背景技术
射频(RF)装置用于非特定地和非选择性地消融或加热不同种类的组织。例如,在皮肤学领域中利用RF设备来处理老化皮肤。皮肤老化与皮肤上层中的变化相关,例如由于角质层和表皮中的变化以及表皮中的不均匀色素沉着导致的皮肤粗糙。在真皮中,老化和环境因素导致胶原纤维和弹性蛋白纤维损坏和发生障碍,从而导致形成皱纹。表皮中皮肤老化的症状通常通过诸如化学去皮或激光表面修整的消融方法来处理。诸如激光的光辐射装置用于对大面积的皮肤进行表面修整。虽然这些激光在皮肤老化症状的处理中是有效的,但是整个表皮的表面修整通常与副作用相关联,诸如伤口感染、治愈时间延长、色素沉着过多、色素沉着减少以及形成疤痕。

射频(RF)装置用于消融局部的感染皮肤或用于破坏皮肤的整个上层。但是,整个的皮肤表面修整方法和装置导致灼伤,像与治愈时间延长、增加感染、长时期红斑、形成疤痕、色素沉着过多和色素沉着减少的危险相关联的后处理反应。
US6,711,435公开了用于消融对象的角质层表皮的装置,该装置包括多个电极,其在相应部位处施加到对象的皮肤。但是,该装置不消融表皮,因此对皮肤老化症状没有任何作用。

其它装置利用光能和单电极RF能的结合来处理皮肤。
前述的装置缺乏控制空间方向、影响处理区域的能量以及电能性质的能力,并且因此缺乏在其相应治疗适应症(indication)中的最大效力所需的选择性和特异性。此外,单电极和单极RF设备缺乏处理表皮中老化症状的能力。控制电子在处理的生物组织中的空间方向和流动模式的增强能力将允许对另外的皮肤病和非皮肤病进行有效处理,诸如去除毛发、痤疮、痤疮疤痕、牛皮癣、骨移植以及更多。

发明内容

在一个实施例中,本发明公开了一种用于将能量传递到患者目标部位的方法。该方法包括将电外科探针放入目标部位的紧密邻近处中,并且将相位控制的RF能量传递到电外科探针。

应该注意在该说明书和所附权利要求中使用的单数形式“一”、“一个”以及“该”包括复数对象,除非在上下文中另外明确指出。这样例如“功率源”不仅指代单一功率源而且指示两个或更多个功率源的组合,“电极”指代电极组合以及单一电极等。

在不希望受到理论的限制的情况下,相信在此公开的相位控制的RF设备在目标部位产生不同和可调节的电场。电场能够操纵目标部位内的电子,由此产生温度升高的选择区域。
在此公开的电外科探针应用设置到处理表面上并适于施加到目标生物组织的多个电极。所述电极可以是任何合适的尺寸或形状,并且应该意识到例如上述依赖于预期用途而改变。处理表面可适于处理各种生物组织表面。因此,处理表面可以是平坦的或弯曲的。所述电极可跨过整个处理表面均匀设置,或者可以聚集在处理表面的特定部分中。具体的,通过将电极分布在处理表面上将形成规则的图案。例如,电极间的间隔可取决于探针的几何形状和电极尺寸。通常,任何两个相邻电极中心间的间隔将在电极直径的大约110%到大约1000%之间,或者对于非圆形的电极而言,该间隔将在电极的最大宽度的大约110%到大约1000%之间。例如,对于人类皮肤处理而言,相邻电极间的中心到中心的距离可在大约0.001毫米到大约100毫米之间或者在大约0.01毫米到大约25毫米之间。在一个实施例中,相邻电极平均间隔开大约0.01毫米到大约0.1毫米。
在图2a中示出处理表面上的电极配置的一个实例。具有圆形截面的电极以规则图案设置在平坦的处理表面上。电极可与处理表面平齐,或者电极可从处理表面突出。
电外科探针包括至少三个电极,并且可包括大于3的任何数目的电极,诸如4,5,6,7,8,9,10,15,20,50,100或者更多。例如,在图2a中探针包括二十八个电极。
