背景技术
脉管炎是一种以肢体经络阻塞,气血凝滞,血管游塞,致使肢趾指缺血坏死为基本特征的血管性疾病,中医称之为脱疽。多因风邪、寒湿、湿热及血瘀等原因,造成瘀阻经脉,致气血不能到达肢端,肢端失去气血之濡养所致。病变进程缓慢,以肢冷、麻木、间歇性跛行、疼痛、肢端坏疽等为主要症状,多发男性青壮年,下肢多见。脉管炎是日常生活中比较常见的一种疾病,近年来发病率是逐年增高,给人们的生活带来了很大的影响,病情轻者生活不便,严重者可造成患肢缺血坏死,甚至致残,给患者造成的痛苦及危害都极大,其临床治疗较棘手。目前,治疗脉管炎的方法主要有西医疗法,中西医结合疗法,手术疗法及中药疗法,这些疗法有一定的疗效,但总体治疗效果(包括治愈率)尚欠满意, 易复发,且某些药物的副反应较多。而目前还没有一种针对于治疗湿热证型血栓闭塞性脉管炎的专用中成药。

白残花【来源】蔷薇科白残花rosa multiflora thunb. var. cathayensis rehd. et wils.,以根入药。【性味】苦、涩,寒。【功能主治】 活血通络。主治关节炎,面神经瘫痪。
石斛【来源】本品为兰科植物环草石斛Dendrobium loddigesii Rolfe. 、马鞭石斛Dendrobium fimbriatum Hook. Var. Oculatum Hook 、黄草石斛Dendrobium chrysanthum Wall. 、铁皮石斛Dendrobium candidum . Wall. Ex Lindl. 或金钗石斛Dendrobium nobile Lindl 的新鲜或干燥茎。【性昧】甘,微寒。【归经】归胃、肾经。【功能主治】益胃生津,滋阴清热。用于阴伤津亏,口干烦渴,食少干呕,病后虚热,目暗不明。
血竭【来源】本品为棕榈科植物麒麟竭果实和藤茎中的树脂。【性昧】咸,性平,小毒。【归经】心经、肝经。【功能主治】散瘀定痛,止血,生肌敛疮。主治跌打损伤、内伤瘀痛、痛经、产后瘀阻腹痛、外伤出血不止,瘰疬、臁疮溃久不合及痔疮。
锻自然铜【来源】本品为硫化物类矿物黄铁矿族黄铁矿。煅自然铜: 取净自然铜,照煅法煅至暗红,醋淬至表面呈黑褐色,光泽消失并酥松,每100kg自然铜,用醋30kg。【性味】辛,平。【归经】归肝经。【功能主治】散瘀,接骨,止痛。用于跌扑肿痛,筋骨折伤。
七爪风【来源】为双子叶植物药蔷薇科植物七爪风的根。【性味】苦,涩,酸,平。【归经】肺,肝经。【功能主治】祛风除湿,活血通络。主风寒湿痹,四肢关节痛,中风偏瘫,肢体麻木,活动障碍。
穿山甲【来源】本品为鲮鲤科动物穿山甲 Manis pentadactyla Linnaeus的鳞甲。【性味】咸,微寒。【归经】归肝、胃经。【功能主治】通经下乳,消肿排脓,搜风通络。用于经闭癓瘕,乳汁不通,痈肿疮毒,关节痹痛,麻木拘挛。
水蛭【来源】为水蛭科动物日本医蛭、宽体金线蛭、茶色蛭等的全体。【性味】咸苦,平,有毒。【归经】入肝、膀胱经。【功能主治】破血,逐瘀,通经。用于症瘕痞块,血瘀经闭,跌扑损伤。
当归【来源】本品为伞形科植物当归Angelica sinensis (Oliv.) Diels 的干燥根。【性味】甘、辛,温。【归经】归肝、心、脾经。【功能主治】补血活血,调经止痛,润肠通便。用于血虚萎黄,眩晕心悸,月经不调,经闭痛经,虚寒腹痛,肠燥便秘,风湿痹痛,跌扑损伤,痈疽疮疡。
赤芍【来源】该品为毛茛科植物赤芍Paeonia lactiflora Pall.或赤芍的图片(18张)川赤芍Paeonia veitchii Lynch 的干燥根。【性味】苦,微寒。【归经】归肝经。【功能主治】清热凉血,散瘀止痛。用于温毒发斑,吐血衄血,目赤肿痛,肝郁胁痛,经闭痛经,症瘕腹痛,跌扑损伤,痈肿疮疡。
丹参【来源】本品为唇形科植物丹参Salvia miltiorrhiza Bge. 的干燥根及根茎。【性味】苦,微寒。【归经】归心、肝经。【功能主治】祛瘀止痛,活血通经,清心除烦。用于月经不调,经闭痛经,症瘕积聚,胸腹刺痛,热痹疼痛,疮疡肿痛,心烦不眠;肝脾肿大,心绞痛。
