技术领域
本发明总体上涉及被植入眼睛的装置和用于这种装置的输送系统。具体地,本发明涉及有助于流体从眼睛内某一区域移动到眼睛内另一区域的装置的输送系统。 
背景技术
根据美国国家卫生研究院(NIH)的国家眼睛研究院(NEI)发表的规划报告,青光眼是目前全球范围内导致不可逆性失明的主要原因,是世界上仅次于白内障导致失明的第二大主要原因。因此,NEI的规划报告认为,“必须重视并继续投入大量的资源以确定这种疾病的病理生理机制”。青光眼研究已经发现眼内高压与青光眼之间存在较强的联系。因此,眼保健专家通过使用已知为气压计的装置测量病人的眼内压力来例行地筛查出青光眼患者。许多现代的气压计通过向眼睛的外表面吹一股突然的空气进行眼内压力的测量。 
眼睛可以理解为充满流体的球体。眼睛内有两种类型的流体,晶状体后面的腔内充满已知为玻璃体的粘性流体,晶状体前面的腔内充满已知为水状体的流体。当人观看物体时,他或她同时通过玻璃体和水状体观看该物体。 
当人观看物体时,他或她还通过眼镜的角膜和晶状体观看该物体。为 保持透明,角膜和晶状体均没有血管。因此,没有血液流经角膜和晶状体以向这些组织提供营养并从这些组织带走垃圾。相反,这些功能由水状体执行。水状体连续流经眼睛,向没有血管的眼睛的部分(例如,角膜和晶状体)提供营养。水状体的流动还从这些组织处除去垃圾。 
水状体由已知为睫状体的器官产生。睫状体包括连续地分泌水状体的上皮细胞。在健康的眼睛内,随着睫状体的上皮细胞分泌出新的水状体,水状体流从眼睛前房流出,经过小梁网流进施累姆氏管(Schlemm’scanal)。这些多余的水状体从施累姆氏管进入静脉血流并由静脉血携带离开眼睛。 
当眼睛的自然排出系统不能正常工作时,眼睛内的压力开始上升。研究者已经提出理论认为,眼内长期高压将损害从眼睛向大脑传递感觉信息的视神经。这种视神经的损害导致周围视觉的损失。随着青光眼的发展,越来越多的视觉区域损失,直到病人完全失明。

发明内容
本发明总体上涉及(例如用于青光眼治疗的)眼睛植入体及其输送系统。特别地,本发明涉及用于治疗青光眼的眼睛植入体及其输送系统。 
2007年9月24日申请的名称为“(Ocular Implants)眼睛植入体”的共有专利US11/860318公开了新的青光眼治疗植入体,该公开纳入本文。现有技术的眼睛植入体输送系统不能有效地用于输送和配置(操纵)专利US11/860318中描述的植入体。另外,用于输送和配置较早的青光眼治疗植入体的输送系统无法满足一些输送系统需求。 
本发明一方面提供一种将眼睛植入体插入病人眼睛的方法,该眼睛植入体安装在载体上,所述方法包括下述步骤:将套管插入眼睛的前房;移动套管的远端出口使之与施累姆氏管连通;推动眼睛植入体和载体穿过套管的出口进入施累姆氏管。在眼睛植入体具有多个孔的实施方式中,该方法还包括在载体封闭植入体孔的情况下推动眼睛植入体和载体进入施累姆氏管的步骤。

