Abstract
본 발명은 비알코올 지방간염을 진단할 수 있는 바이오마커 miRNA-4449과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA에 관한 것으로서, 비알코올 지방간 질환 중 지방간염은 단순 지방증과 달리 예후가 좋지 아니한바, 본 발명은 환자의 혈청에서 상기 miRNA의 수준을 측정함으로써 비알코올 지방간염의 정확한 진단 결과를 제공하여 종래 침습적 검사방법에 의한 부작용을 배제할 수 있고 간단하고 경제적으로 지방간염의 조기 진단과 적절한 치료가 가능하게 한다.
Description
본 발명은 비침습적 체외 진단을 위한 비알코올 지방간염 진단용 바이오마커 조성물 등에 관한 것으로서, 보다 구체적으로 비알코올 지방간 질환 중에서 단순 지방간과 지방간염을 구별하여 진단하기 위한 바이오마커로서의 microRNA (miRNA) 조합에 관한 것이다.
비알코올 지방간 질환(Nonalcoholic fatty liver disease, NAFLD)은 영상의학 검사나 간 조직검사에서 간 조직에 지방 침착이 나타나는 경우, 이러한 지방간의 원인으로서 알코올, 약물 및 다른 요인들이 배제되었을 때 진단할 수 있는 질환이다. 병리 기전상 단순 지방간(simple steatosis)에서 비알코올 지방간염(Nonalcoholic steatohepatitis, NASH)을 거쳐 비알코올 지방간 연관 간경변증으로 진행할 수 있는데 비알코올 지방간 질환은 이러한 일련의 질환을 모두 포괄하는 용어이다. 국내 연구에서의 비알코올 지방간 질환 유병률은 진단 방법에 따라 큰 차이가 있다. 비알코올 지방간질환을 간 조직검사로 확진할 때 유병률은 51.4%라는 연구 결과가 있으며 초음파검사로 진단한 경우 그보다 낮은 16~33%정도로 나타난 바 있다.
간 조직검사는 비용이 비싸고, 해석하는데 있어서 전문가가 필요하며, 합병증의 위험이 있기 때문에, 2018년 미국간학회 가이드라인에서는 비알코올 지방간 질환 선별검사로서 간 조직검사를 권고하지 않고, 지방간염이나 진행된 간 섬유화증이 의심되는 경우, 다른 질환의 배제가 필요한 경우에만 권고하고 있다. 하지만, 비알코올 지방간 질환 환자군은 정상 대조군에 비해 전체 사망률이 높고, 특히 지방간염으로 진행한 경우 심혈관 질환 사망률이 증가한다. 비음주자들의 건강검진에서 복부 영상검사상 지방간 질환이 관찰되고 간 기능 검사가 이상을 보이는 경우가 많으므로 비알코올 지방간질환의 가능성을 염두에 두고 진단과 치료법에 대해 생각해 볼 필요가 있다.
비알코올 지방간 질환은 조직학적인 염증이나 섬유화 여부가 예후에 큰 영향을 주기 때문에 아직까지는 간 조직검사가 기준 진단 검사이다. 그러나, 간 조직검사가 갖고 있는 고비용, 침습성 및 합병증 발생의 위험 때문에 일차 의료에서 이용하기에는 무리가 있다. 또한, 적은 간 조직으로 전체 간 상태를 반영할 수 없고 병리 검사자 간의 진단 일치도가 낮을 수 있다. 그러므로 간 조직검사 대신 영상의학 검사와 생화학 검사를 통해 비알코올 지방간 질환의 조직학적 중증도를 평가하려는 노력이 많이 진행되어 왔다.
이에, 비알코올 지방간 질환의 진단과 중증도 구분을 위한 비침습적인 방법에 대한 관심이 증가하고 있으나, miRNA를 이용한 연구는 아직도 많이 부족한 실정이다. 종래 PCR을 이용하여 miRNA 분석을 하고, 이를 진단에 이용하고자 하는 시도가 있었으나, 수 천개의 miRNA 중 일부를 선택하여 PCR을 통하여 분석한 결과라는 한계가 있다. 또한, 국내 연구진에 의하여 비알코올 지방간염 진단을 위한 특허가 출원되어 있으나, 이는 동물실험을 통한 결과라는 한계가 있다.