牛膝【来源】为苋科植物牛膝的根【性味】苦、酸,平。【归经】归肝、肾经。【功能主治】补肝肾,强筋骨,逐瘀通经,引血下行。用于腰膝酸痛,筋骨无力,经闭症瘕,肝阳眩晕。
白术【来源】本品为菊科植物白术Atractylodes macrocephala Koidz.的干燥根茎。【性味】苦、甘,温。【归经】归脾、胃经。【功能主治】健脾益气,燥湿利水,止汗,安胎。用于脾虚食少,腹胀泄泻,痰饮眩悸,水肿,自汗,胎动不安。
甘草【来源】为豆科甘草Glycyrrhiza uralensis Fischer、胀果甘草Glycyrrhiza inflata Batal、光果甘草Glycyrrhiza glabra L.根及根茎。【性味】甘,平。【归经】归心、肺、脾、胃经 。【功能主治】补脾益气,清热解毒,祛痰止咳,缓急止痛,调和诸药。用于脾胃虚弱,倦怠乏力,心悸气短,咳嗽痰多,脘腹、四肢挛急疼痛,痈肿疮毒,缓解药物毒性、烈性。
气虚血瘀证型血栓闭塞性脉管炎的临床表现为:面色淡白或晦暗,消瘦乏力,气少懒言,舌质紫暗或淡紫,苔薄,脉涩无力。患肢皮色苍白,疼痛,紫暗,皮薄干裂,腿肌肉松驰萎缩、变细,触之不温。趾甲增厚、变形,趾溃烂、坏疽,久不愈合,脓液清稀。
中医认为,气虚血瘀证型血栓闭塞性脉管炎多由先天不足,正气虚弱,因受外邪侵袭,致使脉络瘀组,气血不畅,气虚推动无力,导致血行缓慢而脉络瘀阻,形成瘀血,痹阻不通,肢端失于濡养而发病。气虚血瘀证型血栓闭塞性脉管炎,是以气虚和血瘀为主要表现症状。身体虚弱,正气耗伤,营卫不和,与患者的情志变化及五脏六腑虚损有关。“疽者,皆因五脏六腑不调而生”,心主血脉,有推动血液运行之能,脾为后天之本,能生血益气,肝主疏泄,调肝能够调畅全身的气机,调节气机的升降出入,气机调畅,则血液和津液的运行正常,肾为先天之本,能藏精化气,对人体脏腑起着濡润滋养和温煦生化作用。由此可见,气虚血瘀证型血栓闭塞性脉管炎与心肝肾脾有着明显的关系。基于气虚血瘀病机,根据“虚则补之,瘀则化之”的中医理论,本发明是以益气行血为治疗原则,以行血祛瘀,化瘀通脉,达到推动血瘀运行使瘀血得以消散为目的,按照本发明的配方所制成的中药,能有效改善血液循环,调节血液分布,畅通微循环障碍,改善局部组织缺氧状态,解除血管痉挛及溶解血栓,从而有效治疗气虚血瘀证型血栓闭塞性脉管炎。多年来,通过对40例患者的使用表明,有效率达95%。
下面结合临床实验数据说明本发明的有益效果:
1、一般资料
2008年2月~2013年1月,共收集40例气虚血瘀证型血栓闭塞性脉管炎的患者,其中,女性6例,男性34例,年龄在25~59岁,病情3个月至3年,右足19例,左足18例,左右足3例。患者临床症状:面色淡白或晦暗,消瘦乏力,气少懒言,舌质紫暗或淡紫,苔薄,脉涩无力。患肢皮色苍白,疼痛,紫暗,皮薄干裂,腿肌肉松驰萎缩、变细,触之不温。趾甲增厚、变形,趾溃烂、坏疽,久不愈合,脓液清稀。服用本发明按照实施例1制得的成药,早、中、晚各一次,疗程60天。
2观察项目:治疗患肢创面、血液循环及身体恢复情况等的先后变化。
3疗效标准与治疗结果
疗效标准:痊愈:临床症状消失,肢体疮面完全愈合,肢体末梢血液循环明显改变,步行速度100-120步/分,能持续行走1km左右,能正常工作、生活;显效:临床症状明显改善,肢体创面愈合或接近愈合,肢体末梢血液循环障碍改善,步行速度100-120步/分,能持续行走500m左右,能正常工作、生活;好转:临床症状减轻,肢体创面接近愈合或缩小,肢体末梢血液循环略有改善,步行速度100-120步/分,能持续行走300m左右,能正常生活;无效:治疗一个疗程后,症状及体征无改善或病情继续发展者。
3.2治疗统计结果见下表
可见,本发明制得的药物能够安全有效治疗气虚血瘀证型血栓闭塞性脉管炎。
下面结合具体病例进一步说明本发明的效果。
