背景技术
腹腔镜和内窥镜外科手术操作是微创操作,在该微创操作中,依靠通过身体中的小入口或进入口(例如,由身体的固有出入口、通过组织刺穿器械(如套针等)生成的开口所限定)插入的细长器械在身体内实施手术。
微创操作经常用来从身体内部局部地或完全地移除身体组织或器官,例如,肾切除术、胆囊切除术、十二指肠切除术、回肠切除术、空肠切除术以及其它的这种操作。在这种操作期间,通常是染病的组织或器官必须通过皮肤中的进入口或通过套管移除。已知本领域中的各种捕集装置来帮助进行这个操作。

参考图1至图3,并且最初参考图1,图示出依照本公开实施例的标本取出装置10。标本取出装置10包含壳体12、外轴14以及内轴16(内轴16在图1中以虚线示出)。标本取出装置10(以及与其有关的部件)可由任何合适的生物相容的材料(如塑料)形成。在一个实施例中,注模制造工艺可用来形成壳体12、外轴14和内轴16。
继续参考图1,壳体12包含大体细长的结构且可形成为单一的部件或为通过一个或多个合适的联接方法(如,超声焊接、一个或多个合适的粘合剂)来与彼此联接的两个单独的半部件。在后面的实例中,凹槽/定位槽结构(未明确示出)可用来促进联接两个分离的半部件。壳体12包含一对相对的侧面延伸部13a、13b,所述侧面延伸部13a、13b可起到让用户抓住的手柄的作用,以有助于标本取出装置10的操纵(图1和图2)。
外轴14从壳体12向远侧延伸且包含具有从中贯穿的纵轴线“A-A”的大体管状结构。当小袋18与内轴16联接且处在开口结构中时,相对于穿过小袋18的纵轴线“B-B”,纵轴线“A-A”以不同(如基本上正交或垂直)的方向来定向(图1)。
外轴14被定尺寸以通过套针管(或固有的身体孔口)插入,用来进行内窥镜和腹腔镜操作。通过一个或多个合适的联接方法(例如,粘合剂、超声焊接等),外轴14与壳体12可操作地联接。然而,外轴14可与壳体12一起整体形成。
外轴14包含第一腔15a和第二腔15b(在图1中以虚线示出)。第一腔15a沿外轴的长度延伸且配置为支撑内轴16,以允许内轴16在第一腔15a内平移。第二腔15b沿外轴14的长度延伸且相对于第一腔15a平行地定向。第二腔15b配置为在小袋18与内轴16联接时来接纳小袋18的肚带,这将在下文更详细地描述。
内轴16包含大体细长的结构且在外轴14内是可平移的,以使小袋18移动靠近目标组织。内轴16的近侧端11包含配置为接纳临床医生手指的指环19。
内轴16的远侧端包含支撑构件,通过一个或多个合适的联接方法,该支撑构件可用弹簧20(或其它合适的装置)的形式。例如,在图示的实施例中,内轴16的远侧端与弹簧20的近侧端二次成型(overmolded)。其它联接的方法也可用来使弹簧20与内轴的远侧端联接。例如,一个或多个销钉、铆钉或类似物可用来使弹簧20的近侧端与内轴16的远侧端联接。
参考图2,弹簧20包含形成开叉结构(open fork configuration)的两个柔性的或弹性的构件22和24。弹性的构件22、24配置为从受力的或未扩张的状态(未明确示出)移动至未受力的或扩张的状态。具体地,当用户使指环19向近侧移动时,内轴16在第一腔15a内向近侧移动,并且弹性构件22、24接触其内表面(例如,在外轴14的远侧端处)。内轴16在第一腔15a内的继续近侧移动使弹性构件22、24向彼此接近,以便弹性构件22、24变成基本上平行于彼此的定向。弹性构件22、24在该定位的情况下,用户可以能释放地将小袋18与弹簧20的弹性构件22、24联接。
弹性构件22、24各自包含凸起23a、23b形式的支撑构件连接构件,支撑构件连接构件配置为使小袋18固定至弹簧20的弹性构件22、24;也可使用其它支撑构件连接构件。例如,支撑构件连接构件可用定位槽、钩状物、突起以及类似物的形式。一旦小袋18固定至弹性构件22、24,那么弹性构件22、24将自动呈现其未受力的或扩张的状态,以共同形成用于支撑小袋18的开口25外围的大体圆形或“类似箍(hoop-like)”的结构(图1和图2)。
参考图3,小袋18由能够形成不能渗透的柔性膜的任何合适的生物相容的材料(如尼龙)制成。小袋18包含由能打开能闭合的上部或嘴部26和闭合的下部21所限定的大体管状或细长的结构。上部26包含环形套筒17。通过使上部26折入小袋18的内部并随后把上部26焊接至小袋上,套筒17可形成在小袋18上。