이에, 본 발명자들은 현장에서 즉시 적용 가능한 진단방법으로서 비알코올 지방간 질환 환자 중에서도 지방간염으로 진행된 환자를 구분하기 위한 비침습적 진단방법에 대해 예의연구하여 본 발명을 완성하였다.
본 발명이 이루고자 하는 기술적 과제는 비알코올 지방간 질환 중에서 단순 지방간과 지방간염을 감별할 수 있는 바이오마커로서 miRNA-4449과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA를 추가로 제공하는 것이다.
또한, 본 발명은 비알코올 지방간 질환 중에서 단순 지방간과 지방간염을 감별하여 진단할 수 있는 조성물 및 키트를 제공하는 것을 목적으로 한다.
또한, 본 발명은 miRNA-4449의 발현 또는 활성을 저해하는 물질과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현 또는 활성을 저해하는 물질을 포함하는 비알코올 지방간염의 예방 또는 치료용 약학적 조성물 제공을 목적으로 한다.
그러나 본 발명이 이루고자 하는 기술적 과제는 이상에서 언급한 과제에 제한되지 않으며, 언급되지 않은 또 다른 과제들은 아래의 기재로부터 당해 기술분야의 통상의 기술자에게 명확하게 이해될 수 있을 것이다.
상기 과제를 해결하기 위하여, 본 발명은 miRNA-4449를 포함하고, miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA를 추가로 포함하는, 비알코올 지방간 질환 중 단순 지방간과 지방간염 감별진단용 바이오마커를 제공한다.
상기 바이오마커는 miRNA-4449, miRNA-21, miRNA-151, 및 miRNA-192의 조합인 것이 바람직하며, 더욱 바람직하게는 miRNA-4449, miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192의 조합일 수 있다.
또한, 본 발명은 상기 바이오마커의 발현 수준을 측정할 수 있는 제제, 즉, miRNA-4449의 발현 수준을 측정할 수있는 제제와 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현 수준을 측정할 수 있는 제제를 추가로 포함하는 비알코올 지방간 질환 중 단순 지방간과 지방간염 감별진단용 조성물을 제공한다.
본 발명의 감별진단용 조성물은 miRNA-4449, miRNA-21, miRNA-151, 및 miRNA-192의 발현 수준을 측정할 수 있는 제제를 포함하는 것이 바람직하며, 더욱 바람직하게는 miRNA-4449, miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192의 발현 수준을 측정할 수 있는 제제를 포함할 수 있다.

본 발명의 다른 구현예로서, 상기 방법은 상기 측정하는 단계 이후에 측정된 miRNA 발현수준과 단순 지방간 환자에서 상기 측정된 miRNA와 상응하는 miRNA의 발현수준을 비교하여 단순 지방간 환자보다 상기 측정된 miRNA의 발현 수준이 높은 경우 비알코올 지방간염으로 판정하는 단계를 추가로 포함할 수 있으며, 비알코올 지방간염으로 판정하기 위한 miRNA의 발현 수준은 단순 지방간 환자와 비교하여 1.5배 이상인 것일 수 있고, 바람직하게는 2배 이상인 것일 수 있다.
또한, 본 발명은 miRNA-4449의 발현을 억제하는 제제 및 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현을 억제하는 제제를 추가로 포함하는, 비알코올 지방간염의 예방 또는 치료용 약학적 조성물을 제공한다.
본 발명의 일 구현예로서, 상기 제제는 siRNA, 앱타머(aptamer), 안티센스 올리고뉴클레오티드, 및 화합물로 이루어진 군으로부터 선택되는 1종 이상일 수 있다.
또한, 본 발명은 비알코올 지방간염 예방 또는 치료용 약제의 제조를 위한, miRNA-4449와 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 용도를 제공한다.
또한, 본 발명은 miRNA-4449의 발현을 억제하는 제제 및 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현을 억제하는 제제를 추가로 개체에 투여하는 단계를 포함하는 비알코올 지방간염 예방 또는 치료방법을 제공한다.
또한 본 발명은 하기 단계를 포함하는, 비알코올 지방간염의 예방 또는 치료용 약제의 스크리닝 방법을 제공한다.
