发作时喉间哮鸣,呼吸困难,咯痰不爽,甚则不能平卧,烦躁不安等。
2)常因气候转变、受凉,或接触某些过敏物质等因素诱发。
3)可有婴儿期湿疹史,或家族过敏史。
4)心肺听诊:两肺满布哮鸣音,呼气延长,或闻及湿罗音,心率增快。
5)支气管哮喘,血白细胞总数正常,嗜酸性粒细胞可增高,可疑变应原皮肤试验常呈阳性。伴肺部感染时,血白细胞总数及中性粒细胞可增高。
符合以上标准者,年龄在16岁以下并知情同意愿意受试者均可纳入试验病例。
2、症候分类
发作期
1)寒饮停肺:咳喘哮鸣,恶寒怕冷,鼻流清涕,痰液清稀,四肢欠温,面色淡白。舌质淡胖,苔薄白或白腻,脉浮滑。
2)痰热壅肺:咳喘哮鸣,痰稠色黄,口干咽红,或发热面红。舌质红,苔薄黄或黄腻,脉滑数。
3)外寒肺热:咳喘哮鸣,恶寒发热,流涕喷嚏,咽红,口渴,痰粘色黄。舌质偏红,苔薄白,脉滑数。
4)虚实夹杂:哮喘持续发作,喘促胸满,端坐抬肩,不能平卧。面色晦滞带青,畏寒肢冷,神疲纳呆,小便清长。舌质淡,苔薄白,脉无力。
缓解期
1)肺气亏虚:面色淡白,乏力,自汗,易于感冒。舌质淡,苔薄白,脉细无力。
2)脾气亏虚:食少便溏,面色少华,倦怠乏力。舌质淡,苔少,脉缓无力。
3)肾气亏虚:动则气促,面色淡白,形寒畏冷,下肢欠温,小便清长。舌淡,苔白,脉细无力。
3、排除及剔除病例标准
1)对本药过敏者;
2)合并有心脑血管、肝肾和造血系统等严重原发性疾病,精神病患者;
3)不符合纳入标准、未按规定用药、无法判断疗效或资料不全影响疗效或安全性判断者。
(二)临床试验方案
1、一般资料:
按诊断标准筛选出120例符合小儿哮喘诊断标准的临床志愿者患者,男68例,女52例,年龄6个月~14岁,平均年龄5.5岁。随机分成治疗组和对照组两组,每组各60例,两组年龄、病情分布等影响因素,经统计学处理,无显著性差异,符合分组条件。
2、试验药物及试验方法:
治疗组:服用本发明实施例3制备的本发明中药组合物口服液,2岁以下每次5mL,3~4岁每次7.5mL,4~7岁每次10mL,8岁以上每次15mL,每天3次。
对照组:服用小儿咳喘灵口服液(国药准字Z33020522,杭州老桐君制药有限公司),服用方法同治疗组。
两组均以1周为1 个疗程,3个疗程后评定疗效。
3、疗效判定标准:
痊愈:哮喘症状完全缓解,平时基本不发作,即使偶有轻度发作也不需用药即可缓解,两肺听诊无喘鸣音。
显效:哮喘发作次数明显减少,发作时症状较前明显减轻,两肺听诊偶闻及喘鸣音。
有效:哮喘发作次数有所减少,发作时症状较前有所减轻,两肺听诊可闻及少许喘鸣音。
无效:哮喘发作次数及发作时症状均无改善,两肺听诊时闻及喘鸣音。
4、治疗结果:如表3所示。

5、试验结论:
临床试验数据显示,本发明中药组合物对于小儿哮喘的治疗效果显著,痊愈率达71.67%,愈显率超过80%,总有效率超过90%,均远高于对照组。同时,经临床观察,治疗过程未见任何不良反应,服药一疗程后,患者喘息、呼吸困难、胸闷、咳嗽等临床症状均有明显改善。临床实验证明本发明中药组合物治疗小儿哮喘起效快,治疗效果显著,可作为治疗小儿哮喘的药物使用,临床推广价值高。
Claims (6)
1.一种治疗小儿哮喘的中药组合物,其特征在于,所述的中药组合物包括以下重量份数的制备原料:紫苏子27-36份、款冬花26-32份、麻黄19-25份、桔梗20-25份、川贝母21-27份、胖大海18-22份、百部16-21份、杏仁10-17份、乌梅14-18份、半夏8-13份、山楂11-16份、地龙8-12份、生姜7-13份和甘草5-12份。
2.如权利要求1所述的治疗小儿哮喘的中药组合物,其特征在于,所述的中药组合物包括以下重量份数的制备原料:紫苏子32份、款冬花27份、麻黄22份、桔梗23份、川贝母23份、胖大海20份、百部18份、杏仁15份、乌梅16份、半夏10份、山楂13份、地龙11份、生姜9份和甘草9份。
3.如权利要求1所述的治疗小儿哮喘的中药组合物,其特征在于,所述的中药组合物包括以下重量份数的制备原料:紫苏子27份、款冬花26份、麻黄19份、桔梗20份、川贝母21份、胖大海18份、百部16份、杏仁10份、乌梅14份、半夏8份、山楂11份、地龙8份、生姜7份和甘草5份。
4.如权利要求1所述的治疗小儿哮喘的中药组合物,其特征在于,所述的中药组合物包括以下重量份数的制备原料:紫苏子36份、款冬花32份、麻黄25份、桔梗25份、川贝母27份、胖大海22份、百部21份、杏仁17份、乌梅18份、半夏13份、山楂16份、地龙12份、生姜13份和甘草12份。
5.如权利要求1-4任一所述的治疗小儿哮喘的中药组合物,其特征在于:所述的中药组合物被制成口服液、糖浆剂、颗粒剂或散剂。
6.一种制备如权利要求1-4任一所述的治疗小儿哮喘的中药组合物的方法,其特征在于,所述的制备方法包括以下步骤:
(1)取紫苏子、款冬花、麻黄、桔梗、川贝母和杏仁,碎成粗粉,按照粗粉总重量的4-9倍加入体积浓度为60%-95%的乙醇溶液,回流提取2-3次,每次回流时间为1.5-3h,合并提取液,回收乙醇,减压浓缩至60-65℃环境下相对密度为1.20-1.40的浸膏,减压干燥,粉碎,过80-100目筛,得浸膏细粉A;
(2)取胖大海、百部、乌梅、半夏、山楂、地龙、生姜和甘草,加入药材总重量6-8倍量的水煎煮2-3h,过滤;滤渣加入药材总重量4-6倍量的水煎煮2-2.5h,过滤;合并两次滤液,减压浓缩至60-65℃环境下相对密度为1.05-1.20的浸膏,加入乙醇至含醇量为60-90%v/v,静置12-24小时,取上清液,回收乙醇,减压浓缩至60-65℃环境下相对密度为1.15-1.25的浸膏,减压干燥,粉碎,过80-100目筛,与上述浸膏细粉A混合,充分混匀后即得本发明中药组合物。
