Description
Die Erfindung betrifft ein bioaktives Verbundmaterial für prothetische Zwecke und ein Verfahren zu dessen Herstellung. 60
Es ist bekannt, zur Fixierung beispielsweise von Endoprothesen und anderen Knochenersatzstücken im tierischen oder menschlichen Organismus einen sog. «Knochenzement)) aus Kunststoff auf Methaciylatbasis zu verwenden. So wird in der «Zeitschrift für Orthopädie» 112 (1974), 419-426, ein Kirnst- 65 Stoff «Palakav» beschrieben, der neben Methylmethacrylat und einem organischen Katalysator Kieselsäuregele enthält, wobei das Verhältnis Methylmethacrylat : Kieselsäure sich wie
58 : 42 verhält. Die zugesetzten anorganischen Gele dienen dabei lediglich als Füllmaterial der Kunststoffmatrix.
Zwar werden mit diesem Composite-Material bereits günstigere Haftungsverhältnisse im Sinne einer Mikroverzahnung des «Palakav» an der anliegenden Knochensubstanz erzielt als bei den bislang üblichen Knochenzementen, wie z.B. dem «Palacos», der lediglich eine gewisse Makroverzahnimg mit dem angrenzenden Knochenteil herbeizuführen vermag; das Grundproblem einer voll ausreichenden Verankerung zwischen Material und Knochenwand im Sinne stabiler chemischer Hauptvalenzverknüpfungen, die auch immerwährenden Druck-, Zug-, Scher- und/oder Torsionsbeanspruchungen in erheblichem Masse standzuhalten vermögen, konnte indes mit den bislang bekannten Verbundmaterialien nicht gelöst werden.
Es ist daher die Aufgabe der vorliegenden Erfindung, ein biokompatibles Verbundmaterial aufzuzeigen, das nicht nur eine gewisse «Klebefreundlichkeit» aufweist, sondern das unter Vermeidung der den bekannten Materialien anhaftenden Nachteile bioaktiv ist und zu einem vollständigen Anwachsen an den jeweiligen Kontaktflächen Material/Knochenwand führt. Aufgabe der vorliegenden Erfindung ist ferner ein entsprechendes Verfahren zur Herstellung eines solchen bioaktiven Verbundmaterials anzugeben.
Die Aufgabe wird gemäss der vorliegenden Erfindung dadurch gelöst, dass das Verbundmaterial aus einer Kunststoffmatrix auf Methacrylatbasis und mindestens einem bioaktiven Material besteht. Dabei kann das bioaktive Material eine Glaskeramik sein, wie sie in der DE-OS 2 326 100 beschrieben und beansprucht ist. Es handelt sich hierbei um ein glaskeramisches Material mit Apatit-Kristallphase, das aus 20 bis 60 Gew.-% Si02, 5 bis 40 Gew.-% Pz05,2,7 bis 20 Gew.-% Na20, 0,4 bis 20 Gew.-% K20,2,9 bis 30 Gew.-% MgO und 5 bis 40 Gew.-% CaO besteht, das zusätzlich noch bis 0,005 bis 3,0 Gew.-% Fluor enthalten kann. Die Verteilung der vorzugsweise in Korngrössen bis zu 200 (im der Kunststoffmatrix beigegebenen pulverisierten Glaskeramik kann entweder eine homogene (isotrope Korn Verteilung) oder eine gezielt inhomogene (anisotrope Kornverteilung) sein. Es ist vorteilhaft, dass der Anteil des bioaktiven Materials zwischen 20 und 70 Vol-% ausmacht.
Auch ist es möglich, dass das erfindungsgemässe Verbundmaterial zur Erhöhung seiner mechanischen Eigenschaften zusätzlich mindestens eine aus einem Fasermaterial, beispielsweise Glasfasern bestehende Armierung aufweist.
Bei dem erfindungsgemässen Verfahrensablauf wird zunächst einem flüssigen monomeren Methylmethacrylat ein als Härter wirkendes pulverförmiges oder flüssiges organisches Material, vorzugsweise ein organisches Peroxid, und sodann bevorzugt die bereits beschriebene vorzugsweise pulverisierte bioaktive Glaskeramik beigegeben. Es kann dabei zur Erreichung einer gezielt anisotropen Kornverteilung vorteilhaft sein, das pulverisierte Bio-Material erst dann zu dem bereits mit dem Härter versehenen Methylmethacrylat zu geben, wenn der bereits ablaufende Polymerisationsvorgang schon zu einer gewissen Viskosität der organischen Mischsubstanz (Methylmethacrylat + organischer Härter) geführt hat. Nach einer weiteren Ausgestaltung der Erfindung kann als weitere Komponente ein Katalysator zugegeben werden. Wegen der stark exothermen Polymerisationsreaktion kann eine Kühlung während des Verfahrensablaufs von Vorteil sein.
Es ergibt sich folgender Anwendungsbereich des neuen Verbundmaterials. Es kann als Knochen- oder Zahnzement zur Fixierung künstlicher Implantate oder körpereigener Knochen- bzw. Zahnteile dienen; zweitens kann das vollständig auspolymerisierte Verbundmaterial selbst zu Prothesen oder Prothesenteilen verarbeitet werden, die aufgrund ihrer bioaktiven Einlagerungen ein dauerhaftes Anwachsen ermöglichen.

