본원은 성인형 스틸병 진단용 바이오마커 조성물에 관한 것이다.
스틸병(Still’s disease)은 연소성 류마티스 관절염의 전신형으로 스틸(Still)이 처음 기술한 후 아직 원인이 밝혀지지 않은 질환이며 소아와 성인에서 모두 발병한다. 성인에서 발병하는 성인형 스틸병은 1971년 바이워터(Bywaters)에 의해서 보고된 이래, 세계 각지에서 보고되고 있으며, 드문 질환이나 불명열의 한 원인 질환으로서 알려져 있다.
대개 성인형 스틸병은 양성 질환으로 예후가 좋다고 보고되고 있으나, 일부의 환자가 이로 인해 사망한 치명적인 경우도 보고되고 있으며, 혈구 탐식 증후군, 간 기능 악화, 감염 등이 동반될 경우 환자가 사망할 수 있는 상태에 이를 수도 있다. 국내의 연구 결과에서도 발병 환자의 약 10%에서 사망하는 예후를 보고한 바 있다.
스틸병의 진단은 주로 임상소견에 기초하는데, 고열, 관절통이나 관절염, 특징적인 피부 병변, 림프절 종대, 간종대, 비종대, 장막염과 인후통 등의 임상소견 및 백혈구 증가증, 혈침속도의 증가, 항핵항체 음성, 류마티스 인자 음성 등의 검사실 소견을 통해 진단을 한다.
성인형 스틸병에서 이루어지고 있는 연구의 대부분은 환자의 혈청을 이용한 염증성 사이토카인, 급성반응기 물질에 대한 간단한 효소면역분석법(Enzyme-LinKed ImmunoSpecific Assay, ELISA) 연구, 또는 유전자 연구이며, 체계적인 바이오마커에 대한 보고는 없었다. 더불어 병인 기전에 대한 이해의 부족으로 경험에 의존하여 치료하고 있으며 이에 대한 합의(consensus)도 없는 실정이다.
따라서, 성인형 스틸병 진단에 유용하게 활용될 수 있는 바이오마커에 대한 연구의 필요성이 대두되고 있다.
본원의 배경이 되는 기술인 한국 등록특허공보 제 10-1806681 호는 스틸씨병 진단용 바이오마커 조성물, 진단 키트 및 진단 방법에 관한 것으로서, 구체적으로, CD11b 또는 CD32의 발현 수준과, 림프구에 대한 과립구의 빈도 비율을 지표로 하여 소아와 성인에게서 스틸씨병을 진단하는 할 수 있음을 개시하고 있다. 그러나, TLR2 를 이용하여 간편하게 성인형 스틸병을 진단하는 방법, 진단용 바이오마커 조성물 및 키트에 대해서는 언급하지 않고 있다.
본원은 전술한 종래 기술의 문제점을 해결하기 위한 것으로서, 성인형 스틸병 진단용 바이오마커 조성물을 제공하는 것을 목적으로 한다.
또한, 본원은 TLR2 항원 단백질 또는 TLR7 항원 단백질에 특이적으로 결합하는 항체를 포함하는 성인형 스틸병 진단용 키트를 제공하는 것을 목적으로 한다.
또한, 본원은 성인형 스틸병 진단을 위한 정보제공 방법을 제공하는 것을 목적으로 한다.
또한, 본원은 성인형 스틸병 치료제의 스크리닝 방법을 제공하는 것을 목적으로 한다.
다만, 본원의 실시예가 이루고자 하는 기술적 과제는 상기된 바와 같은 기술적 과제들로 한정되지 않으며, 또 다른 기술적 과제들이 존재할 수 있다.
상기한 기술적 과제를 달성하기 위한 기술적 수단으로서, 본원의 제 1 측면은, TLR2 단백질을 포함하는 성인형 스틸병 진단용 바이오마커 조성물을 제공한다.