(1) 간 세포에 후보물질을 처리하는 단계; (2) 상기 세포의 miRNA-4449 발현 수준과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA 발현 수준을 측정하는 단계; 및 (3) 상기 (2) 단계에서 측정한 miRNA 발현 수준이 후보물질 처리 전보다 감소한 경우 상기 후보물질을 비알코올 지방간염의 예방 또는 치료용 약제로 선정하는 단계.
본 발명은 비알코올 지방간염을 진단할 수 있는 바이오마커 miRNA-4449과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA에 관한 것으로서, 비알코올 지방간 질환 중 지방간염은 단순 지방증과 달리 예후가 좋지 아니한바, 본 발명은 환자의 혈청에서 상기 miRNA의 수준을 측정함으로써 비알코올 지방간염의 정확한 진단 결과를 제공하여 종래 침습적 검사방법에 의한 부작용을 배제할 수 있고 간단하고 경제적으로 지방간염의 조기 진단과 적절한 치료가 가능하게 한다.
도 1a는 비알코올 지방간염에서 발현량이 증가한 8가지 miRNA 각각의 비알코올 지방간염 진단능력을 나타낸 것이다.
도 1b는 miRNA 4가지 조합(miRNA-4449, miRNA-21, miRNA-151, 및 miRNA-192)과 miRNA 8가지 조합(miRNA-4449, miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192)에 따른 비알코올 지방간염 진단능력을 나타낸 것이다.
본 발명자들은 간 조직검사를 대체할 수 있는 비알코올 지방간염의 진단을 위한 비침습적 검사방법에 대해 예의 연구한 결과 단순 지방간 환자에 비하여 비알코올 지방간염 환자에서 miRNA-4449, miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192이 과발현 됨을 확인하여 본 발명을 완성하였다.
이에, 본 발명은 비알코올 지방간 질환 환자 중에서 지방간염으로 진행을 확인할 수 있는 바이오마커로서 miRNA-4449과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA를 추가로 포함하는. miRNA의 조합을 제공한다.
또한, 본 발명은 환자의 생물학적 시료로부터 miRNA-4449의 발현 수준을 측정할 수 있는 제제와 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현 수준을 측정할 수 있는 제제를 추가로 포함하여 비알코올 지방간 질환 환자 중에서 단순 지방간과 지방간염을 감별 진단할 수 있는 조성물을 제공한다.
본 발명에서 “진단”이란 특정 질병 또는 질환에 대한 한 개체의 감수성(susceptibility)을 판정하는 것, 한 개체가 특정 질병 또는 질환을 현재 가지고 있는지 여부를 판정하는 것, 특정 질병 또는 질환에 걸린 한 개체의 예후(prognosis)를 판정하는 것, 또는 테라메트릭스(therametrics)(예컨대, 치료 효능에 대한 정보를 제공하기 위하여 개체의 상태를 모니터링하는 것)을 포함한다.
본 발명에서 “감별”이란 특정 질환 상태를 진단, 판단 또는 판별하는 것을 말하며, 특히 임상적으로 유사한 특징을 일부 공유하는 질환들 가운데 어떤 질환인지를 정확하게 구별해낸다는 의미에서, 본 발명에서는 "질환을 감별한다"는 용어를 사용하였다. 그러나, 특정 질환을 확인한다는 측면에서, 상기 용어는 진단, 판단, 판별, 식별 등의 용어와 혼용될 수도 있다.
본 발명의 목적상, 본 발명에서 감별은 비알코올 지방간 질환 중에서 단순 지방간인지 비알코올 지방간염인 지를 구분하는 것을 의미한다. 단순 지방간은 간 내에 중성지방이 축적되는 단순 양성 질환을 의미하나, 비알코올 지방간염은 지방간과 함께 염증 또는 섬유화를 동반하는 것을 특징으로 하는 진행성 간질환을 말하는 것으로, 양 질환의 예후와 치료 요법의 적용 등이 상이하기 때문에, 단순 지방간인지 비알코올 지방간염인지를 구분하는 것이 중요하다.
