Description
この発明は,miR-140を含む医薬組成物及びその製造方法に関する。
特許6726726号公報には,miR-140-3pを含むエキソソームを有する医薬組成物(請求項2)が記載されている。この公報のエキソソームは,心筋球由来細胞(CDC)から回収されたものである。特許第6035010号公報や特許第5395439号公報にもmiR-140を含む医薬組成物が記載されている。
この発明は,miR-140を含むエキソソームを有効成分として含む医薬組成物や多くmiR-140を含むエキソソームを医薬組成物の製造方法を提供することを目的とする。
最初の発明は,耳介軟骨細胞由来のエキソソームを含む医薬組成物に関する。そのエキソソームはmiR-140を含む。
次の発明は,上記の医薬組成物を製造する方法に関する。
この方法は,耳介軟骨細胞からエキソソームを得るエキソソーム取得工程を含む。

以下,本発明を実施するための形態について説明する。本発明は,以下に説明する形態に限定されるものではなく,以下の形態から当業者が自明な範囲で適宜修正したものも含む。

エキソソームは,エクソソームともよばれる。エキソソームは細胞から放出される核を持たない脂質二重膜粒子である。エキソソームは,mRNAやmiRNAを含む核酸物質を内包する。例えば,特許第6524162号公報には,間葉系幹細胞エキソソームを有効成分とする医薬組成物が記載されている。このようにエキソソームは各種疾患の治療用又は予防用組成物の有効成分として利用されている。

医薬組成物の対象疾患の例は,上記の特許文献において紹介される通り,固形癌や腫瘍である。その例は,脳腫瘍・咽頭癌・上顎癌・口腔癌・舌癌・口唇癌・甲状腺癌・肺癌・乳癌・食道癌・胃癌・大腸癌・肝細胞癌・胆道癌・膵臓癌・腎細胞癌・膀胱癌・子宮頸癌・子宮体癌・卵巣癌・皮膚癌・悪性黒色腫・骨髄腫瘍・骨肉腫・軟部腫瘍・血管肉腫・小児固形腫瘍」である。医薬組成物は,変形性関節症の治療にも有効であると考えられる。
医薬組成物の対象疾患の例は,上記の特許文献において紹介される通り,固形癌や腫瘍である。その例は,脳腫瘍・咽頭癌・上顎癌・口腔癌・舌癌・口唇癌・甲状腺癌・肺癌・乳癌・食道癌・胃癌・大腸癌・肝細胞癌・胆道癌・膵臓癌・腎細胞癌・膀胱癌・子宮頸癌・子宮体癌・卵巣癌・皮膚癌・悪性黒色腫・骨髄腫瘍・骨肉腫・軟部腫瘍・血管肉腫・小児固形腫瘍」である。医薬組成物は,変形性関節症の治療にも有効であると考えられる。
医薬組成物の成分
医薬組成物は公知の担体(例えば,水,生理食塩水,クリーム,賦形剤)を含んでもよい。また,医薬組成物は,公知の材料を適宜含んでもよい。また,例えば,医薬組成物は,公知の抗がん剤を含んでもよい。有効成分を適宜担体と混合することで医薬組成物を得ることができる。
医薬組成物は公知の担体(例えば,水,生理食塩水,クリーム,賦形剤)を含んでもよい。また,医薬組成物は,公知の材料を適宜含んでもよい。また,例えば,医薬組成物は,公知の抗がん剤を含んでもよい。有効成分を適宜担体と混合することで医薬組成物を得ることができる。
本発明の用医薬組成物の投与経路は限定されず、全身投与であっても、局所投与であってもよい。
本発明の用医薬組成物の剤型は、前記投与経路に応じて適宜決定してよく、限定はされないが、注射剤、点滴剤、錠剤、カプセル剤、細粒剤、散剤、液剤、シロップ等に溶解した水剤、貼付剤、座薬剤等を挙げることができる。
本発明の医薬組成物を投与する対象は限定されず、例えば、哺乳動物(ヒト、ブタ、ウシ、サル、ヒヒ、イヌ、ネコ、ラット、マウス等)に対して本発明を用いることができる。
本発明の医薬組成物の対象への投与方法(投与経路、投与量、1日の投与回数、投与のタイミング、等)は限定されず、対象の健康状態、疾患の程度、併用する薬剤の種類等に応じて、当業者(例えば、医師)が適宜決定することができる。miR-140の投与量の例は1μg以上1mg以下/日である。
次の発明は,上記の医薬組成物を製造する方法に関する。この方法は,耳介軟骨細胞からエキソソームを得るエキソソーム取得工程を含む。
エキソソーム取得工程の好ましい例は,耳介軟骨細胞の培養工程と,エキソソーム採取工程とを含む。
耳介軟骨細胞の培養工程は,耳介軟骨細胞を培養し,培養軟骨細胞を得るための工程である。耳介軟骨細胞を培養する培地にエストロゲンを含むことが好ましい。エストロゲンは,エストラジオールを含むものが好ましい。耳介軟骨細胞は、例えば、人や動物等から得た耳介から、皮膚、結合織、軟骨膜を除去したうえで、5mm×5mm程度の小片に細切した後、コラゲナーゼ処理することにより、耳介軟骨細胞を単離することができる。単離した耳介軟骨細胞をそのまま用いてもよいし、適宜培養した後に医薬組成物に用いてもよい。