본원의 일 구현예에 따르면 상기 성인형 스틸병은 관절염이 발현된 것을 포함하는 것일 수 있으나, 이에 제한되는 것은 아니다.
본원의 일 구현예에 따르면 상기 조성물은 TLR7 단백질을 추가로 포함하는 것일 수 있으나, 이에 제한되는 것은 아니다.
본원의 제 2 측면은 TLR2 항원 단백질 또는 TLR7 항원 단백질에 특이적으로 결합하는 항체를 포함하는 성인형 스틸병 진단용 키트를 제공한다.
본원의 일 구현예에 따르면 상기 항체는 모노클로날 항체 또는 폴리클로날 항체인 것일 수 있으나, 이에 제한되는 것은 아니다.
본원의 일 구현예에 따르면 상기 키트는 항원-항체 결합반응을 이용하여 시료 내 TLR2 단백질 또는 TLR7 단백질을 검출하여 성인형 스틸병을 진단하는 것일 수 있으나, 이에 제한되는 것은 아니다.
본원의 일 구현예에 따르면 상기 시료는 개체로부터 분리된 전혈, 혈청, 혈장, 조직, 타액, 눈물, 소변, 객담, 림프액, 뇌척수액, 세포간액, 골수액, 복수 및 이들의 조합들로 이루어진 군으로부터 선택되는 것을 포함하는 것일 수 있으나, 이에 제한되는 것은 아니다.

성인형 스틸병(Adult-onset Still's disease, AOSD)은 1971년 바이워터(Bywaters)에 의해서 보고된 이래, 세계 각지에서 보고되고 있으며, 발열, 관절염, 소실성 발진, 관절통 혹은 관절염, 백혈구 증가증을 특징적인 증상으로 발현되는 염증질환으로서 발병 기전은 명확하게 밝혀진 바 없다.
스틸병의 진단은 주로 임상소견에 기초하는데, 고열, 관절통이나 관절염, 특징적인 피부 병변, 림프절 종대, 간종대, 비종대, 장막염과 인후통 등의 임상소견 및 백혈구 증가증, 혈침속도의 증가, 항핵항체 음성, 류마티스 인자 음성 등의 검사실 소견을 통해 진단을 한다.
성인형 스틸병에서 이루어지고 있는 연구의 대부분은 환자의 혈청을 이용한 염증성 사이토카인, 급성반응기 물질에 대한 간단한 효소면역분석법(Enzyme-LinKed ImmunoSpecific Assay, ELISA) 연구, 또는 유전자 연구이며, 체계적인 바이오마커에 대한 보고는 없었다. 더불어 병인 기전에 대한 이해의 부족으로 경험에 의존하여 치료하고 있으며 이에 대한 합의(consensus)도 없는 실정이다.
이에, 조직검사 또는 영상촬영 기술을 사용하지 않고도 초기에 간편하게 성인형 스틸병을 진단할 수 있는 바이오마커에 대한 연구의 필요성이 대두되고 있다.
바이오마커란 DNA, RNA, 대사물질, 단백질 및 단백질 조각 등에서 유래된 단일 분자 또는 분자들의 패턴을 근거로 한 분자적 정보로서 체내에서 유전적 또는 후생 유전적 변화의 영향으로 유발된 신체의 변화를 감지할 수 있는 지표를 의미한다. 단백질체는 단백질 바이오마커들로써 환자의 혈청 내에 존재하며 검출이 용이한 장점으로 인해 바이오마커로의 중요한 가치를 나타낸다.
전술한 본원의 과제 해결 수단에 의하면, 본원에 따른 성인형 스틸병 진단용 바이오마커 조성물은 진단이 어려운 성인형 스틸병을 정확하고 신속하게 진단할 수 있다.
본원의 일 구현예에 따르면 상기 조성물은 TLR7 단백질을 추가로 포함하는 것일 수 있으나, 이에 제한되는 것은 아니다.