보다 바람직하게, 본 발명에서 감별이란, 비알코올 지방간 질환 환자를 대상으로 하여 단순 지방간 상태에 머물러 있는지 아니면 비알코올 지방간염으로 발전했는지를 확인하는 것, 즉, 단순 지방간으로부터 비알코올 지방간염을 구별하는 것을 의미할 수 있다.
본 발명에서는 단순 지방간으로부터 비알코올 지방간염을 감별하기 위하여, 비침습적인 도구로서 마이크로RNA(miRNA) 바이오마커를 사용함을 특징으로 한다.
보다 구체적으로, 본 발명은 시료 내 존재하는 miRNA-4449과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현 수준을 측정하여, 상기 miRNA가 상대적으로 과발현된 경우 해당 시료의 환자가 비알코올 지방간염인 것으로 감별하는 기술을 제공한다. 본 발명에서 상기 miRNA가 상대적으로 과발현된 경우란 단순 지방간 환자에서 상기 측정한 miRNA와 상응하는 miRNA의 발현량을 기준으로 하며, 해당 시료의 환자가 비알코올 지방간염인 것으로 판정하기 위해서는 단순 지방간 환자의 miRNA의 발현량 보다 약 1.5배 이상의 miRNA 발현 수준을 나타내야 하며, 바람직하게는 약 2배 이상의 과발현을 확인하여야 한다.
시료 내 존재하는 miRNA-4449의 발현 수준과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현 수준을 측정하기 위해서 miRNA를 검출할 수 있는 각종 제제들을 사용할 수 있다.
또한, 본 발명의 단순 지방간과 비알코올 지방간염 감별 진단용 키트는 생물학적 시료로부터 miRNA-4449의 발현 수준을 측정할 수 있는 제제와 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현 수준을 측정할 수 있는 제제를 추가로 포함할 수 있으며, 상기 제제란 miRNA의 발현 수준을 확인함으로써 본 발명의 바이오마커 검출에 사용될 수 있는 분자를 의미한다.
본 발명에서 상기 miRNA의 발현 수준을 측정할 수 있는 제제로서 miRNA에 특이적으로 결합하는 안티센스 올리고뉴클레오티드, 프라이머, 프로브, 및/또는 항체를 사용할 수 있으며, 유전자 발현 정도를 측정하는데 이용되는 공지의 방법, 예를 들면 프라이머를 이용하여 PCR을 수행하거나, 프로브를 이용한 혼성화 반응을 통하여 실시하여 miRNA의 발현 수준을 측정할 수 있다. 또한, 상기 키트에는 상기 측정할 수 있는 제제 이외에도 검출을 용이하게 하는 당업계에 공지된 여러 도구, 시약, 예를 들어 적합한 담체, 검출 가능한 신호를 생성할 수 있는 표지 물질, 안정화제 등이 포함될 수 있다.
본 발명에서 생물학적 시료는 비침습적 시료일 수 있으며, 상기 비침습적 시료는 혈액, 혈청, 소변, 또는 타액일 수 있고, 바람직하게는 혈액 또는 혈청일 수 있고, 더욱 바람직하게는 혈청일 수 있으나, 이에 제한되는 것은 아니다.
본 발명에서 miRNA는 이를 구성하는 올리고뉴클레오티드의 작용성 등가물, 예를 들어, 상기 miRNA 올리고뉴클레오티드의 일부 염기서열이 결실(deletion), 치환(substitution) 또는 삽입(insertion)에 의해 변형되었지만, 상기 miRNA 핵산분자와 기능적으로 동일한 작용을 할 수 있는 변이체(variants) 및 모방체(mimic)를 포함하는 개념이다.
본 발명에서 "안티센스 올리고뉴클레오티드"는 상기 miRNA에 상보적으로 결합하여 발현을 억제하는 염기서열로서, 이에 제한되지는 않으나 antisenseRNA, antagonist miRNA를 포함한다.
본 발명에서 “프라이머”는 짧은 자유 수산화기를 가지는 핵산서열로서 상보적인 템플레이트와 염기쌍을 형성할 수 있고 템플레이트 가닥 복사를 위한 시작 지점으로 기능하는 짧은 핵산서열을 말한다. 본 발명의 프라이머는 예를 들면, 포스포르아미다이트 고체 지지체 방법과 같은 당 분야에 공지된 방법을 이용하여 화학적으로 합성할 수 있다.