电极电耦合到能够提供多个功率输出的发生器。功率发生器可包括多个RF源。发生器还可包括单一的RF源,在该情况下,发生器还包括合适的电路,以便将RF源的输出分离成多个RF信号。发生器还包括用于控制任何两个功率输出之间的相位的装置。这种用于控制的装置将通常由相位偏移电路等构成,如由本领域内的任何普通技术人员意识到的那样。
在公开于此的电外科设备中的至少两个RF源之间的相位角是可以调节的,但是应该意识到电外科设备的构造是可以变化的。在一个实施例中,功率发生器包括两个RF源和相位偏移电路以便调节两个RF源的RF输出之间的相位角。在另一个实施例中,功率发生器包括第一、第二和第三RF源。在该实施例的一个实例中,每一RF源的相位是可以调节的,这样第一和第二RF源之间的相位角、第二和第三RF源之间的相位角以及第一和第三RF源之间的相位角可独立变化。在该实施例的另一实例中,第一RF源具有固定的输出,以及第二和第三RF源的相位是可以调节的。该构造还允许调节任何两个RF源之间的相位角。在该实施例的另一实例中,第一和第二RF源具有固定输出,并且第三RF源的相位是可以调节的。该构造允许调节第一和第三RF源之间的相位角以及第二和第三RF源之间的相位角。如在此所述的,RF源之间的相位角调节可通过响应于所测量的电参数(例如,在目标部位处的阻抗等)的反馈回路自动完成,或者可通过调节控制手动完成。
电外科探针可以是一次性的,这样在其制造后进行消毒,并预期单次使用。可选的,电外科探针可进行消毒(例如经受压热器作用),这样其适于多次使用,具体地说是供多个患者使用。
在一个实施例中,提供电外科设备,其包括用于将光能施加到处理部位的装置。这种用于施加光能的装置包括相干源和非相干源,并且可包括诸如激光器、紫外灯、红外灯、白炽灯和荧光灯、发光二极管等的源。用于施加光的装置可附着到电外科探针或可与电外科探针分离。
在另一个实施例中,电外科设备可包括用于降低目标部位的温度的装置。这种装置包括电冷却设备,诸如用于将冷却液体或气体传递到目标部位和周围组织的散热器(heat sink)和传递端口。例如,电接触冷却允许冷却目标部位的部分,诸如表皮,由此降低对周围(也就是,病变周围)皮肤的疼痛和热损伤。

在另一实施例中,该设备的电外科探针的处理部分(例如头部或尖端)包括允许电外科探针的所有部分或一部分在使用中机械振动的机构。这种振动允许处理部位受到按摩或其他抚慰。当如在此所述的该设备用于处理脂肪团时该特征是尤其优选的。
电外科设备可包括用于测量电特性的装置,并且可任选地包括允许电外科设备响应于所测量的电特性调节供应的电能的反馈回路。所述电特性包括目标部位的电阻抗和/或导纳,在电极间流动的电流,电极间的电势,RF源的输出电压和相位,RF源之间的相位差异。所述测量可以当电外科探针紧密邻近目标部位时实时进行,允许反馈回路调节由电外科设备供应的能量以便获得所需结果。
在一个实施例中,电外科设备适于处理皮肤。该设备产生电场,其使得电流流动通过角质层、表皮和/或真皮,并且包括用于响应于所测量的电特性变化降低或增加角质层中消散功率的装置。所述电特性可选自于:电流的量值;电流的时间积分;电流的第一时间导数;以及电流的第二时间导数。应该意识到当皮肤不是目标部位时,这些电特性可在角质层外的生物组织中测量。
电极的特性可由合适的电路独立测量和监测。此外,RF功率源可适于改变由电极产生的电场,以便降低通过一个或多个电极的电流,该电流基本独立于通过任何其它电极的电流。
公开于此的电外科设备在用于将能量传递到患者目标部位的方法中是有用的。适用于利用公开于此的设备应用电能的目标部位包括生物组织,诸如皮肤、黏膜、器官、血管等。能量经由电外科探针传递到目标部位,该探针紧密邻近目标部位放置。至于“紧密邻近”意味着探针足够紧密地放置到目标部位以便具有希望的效果(例如,组织消融,加热目标部位等)。在一个实施例中,电外科探针与目标部位接触放置。