구체적으로, 본원에 따른 성인형 스틸병 진단용 바이오마커 조성물은 정상인과 성인형 스틸병 환자에게서 TLR2 또는 TLR7 을 발현하는 세포의 빈도 수 차이를 발견하고, 이를 발굴하고 검증한 것이다.
구체적으로, 본 발명의 일 실시예에서는 성인형 스틸병 환자의 말초혈액 단핵세포(peripheral blood mononuclear cell, PBMC)를 정상 대조군과 비교하였을 때, TLR2 를 발현하는 세포의 빈도 및 TLR7 을 발현하는 세포의 빈도가 더 높음을 확인하였다(실시예 2).

본원의 일 구현예에 따르면 상기 성인형 스틸병은 관절염이 발현된 것을 포함하는 것일 수 있으나, 이에 제한되는 것은 아니다.
구체적으로, 본 발명의 일 실시예에서는 성인형 스틸병 환자의 전체 세포의 TLR2 빈도는 전신 점수, LDH 수준 및 페리틴 수준과 상관 관계가 있음을 확인하였으며, 관절염(Arthritis)이 발현된 AOSD 환자에서 TLR2 발현 수준이 유의하게 높음을 확인하였다(실시예 3).
따라서, 본원에 따른 성인형 스틸병 진단용 바이오마커 조성물은 특정 질병 활성 마커(전신 점수, LDH 수준 및 페리틴 수준) 및 특정 임상 증상(관절염)이 발현된 성인형 스틸병을 정확하고 신속하게 진단할 수 있는 장점이 있다.

본원의 제 2 측면은 TLR2 항원 단백질 또는 TLR7 항원 단백질에 특이적으로 결합하는 항체를 포함하는 성인형 스틸병 진단용 키트를 제공한다.
본원의 제 2 측면의 상기 성인형 스틸병 진단용 키트에 대하여, 본원의 제 1 측면과 중복되는 부분들에 대해서는 상세한 설명을 생략하였으나, 그 설명이 생략되었더라도 본원의 제 1 측면에 기재된 내용은 본원의 제 2 측면에 동일하게 적용될 수 있다.
상기 항원은 항체와 항원-항체 결합을 수행할 수 있는 단백질성 면역원을 의미한다. 이러한 항원은 통상적인 방법에 의해 유전자 재조합 방법을 통해 합성 분리 및 정제될 수 있다.
본원의 일 구현예에 따르면 상기 항체는 모노클로날 항체(단일클론 항체) 또는 폴리클로날 항체(다클론성 항체)인 것일 수 있으나, 이에 제한되는 것은 아니다.
상기 단일클론 항체는 당업자에 알려진 융합에 의한 불멸화된 세포주 생성기술에 의해 제조될 수 있다. 예를 들어, 상기 유전자에 의해 발현된 단백질을 마우스에 면역화시키거나 펩타이드를 합성하여 소혈청 알부민과 결합시켜 마우스에 면역화시킨다. 마우스에서 분리된 항원-생산 B 임파구를 인간 또는 마우스의 골수종(myeloma)과 융합하여 불멸화된 하이브리도마(hybridoma)를 생성하며, 상기 하이브리도마 세포를 가지고 간접적인 효소 결합 면역흡착 분석법(enzyme-linked immunoabsorbent assays, ELISA)을 사용하여 모노클노날 항체의 생성 여부를 확인하고 양성 클론을 택하여 배양한 후 항체를 분리정제하거나 랫트의 복강에 주입한 후 복수를 채취함으로써, 단일클론 항체를 제조할 수 있다.

상기 다클론성 항체는 당업자에 알려진 방법에 따라 면역원으로 TLR2 또는 TLR7 단백질 또는 그 단편을 외부 숙주에 주사함으로써 제조할 수 있다. 외부 숙주는 마우스, 랫트, 양 또는 토끼와 같은 포유동물을 포함한다. 면역원은 근내, 복강내 또는 피하 주사방법으로 주사되며, 일반적으로 항원성을 증가시키기 위한 보조제(adjuvant)와 함께 투여된다. 외부숙주로부터 정기적으로 혈청을 채취하여 향상된 역가 및 항원에 대한 특이성을 보이는 혈청을 수거하거나 이로부터 항체를 분리정제한다.