본 발명에서 “프로브”는 miRNA와 특이적으로 결합할 수 있는 수개 내지 수백개의 염기로 이루어진 RNA 또는 DNA 등의 핵산 단편을 의미하며 라벨링되어 있어 특정 miRNA의 존재유무를 확인할 수 있다. 프로브는 올리고뉴클레오타이드 프로브, 단쇄 DNA 프로브, 이중쇄 DNA 프로브, RNA 프로브 등의 형태로 제작될 수 있고 비오틴, FITC, 로다민, DIG 등으로 표지되거나 방사선 동위 원소 등으로 표지될 수 있다.
본 발명에서 키트는 RT-PCR을 수행하기 위해 필요한 필수 요소를 포함하는 키트일 수 있으나, 이에 제한되지 않는다. 일 예로서 RT-PCR 키트의 경우, 마커 유전자에 대한 특이적인 각각의 프라이머 쌍 외에도 테스트 튜브 또는 다른 적절한 컨테이너, 반응 완충액, 데옥시뉴클레오티드(dNTPs), Taq-중합효소 및 역전사효소, DNase, RNase 억제제, DEPC-물(DEPC-water), 멸균수 등을 포함할 수 있다.
또한, 본 발명은 환자의 생물학적 시료에서 miRNA-4449의 발현 수준과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현 수준을 측정하는 단계를 포함하는 비알코올 지방간 질환 중 단순 지방간과 지방간염을 감별하여 진단하기 위한 정보제공방법을 제공할 수 있으며, 상기 정보제공방법은 상기 측정된 miRNA의 발현 수준을 단순 지방간 환자의 발현 수준과 비교하여 이보다 높은 경우 지방간염으로 판정하는 단계를 추가로 포함할 수 있다.
상기 miRNA 발현 수준 측정은 RT-PCR, 경쟁적 RT-PCR(Competitive RT-PCR), 실시간 RT-PCR(Real-time RT-PCR), 정량적 중합효소반응(quantitative RT-PCR), RNase 보호 분석법(RPA; RNase protection assay), 노던 블롯팅(Northern blotting), DNA 칩, DNA-silver nanoclusters(DNA/AgNC) 센서, 또는 직접적으로 결합하여 발현량을 측정하는 키트 방식으로 수행될 수 있으나, 이에 제한되는 것은 아니다.
본 발명에 있어서 상기 miRNA의 발현 억제제는 상기 miRNA에 특이적인 억제제를 사용하는 것을 특징으로 하며, 상기 발현 억제제는 상기 miRNA의 발현량을 감소시키거나 분해를 촉진함으로써 세포 내 상기 miRNA의 수준을 감소시키거나 상기 miRNA와 특이적으로 결합하여 그 활성을 감소시키는 것일 수 있으며, siRNA, 앱타머(aptamer), 안티센스 올리고뉴클레오티드 및 화합물로 구성된 군에서 선택되는 것을 특징으로 한다. 상기 화합물은 항산화제, 세포성장에 영향을 미치는 물질 및 세포신호전달물질 중 miRNA의 발현을 억제시키는 화합물이라면 어느 것이든지 사용할 수 있다. 하지만 이에 한정되지 않으며 상기 miRNA의 발현을 억제하는 물질이라면 어느 것이든지 사용할 수 있다.
본 발명에서 "siRNA"는 이중가닥의 RNA가 다이서에 의해 절단되어 생성되는 21-25 뉴클레오티드 크기의 작은 RNA 조각으로 상보적인 서열을 갖는 miRNA에 특이적으로 결합하여 발현을 억제하는 것을 말한다. 본 발명의 목적상 miRNA-4449과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA에 특이적으로 결합하여 상기 miRNA의 발현을 억제하는 것을 의미한다. siRNA는 화학적으로 또는 효소학적으로 합성될 수 있다. siRNA의 제조방법으로는 특별히 한정되지 않으며, 당업계에 공지된 방법을 사용할 수 있다. 예를 들면, siRNA를 직접 화학적으로 합성하는 방법, 시험관 내(in vitro) 전사를 이용한 siRNA의 합성법, 시험관 내(in vitro) 전사에 의해 합성된 긴 이중-가닥 RNA를 효소를 이용하여 절단하는 방법, shRNA 발현 플라스미드나 바이러스성 벡터의 세포내 전달을 통한 발현법 및 PCR(polymerase chain reaction) 유도 siRNA 발현 카세트(cassette)의 세포 내 전달을 통한 발현법 등이 있으나 이에 한정되는 것은 아니다.
