Description
本願は、米国特許法第119条(e)項に基づき、2010年4月22日に出願した米国特許仮出願第61/326667号及び2010年2月23日に出願した米国特許仮出願第61/307004号の利益を主張し、2009年9月30日に出願したイスラエル特許出願第201246号および2009年7月27日に出願したイスラエル特許出願第200081号からの優先権を主張する。上述の出願の開示内容は参照として本明細書中に組み入れられる。
本発明は、その一部の実施形態では、外科的方法および装置に関し、さらに詳しくは、組織の正確な焼灼のための方法および装置に関するが、それに限定されない。

本発明の一部の実施形態では、前進および後退はコイルおよび磁石によって実行される。
金属の熱容量に基づいて、約400℃の直径300ミクロンの金属ロッドから組織10に約30ミリジュールを蓄熱送達するために、典型的には約300ミクロンから5000ミクロンの範囲の長さLのロッドの遠位部からエネルギーが流れなければならないことが分かる。しかし、ステンレス鋼のような多くの金属の熱伝導率は低すぎるので、上述した断面を有する上述した距離から組織に約100℃で約5ミリ秒間必要なエネルギー(30ミリジュール)を流すことができない。
式中Hνは1立方センチメートルの組織の気化エネルギーである(約3000ジュール/cm3)。
熱流量は時間t内のエネルギーEの組織内への流れを可能にし、E=W*tである。
式中、C=熱容量、ρ=気化ロッドの材料密度である。
式中、Bは組織の熱拡散係数である。
方程式7は、気化ロッドの長さの下限を決定する。
方程式9は気化ロッドの長さLの上限を設定する。
実施例1に記載した実施形態と一般的に同様の部分的皮膚再建術ユニットは、毎秒2処置部位ないし3秒当たり1処置部位、またはより遅い例示的速度で動作する。毎秒2処置部位の処置速度は一般的に高速とみなされる。本発明に係る装置の処置速度の1つの制限は、気化ロッドが気化直後に組織から上昇した後、熱が気化ロッドの遠位端に拡散するのに要する時間である。
気化ロッドは、カテーテルを介して張り出した椎間板内に挿入され、椎間板で穴を気化させる。
Claims (12)
- 組織に穴を形成するように組織を気化させるための装置であって、
気化要素と、
気化要素を200℃から600℃までの範囲の温度に加熱するように構成された加熱要素と、
穴からの予め定められた付帯的損傷距離を越える距離で組織の熱緩和時間未満の短い時間内に、気化要素を組織の特定の深さまで前進させかつ気化要素を組織から後退させるように構成された機構と
を含み、
付帯的損傷距離が、組織の熱拡散係数に、気化要素が組織に熱を提供する短い時間の平方根を掛けたものに基づく、装置。 - 気化要素が組織中に前進されている間、組織内にある間、及び組織から後退されている間、加熱要素が、気化要素を加熱しないように構成される、請求項1に記載の装置。
- 組織と接触して配置されるように構成された保護板をさらに含み、気化要素は特定の深さを、保護板を越えて組織内に延長するように構成される、請求項1または2に記載の装置。
- 装置内の移動要素に連結されたばねをさらに含み、移動要素は気化要素を含み、移動要素は調和振動子を含み、調和振動子は10ミリ秒から100ミリ秒の範囲の、移動要素の質量およびばねのばね定数に基づく振動周期を有する、請求項1〜3のいずれかに記載の装置。
- 気化要素の前進行程を制限するように構成されたスペーサと、機構が複数の気化要素を短時間内に前進および後退させるように構成され、かつスペーサが複数の穴を含み、そこを介して複数の気化要素を組織内に送り込むことができるようにした、複数の気化要素とをさらに含む、請求項1〜4のいずれかに記載の装置。
- 特定の深さは、以下のものからなる組織の群から選択される組織に穴を形成するように組織を気化するために調整可能である、請求項5に記載の装置:
鼓膜;
ファロビウス管;
表皮;
真皮乳頭層;
しわの付いた組織;
声帯;
膣頸管組織;および
歯茎組織。 - 加熱、前進、および後退の速度は、1〜100パルス/秒の繰返し率の気化の繰返しパルスを生成する、請求項1〜6のいずれかに記載の装置。
- 気化要素が加熱される温度および短い持続時間の組合せは、付帯的損傷距離を150ミクロン未満の距離に制限するように選択される、請求項1〜7のいずれかに記載の装置。
- 気化要素の先端は気化要素の本体より細い、請求項1〜8のいずれかに記載の装置。
- 気化要素は、第一材料と、第一材料を被覆する第二生物適合性材料とを含む、請求項1〜9のいずれかに記載の装置。
- 第一材料は銅を含む、請求項10に記載の装置。
- 気化要素が組織を気化する間に組織を延伸するための組織延伸機構をさらに含む、請求項1〜11のいずれかに記載の装置。