通向套筒17的一对开口28a、28b(开口28b在图3中未明确示出)设在小袋18上。开口28a、28b配置为接纳弹簧20的相应的弹性构件22、24,以允许临床医生在弹性构件22、24上滑动套筒17。具体地,当弹性构件22、24处于受力状态(例如,弹性构件22、24向彼此接近)时,临床医生可在弹性构件22、24上滑动套筒17直到凸起23a、23b接合设于邻近套筒17的上部26的相应的凹处30a、30b为止。一旦凸起23a、23b接合凹处30a、30b,那么临床医生可向远侧移动包含弹簧20的弹性构件22、24的内轴16。内轴16的继续远侧移动导致弹性构件22、24自动地移回到未受力的结构,这接下来导致小袋18处于用于接纳组织的开口结构。
小袋18的穿孔部31设在套筒17和沿小袋18圆周地延伸的沟槽33之间。沟槽33中包含肚带一部分(例如,缝合线“S”、丝线、金属丝、线缆或类似物),该肚带的一部分用来在组织定位于小袋18内之后来捆紧小袋18。
具体地,缝合线“S”的远侧端通过一个或多个合适的联接方法(例如,粘合剂、焊接等)在沟槽33内与小袋18联接,而缝合线“S”的近侧端与定位在外轴14的第二腔15b内的缝合线导引件32联接。缝合线导引件32能通过第二腔15b的远侧端插入,且能沿第二腔15b的内表面滑动,以退出第二腔15b的近侧端。在图示的实施例中,缝合线导引件32是加重的,以允许在缝合线32插入第二腔15b之后且当临床医生将外轴14的远侧端移在纵轴线“A-A”上方(例如,将外轴14的远侧端移在大体竖立的结构)时,缝合线导引件32滑动穿过第二腔15b。
在使用中,临床医生可将内轴16向近侧移动,以使弹簧20和弹性构件22、24收缩,以便弹性构件22、24以按照上文所描述的方式向彼此移动。此后,弹簧20的弹性构件22、24可插入套筒17的相应的开口28a、28b,并且可使小袋18滑到弹簧20上面(反之亦然),直到弹性构件22、24上的凸起23a、23b接合套筒17上的凹处30a、30b的这种时刻为止。
一旦凸起23a、23b接合凹处30a、30b,那么临床医生然后可将缝合线导引件32插入外轴14的第二腔15b。在实施例中,临床医生可以按照上文描述的方式来升起外轴14,以使缝合线导引件32滑动穿过第二腔15b,以便缝合线导引件32退出第二腔15b的近侧端,以便被临床医生抓住。可以理解,缝合线导引件32可首先通过第二腔15b插入,且然后小袋18可与弹簧20联接。
在小袋18与弹簧20联接之后,组织可被定位于小袋18内。此后,临床医生可抓住缝合线导引件32然后向近侧拉它,这接下来将使小袋18闭合且沿穿孔部31撕开小袋18,留下套筒17仍然与弹性构件22、24联接。缝合线“S”于是可被切断并且内轴16和外轴14可相应地从患者移除。捆紧的小袋18于是可从患者移除。
在需要安装另一个小袋18的情况中,小袋18的上部26的套筒17可从弹性构件22、24移除,并且新的小袋18可以按照上文描述的方式来与弹簧20联接。可以理解,新的小袋18可在患者体腔的内部或外部来与弹簧20联接。
不同于传统的标本取出装置,标本取出装置10的内轴16可与新的小袋18联接,且被用来移除另外的目标组织。可以理解,这可减少与组织移除操作有关的总的手术成本。标本取出装置10也允许外科医生使用单一的器械来移除多个组织标本;因此,减小浪费。
从前述内容以及各幅附图,本领域的技术人员将意识到也可对本公开作出某些修改,而不脱离相同的范围。例如,切断机构(未示出)可以是壳体12和/或指环19的一部分,并且可配置为在组织定位于小袋18内以及小袋18闭合且沿穿孔移除之后来切开缝合线“S”。
在实施例中,上部26可由允许小袋18的上部26在被压缩的状态和未被压缩的状态之间移动的可压缩材料所形成。在这个实施例中,临床医生可挤压上部26来压缩上部26且定位开口28a、28b,用于弹性构件22、24的接收。换句话说,在被压缩的状态中,开口28a、28b将被定向成基本上平行于彼此,并且处于与对应的弹性构件22、24大致对齐的位置。可以理解,这可利于小袋18与弹簧20的联接。
而且,在实施例中,弹性构件22、24的凸起23a、23b可省略。具体地,小袋18可设有具有开口或孔的近侧联接部(未示出),所述开口或孔接合可设在内轴16远侧端的止动销。在这个实施例中,内轴16具有靠近其远侧端的凹口,该凹口限定近侧凹口面和远侧凹口面。
弹性构件22、24可通过小袋的套筒17来进给。止动销可在凹口面之间完全地延伸,以便小袋18附接至内轴16的远侧端,并且将不会与其意外地分开。

Claims (20)
1.一种标本取出装置,包括:

小袋,其与所述至少两个支撑构件能释放地联接,所述小袋包含具有能够穿过所述外轴以便用户抓住的近侧端的肚带,所述小袋包含通向设在所述小袋上的套筒的一对开口,所述开口各自配置为接纳所述至少两个支撑构件中的相应的一个,所述小袋进一步包含用于接合所述支撑构件连接构件且阻止所述小袋从所述至少两个支撑构件意外分开的至少一个小袋连接构件。