상기 개체는 성인형 스틸병이 의심되는 사람 또는 성인형 스틸병에 걸린 사람 또는 성인형 스틸병으로 진단 후 치료를 받고 있는 사람을 의미하는 것이다.
구체적으로, 상기 시료는 상기 개체에서 분리되어 상기 TLR2 단백질 또는 TLR7 단백질의 발현수준을 측정하는 직접적인 대상을 의미한다. 이러한 시료는 상기 TLR2 단백질 또는 TLR7 단백질을 검출 할 수 있는 것이면 특별히 제한되지 않으며, 검출이 가능한 하나 이상의 성분을 포함하는 물질 또는 물질의 혼합물을 일컫는 것으로 생물체, 특히 인간 유래의 세포, 조직 또는 체액, 예를 들면 타액, 눈물과 같은 체액, 전혈, 혈장, 및 혈청을 포함하나 이로 제한하는 것은 아니다.

이하 실시예를 통하여 본 발명을 더욱 상세하게 설명하고자 하나, 하기의 실시예는 단지 설명의 목적을 위한 것이며 본원의 범위를 한정하고자 하는 것은 아니다.
[실시예 1] 실험준비
[실시예 1-1] 대상체 선별
도 1 은 임상 시험 대상체의 임상 특성(clinical characteristic)을 나타낸 표이다.
구체적으로, 20 명의 성인형 스틸병(Adult-onset Still’s disease, AOSD) 환자와 15 명의 건강한 대조군(Healthy controls, HC)이 참여하였다. AOSD 환자는 감염, 기타 자가 면역 질환, 악성 종양을 배제한 후 야마구치(Yamaguchi) 진단 기준에 따라 진단되었다. HC 는 자가 면역, 류마티스 또는 기타 악성 질환의 병력이 없는 건강한 개인이었다.
[실시예 1-2] 말초혈액 단핵세포(peripheral blood mononuclear cell, PBMC) 분리
상기 실시예 1-1 의 각각의 대상체의 PBMC는 BD Vacutainer CPTTM(BD Biosciences, Franklin Lakes, NJ)을 이용하여 분리하였다.
[실시예 2] 말초혈액 단핵세포에서 TLR2 또는 TLR7 을 발현하는 세포의 빈도 측정
도 2 는 전체 말초혈액 단핵세포에서 TLR1, TLR2, TLR4, TLR7 및 TLR9 를 발현하는 세포의 빈도를 측정한 결과를 나타낸 표이고, 도 3 은 AOSD 환자 1 명의 전체 말초혈액 단핵세포에서 다양한 TLR 마커를 제시하는 표면 염색 세포의 유세포 분석 히스토그램의 대표예를 나타낸 이미지이다.
구체적으로, 상기 실시예 1-2 에서 분리한 PBMC에서 여러 종류의 TLR 을 발현하는 세포의 빈도를 각각 측정하였다.
더욱 구체적으로, 각 1 x 106 세포를 FACS 블로킹 버퍼와 함께 배양한 후, 각각 다른 형광이 붙어 있는 항-TLR1, 항-TLR2, 항-TLR4, 항-TLR7 또는 항-TLR9 항체와 반응시킨 후 유세포분석기(flow cytometry, FACS)를 이용하여 항체가 붙은 세포의 수를 카운트하고, 이 세포가 전체 세포(10,000 개)에서 차지하는 빈도를 분석하였다.
이를 통하여, 전혈에서 TLR2 를 나타내는 표면 염색 세포의 평균 빈도가 HC(0.98 ± 0.04, p <0.001)보다 AOSD(1.27 ± 0.43) 환자군에서 유의하게 더 높음을 확인할 수 있었다.
