本发明涉及一种用于恶性肿瘤治疗的培养自身活化淋巴细胞的培养基组合物。更具体地说,本发明涉及一种培养基组合物,所述培养基内除了含有白细胞介素2(IL-2)以外还含有抗CD3、抗CD16以及抗CD56抗体,可以均匀地增值和活化NK细胞、T细胞和NKT细胞,从而可以培养在多种多样的恶性肿瘤治疗中具有显著功能的免疫细胞。
背景技术
最近在癌治疗中,可以替代现有的手术、放射治疗以及化学疗法的所谓过继免疫疗法(adaptive immunotherapy)的新疗法正引起人们的注目。
过继免疫疗法是一种将从患者的血液中分离而获得自然杀伤细胞(natural killer cell;NK细胞)、树突状细胞(De)、B细胞、T细胞等在治癌过程中最重要的免疫细胞,利用多种刺激剂,扩增出具有高度抗肿瘤活性的免疫细胞,并回输患者体内的方法,该方法由于使用患者自己的血液,与传统的化学疗法等治疗方法相比副作用少、给药简便,因此,最近被广泛研究。
在过继免疫疗法中进行活化的免疫细胞当中,特别是属于淋巴细胞一种的NK细胞具有可以有效地杀伤感染病毒和肿瘤细胞、但不杀伤大部分正常细胞的特征。众所周知,这种NK细胞在初期机体防御机构和人体的肿瘤免疫中起重要的作用。即,即使无由MHC(MajorHistocompatibility Complex)表达而获得的免疫过程,NK细胞也可以杀伤特异性自身细胞、同种细胞以及异种癌细胞,特别是优先杀伤很少表达或不表达Class 1 MHC的靶细胞。为此,NK细胞不仅可以有效的杀伤不表达MHC的大部分癌细胞,还可以杀伤某些病毒感染细胞和如伤寒沙门菌(salmonella typhi)等细菌。
但是,这种具有高度杀伤癌细胞功能的NK细胞只占正常外周血淋巴细胞的5%~15%,特别是在癌患者的情况下,该比例下降到不足1%,因此,如果没有通过过继免疫疗法的另外扩增过程,就无法有效地攻击癌细胞。
因此,为了适用于以癌治疗为目的的过继免疫疗法,需要进行针对癌进展程度大量培养和活化以NK细胞为首的免疫细胞的培养基的研究,但是,用于培养适用于过继免疫疗法的免疫细胞的现有的培养基主要是针对大量扩增免疫细胞中的T细胞。
韩国专利第0735081号(发明名称:活化CD4T细胞的方法)公开了这种现有的免疫细胞培养用培养基。根据上述的活化CD4 T细胞的方法,从血液等生物试样分离CD4 T细胞之后,在含有GM-CSF、IFN-gamma、TNF-alpha、Lectin以及IL-4等细胞因子的培养基中进行培养,并在试管中活化CD4 T细胞,从而获得预防或治疗细菌感染的组合物。
但是,在上述活化CD4 T细胞的方法中所使用的培养基组合物具有以下技术问题;因为该培养基组合物只能选择性地激活免疫细胞中参与获得免疫的T细胞,所以当使用该方法治疗肿瘤时,虽然通过大量被激活的T细胞可以有效地攻击或杀伤已被记忆的癌细胞,但是在攻击未被记忆的各种癌细胞而治疗恶性肿瘤方面,受到了限制。
发明内容
本发明的目的是解决上述现有技术中存在的问题。即,本发明目的是提供一种培养基组合物,该培养基中除了含有白细胞介素2之外,还含有抗CD3、抗CD16、以及抗CD56抗体,能均匀地增殖和活化NK细胞、T细胞以及NKT(Natural Killer T)细胞,从而可以培养在多种多样的恶性肿瘤治疗中具有显著功能的免疫细胞。
作为为了达到上述目的的技术思想的本发明提供自身活化淋巴细胞培养用培养基组合物,该培养基组合物由细胞培养用培养基和添加于所述细胞培养用培养基的添加剂构成,用于大量培养自身活化淋巴细胞,其特征在于,所述添加剂包含白细胞介素-2、抗CD3抗体、抗CD16抗体以及抗CD56抗体。
根据本发明的自身活化淋巴细胞培养用培养基组合物,可以大量激活对不表达MHC的大部分癌细胞具有较强杀伤活性的NK细胞,适用于副作用少、给药简便的过继免疫疗法,为此,具有极大地改善因各种恶性肿瘤而受痛苦的癌患者的预后的效果。

具体实施方式
本发明涉及一种培养基组合物,为了培养来源于人外周血的淋巴细胞而产生在多种多样的恶性肿瘤治疗中具有显著效果的免疫细胞组成比的自身活化淋巴细胞,向培养基内添加白细胞介素2(IL-2)、抗CD3(anti-CD3)、抗CD16(anti-CD16)以及抗CD56(anti-CD56)抗体。在详细说明本发明之前,先探讨各种免疫细胞的特征以及在免疫细胞的扩增以及活化中所使用的上述各种添加物的作用机制。