본 발명에 있어서, 상기 miRNA 억제제의 세포 내로의 도입은 미세 주입법, 칼슘 포스페이트 공동-침전법, 전기천공법 및 리포좀 이용법 등을 이용하여 실시할 수 있으나, 이에 제한되는 것은 아니다.
본 발명에서 “예방”이란 본 발명에 따른 조성물의 투여에 의해 간 조직의 염증과섬유화 또는 그에 따른 이상을 지연시키는 모든 행위를 의미하고, “치료”란 본 발명에 따른 약학적 조성물의 투여에 의해 간 조직의 염증과 섬유화와 그에 따른 대사 이상 증세가 호전되거나 이롭게 변경되는 모든 행위를 의미한다. 또한, 본 발명의 비알코올 지방간염 치료용 약학적 조성물의 투여 대상 개체는 포유류라면 제한되지 아니하나, 바람직하게는 인간일 수 있고, 보다 구체적으로 비알코올 지방간질환 환자일 수 있다.
본 발명에서 약학적 조성물(pharmaceutical composition)은 약학적 조성물의 제조에 통상적으로 사용되는 적절한 담체, 부형제, 및 희석제를 더 포함할 수 있다.
본 발명에서 "담체(carrier)"란 비이클(vehicle)이라고도 불리며, 세포 또는 조직 내로의 단백질 또는 펩타이드의 부가를 용이하게 하는 화합물을 의미하는 것으로서, 예를 들어 디메틸술폭사이드(DMSO)는 생물체의 세포 또는 조직 내로의 많은 유기물의 투입을 용이하게 하는 통상 사용되는 담체이다.
본 발명에서 "희석제(diluent)"란 대상 단백질 또는 펩타이드의 생물학적 활성 형태를 안정화시킬 뿐만 아니라, 단백질 또는 펩타이드를 용해시키게 되는 물에서 희석되는 화합물로 정의된다. 버퍼 용액에 용해되어 있는 염은 당해 분야에서 희석제로 사용된다. 통상 사용되는 버퍼 용액은 포스페이트 버퍼 식염수이며, 이는 인간 용액의 염 상태를 모방하고 있기 때문이다. 버퍼 염은 낮은 농도에서 용액의 pH를 제어할 수 있기 때문에, 버퍼 희석제가 화합물의 생물학적 활성을 변형하는 일은 드물다. 여기에 사용된 아젤라산을 함유하는 화합물들은 인간 환자에게 그 자체로서, 또는 결합 요법에서와 같이 다른 성분들과 함께 또는 적당한 담체나 부형제와 함께 혼합된 약학적 조성물로서, 투여될 수 있다.
또한, 본 발명에 따른 비알코올 지방간염 예방 또는 치료용 약학적 조성물은 각각 통상의 방법에 따라 산제, 과립제, 정제, 캡슐제, 현탁액, 에멀젼, 시럽, 에어로졸 등의 외용제 및 멸균 주사용액의 형태로 제형화하여 사용될 수 있으며, 상기 조성물에 포함될 수 있는 담체, 부형제 및 희석제로는 락토즈, 덱스트로즈, 수크로스, 올리고당, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로즈, 메틸 셀룰로즈, 미정질 셀룰로스, 폴리비닐 피롤리돈, 물, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 탈크, 마그네슘 스테아레이트 및 광물유를 들 수 있다. 제제화할 경우에는 보통 사용하는 충진제, 증량제, 결합제, 습윤제, 분해제, 계면활성제 등의 희석제 또는 부형제를 사용하여 조제된다. 경구투여를 위한 고형제제에는 정제, 환제, 산제, 과립제, 캡슐제 등이 포함되며, 이러한 고형제제는 상기 화합물에 적어도 하나 이상의 부형제 예를 들면, 전분, 칼슘카보네이트(calcium carbonate), 수크로스(sucrose) 또는 락토오스(lactose), 젤라틴 등을 섞어 조제된다. 또한 단순한 부형제 이외에 마그네슘 스티레이트 탈크 같은 윤활제들도 사용된다. 경구를 위한 액상제제로는 현탁제, 내용액제, 유제, 시럽제 등이 해당되는데 흔히 사용되는 단순 희석제인 물, 리퀴드 파라핀 이외에 여러 가지 부형제, 예를 들면 습윤제, 감미제, 방향제, 보존제 등이 포함될 수 있다. 비경구 투여를 위한 제제에는 멸균된 수용액, 비수성용제, 현탁제, 유제, 동결건조제제, 좌제가 포함된다. 비수성용제, 현탁제로는 프로필렌글리콜 (propylene glycol), 폴리에틸렌 글리콜, 올리브 오일과 같은 식물성 기름, 에틸올레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기제로는 위텝솔(witepsol), 마크로골, 트윈 (tween) 61, 카카오지, 라우린지, 글리세로제라틴 등이 사용될 수 있다.