또한, 전혈에서 TLR7을 나타내는 세포의 빈도가 HC(1.06 ± 0.86, p = 0.006)보다 AOSD(1.28 ± 0.29) 환자군에서 훨씬 더 높음을 확인할 수 있었다.
한편, AOSD 군과 HC 사이에 TLR1, TLR4 또는 TLR9 를 나타내는 세포의 빈도에는 유의한 차이가 없음을 확인할 수 있었다.
이는 성인형 스틸병 환자의 PBMC 를 정상 대조군과 비교하였을 때, TLR2 를 발현하는 세포의 빈도 및 TLR7 을 발현하는 세포의 빈도가 더 높음으로, 이를 분석하여 진단이 어려운 성인형 스틸병을 정확하고 신속하게 진단할 수 있음을 시사하는 것이다.
[실시예 3] AOSD 환자에서 질병 활성 마커(disease activity markers) 또는 임상 증상(clinical manifestations)에 따른 TLR2 또는 TLR7 을 발현하는 세포의 빈도 측정
도 4 는 AOSD 환자에서 여러 TLR 마커를 나타내는 염색된 세포의 빈도와 질병 활성 마커 또는 전신 점수(systemic score)의 상관 관계를 나타낸 표이다.
이를 통하여, 전체 세포의 TLR2 빈도는 전신 점수(r = 0.446, p = 0.049), LDH 수준(r = 0.728, p <0.001) 및 페리틴(r = 0.829, p <0.001) 수준과 상관 관계가 있음을 확인하였고, TLR1 빈도는 LDH (r = 0.716, p <0.001) 및 페리틴(r = 0.804, p <0.001)과 상관 관계가 있음을 확인할 수 있었다.
또한, AOSD 환자에서 각 세포 표면 TLR 마커 간의 상관 관계를 평가한 결과, PBMC 에서 TLR2 의 빈도는 TLR4(r = 0.838, p <0.001), TLR7(r = 0.588, p <0.001) 및 TLR9(r = 0.932, p <0.001)와 양의 상관 관계가 있음을 확인할 수 있었다.
도 5 는 AOSD 환자에서 임상 증상에 따른 여러 TLR 발현 수준을 나타낸 표이다.
이를 통하여, 임상 증상으로 관절염(Arthritis)이 발현된 AOSD 환자에서 TLR2 발현 수준이 유의하게 높음을 확인할 수 있었다.
이는 본원에 따른 성인형 스틸병 진단용 바이오마커 조성물이 관절염이 발현된 성인형 스틸병을 정확하고 신속하게 진단할 수 있음을 시사하는 것이다.
[실시예 4] AOSD 환자의 피부 시료의 조직 병리학적 평가
도 6 은 AOSD 환자의 피부 조직 검사 결과를 나타낸 표이다.
구체적으로, 피부 조직 검사 시료는 2000 년 내지 2019 년에 피부 발진 평가를 위해 32 명의 AOSD 환자, 5명의 습진(eczema) 환자, 5명의 건선(psoriasis) 환자 및 5명의 대조군(normal controls, NC)으로부터 수집되었다. 림프절 조직 검사 시료는 9 명의 AOSD 환자로부터 수집되었다. 이어서, 각 시료에 H&E staining 을 진행하여 3 명의 병리학자가 독립적으로 피부 병리학적 매개 변수를 평가하였다.
도 7 은 AOSD 환자, 습진 환자, 건선 환자 및 대조군에서 수행한 TLR1, TLR2, TLR4, TLR7 및 TLR9 면역조직화학염색(Immunohistochemical staining) 결과를 나타낸 표이다.