i Der zeitliche Ablauf der einzelnen Verfahrensstufen hängt von dem jeweils erwünschten Endprodukt ab. Will man das erfindungsgemässe Verbundmaterial als Knochenzement zur Verankerung einer Endoprothese in einem Röhrenknochen verwenden, so wird beispielsweise dem in einem Gefäss befindlichen flüssigen Methylmethacrylat nach Zugabe des Härters alsbald eine feingekörnte Fraktion der Bio-Glaskeramik zugegeben, die in Abhängigkeit von der Masse der einzelnen Pulver-Partikeln und des sich stetig steigernden Viskositätsgrades in die sich «eindickende» Substanz eindringen und teilweise oder ganz absinken. Wenn die Auspolymerisation soweit fortgeschritten ist, dass sich eine knetfahige, teigige Masse ergibt, kann ein für den beabsichtigten Zweck ausreichend grosser plastischer «Klumpen» entnommen werden, der sodann mit einem Bio-Glaskeramik-Pulver gewissennassen «paniert» wird, so dass in der Oberfläche der Knetmasse eine Anreicherung des bioaktiven Materials vorhanden ist. Die so präparierte Masse wird dann als bioaktiver Zement funktionsgerecht zwischen Knochen und Implantat plaziert.
Eine weitere Möglichkeit der Zubereitung des erfindungs-gemässeh Verbundmaterials für die anschliessende Verwendung als Knochenzement ist dadurch gegeben, dass der mit dem Härter und einer gewissen Menge von pulverfönnigem bioaktivem Material versehene niederviskose Kunststoff alsbald allseitig dünnschichtig aufgetragen wird - beispielsweise auf die zylinderförmige Innenwandung eines Prothesenteils -, dass danach in kurzem zeitlichen Abstand diese dünne Schicht J aus Verbundmaterial mit zusätzlichem pulverisiertem Bio-Glaskeramik-Material angereichert wird und daran anschliessend die Hauptmasse des bereits vorbehandelten erfindungs-gemässen Knochenzements eingeführt wird.
Damit wird eine anisotrope Kornverteilung des für den chemischen Haftmechanismus ausschlaggebenden Bio-Pulvers