モネンシン
モネンシンは、ストレプトマイセス属(genus Streptomyces)からバクテリアで単離されたポリエーテル系抗生物質である。この分子は、コクシジウム型感染症を予防するために、反芻動物飼料(商標Rumensin(登録商標))の添加物として非常に広く使用されている。モネンシンの塩の例は,ナトリウム塩,カリウム塩である。

エキソソーム取得工程の好ましい上記とは別の例は,耳介軟骨細胞の培養工程と,不死化工程と,エキソソーム採取工程とを含む。
不死化工程は,耳介軟骨細胞の培養工程で培養された培養軟骨細胞を不死化し不死化細胞を得るための工程である。
不死化工程
培養軟骨細胞を不死化するためには,レンチウイルス感染を引き起こせばよい。例えば,不死化工程は,SV40又はhTERTを用いて培養軟骨細胞を不死化する工程を含むものが好ましい。レンチウイルスを用いた不死化は,例えば特許第6302356号公報,及び特許第6868565号公報に記載されている。これらの特許文献や実施例の記載を参考にして,培養軟骨細胞を不死化すればよい。
培養軟骨細胞を不死化するためには,レンチウイルス感染を引き起こせばよい。例えば,不死化工程は,SV40又はhTERTを用いて培養軟骨細胞を不死化する工程を含むものが好ましい。レンチウイルスを用いた不死化は,例えば特許第6302356号公報,及び特許第6868565号公報に記載されている。これらの特許文献や実施例の記載を参考にして,培養軟骨細胞を不死化すればよい。
この例では,エキソソーム採取工程は,不死化細胞からエキソソームを採取するための工程である。エキソソーム採取工程は,モネンシン又はモネンシンの塩を用いて不死化細胞からエキソソームを採取する工程を含むものが好ましい。
この明細書は,ヒト(例えば患者)に上記した医薬組成物を投与する工程を含む,対象疾患の治療方法や予防方法をも提供する。
ヒト耳介軟骨細胞の不死化
レンチウイルス感染(Lentiviral infection)
正常女性耳介軟骨細胞を感染24時間前に30-40% コンフルエントの状態に6-well plateに播種した。
翌日に、細胞をLenti-SV40 (LV613)とLenti-hTERT (LV615)のレンチウイルスでMOI 5の条件で感染させた。
72時間後に、細胞を継代しqRT-PCRで遺伝子の導入をチェックした。
感染及び非感染細胞から5X All-In-One MastermixとEXCellentCT Lysis Kit(G916)を用いてRNAを抽出して、そこからcDNAを合成した。
遺伝子導入の可否は、
SV40 Forward Primer:
5’GACTCAGGGCATGAAACAGG3’,(配列番号1)
SV40 Reverse Primer:
5’ ACTGAGGGGCCTGAAATGA 3’(配列番号2)
を用いて上記のcDNAをqRT-PCRで検出した。
その結果、非感染細胞には遺伝子の導入はなく、感染細胞には導入されていた。感染細胞の細胞形態は非感染細胞とあまり変わらなかった。しかし、継代培養を続けると、感染させなかった細胞は15継代で増殖が止まったが、感染させた細胞は25継代を過ぎても生きてた。以上から、感染は成功したと結論された。
レンチウイルス感染(Lentiviral infection)
正常女性耳介軟骨細胞を感染24時間前に30-40% コンフルエントの状態に6-well plateに播種した。
翌日に、細胞をLenti-SV40 (LV613)とLenti-hTERT (LV615)のレンチウイルスでMOI 5の条件で感染させた。
72時間後に、細胞を継代しqRT-PCRで遺伝子の導入をチェックした。