由于电外科探针紧密邻近目标部位,跨过存在于电外科探针上的两个或更多个(通常为三个或四个或更多个)电极施加RF电势。在一些情况下,该电势可导致电流在目标部位内和电极间流动。另外或可选的,电势导致电场施加到目标部位。通过应用多个RF源和至少三个电极,电场的特性(例如,强度、方向等)可通过控制RF源之间的相位角()来操纵。由电外科探针产生的电场(F)与RF源间的相位和每一RF源的其它电参数成比例。该电场的极性可根据RF源变化。这些变化将吸引自由电子并且最终移动自由电子,从而加热至少一部分目标组织。在该设备的另一实施例中,这些自由电子将趋于在处理区域中的更为加热的路径上流动,该处理区域利用光、闪光(flash)或激光束建立,如在此公开的那样。
在一个实施例中,目标部位是皮肤,并且电外科设备紧密邻近皮肤表面放置,以便产生电场,该电场导致电流流动通过角质层、表皮和真皮。引起的电流可在电极间流动,但是也可具有在垂直于皮肤表面的方向上的明显分量(例如,10%,25%,35%,50%,75%或更多)。通过在皮肤中产生电流,公开于此的设备能够增加皮肤温度,并且在一些情况下,可消融皮肤的一层或多层。例如,所述设备在全部或部分消融皮肤表面中是有用的。所述设备在部分或全部消融皮肤表面之下的一层或多层中也是有用的。
在一个实施例中,如上所述,可利用电外科设备来非均匀地升高生物组织的温度。在另一个实施例中,可利用电外科设备来升高相对于应用的电外科探针尺寸的狭窄区域内的生物组织的温度。
在一个实施例中,公开的电外科设备可适于在目标部位处产生一个或多个聚焦损伤区域(focal damage region)。聚焦损伤区域是在其中发生组织坏疽的目标部位内的分离区域。聚焦损伤区域的尺寸、位置、数目、相对布置以及其它因素通过电外科设备的物理和电参数以及设备操作中的操作条件来确定。虽然聚焦损伤区域可在公开于此的任何目标部位处产生,但是属于该实施例的剩余论述将主要利用人皮肤作为示例性但非限制性的实例。图3示出在皮肤组织中产生的多个聚焦损伤区域9的实例。
在不希望受到理论的限制的情况下,相信由于RF源之间的相位角的可调节性,公开于此的电外科设备能够产生聚焦损伤区域。RF源电耦合到电外科探针上的电极;RF源之间的相位角调节导致在电极附近处产生的电场的变化。所述变化包括电场强度中的强度区域和弱度区域,并且可用于操纵目标部位内的电子。这样在本发明中利用RF源之间的相位的合适调制和调节来在目标部位中产生非均匀的电流。所述电流产生温度升高的区域,并且能够在聚焦损伤区域中产生组织坏疽。因此,目标部位的温度与连接到电极的RF源的相位成比例。
聚焦损伤区域的尺寸可根据需要以及按照预期用途合适地进行改变。例如,在处理人皮肤时,聚焦损伤区域可基本是柱状的并且垂直于被处理的皮肤表面。所述柱可在皮肤表面处或之下开始,并延伸到表面之下的某一深度。因此,这些柱具有近端和远端,其中近端在皮肤表面处或最靠近皮肤表面,并且远端离皮肤表面最远。当不在皮肤表面处时,所述柱的近端可位于皮肤表面之下的0.1,1,2,3,4,5,10,25或50微米的距离处。所述柱的远端可位于皮肤表面之下的1,5,10,25,50,100,1000,2000或4000微米的距离处。所述柱的宽度(也就是直径)也可变化,并且可在大约1微米和大约7000微米之间,或可在大约10微米到大约4000微米之间。例如,所述柱的宽度可至少为1,10,20,30,40,50,100,150,200,250,500,800,1000,2000或5000微米。