病例1:张某,男,53岁,国家公务员。2010年4月7日就诊。患有脉管炎近三年,始发于左下肢,继而累入右下肢,间歇性跛行。曾在多家医院治疗,时有好转,但反复不愈,近一年来疼痛不断加重,且左侧第5趾逐渐溃烂坏死。自述双下肢发凉、持续性疼痛,左足第5趾为甚,疼痛剧烈,夜间更甚,难以入睡。查:双下肢青紫,肌肉萎缩,腿变细,汗毛脱落,双足紫暗,趾甲增厚、粗糙而脆。左足更甚,第5趾溃烂坏死,有少量清稀脓液。双足触痛明显,触之冰凉,足背动脉、胫后动脉搏动消失。伴消瘦、精神差、食欲不佳。 舌淡紫,苔薄,脉细涩无力。诊断为气虚血瘀证型血栓闭塞性脉管炎。服用本发明按实施例1制得的中药,服用20天后,即明显见效,继续服用至60天,所有的临床症状消失,肢体疮面完全愈合,肢体末梢血液循环明显改变,步行速度100-120步/分,能持续行走1km左右,能正常工作、生活痊愈,随访至今没复发过。 病例2:黄某,男,34岁,工人。2011年7月份9日就诊。患有脉管炎近二年。始发于右下肢,继而累计左下肢,曾在多家医院治疗,时有好转,但反复不愈,近半年来疼痛不断加重,且左侧第4趾逐渐溃烂坏死。自述双下肢发凉、持续性疼痛,左足第4趾为甚,疼痛剧烈,夜间更甚,难以入睡。查:双下肢青紫,肌肉萎缩,汗毛脱落,双足紫暗,趾甲增厚、粗糙变形。左足更甚,第4趾溃烂坏死,有少量清稀脓液。双足触痛明显,触之冰凉,足背动脉、胫后动脉搏动消失。伴消瘦、精神差、气少懒言,舌质紫暗,苔薄,脉涩无力。诊断为气虚血瘀证型血栓闭塞性脉管炎。服用本发明按实施例1制得的中药,服用20天后,即明显见效,继续服用至60天,所有的临床症状消失,肢体疮面完全愈合,肢体末梢血液循环明显改变,步行速度100-120步/分,能持续行走1km左右,能正常工作、生活痊愈,随访至今没复发过。 病例3,曲某,女,44岁,自由职业。2012年2月21日就诊。患有脉管炎近一年。发于右下肢,曾在多家医院治疗,时有好转,但反复不愈,近二个月来疼痛不断加重。自述双下肢发凉、持续性疼痛,右足第4趾为甚,疼痛剧烈,夜间更甚,难以入睡。查:右下肢青紫,趾甲增厚、粗糙变形,第4趾溃烂坏死,有少量清稀脓液。触痛明显,触之冰凉,足背动脉、胫后动脉搏动消失。伴面色淡白,精神差、气少懒言,舌质紫暗,苔薄,脉涩无力。诊断为气虚血瘀证型血栓闭塞性脉管炎。服用本发明按实施例1制得的中药,服用20天后,即明显见效,继续服用至60天,所有的临床症状消失,肢体疮面完全愈合,肢体末梢血液循环明显改变,步行速度100-120步/分,能持续行走1km左右,能正常工作、生活痊愈,随访至今没复发过。 具体实施方式

具体实施例1:
称取黄芪50克、党参50克、白残花30克、七爪风8克、当归25克、赤芍25克、丹参25克、牛膝25克、白术25克、石斛50克、血竭15克、锻自然铜15克、穿山甲20克、水蛭20克、甘草10克,加8倍量水,煎煮1.5小时,过滤;滤渣加6倍量水,煎煮1小时,过滤;合并滤液,浓缩,干燥,装入胶囊壳,即得符合正常成人1天用量的胶囊剂。
用于气虚血瘀证型血栓闭塞性脉管炎。口服,每日分早、中、晚三次服用。
具体实施例2:
称取黄芪50克、党参50克、白残花30克、七爪风8克、当归25克、赤芍25克、丹参25克、牛膝25克、白术25克、石斛50克、血竭15克、锻自然铜15克、穿山甲20克、水蛭20克、甘草10克,加10倍量水,煎煮2小时,过滤;滤渣加8倍量水,煎煮1.5小时,过滤;合并滤液,浓缩,干燥,包装,即得符合正常成人1天用量的散剂。
用于治疗气虚血瘀证型血栓闭塞性脉管炎。口服,每日分早、中、晚三次服用。
具体实施例3:
称取黄芪50克、党参50克、白残花30克、七爪风8克、当归25克、赤芍25克、丹参25克、牛膝25克、白术25克、石斛50克、血竭15克、锻自然铜15克、穿山甲20克、水蛭20克、甘草10克,加8倍量水,煎煮1.5小时,过滤;滤渣加6倍量水,煎煮1小时,过滤;合并滤液,浓缩,干燥,按照本领域常规工艺制粒,包装,即得符合正常成人1天用量的颗粒剂。
用于治疗气虚血瘀证型血栓闭塞性脉管炎。口服,每日分早、中、晚三次服用。

1.一种治疗气虚血瘀证型血栓闭塞性脉管炎的中药,其特征是由下列重量配比的药材制成的:
黄芪50份、党参50份、白残花30份、七爪风8份、当归25份、赤芍25份、丹参25份、牛膝25份、白术25份、石斛50份、血竭15份、锻自然铜15份、穿山甲20份、水蛭20份、甘草10份。