구체적으로, TLR1, TLR2, TLR4, TLR7 및 TLR9 항체를 각각 1 차 항체로 사용하고, Ventana Optiview DAB 키트 (Ventana Medical Systems Inc.)를 이용하여 면역 조직 화학 염색을 수행하였다. IHC 결과값은 양성 임파상 세포(positive lymphoid cells) 및 histiocytes 의 백분율에 따라 등급을 산정하였다.
이를 통하여, AOSD 환자의 피부 병변에서 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율은 각각 81.9 ± 8.2, 34.8 ± 24.1, 52.3 ± 29.8, 19.3 ± 15.7 및 40.2 ± 27.9 이고, 정상 대조군의 피부에서 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율은 각각 70.0 ± 18.4, 6.2 ± 3.6, 77.0 ± 11.0, 9.6 ± 6.6 및 5.4 ± 4.5 이며, 습진으로 인한 피부 병변에서 TLR1, TLR2, TLR4, TLR7, TLR9를 발현하는 염증 세포의 평균 백분율은 각각 70.0 ± 16.0, 27.0 ± 30.9, 75.0 ± 10.0, 24.4 ± 31.2 및 17.6 ± 20.2 이고, 건선으로 인한 피부 병변에서 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율은 각각 72.0 ± 5.7, 27.2 ± 21.4, 64.0 ± 11.4, 19.3 ± 15.7 및 7.4 ± 9.9 임을 확인할 수 있었다.
비교 분석에 따라, AOSD 환자의 피부 병변에서 TLR2 를 발현하는 염증 세포의 비율은 대조군(HC)에서보다 유의하게 더 큼을 확인할 수 있었다(p = 0.002).
도 8 은 AOSD 환자의 피부에서 염증 세포 등급(CD4/CD8/CD68 염색); 및 CXCL9, CXCL10, CXCL11, CXCR3, CXCL12 및 CXCR4 를 발현하는 염증 세포의 평균 백분율과 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율의 상관 관계를 나타낸 표이다.
이를 통하여, AOSD 환자의 피부에서 TLR2 를 발현하는 염증 세포의 비율은 CXCL10을 발현하는 염증 세포의 평균 백분율과 양의 상관 관계(r = 0.467, p = 0.021)가 있음을 확인할 수 있었다.
도 9 는 AOSD 환자의 피부에서 TLR 마커 간의 상관 관계를 평가한 결과를 나타낸 그래프이다.
이를 통하여, AOSD 환자의 피부에서 TLR2 의 빈도는 TLR4(r = 0.370, p = 0.037) 및 TLR9(r = 0.398, p = 0.024)와 양의 상관 관계가 있음을 확인할 수 있었다.
종합하면, 이는 본원에 따른 성인형 스틸병 진단용 바이오마커 조성물이 다른 유사 병변(mimickers)으로부터 성인형 스틸병을 구분하는데 유용하게 활용될 수 있음을 시사하는 것이다.
[실시예 5] AOSD 환자의 림프절(LN) 시료의 면역 조직 화학 염색
도 10 은 AOSD 환자의 림프절 조직 검사 결과를 나타낸 표이다. 구체적으로, AOSD 환자, T 세포 림프종 환자, 조직세포 괴사 림프선염 환자, 결핵 임파선염 환자, 비특이 반응성 과형성증 환자 및 대조군에서 수행한 TLR1, TLR2, TLR4, TLR7 및 TLR9 면역조직화학염색 결과를 나타낸 표이다.
상기 림프절 조직 검사 시료는 9 명의 AOSD 환자, T 세포 림프종(T cell lymphoma) 환자, 조직세포 괴사 림프선염(histiocytic necrotizing lymphadenitis, HNL, kikuchi 병) 환자, 결핵 임파선염(tuberculous lymphadenitis, Tbc lymphadenitis) 환자 및 비특이 반응성 과형성증(nonspecific reactive hyperplasia) 환자로부터 수집되었다. 이어서, 각 시료에 H&E staining 을 진행하여 3 명의 병리학자가 독립적으로 피부 병리학적 매개 변수를 평가하였다.