在一个实施例中,聚焦损伤区域可具有大约50-100微米或大约50-70微米范围内的宽度。在聚焦损伤区域内的组织损伤可在皮肤的上层中被隔离,诸如在角质层中,或可限定到位于角质层下的皮肤细胞。组织损伤还可延伸跨过皮肤的多个层。因此由公开于此的电外科设备产生的聚焦损伤区域可延伸通过角质层并进入表皮和真皮的下面层内。聚焦损伤区域还可限定到角质层之下的表皮和真皮层。聚焦损伤区域可限定到角质层。聚焦损伤区域还可限定到角质层和表皮。聚焦损伤区域还可限定到角质层、表皮和真皮。通常,聚焦损伤区域的深度可由设备操作员选择。
应该意识到聚焦损伤区域可具有除了柱状之外的其它形状,所述的其它形状包括锥形、蛋形或球形。此外,聚焦损伤区域的截面(也就是,平行于皮肤表面所取的截面)可具有任何形状,包括规则形状,诸如圆形、正方形、椭圆形、三角形、多边形,以及不规则形状。
将组织坏疽限制到聚焦损伤区域允许精密控制受损的组织总面积。通过调节聚焦损伤区域的强度和物理尺寸(其通过调节电极间的相位关系、传递到电极的RF功率以及公开于此的其它因素完成),可以控制受损皮肤的量。例如,利用公开于此的方法,导致处理区域中的至少大约1%,5%,10%,15%,20%,25%,50%或75%的组织受损。
聚焦损伤区域的另一特性是密度-也就是,在目标部位处的每单位组织面积上产生的聚焦损伤区域的数目。通常的密度是至少大约10,100,200,500,1000,2000或3000每平方厘米。在一个实施例中,聚焦损伤区域的密度在大约100-3000每平方厘米的范围内。由于聚焦损伤区域可完全位于目标部位处的组织表面之下,聚焦损伤区域的密度还可指的是目标部位处的单位面积组织切片中的区域数目。最便利的是,所述组织切片将平行于目标部位处的组织表面。再次在不希望受到理论的限制的情况下,聚焦损伤区域的密度是电极数目和密度、施加到电极的RF能量的相位关系、操作条件以及由本领域技术人员意识到的其它因素的函数。
此外,聚焦损伤区域可以一定的图案在目标区域中产生。由于具有聚焦损伤区域密度,目标部位处的聚焦损伤区域的取向是电极数目和密度、施加到电极的RF能量的相位关系、操作条件以及由本领域技术人员意识到的其它因素的函数。
产生每一聚焦损伤区域所需的能量的量将随操作条件、生物组织的类型、损伤区域的尺寸以及其它因素而改变。在一个实例中,被传递以便产生每一损伤区域的能量的量是大约1mJ*cm-3。
应该意识到聚焦损伤区域的物理尺寸、密度、总数以及分布图案可依赖于预期应用而改变。电极的数目和布置、施加到电极的RF能量的相位以及其它因素基于所需的处理效果来进行选择。
图4示出代表聚焦损伤区域的图,所述区域具有位于由处理探针10处理的组织表面之下的近端。聚焦损伤区域12的近端和组织表面之间的组织区域11与聚焦损伤区域中的组织相比保持在更低的温度下。位于聚焦损伤区域12之间的组织区域13也比聚焦损伤区域中的组织温度更低。图5示出代表从组织表面向下延伸(也就是,进入组织更深)的聚焦损伤区域12的图。
在将电能施加到皮肤的过程中,电外科探针可平行于皮肤平移(也就是移动)。这种平移可在探针与皮肤接触时或紧密邻近皮肤时发生。探针平移允许处理区域扩大、改善热消散以及本领域技术人员可意识到的其它益处。RF源还可利用标准控制电路被编程和控制,以便将RF能量以依赖时间的方式施加到电极,这样可基于电外科探针平移的速度和方向产生聚焦损伤区域的特定图案。