erzielt, derart, dass an der dem Knochen zugewandten Seite des in den Prothesenteil eingebrachten Zements eine Anhäufung des Pulvers vorhanden ist.
Die Wahl der Pulver-Partikel-Grösse hängt ebenfalls vom 5 jeweils vorgesehenen Einsatzzweck des Verbundmaterials ab; es hat sich gezeigt, dass Korngrössendurchmesser über 200 [im aufgrund der reduzierten spezifischen Oberfläche der eingebetteten Partikel nicht mehr zu optimalen Ergebnissen führen.
Will man das erfindungsgemässe Verbundmaterial bei-io spielsweise selbst als Prothesenwerkstoff verwenden, so kann das flüssige Ausgangsmaterial (Kunststoff + Härter) sogleich in vorgefertigte Formen eingegeben werden, wobei bei der Einrichtung der Formen auf eine Volumen-Vorgabe (Toleranz) zu achten ist, da das Polymerisat einer gewissen 15 Schrumpfung unterworfen ist, die allerdings durch die Zugabe von bis zu 70 Vol.-% Bio-Glaskeramik-Material gezielt «abgemildert» werden kann. Das erhärtete Material kann im übrigen in beliebiger Weise (sägen, fräsen, bohren, schneiden, läppen, schleifen, pressen) einer anschliessenden mechanischen Form-20 bearbeitung auf gewünschte Endmasse unterzogen werden. Bei diesem letztgenannten Beispiel kann es von Vorteil sein, nach Art der faserverstärkten Kunststoffe Glas- oder andere mineralische Fasern (Whiskers, z.B. Kohlenstoffasern) gerichtet (anisotrop) oder ohne Vorzugsrichtung (isotrop) einzula-25 gern.
Auch ist es für spezielle Anfertigungen zweckmässig, an bestimmten Stellen eines Prothesenteils oder einer Prothese eine Anhäufung der pulverförmigen bioaktiven Partikel beispielsweise durch Seigerung infolge der Schwerkrafteinwir-30 kung oder durch Nach-Beschichtung («Panieren») zu erzielen.
Als Füllmaterial - beispielsweise für Zahnfüllungen oder für lokal beschädigte Knochenpartien - lässt sich das neue Material ebenfalls in vielgestaltigerweise verwenden.

Claims (10)
1. Verbundmaterial für prothetische Zwecke, dadurch gekennzeichnet, dass es aus einer Kunststoffmatrix auf Metha-ciylatbasis und mindestens einem bioaktiven Material besteht.
2. Verbundmaterial nach Anspruch 1, dadurch gekennzeichnet, dass das bioaktive Material eine bioaktive Glaskeramik mit Apatit-Kristallphase der Zusammensetzimg

PATENTANSPRÜCHE
3. Verbundmaterial nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass das bioaktive Material in homogen isotroper Kornverteilung, vorzugsweise in Korngrössen bis zu 200 um, in der Kunststoffmatrix eingelagert ist.
4. Verbundmaterial nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass das bioaktive Material in inhomogen anisotroper Kornverteilung in der Kunststoffmatrix eingelagert ist.
5. Verbundmaterial nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass das bioaktive Material in einem Anteil von 20 bis 70 Vol.-%, bezogen auf das Gesamtvolumen des Verbundmaterials, vorhanden ist.
6. Verbundmaterial nach den Ansprüchen 1 bis 5, dadurch gekennzeichnet, dass es zur Erhöhung seiner mechanischen Eigenschaften zusätzlich mindestens eine aus einem Fasermaterial, beispielsweise Glasfasern bestehende Armierung aufweist.
7. Verfahren zur Herstellung eines Verbundmaterials nach Anspruch 1, dadurch gekennzeichnet, dass zunächst einem flüssigen monomeren Methylmethaciylat ein als Härter wirkendes pulverförmiges oder flüssiges organisches Material, und sodann das bioaktive Material beigegeben wird, worauf man die Masse durch Polymerisation härtet.

Verfahren nach Anspruch 7 zum'Herstellen eines Materials nach Anspruch 2 und 4, dadurch gekennzeichnet, dass die Zugabe der pulverisierten Glaskeramik zum Zweck einer gezielten fraktionierten Seigerung mit anisotroper Kornverteilung erst dann zu dem bereits mit dem Härter versehenen Me-thylmethacrylat erfolgt, wenn der einsetzende Polymerisationsvorgang bereits zu einer gewissen Viskosität der organischen Mischsubstanz geführt hat.
9. Verfahren nach Anspruch 7 oder 8, dadurch gekennzeichnet, dass als weitere Komponente ein Katalysator zugegeben wird.
10. Verfahren nach einem der Ansprüche 7 bis 9, dadurch gekennzeichnet, dass während des Polymerisationsvorgangs eine Kühlung des aushärtenden Verbundmaterials erfolgt.