图2-8示出眼睛植入体100,该眼睛植入体通过套管102被输送进入施累姆氏管104内(为易于说明,附图中示出的施累姆氏管为直的,而非弯曲的)。2007年9月24日申请、名称为“Ocular Implants(眼睛植入体)”、美国序列号为11/860,318的专利中更加详细地介绍了图中所示的眼睛植入体。应当理解,本发明的输送系统也可输送和配置其它眼睛植入体。 
如图2所示,套管102的远端部分已经穿过角膜进入眼睛的前房101,并且已经穿透小梁网106从而使得套管102的远端出口108与施累姆氏管104连通。在该实施方式中,如图3和图4所详细示出的,套管102是刚性弯曲管,其在出口108处具有切割部分110。在一些实施方式中,例如通过构成曲率半径小于大约0.1英寸的圆弧,套管102被弯曲以便切线进入施累姆氏管。在其它实施方式中,套管102可以具有其它的形状和曲线。 
在该实施方式中,切割部分110由两个凸起的边缘112形成,这两个边缘112在顶端114处相交。在其它的实施方式中,切割边缘可以是凹陷的或者直的。如图所示,边缘112从顶端114延伸至一对可选的止动部116,该止动部在边缘112与可选的套管延伸部118的相交处形成。如图2所示,套管102的远端可以朝向小梁网106被送进前房101内。当套管102的远端接触到小梁网时,切割部分110的顶端114和边缘112被推进以便延伸穿过小梁网进入施累姆氏管,而延伸部分118向后弯曲并保留在前房101内。当止动部116与小梁网接合时,套管102远端停止移动。 
在一些实施方式中,套管102由透明的聚碳酸酯管形成,其直径小于大约0.030英寸,例如外径为0.028英寸,内径为0.014英寸。在由切割边缘引导至止动部的实施方式中,切割边缘与套管的中心轴线可形成约10°和80°之间的角度,并且止动部可位于顶端114向内大约半个直径处。在具有套管延伸部的实施方式中,延伸部118可从顶端114向外延伸大约1.5mm。除了其它功能,(例如如图2所示)当向前的压力继续保持在套管上时延伸部分118的弯曲向使用者提供与小梁网强力接合以及套管远端的精确位置的反馈。 
在输送过程中,眼睛植入体100被安装在载体120上,该载体与植入体100可在套管102内移动。除了其它功能,载体120的一个特殊功能是封闭在植入体100上形成的孔122,从而减少在植入体被送进的过程中植入体和施累姆氏管内组织之间的干涉。在该实施方式中,眼睛植入体100的远端124为钝的,从而避免损伤眼睛组织。在其它实施方式中,可由载体的至少一部分形成钝的远端。 
在该实施方式中,如图6所示,推进器126与眼睛植入体100的近端128接合,以便推动植入体使之通过套管102的出口108进入施累姆氏管。载体120朝向近端延伸进入推进器126到达例如眼睛外部的手持致动器(未示出)。 
只有当植入体100的近端128保留在前房101内时,植入体向施累姆氏管的移动才停止。根据眼睛植入体的设计的不同,此时植入体可围绕施累姆氏管延伸60°-180°。并且,此时或者在这之前,植入体可在施累姆氏管内转动以获得合适的定向。可(通过例如外部致动器或者控制装置)在载体120上施加近端(导向)力以便从植入体100朝向近端撤回载体,同时推进器126(又通过例如外部致动器或者控制装置)施加远端导向力以便将植入体100保持在合适的位置,如图7所示。然后可将载体120推进器126和套管102从眼睛内撤出,使植入体保留在施累姆氏管内,植入体的进口近端128位于前房101内。 
图9-11示出眼睛植入体(例如图2-8中所示的植入体100)与输送系统之间的接合机构的一个实施方式的细节。在该实施方式中,载体200具有远端直径减小部分202和近端直径增大部分204。推进器206的远端具有与植入体的近端128接合的内部凸缘207和围绕植入体近端128的套环。如图10所示,套环208内形成有一条或者多条允许套环208径向膨胀的缝隙210。另外,本实施方式中的植入体100具有打开的通道近端128,如图8所示,该近端也可径向膨胀。在图9所示的接合构型中,载体的直径增大部分204位于植入体100的近端,该近端又被置于推进器206的套环208内。载体部分204的直径大于套环208和植入体部分128的静止(at-rest)直径,因此使得套环208和植入体部分128从其静止形状径向膨胀。因此,在该构造中,推进器、植入体和载体是摩擦配合,从而使它们作为一个单元移动。 
为使植入体与输送系统脱离接合,载体200被从近端撤回(或者,植入体向远端移动)直到直径减小部分202位于植入体近端部分128和套环208内,如图11所示。由于直径减小部分202的直径小于植入体远端部分 128的静止内径,植入体被从输送系统的载体释放。然后只需通过从近端撤回推进器,便可使推进器与植入体脱离接合。 
图12和13示出本发明的植入体和输送系统的手持致动器的实施方式。在该实施方式中,致动器的功能在两个手柄——近端手柄300和远端手柄302——之间被分开。为便于说明,图12和13省略了套管和植入体。眼睛植入体载体304向近端延伸穿过推进器306进入远端手柄302。在该实施方式中,推进器306具有近端推管308和与近端推管308内表面结合的远端直径减小推管310。载体304向近端延伸穿过远端套筒312并穿过近端中心管314的远端部分(近端中心管314在图12的俯视图和图13的剖视图中示出)。载体304的扩大近端316位于近端中心管314内远端套筒312的近端和远端止动元件318之间。载体304的扩大端316比套筒312和止动元件318的内部直径大。由此,载体304可相对于远端中心管314纵向移动有限的距离。 
在远端手柄302的远端设置与例如上述套管(未示出)的近端接合的小型快速接头(luer fitting)320(或者其它合适的连接件)。因此,可通过相对于眼睛移动远端手柄302来控制套管和植入体移动进入病人的眼内。在一些实施方式中,远端推管308的外表面具有至少一个平坦表面(例如六边形表面),该表面与远端手柄302的内表面上的相应的形状配合,从而手柄302相对于套管转动可使得推进器和植入体旋转。 
编织管322从远端手柄302的近端向近端附近延伸,分别穿过远端和近端压力释放部分324和326到达近端手柄300的远端。编织管322允许手柄300和302相对于彼此转动,由此防止手柄300的无意识转动导致手柄302的转动。