为了处理病变、疤痕、色素沉着区域等,例如可利用通过数字成像技术所获取并传送到与电外科设备集成在一起的控制单元的病变图像来预先确定聚焦损伤区域的图案。例如,在用于处理患者痤疮的方法中,可对痤疮进行照相并且对电外科设备进行合适的预编程以便对具有具体病变或疙瘩的病变处进行消融。其它实例包括仅仅在鳞皮病病变处或其附近或仅仅在皮肤刺纹的区域中产生聚焦损伤区域。在另一实例中,利用该设备来处理在脸上具有色素沉着过多区域的黄褐斑(melasma)的患者。电外科设备可被编程,以便消融具有低消融深度的整个脸。可选的,具有更为过多色素沉着特征的脸区域可用较高密度的聚焦损伤区域进行处理,而具有较少的过多色素沉着特征的脸区域可用较低密度的聚焦损伤区域进行处理。
在聚焦损伤区域内的组织可被全部或部分消融或损伤。聚焦损伤区域之间的组织区域将通常由于来自电极消散的热量进行加热,虽然这些区域将通常不被消融或永久损伤。
在一些实施例中,利用如在此所述的聚焦损伤区域的皮肤状况处理由于对聚焦损伤区域周围组织的损伤最小化而具有使得治愈时间最小化的优势。
另外或可选的,为了产生聚焦损伤区域,通过在此公开的电外科设备施加的电能可用于加热但不是破坏和/或损伤目标部位。例如,当目标部位是皮肤时,可施加热量,以便使得用于处理皱纹的方法中有效改造胶原质。
公开于此的相位控制的RF设备和方法可结合其它能量源。在一些实施例中,利用另外形式的能量允许协助用于处理诸如皮肤疾病、皮肤老化和去除毛发的效果。例如,聚焦的超声能量可导致易受影响的活组织中的微振动。由超声导致的微振动随不同类型的组织(例如,皮肤;角化细胞或表皮细胞,诸如发干的硬角蛋白等)而不同。由于聚焦的超声能量可使得活组织的物理性能不同(例如,对电外科过程中未经处理的组织与处理过的组织的物理性能不同,皮下脂肪细胞与连接性组织细胞的物理性能不同等),其可扩大相位控制的RF能量的选择性。在公开于此的方法和设备的一个实施例中,利用相位控制的RF和超声能量来处理组织。适用于将相位控制的RF和超声能量结合的实例包括去除头发以及脂肪团毛发的处理(也就是,比已经存在的方法更安全和更有效的毛发去除或处理)。
公开于此的方法还可包括预处理的步骤,诸如:用局部麻醉处理;冷却;以及用光能进行处理。按所需在用电外科设备处理之前可施加30-60分钟的诸如利多卡因等的局部麻醉。作为预处理步骤的目标部位冷却可包括应用诸如凝胶、液体或气体的冷却剂。实例包括水和盐溶液、液氮、二氧化碳、空气等。冷却还可包括电接触冷却。通常恰好在用电外科探针处理前完成目标部位的冷却,并且具有降低目标部位周围组织的疼痛和不希望的热损伤的效果。如在此所述的,用光能的处理可利用与电外科探针集成在一起的光源或用独立的光源来完成。光能能够导致光散热,并且有利于选择性地加热目标区域的区域。因此,可结合光能来使用电外科设备。例如,可选择性地加热更暗的着色区域,诸如以存在相对大量黑色素为特征的毛发和皮肤(例如,痣,色素沉着过多病变等),原因在于所述区域与具有较少色素沉着区域相比将吸收更多的光能。光能还可用于产生优选的适于由公开于此的电外科探针产生的电流的传导路径。利用光能以及在此公开的电外科设备的处理方法特别适于处理色素沉着过多的病变、黄褐斑、痣、皱纹以及痤疮疤痕以及去除毛发和去除脉管病变。
在用公开于此的电外科设备处理目标部位之后,还可进行特定的后处理步骤。所述后处理步骤包括用如上所述的局部麻醉处理以及如上所述的冷却目标部位和周围组织。
公开于此的电外科方法和设备还可结合用于将诸如光和/或超声能量施加到目标部位的附加装置。例如,电外科探针可包括光学光源(例如,激光器、白炽灯、气体放电灯等)、高频超声源、红外光源、紫外光源、或者其任何组合。这种用于施加能量的附加装置可电耦合到相同功率源,所述功率源将功率提供到电外科探针的电极,或者可电耦合到单独的功率源。