TLR1, TLR2, TLR4, TLR7 및 TLR9 항체를 각각 1 차 항체로 사용하고, Ventana Optiview DAB 키트 (Ventana Medical Systems Inc.)를 이용하여 면역 조직 화학 염색을 수행하였다. IHC 결과값은 양성 임파상 세포(positive lymphoid cells) 및 histiocytes 의 백분율에 따라 등급을 산정하였다.
이를 통하여, AOSD 환자의 림프절에서 TLR1, TLR2, TLR4, TLR7 및 TLR9 를 발현하는 염증 세포의 평균 백분율은 각각 34.4 ± 17.9, 17.3 ± 16.6, 31.1 ± 13.6, 27.2 ± 20.0 및 18.3 ± 9.7 이고, 반응성 임파절(Reactive Lymph Node, reactive LNs) 환자에서 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율은 각각 7.6 ± 7.6, 8.6 ± 4.7, 12.2 ± 7.2, 8.0 ± 2.7 및 2.6 ± 1.7이며, 결핵 임파선염 환자의 림프절에서 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율은 각각 13.2 ± 13.2, 6.8 ± 5.7, 44.0 ± 26.0, 32.0 ± 4.5 및 11.2 ± 11.2 이며, T 세포 림프종 환자에서 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율은 각각 9.2 ± 6.2, 20.0 ± 31.8, 4.4 ± 3.4, 8.2 ± 7.2 및 3.6 ± 4.0 이고, 조직세포 괴사 림프선염 환자의 림프절에서 TLR1, TLR2, TLR4, TLR7 및 TLR9를 발현하는 염증 세포의 평균 백분율은 각각 24.0 ± 12.9, 4.8 ± 3.0, 52.0 ± 17.5, 49.0 ± 12.5 및 21.0 ± 19.2 임을 확인할 수 있었다.
비교 분석에 따라, AOSD 환자의 림프절에서 TLR2 를 발현하는 염증 세포의 비율은 림프선염 환자의 림프절(kikuchi)에서보다 유의하게 더 큼을 확인할 수 있었으며(p = 0.042), AOSD 환자의 림프절에서 TLR4, TLR7 및 TLR9 를 발현하는 염증 세포의 비율은 T 세포 림프종 환자(p = 0.001) 및 반응성 임파절 환자(p = 0.012)에서보다 유의하게 높음을 확인할 수 있었다.
종합하면, 이는 본원에 따른 성인형 스틸병 진단용 바이오마커 조성물이 다른 유사 병변(mimickers)으로부터 성인형 스틸병을 구분하는데 유용하게 활용될 수 있음을 시사하는 것이다.

- 제1항에 있어서,상기 조성물은 TLR7 단백질을 추가로 포함하는 것인, 성인형 스틸병 진단용 바이오마커 조성물.
- TLR2 항원 단백질 또는 TLR7 항원 단백질에 특이적으로 결합하는 항체를 포함하는 성인형 스틸병 진단용 키트.
- 제 4 항에 있어서,상기 항체는 모노클로날 항체 또는 폴리클로날 항체인 것인, 성인형 스틸병 진단용 키트.
- 제 4 항에 있어서,상기 키트는 항원-항체 결합반응을 이용하여 시료 내 TLR2 단백질 또는 TLR7 단백질을 검출하여 성인형 스틸병을 진단하는 것인, 성인형 스틸병 진단용 키트.
- 제 6 항에 있어서,상기 시료는 개체로부터 분리된 전혈, 혈청, 혈장, 조직, 타액, 눈물, 소변, 객담, 림프액, 뇌척수액, 세포간액, 골수액, 복수 및 이들의 조합들로 이루어진 군으로부터 선택되는 것을 포함하는 것인, 성인형 스틸병 진단용 키트.
- 제 6 항에 있어서,상기 시료는 개체로부터 분리된 말초혈액 단핵세포(peripheral blood mononuclear cell, PBMC)인 것인, 성인형 스틸병 진단용 키트.