その結果、非感染細胞には遺伝子の導入はなく、感染細胞には導入されていた。感染細胞の細胞形態は非感染細胞とあまり変わらなかった。しかし、継代培養を続けると、感染させなかった細胞は15継代で増殖が止まったが、感染させた細胞は25継代を過ぎても生きてた。以上から、感染は成功したと結論された。
不死化軟骨細胞への各種エキソソーム放出増加剤添加及び培養上清の処理
5継代目の不死化軟骨細胞を継代し、75cm2フラスコに1フラスコ当たり1.5x106 個の細胞の割合で、4個のフラスコに播種した。このうち3個には10nM エストラジオール(エストラジオール)を添加し、1個には添加せずに培養を開始した。培養4日目に、エストラジオールを添加しているフラスコにエキソソーム分泌促進剤として各フラスコに10uM モネンシン(Monensin), 20nM アプリモド(Aplimod), 10uM D609を添加し、エストラジオールを添加していないフラスコをコントロールとして翌日まで培養を行った。翌日に各フラスコから培地を採取し、次に1,500rpm, 5分遠心した。この上清を更に0.22umの濾過フィルターで濾過し、エキソソーム画分としてmiRの分析まで-80°Cで保存した。

サンプルからのmiR抽出及び抽出核酸の確認
実施例2のエキソソーム画分からexoRNAeasy Serum/Plasma Maxi Kit (キアゲン)を用いてmicroRNAを抽出した。抽出されたRNAのA260/A280及びA260/A230を分光光度計Nano Drop One (サーモフィッシャーサイエンス社)を用いて測定し、このサンプルには蛋白及び塩の夾雑物がないことが判明した。さらにAgilent 2100 Bioanalyzer (Aligent Technologies)でサンプルの電気泳動を行い、ターゲットの核酸のみが含まれていることを確認した。以下の表のように、採取された核酸量はモネンシン処理が一番多くなることが判明した。
実施例2のエキソソーム画分からexoRNAeasy Serum/Plasma Maxi Kit (キアゲン)を用いてmicroRNAを抽出した。抽出されたRNAのA260/A280及びA260/A230を分光光度計Nano Drop One (サーモフィッシャーサイエンス社)を用いて測定し、このサンプルには蛋白及び塩の夾雑物がないことが判明した。さらにAgilent 2100 Bioanalyzer (Aligent Technologies)でサンプルの電気泳動を行い、ターゲットの核酸のみが含まれていることを確認した。以下の表のように、採取された核酸量はモネンシン処理が一番多くなることが判明した。
サンプル抽出核酸からのcDNA合成
抽出したRNAをもとに、miScript II RT Kit (カタログ番号:218160;キアゲン社)を使用して下記の条件でcDNA合成を行った。

リアルタイムPCRによるmiR-140発現の検討
上記で合成されたcDNAをもとに、miScript SYBR Green PCR Kit (カタログ番号218073 キアゲン社)を使用して下記の条件でリアルタイムPCR装置(LightCycler480:ロシュ社)にてリアルタイムPCR反応を行った。その時のmiR-140測定のためのプライマーはHs_miR-140_1 miScript Primer Assay(Cat No.MS00003500; has-miR-140-5p: MIMAT0000431)を使用した。
上記で合成されたcDNAをもとに、miScript SYBR Green PCR Kit (カタログ番号218073 キアゲン社)を使用して下記の条件でリアルタイムPCR装置(LightCycler480:ロシュ社)にてリアルタイムPCR反応を行った。その時のmiR-140測定のためのプライマーはHs_miR-140_1 miScript Primer Assay(Cat No.MS00003500; has-miR-140-5p: MIMAT0000431)を使用した。
その結果、下記の値を得た。したがって、モネンシン処理群が他より2倍近くのmiR-140を含んでいることが判明した。Cp値が低いことは、早く検出されたことを意味し、多くのmiR-140を含んでいることを意味する。
考察
従来の抗癌剤は1種で1つの標的分子を抑制することでその抗癌作用が現われていた。一方、1つの分子で多種類の標的分子種に作用するという新しい生体内物質が見出され始めた。この中で、micro-RNA(miR)は種々の蛋白質の転写や翻訳を抑制することが分かってきたが、その中で癌に増殖抑制に有効なデータが実験的に示されているmiRがmiR-140である。このmiRは細胞からExosomeの形で分泌され、その内容物は血中でも分解されることなく周囲の細胞に取りこまれ、取り込んだ細胞では、その支配下にある蛋白質の生合成が停止する。その結果、その蛋白質が癌の増殖に必要なものであれば、取り込んだ癌細胞は増殖を止めることとなり、結果として癌治療に繋がる。また、Exosomeは血液脳関門を通過できるので脳関連の腫瘍にも有効に使用できる。