在此公开的方法和设备通常在电外科领域内是有用的,并且更具体的在适于利用RF能量进行处理的步骤中是有用的。例如,公开于此的方法和设备可在有利于处理影响患者皮肤和皮下组织的医学和美学的疾病和状况的步骤中应用,所述步骤包括下述:皮肤表面修整步骤;减轻色素沉着表现或去除色素沉着;减轻脂肪团、去除脂肪团或另外处理脂肪团;处理或去除皱纹、脉管病变、疤痕和纹身;头发去除和头发移植步骤;皮肤癌的处理;痤疮和牛皮癣的处理;去除慢性皮肤溃疡;以及眼睑整容步骤。
在此公开的所述方法和设备还有利于处理皮肤老化症状,包括处理粗糙皮肤、不均匀的色素沉着、皱纹以及扩张的毛细管。
适于在此公开的设备和方法的其它应用,并且具体是在目标部位中的聚焦损伤区域的产生包括去除老化或生病的皮肤,由此允许通过不消融周围区域的皮肤来快速再生。
许多处理条件和方法利用在此公开的设备以及利用其能力来选择性地加热待处理的组织表面之下的组织。例如,在此公开的设备在处理皱纹和其它老化症状的方法中是有用的。加热皮肤表面之下的胶原质导致胶原质分子重新取向于分子水平,由此消除或减少皱纹存在。利用相位控制的RF允许选择性地加热胶原质区域而不导致加热或损伤周围区域。
在另一实例中,在此公开的设备在去除毛发的方法和毛发移植的方法中是有用的。利用相位控制的RF可产生特定和选择性加热毛发和毛囊的电场,特别是当如图6中所示的毛发和毛囊位于电极间时。此外,通过利用除了电外科探针之外的光能可有益于毛发处理(除了使用的其它方法之外),如在此所述和在图7中所示。图8还示出其中在处理皮肤和毛发或毛囊的过程中将光能与定相的RF能结合的效果。在图8中,来自光源的热量积累在富含黑色素的毛发和毛囊(区域1)中,从RF能量补充热量。这增强了选择性,从而选择性加热和/或损伤毛发,从而减少对周围相对黑色素含量少的区域(区域2)的加热和/或损伤。

实例1
电外科设备的实例构造
在图9a,9b和9c中示出相位控制的RF设备的三个实例构造。在图9a中,示出仅仅具有四个电极的相位控制的RF(PCRF)设备。所述电极被连接成两对,标示为RF1和RF2。利用两个独立的RF发生器分别将电压V1和V2施加到RF1和RF2。在图10中示出V1和V2的实例。V1和V2之间的电压关系由等式(1)描述:
允许按如下假设等式(2)-(5):
(2) V1=V0sin(2πft+θ)
(3) V2=V’0sin(2πft+ψ)
(4) V0=V’0

V01是用于操纵目标部位中的电子的等价内电势。由也在图9a中示出的RF相位控制单元来控制RF发生器之间的相位。例如每一RF发生器能够以1MHz的频率传递高达500瓦特到50欧姆的典型负载,但是应该意识到所述值可适当地更大或更小。每一RF发生器具有不同的输入,以便控制:(1)所述单元的功率输出;(2)RF信号的相位;(3)脉冲持续时间;以及(4)开/关功能。可以多种方式来实施图9a中所示的设备,并且应该意识到所述单元的效率(也就是,输出功率和输入功率的比值)将相应地改变。在一个实例中使用D类类型的RF发生器以提供具有高效率(~90%)的设备。图11示出典型D类类型的RF发生器。
RF相位控制单元是设定每一RF发生器单元的相位的低功率单元。该单元实施简单的方波发生器,其中该方波的相位利用电阻器-电容器(RC)电路或以数字方式通过设定方波发生器芯片的合适寄存器来设定。这种半导体芯片的实例是由数字555或556(双555)命名的芯片或是任何普通的锁相环(PLL)芯片。RF相位控制单元的另一功能是控制RF发生器单元(RFGU)的功率输出,其通过控制供应到RFGU的输入电压来实现。RFGU的输出功率与所述输入电压成比例(见图12)。在AC电源单元上设定所述输入电压。