NK细胞是淋巴细胞中的一类细胞,是具有特征性形态的大颗粒淋巴细胞(LGL,Large granular lymphocytes),其抗肿瘤效果是由细胞坏死(necrosis)或细胞凋亡(apoptosis)或这两种作用机制同时发生而产生。NK细胞在IL-2、IL-12、干扰素(interferon)等细胞因子的存在下,能提高细胞溶解(cytolytic)、分泌(secretory)以及增值(proliferative)等功能。人的NK细胞的表型(phenotype)为CD16(FcrRIII)与CD56,其细胞表面无TRC(T细胞受体复合体,T-cell receptor complex),为此CD16与CD56作为NK细胞的标志。
一般认为NK细胞对癌细胞的作用机制是抗体依赖性细胞介导的细胞毒性(Antibody-dependent cell mediated cytotoxicity;ADCC)。NK细胞表达免疫球蛋白G(IgG)的Fc受体CD16,通过此受体可以进行MHC非限制性杀伤。即,NK细胞的ADCC依赖于识别靶细胞的抗体,当抗体和抗原相结合时会暴露其抗体的Fc区,并且,该暴露出的Fc区与NK细胞上的Fc受体结合形成一个桥,一旦此桥形成,根据受体接合时发生的信号传递,NK细胞就产生细胞损伤物质,引起靶细胞的损伤。
若在抗CD16抗体或抗原抗体复合体的存在下,培养淋巴细胞,则CD16抗原被添加到NK细胞中而引起信号传递,NK细胞受到它们的刺激之后,表达IL-2受体的α链等转铁蛋白(transferrin)受体或产生肿瘤坏死因子(tumor necrosis factor;tnf)或干扰素-γ(IFN-γ)。
CD56抗原(Leu19/NKH1)是一种分子量为180~200kDa的糖蛋白,表达于所有NK细胞以及某些T细胞。CD56抗原与广泛分布在人体神经组织中的NCAM-1(神经细胞黏附分子-1,Neural CellAdhesion Molecule-1)是同一种分子。众所周知,NCAM-1是一种在神经和肌组织中参与细胞之间黏附的分子。由此推测,在NK细胞中表达的CD56也可以起到细胞之间黏附作用。
另一方面,T细胞是指细胞表面具有抗原受体(T cell antigenreceptor;TCR)的细胞。TCR与α链和β链的二聚体CD3抗原形成异二聚体。一部分T细胞(外周血T细胞的5%左右)是由γ链和δ链的二聚体形成,而不是αβ链。TCR与CD3抗原(γ、δ、ε、ζ、ζ或η)形成复合体,若TCR识别抗原,则CD3抗原向细胞内传递其信号。
一般地,促进癌细胞免疫反应的辅助性T细胞分泌多种细胞因子,并活化杀伤T细胞、B细胞以及巨噬细胞等。细胞因子的种类繁多,如细胞和细胞之间的信息传递物质即白细胞介素1、2、3,TNF-α以及干扰素-γ等。根据本发明,在培养液中添加IL-2,活化杀伤T细胞、B细胞、巨噬细胞等免疫细胞。
IL-2是当T细胞识别抗原而被活化时产生的、分子量为14~17kDa的糖蛋白(glycoprotein)。IL-2被分泌到T细胞外之后,与产生IL-2的T细胞自身进行反应,促进该T细胞的增殖。IL-2还可以促进NK细胞的增殖并增强NK细胞的杀伤活性,亦可促进B细胞的增殖。
NKT细胞是固有免疫的成员,是T细胞的一种,其功能近年来逐渐被阐明。从名称可知,它表达T细胞的受体和NK细胞的特异性细胞表面标志(surface marker)。NKT细胞的一种惊人的特征是,它被活化之后,能在短时间内分泌IL-4、IL-10、IL-13、IFN-γ、TNF-α等多种细胞因子。这种特征暗示着NKT细胞对适应性免疫应答产生重大影响。
因此,在本发明中,为了均匀活化从人外周血分离出的NK细胞、T细胞以及NKT细胞,将IL-2和抗CD3、抗CD16以及抗CD-56单克隆抗体作为培养基的添加剂使用。其中,IL-2对T细胞和NK细胞的增殖起作用;各单克隆抗体使免疫细胞表达CD3、CD16、以及CD56而起到抗原的功能。
以下,具体地说明本发明中的自身活化淋巴细胞培养用培养基组合物。