種々の癌細胞にmiR-140を増加させた場合、現在までに実験的に脳腫瘍・咽頭がん・上顎がん・口腔がん・舌癌・口唇癌・甲状腺がん・肺がん・乳がん・食道がん・胃がん・大腸がん・肝細胞癌・胆道癌・膵臓癌・腎細胞がん・膀胱がん・子宮頸がん・子宮体癌・卵巣がん・皮膚がん・悪性黒色腫・骨髄腫瘍・骨肉腫・軟部腫瘍・血管肉腫・小児固形腫瘍等の幅広い癌に有効と報告されている。ここに作用するmiR-140の癌標的分子は、42種類が報告されている。その癌標的分子は、Muc1,Flap,CTSB,Yes,Wnt-1,Glut1,THY1,GRN,TGFBR,IGF1R,CeMP,Six1,Tripartivemotif28,ORC1,TRPM2,Brd9,cMet,CADM3,PD-L1,ZEB1,mTor,VEGF-A,MMP2,Pin1,HMGN,IL6,ADAMSTS5,IGFBP5,IGF2BP,IP3K,ADAM9,Sox4,Septin2,iASP,ADAM10,ATP8A,PDGFRA,MMO,FGF9等が挙げられている。この中のPD-L1とかVEGFは既に抗体癌医薬として使用されているが、分子作用を抗体で抑えることと、その分子の生合成を抑制することは効果に劇的な隔たりが生ずるものと推定される。しかも、この2つの標的分子でも高価な抗体医薬が2種類も必要となる。また、抗体医薬では細胞外にある分子か、または細胞表面にある分子に対してしか抗体を造ることができない重大な欠点を持つ。一方、このmiR/Exosome薬は細胞内の種々の癌作用点を司る酵素・転写因子・シグナル因子等の蛋白質を抑制することができる。
さて、癌の増殖・転移・生存に必要なこれらの標的分子を一度に抑制することは、今までの一点にのみ効果がある分子標的薬とは抗癌効果が明らかに向上する可能性が高く、また標的癌の種類も多種に及ぶこと、及び血液脳関門も通過するmiR-140含有Exosomeは標的器官に着くまでは分解されないので極めて有効な癌治療薬と成り得る。この時は2種類の投与法が考えられる。1つは静脈注射であるが、その場合は体内全般には行き渡るが、患部である関節内への移行率は低いと考えられる。癌治療の場合、特に既に全身に転移している場合は脳への転移も考えて静脈注射が相応しい。しかし、未だ転移していない場合は患部局所への投与が好ましい。
一方、老化に伴って増加する変形関節症の治療は鎮痛薬を使用して、その痛みを止めることを主とする対症療法であり、原因を治す薬は未だ開発されていない。しかし最近、変形関節症ではmiR-140の減少が起こっていることが報告され始めた。更に、miR-140を含有するExosomeの投与は変形関節症の症状回復に有効であることが報告されている。この変形性関節症での投与法は、関節腔内への投与が局所濃度をあげるためにも好ましいと考えられている。
一方、老化に伴って増加する変形関節症の治療は鎮痛薬を使用して、その痛みを止めることを主とする対症療法であり、原因を治す薬は未だ開発されていない。しかし最近、変形関節症ではmiR-140の減少が起こっていることが報告され始めた。更に、miR-140を含有するExosomeの投与は変形関節症の症状回復に有効であることが報告されている。この変形性関節症での投与法は、関節腔内への投与が局所濃度をあげるためにも好ましいと考えられている。
問題は、このmiR-140を含むExosomeを何処から定常的に得るかということである。まず、第一に関節軟骨がmiR-140を産生していることが報告されており、しかも、このmiRを含有するExosomeを変形関節症に投与すると治癒効果が大きいことが報告されている。そこで、発明者は未だmiR-140の発現が報告されていないヒト耳介軟骨細胞からmiR-140含有Exosomeを採取することとした。また、miR-140はEstradiol(女性ホルモン)によって産生を上げることが報告されていることから、出発の軟骨細胞は女性由来のものを用いた。この事により、女性ホルモン受容体を持つ細胞を得ることとなり、培養系にEstradiolを添加することによりmiR-140の産生を増やすことを可能とした。次に定常的にこの細胞からExosomeを採取するために、この細胞をLenti-SV40およびLenti-hTERTを用いて不死化した。最終的にこの細胞からExosomeを多量に採取するために、3種のExosome放出促進剤を添加してExosomeの採取を行い、そこからのmiR-140の存在量を定量した。その結果、今までに報告のない耳介軟骨細胞にmiR-140が多量に且つ安定的に発現する方法を決定できた。以上から得られるmiR-140をExosomeは冷凍で長期間保存できるので薬物として有効な長所を有する。
この発明は、1剤で多種の癌種に効果を及ぼすExosome抗癌製剤の提供及び変形性軟骨症(OA)に対し軟骨細胞再生力を持つ初の関節腔注入型Exosome製剤を提供できることとなる。