본 발명의 약학적 조성물은 경구 또는 비경구, 바람직하게는 비경구로 투여할 수 있고, 비경구 투여인 경우에는 근육 주입, 정맥내 주입, 피하 주입, 복강 주입, 국소 투여, 경피 투여 등으로 투여할 수 있다.
본 발명의 약학적 조성물의 적합한 투여량은 제제화 방법, 투여 방식, 환자의 연령, 체중, 성, 병적 상태, 음식, 투여 시간, 투여 경로, 배설 속도 및 반응 감응성과 같은 요인들에 의해 다양하게 처방될 수 있다.
본 발명의 약학적 조성물은 당해 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약학적으로 허용되는 담체 및/또는 부형제를 이용하여 제제화함으로써 단위 용량 형태로 제조되거나 또는 대용량 용기 내에 내입시켜 제조될 수 있다. 이때 제형은 오일 또는 수성 매질중의 용액, 현탁액 또는 유화액 형태이거나 엑스제, 분말제, 과립제, 정제 또는 캅셀제 형태일 수도 있으며, 분산제 또는 안정화제를 추가적으로 포함할 수 있다.
본 발명은 다양한 변환을 가할 수 있고 여러 가지 실시예를 가질 수 있는 바, 이하 특정 실시예들을 도면에 예시하고 상세한 설명에 상세하게 설명하고자 한다. 그러나, 이는 본 발명을 특정한 실시 형태에 대해 한정하려는 것이 아니며, 본 발명의 사상 및 기술 범위에 포함되는 모든 변환, 균등물 내지 대체물을 포함하는 것으로 이해되어야 한다. 본 발명을 설명함에 있어서 관련된 공지 기술에 대한 구체적인 설명이 본 발명의 요지를 흐릴 수 있다고 판단되는 경우 그 상세한 설명을 생략한다.
[실험 방법]
- miRNA의 비알코올 지방간염 진단능력 분석
조직검사를 통해서 비알코올 지방간 질환으로 진단된 환자 24명의 혈청을 수집하여 분석하였다. 24명 중 12명은 단순 지방간, 12명은 비알코올 지방간염 환자였다. 이들 환자로부터 수집된 혈청에서 RNA를 분리하여 miRNA의 발현을 분석하였다. miRNA의 발현 분석은 RNA sequencing을 통하여 진행되었고, 그 결과를 ROC curve로 도 1에 나타내었다.
[실험 결과]
단순 지방간 그룹과 비알코올 지방간염 그룹에서 miRNA 발현을 비교한 결과, 양군에서 2배 이상 차이를 보이면서 통계적인 유의성을 나타낸 miRNA는 총 38개였다. 비알코올 지방간염 그룹에서 26가지의 miRNA가 유의하게 증가하였고, 12가지의 miRNA는 유의하게 감소하였다. 비알코올 지방간염에서 증가한 miRNA 중에서 충분한 발현량을 보이는 miRNA를 8가지 선정하였으며, 이는 각각 miR-15b-3p, miR-21-5p, miR-29b-3p, miR-126-5p, miR-151a-3p, miR-183-5p, miR-192-5p 및 miRNA-4449였다. 상기 각 miRNA의 비알코올 지방간염 진단능력(AUROC), 통계적 유의성(p-value), 민감도(sensitivity), 특이도(specificity)는 표 2에 나타내었다. 이 중에서 AUROC가 유의한 miRNA는 양군에서 유의한 차이를 보이는 miR-21-5p, miR-151a-3p, miR-192-5p, 및 miR-4449 4가지였다.