在图9a中所示的设备中,由AC电源单元供应的电压由微控制器单元控制。此外,诸如开/关,脉冲持续时间等的函数在微控制单元上限定并传送到相位控制单元(PCU),其又控制RFGU。
图9a中的设备还包括电冷却单元(ECU)。在一个实施例中,该单元是受控的功率源,其产生DC电流以便驱动一个或多个电-热式TEK冷却设备。例如,对于在该设备的两侧之间能够产生30℃增量(del ta)的冷却设备而言,在35℃的环境温度下电极可保持在5℃。这可通过将TEK的一侧保持在环境温度而完成,例如利用图9a中所示的风扇单元,其将环境气流供应到电极。由AC电源单元提供到风扇单元的输入功率。例如AC电源单元可供应与电极温度成比例的电压。这允许优化噪声和功率,这样在目标组织处获得所需温度。应该意识到在公开于此的设备中可利用冷却的替换方法(包括多种散热器液体冷却方法等)。
AC电源单元可为设备的各种部件供应所需的电压和电流。适于AC电源单元的输入电压范围优选在标准范围内,也就是AC95V-230V。输出电压和电流可根据部件的需要进行改变,并且可包括:在5A下的+3.3V,在2A下的+12V,以及在8A下的+0V到60V,并且所述电压和电流被供应到每一部件和/或多个RF发生器的每一个。
控制界面单元允许由使用者控制设备,诸如医学执业者(对于预期用于医院用途的设备)或个人(对于预期个人使用的设备,诸如毛发去除设备),并且可包括计算机界面、数字控制和显示器,模拟刻度盘和控制等。所述控制包括例如电源开/关、菜单设定的控制、紧急电源关闭按钮、电源入口(Power inlet)等。
在图9b中示出另一PCRF设备。该设备类似于图9a中所示的设备,除了使用单一的RF发生器之外,其输出供应到多个相位偏移模块,以便在电极处产生不同的电相位。在希望或需要相对多个不同相位的情况下,由图9b中的设备示例的方法与图9a中的设备示例的方法相比可能相对更经济。
在图9c中,示出另一PCRF设备(特别适于其中需要低功率的情况)。具有VFBFGA(非常细小间距的球栅阵列)类型电路的定制封装的半导体芯片可用于控制电极、RF发生器、RF相位控制单元、微控制器单元、风扇控制单元以及控制界面单元。该系统包括电源单元、定制半导体芯片和用于人界面的控制单元上的相对少的按钮。在示出的实例中,适当地,控制单元和电源单元经由最小数目的线电附着到半导体芯片。
实例2
电外科设备的实例构造
在图13中示出相位控制的RF设备的实例构造。聚焦超声发射器与供应有相位控制的RF能量的两对电极耦合(在示出的实例中,每一侧上有两个)。在处理活组织(例如,去除毛发)过程中同时发射定相的RF和超声能量。所施加的能量和目标组织的物理性导致发干的过热,以及与周围皮肤相比形成隆起。这允许选择性的色素和/或毛发去除系统。
实例3
利用定相的RF能量的毛发模型的处理
利用尼龙钓鱼线作为毛发的模拟物(也就是,模型)。尼龙线类似于毛发,原因在于其具有小的直径,不吸水,并且是非导电性的。尝试的其它模拟物是棉线和金属针。
利用的组织模型是鸡胸。当植入在模型中时,尼龙线保持不导电的电性能。利用具有100微米的分辨率的红外相机来获取模型的实时图像。
在相位控制模式中利用包括四个电极的电外科探针。将结果与从相同电外科探针以规则的两电极双极模式(也就是,只有两个电极被加电,并且RF能量不是相位控制的)运行获得的结果相比。在规则的双极两电极设置中,电外科设备没有显示选择性加热毛发模型。尼龙丝线保持比周围组织温度更低。在相位控制模式中,电外科设备显示选择性加热毛发模型。发现尼龙丝线的温度比周围组织的温度高20-40℃。