在实施本发明的过程中,首先应该从治疗对象的患者外周血液中分离培养对象的免疫细胞,为此,可以使用以下方法:利用人淋巴细胞或单核球等单核细胞的比重低于1.077的特性,将患者的血液与比重为1.077的Ficoll-Paque Plus溶液(淋巴细胞分离液)相混合,并以一定的离心力进行离心,使之沉淀。根据比重的差异,获得包含淋巴细胞的单核细胞层,并从该单核细胞层单离出淋巴细胞。
根据本发明的自身活化淋巴细胞培养用培养基组合物(以下称为第一培养基组合物)由细胞培养用培养基和添加在细胞培养用培养基中的各种添加剂构成。当以39ml细胞培养用培养基为基准时,该培养基组合物含有作为免疫细胞培养用营养成分的20~500ul 190~210mM的L-谷氨酰胺(L-glutamine)和2~5ml来源于自身的血浆(plasma)、用于活化免疫细胞的20~300ul 17×106~19×106IU/ml的IL-2以及用于调节最终培养完成的免疫细胞(NK细胞,NKT细胞,T细胞)的相对组成比的各2-30ul 0.9~1.1mg/ml的抗CD3抗体、抗CD16抗体以及抗CD56抗体。
在使用如上所述培养基组合物培养淋巴细胞的过程中,为了提高培养效率优选使用继代培养方法,首先使用如上所述培养基组合物培养3~4天,然后将此培养3~4天的淋巴细胞的培养基组合物混入到具有下述组成的培养基组合物中,继续培养2~3天。
当以67ml细胞培养用培养基为基准时,在细胞培养用培养基中添加下述的物质而形成继代培养用的另一个培养基组合物(以下称为第二培养基组合物):作为免疫细胞培养用营养成分的20~500ul190~210mM L-谷氨酰胺(L-glutamine)和2~5ml来源于自身的血浆(plasma)、用于活化免疫细胞的20~300ul 17×106~19×106IU/ml的IL-2以及用于调节最终培养完成的免疫细胞(NK细胞,NKT细胞,T细胞)的相对组成比的各2-30ul 0.9~1.1mg/ml的抗CD3抗体、抗CD16抗体以及抗CD56抗体。
在上述第一和第二培养基组合物中所使用的细胞培养用培养基是在含有氨基酸、维生素、有机化合物和无机化合物以及蛋白质等细胞生长和生存所必需的营养成分的通常培养基内添加800~1200IU/ml IL-2的培养基,可以列举NKB6040(NKBIO,KOREA)。
以下,通过具体的实施例进一步详细说明本发明。但是,下述实施例仅用于例示本发明,本发明的范围不受这些实施例的限定。
实施例1:淋巴细胞的细胞增殖培养
图1为表示根据本发明的培养基组合物进行培养之后的免疫细胞数变化的示意图。
将从正常人、实体肿瘤(solid tumor)患者以及白血病(leukemia)患者的外周血60cc中分离出的各淋巴细胞作为实验对象组,将各淋巴细胞在39ml第一培养基组合物中培养4天,然后把包括培养细胞的第一培养基组合物混入在67ml第二培养基组合物中,继续培养2天。另外,为了诱导细胞的大量增殖,向追加培养的细胞和第二培养基组合物中添加7ml来源于自身的血浆,并注入到含有1L包含10~20IU(International Unit)/ml的IL-2的细胞培养用培养基的透气性培养袋中,培养8天。在这里,各培养过程是在37℃条件下利用CO2培养箱而进行的。
经过上述的培养过程,如图1所示,可以确认包含NK细胞、T细胞以及NKT细胞的淋巴细胞总数,在三种对象淋巴细胞中全部由培养前的2.0×106~4.0×107增殖到2.0×109以上。
实施例2:培养前后表型的观察
图2为根据本发明培养基组合物培养的活化淋巴细胞的表型变化示意图,H1区域表示NK细胞、H4区域表示T细胞、H2区域表示NKT细胞、H3区域表示其它免疫细胞的分布。
从60cc正常人外周血中分离淋巴细胞,经过实施例1的培养过程之后,利用流式细胞仪(floweytometry)分析培养的活化淋巴细胞表面抗原,用图2进行说明其结果,如图(a)所示,培养前表面抗原的分布主要在H4区域,而如图(b)所示,培养后表面抗原的分布主要在H1区域,实际计算CD3阳性(Positive)T细胞和CD16+CD56阳性(positive)NK细胞的比率的结果,由培养前的53.60%∶12.74%变成培养后的39.74%∶69.03%。
从上述结果可以确认,在利用本发明的培养基组合物培养的淋巴细胞中NK细胞的比率明显上升,为了评价像这样的最终培养物的组成变化对癌细胞的杀伤能力的提高的影响,进行了下述的细胞毒性分析。