- 제 6 항에 있어서,상기 TLR2 단백질 또는 TLR7 단백질을 검출하는 방법은 웨스턴 블럿, 효소면역측정법(ELISA), 닷 블랏 분석(dot blot assay), 방사선면역분석, 방사 면역 확산법, 오우크테로니 면역 확산법, 로케트 면역 전기영동, 조직 면역 염색, 면역 침전 분석법, 보체 고정 분석법, 유세포분석법 (FLOW CYTOMETRY ANALYSIS, FACS), 단백질 칩, 샌드위치 측정법(Sandwich assay) 및 이들의 조합들로 이루어진 군으로부터 선택되는 방법을 이용하여 수행되는 것인, 성인형 스틸병 진단용 키트.
- 성인형 스틸병이 의심되는 개체의 시료에서 TLR2 또는 TLR7을 발현하는 세포의 빈도를 측정하는 단계;상기 측정된 빈도를 정상 대조군 시료의 빈도와 비교하는 단계; 및상기 대조군에 비하여 상기 성인형 스틸병이 의심되는 개체의 시료에서 상기 TLR2 또는 TLR7을 발현하는 세포의 빈도가 증가한 경우, 성인형 스틸병으로 진단하는 단계;를 포함하는 성인형 스틸병 진단을 위한 정보제공 방법.
- 제 10 항에 있어서,상기 시료는 개체로부터 분리된 전혈, 혈청, 혈장, 조직, 타액, 눈물, 소변, 객담, 림프액, 뇌척수액, 세포간액, 골수액, 복수 및 이들의 조합들로 이루어진 군으로부터 선택되는 것을 포함하는 것인, 성인형 스틸병 진단을 위한 정보제공 방법.
- 제 10 항에 있어서,상기 시료는 개체로부터 분리된 말초혈액 단핵세포(peripheral blood mononuclear cell, PBMC)인 것인, 성인형 스틸병 진단을 위한 정보제공 방법.
- 성인형 스틸병이 의심되는 개체의 시료에서 TLR2 또는 TLR7을 발현하는 세포의 빈도를 측정하는 단계;상기 개체에 성인형 스틸병 치료제 후보 물질을 처리하는 단계; 및상기 성인형 스틸병 치료제 후보 물질의 치료 효과를 측정하는 단계;를 포함하는 성인형 스틸병 치료제의 스크리닝 방법.
- 제 13 항에 있어서,상기 치료 효과를 측정하는 단계는 상기 치료제 후보 물질을 처리한 개체의 시료에서 TLR2 또는 TLR7을 발현하는 세포의 빈도를 측정하는 단계;상기 치료제 후보 물질을 처리한 개체의 시료에서 측정된 상기 TLR2 또는 TLR7을 발현하는 세포의 빈도를 상기 치료제 후보 물질을 처리하는 단계 전 측정된 상기 TLR2 또는 TLR7을 발현하는 세포의 빈도와 비교하는 단계; 및상기 치료제 후보 물질을 처리하는 단계 전 측정된 상기 TLR2 또는 TLR7을 발현하는 세포의 빈도에 비하여 상기 치료제 후보 물질을 처리한 개체의 시료에서 측정된 상기 TLR2 또는 TLR7을 발현하는 세포의 빈도가 감소한 경우 상기 치료제 후보 물질을 성인형 스틸병 치료제로 고려하는 단계;를 포함하는 것인, 성인형 스틸병 치료제의 스크리닝 방법.
- 제 13 항에 있어서,상기 시료는 개체로부터 분리된 전혈, 혈청, 혈장, 조직, 타액, 눈물, 소변, 객담, 림프액, 뇌척수액, 세포간액, 골수액, 복수 및 이들의 조합들로 이루어진 군으로부터 선택되는 것을 포함하는 것인, 성인형 스틸병 치료제의 스크리닝 방법.