Claims (13)
1.一种电外科系统,包括:
(a)包括处理表面和多个电极的电外科探针,用于将RF能量施加到患者的外部目标部位;以及
(b)发生器,其包括:
(i)用于传递RF能量的第一和第二RF功率源或用于传递RF能量的包括第一和第二RF输出的RF功率源;以及
(ii)用于控制由第一和第二RF功率源传递的RF能量之间的相位或由第一和第二RF输出传递的RF能量之间的相位的装置,
其中所述外部目标部位是组织并且
包括处理表面和多个电极的所述电外科探针在目标部位处产生非均匀的电流以通过借助第一和第二RF功率源电耦合到电外科探针的电极调节由第一RF功率源传递的RF能量和由第二RF功率源传递的RF能量之间的相位角或者通过借助第一和第二RF功率源电耦合到电外科探针的电极调节由第一RF输出传递的RF能量和由第二RF输出传递的RF能量之间的相位角来非均匀地提高在目标部位处的组织的温度。
2.根据权利要求1所述的电外科系统,还包括用于将光能施加到目标的装置,用于将超声能施加到目标的装置,用于将调节温度的液体或气体施加到目标的装置,用于响应于所测量的参数调节由一个或多个RF功率源传递的RF能量的装置,或其任何组合。
3.根据权利要求2所述的电外科系统,其中所测量的参数是电阻抗、在第一和第二RF功率源的一个或多个电极间或者在与第一和第二RF输出相连的一个或多个电极间流动的电流和/或RF功率源的输出电压和相位。
4.根据权利要求1所述的电外科系统,其中电外科探针是单次使用的电外科探针。
5.根据权利要求1所述的电外科系统,其中电外科系统适于对皮肤进行表面修整,从皮肤去除色素沉着、毛发、皱纹、疤痕、纹身或病变,更新皮肤,处理脂肪团,处理痤疮、牛皮癣或癌症,去除慢性皮肤溃疡,毛发移植程序,或眼睑整容程序。
6.根据权利要求1所述的电外科系统,其中所述电极的截面基本是圆形的、椭圆形的或正方形的,并且其中所述电极设置在处理表面上,这样在任何两个电极中心之间存在至少在0.01毫米到25毫米之间的距离。
7.根据权利要求1所述的电外科系统,其中所述RF能量导致所述组织的柱内的温度升高,所述组织的柱基本垂直于组织表面。
8.根据权利要求1所述的电外科系统,其中所述目标部位是组织,用于施加RF能量的装置被配置成使得其在目标部位产生非均匀的电场,并且其中所述非均匀的电场导致非均匀地加热所述组织。
9.一种用于处理活组织的电外科系统,所述电外科系统配置成将相位控制的RF能量传递到活组织,所述电外科系统包括:
(a)用于传递RF能量的多个RF功率源或者用于传递RF能量的包括多个RF输出的RF功率源;以及
(b)电外科探针,其包括处理表面和电耦合到所述多个RF功率源或RF输出的多个电极,用于将相位控制的RF能量施加到活组织上的外部目标部位,
其中用于施加相位控制的RF能量的电外科探针在外部目标部位处产生非均匀的电流以通过借助RF功率源电耦合到电外科探针的电极调节由所述多个RF功率源传递的RF能量之间的相位角或者通过借助RF功率源电耦合到电外科探针的电极调节由所述多个RF输出传递的RF能量之间的相位角来非均匀地提高在所述外部目标部位处的所述活组织的温度。
10.根据权利要求9所述的电外科系统,其中电外科系统还包括有效控制RF功率源或RF输出的相位的控制单元。
11.根据权利要求9所述的电外科系统,其中所述相位控制的RF能量在活组织中产生电流。
12.根据权利要求11所述的电外科系统,还包括用于响应于活组织中的电流的测量的特性调节活组织中的电流的装置。
13.根据权利要求12所述的电外科系统,其中所述测量的特性选自于量值,时间积分,相对于时间的第一导数,以及相对于时间的第二导数。