实施例3:对各种癌细胞的毒性(cytotoxicity)分析
从60cc淋巴肉瘤(lymphosarcoma)患者的外周血中分离淋巴细胞,经过前述的实施例1的培养过程培养活化淋巴细胞,以各种比率混合作为靶细胞(target cell)的癌细胞和作为效应细胞(effector cell)的活化淋巴细胞之后,经过6小时之后,使之与乳酸脱氢酶(lactatedehydrogenase;LDH)进行反应,通过酶联免疫吸附试验(Elisa)分析细胞毒性,其结果如图3所示,特别是在白血病细胞株(leukemiacell line;K562)、人淋巴瘤细胞株(human lymphoma cell line;U937)以及人宫颈癌细胞株(human cervical epidermoid cell line;ME180)中,被活化的自身活化淋巴细胞的癌细胞杀伤能力与外周血单核细胞(PBMC)相比显著提高。
比较例1:与T细胞培养用培养基组合物的癌细胞杀伤能力比较
从60cc淋巴瘤(lymphoma)患者的外周血中分离淋巴细胞,经过实施例1的培养过程,培养出NK/NKT细胞和T细胞的比率为6∶4的活化淋巴细胞(NKM),此外,利用现有的T细胞培养用培养基组合物培养出T细胞的比率为90%以上的活化淋巴细胞(CAT),对于上述两种活化淋巴细胞,根据calcein-Am dying方法,对多种靶细胞的各细胞毒性进行比较分析。在这里,T细胞培养用培养基组合物是根据在日本被广泛利用的现有的CAT(anti CD3Antibody-activity T cells)法而制备的,使用了添加有抗CD3抗体和IL-2等的培养液,并将胃癌细胞株(AGS,NCI-N87)、乳腺癌细胞株(MCF-7,MDA-MB-231)、肝癌细胞株(SK-HEP-1,HepG2)以及白血病细胞株(K562)作为靶细胞。
如图4所示,可以确认在除了AGS之外的大部分癌细胞株,根据本发明的培养基组合物培养的活化淋巴细胞的细胞毒性与根据T细胞培养用培养基组合物培养的活化淋巴细胞相比增加了6~34%。
以上所述仅为本发明的优选实施例而已,并不用于限制本发明,对于本领域的技术人员来说,本发明可以有各种更改和变化。凡在本发明的精神和原则之内,所作的任何修改、等同替换、改进等,均应包含在本发明的保护范围之内。
Claims (6)
1.一种自身活化淋巴细胞培养用培养基组合物,该培养基组合物由细胞培养用培养基和添加于所述细胞培养用培养基的添加剂构成,用于大量培养自身活化淋巴细胞,其特征在于,
所述添加剂包含白细胞介素-2、抗CD3抗体、抗CD16抗体以及抗CD56抗体。
2.如权利要求1所述的自身活化淋巴细胞培养用培养基组合物,其特征在于,
所述细胞培养用培养基含有800~1200IU/ml白细胞介素-2。
3.如权利要求1所述的自身活化淋巴细胞培养用培养基组合物,其特征在于,
若以39ml所述细胞培养用培养基为基准时,添加于所述细胞培养用培养基的添加剂包括20~300ul 17×106~19×106IU/ml的白细胞介素-2、2~30ul 0.9~1.1mg/ml的抗CD3抗体、2~30ul 0.9~1.1mg/ml的抗CD16抗体以及2~30ul 0.9~1.1mg/ml的抗CD56抗体。
4.如权利要求3所述的自身活化淋巴细胞培养用培养基组合物,其特征在于,
若以39ml所述细胞培养用培养基为基准时,所述添加剂还包括20~500ul 190~210mM L-谷氨酰胺和2~5ml来源于自身的血浆。
5.如权利要求1所述的自身活化淋巴细胞培养用培养基组合物,其特征在于,
若以67ml所述细胞培养用培养基为基准时,添加于所述细胞培养用培养基的添加剂包括20~300ul 17×106~19×106IU/ml的白细胞介素-2、2~30ul 0.9~1.1mg/ml的抗CD3抗体、2~30ul 0.9~1.1mg/ml的抗CD16抗体以及2~30ul 0.9~1.1mg/ml的抗CD56抗体。
6.如权利要求5所述的自身活化淋巴细胞培养用培养基组合物,其特征在于,
若以67ml所述细胞培养用培养基为基准时,所述添加剂还包括20~500ul 190~210mM的L-谷氨酰胺和2~5ml来源于自身的血浆。