本实施方式的两个手柄的设计允许两个人操作该眼睛植入和输送系统。使用中,眼睛植入体(如上所述)被安装在载体304上并放置在套管(如上所述)内,该套管连接在远端手柄302的小型快速接头320上。使用角镜(goniolen)在可视操作的条件下,外科医生通过推动远端手柄302来推动套管的远端使之穿过病人角膜的孔进入眼睛的前房。当套管已经切割穿过小梁网从而将套管的远端出口与施累姆氏管连通时,辅助保持近端手柄300同时转动致动器332和336,由于致动器332和手柄部分333螺纹配合,这使得推管308和载体304相对于手柄部分333、远端手柄302和角膜向远端附近移动,从而将载体和植入体从套管的远端出口推出。 
当植入体已经被充分地推进施累姆氏管内达一定距离后,通过相对于致动器332转动致动器336从而相对于推管308向近端附近移动载体304使得植入体与输送系统脱离接合,由此当载体被撤回时植入体保持静止不动。在植入体被配置好并与输送系统脱离接合后,推进器、载体和套管被从病人的眼睛内取出。

Claims (13)

1.一种眼睛植入和输送系统,包括:
套管,该套管包括具有远端切割部分的刚性弯曲管,该远端切割部分限定适用于被插入眼睛的施累姆氏管部分内的远端出口,该刚性弯曲管适于穿过眼睛的前房延伸到施累姆氏管并且弯曲成切线进入施累姆氏管;
眼睛植入体;
载体,该载体位于该眼睛植入体内并且能与眼睛植入体一起在套管内移动;和
近端控制装置,能从眼睛外部操作该近端控制装置以便当套管的远端出口位于眼睛内时使眼睛植入体移动通过远端出口进入施累姆氏管。
2.根据权利要求1所述的眼睛植入和输送系统,其特征在于,所述眼睛植入体包括多个孔,并且载体被定向成封闭所述孔。
3.根据权利要求1所述的眼睛植入和输送系统,其特征在于,所述眼睛植入体和载体共同具有钝的远端。
4.根据权利要求1所述的眼睛植入和输送系统,其特征在于,所述套管构成一圆弧。
5.根据权利要求4所述的眼睛植入和输送系统,其特征在于,所述圆弧具有小于0.1英寸的曲率半径。
6.根据权利要求1所述的眼睛植入和输送系统,其特征在于,所述套管的外径小于0.03英寸。
7.根据权利要求1所述的眼睛植入和输送系统,其特征在于,所述载体具有较大直径部分和较小直径部分,所述眼睛植入体与载体的较大直径部分接合。
8.根据权利要求7所述的眼睛植入和输送系统,还包括位于套管内并与眼睛植入体接合的推进器,该推进器与近端控制装置可操作地连接。
9.根据权利要求8所述的眼睛植入和输送系统,其特征在于,所述推进器包括植入体接合机构,该植入体接合机构适用于在眼睛植入体被从 套管的远端出口移出时保持眼睛植入体。
10.根据权利要求9所述的眼睛植入和输送系统,其特征在于,当眼睛植入体设置在载体的较大直径部分和植入体接合机构之间时,所述眼睛植入体与植入体接合机构接合,并且当眼睛植入体设置在载体的较小直径部分和植入体接合机构之间时,所述眼睛植入体与植入体接合机构脱离接合。
11.根据权利要求1所述的眼睛植入和输送系统,其特征在于,所述载体包括与近端控制装置的材料进口连通的材料输送内腔。
12.根据权利要求1所述的眼睛植入和输送系统,其特征在于,所述近端控制装置包括与套管连接的远端手柄和具有载体移动致动器的近端手柄,所述近端手柄和远端手柄可相对彼此移动。
13.根据权利要求12所述的眼睛植入和输送系统,其特征在于,所述近端手柄还包括植入体移动致动器。