한편 상기 8가지 miRNA를 조합하였을 때, 비알코올 지방간염 진단의 AUROC 값은 0.924이고, miR-21-5p, miR-151a-3p, miR-192-5p, 및 miR-4449의 4가지 miRNA를 조합하였을 때, 비알코올 지방간염 진단의 AUROC 값은 0.875로 양군간의 유의한 차이는 없었다.
상기 결과로부터, 비알코올 지방간 질환 환자의 혈청에서 miR-15b-3p, miR-21-5p, miR-29b-3p, miR-126-5p, miR-151a-3p, miR-183-5p, miR-192-5p 및 miRNA-4449의 발현을 분석하고 이들을 조합하여, 비알코올 지방간염을 진단할 수 있으며, 상기 miRNA는 비알코올 지방간 질환 환자에 있어서 단순 지방간 환자와 지방간염 환자를 구별할 수 있는 유용한 바이오마커로 이용될 수 있음을 알 수 있다.
이상으로 본 발명 내용의 특정한 부분을 상세히 기술하였는바, 당업계의 통상의 지식을 가진 자에게 있어서, 이러한 구체적 기술은 단지 바람직한 실시 양태일 뿐이며, 이에 의해 본 발명의 범위가 제한되는 것이 아닌 점은 명백할 것이다. 따라서 본 발명의 실질적인 범위는 첨부된 청구항들과 그것들의 등가물에 의하여 정의된다고 할 것이다.
본 발명은 비알코올 지방간염을 진단할 수 있는 바이오마커 miRNA-4449과 miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA에 관한 것으로서, 환자의 혈청에서 상기 miRNA의 수준을 측정함으로써 단순 지방증과 달리 예후가 좋지 않은 비알코올 지방간염을 정확하게 감별하여 진단할 수 있으므로, 비알코올 지방간염의 예방, 치료 및 진단 분야에서 유용하게 이용될 수 있다.
Claims (15)
- miRNA-4449를 포함하고,miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상을 추가로 포함하는 비알코올 지방간 질환 중 단순 지방간과 지방간염 감별 진단용 바이오마커 조성물.
- 제1항에 있어서,상기 miRNA-4449는 서열번호 1의 염기서열로 이루어진 것을 특징으로 하는, 조성물.
- miRNA-4449 발현 수준을 측정할 수 있는 제제와miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA 발현 수준을 측정할 수 있는 제제를 추가로 포함하는, 비알코올 지방간 질환 중 단순 지방간과 지방간염 감별용 조성물.
- 제3항에 있어서,상기 miRNA 발현 수준을 측정할 수 있는 제제는 상기 miRNA에 상보적인 서열을 갖는 올리고뉴클레오티드, 프라이머, 또는 프로브인 것을 특징으로 하는, 조성물.
- 제3항의 조성물을 포함하는, 비알코올 지방간 질환 중 단순 지방간과 지방간염 감별용 키트.
- 단순 지방간과 비알코올 지방간염 감별진단이 필요한 환자의 생물학적 시료로부터 miRNA-4449과miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA 발현 수준을 측정하는 단계를 포함하는, 단순 지방간과 비알코올 지방간염을 감별하여 진단하기 위한 정보제공방법.
- 제6항에 있어서,상기 환자는 비알코올 지방간 질환으로 진단받은 환자이고,상기 생물학적 시료는 혈액, 혈청, 및 혈장으로 이루어진 군으로부터 선택되는 1종 이상인 것을 특징으로 하는, 정보제공방법.
- 제6항에 있어서,상기 단계 이후에 측정된 miRNA 발현 수준과 단순 지방간 환자에서 상기 측정한 miRNA와 상응하는 miRNA의 발현 수준을 비교하는 단계를 포함하는, 정보제공방법.
- 제8항에 있어서,상기 비교하는 단계 이후에 감별 진단이 필요한 환자의 각 miRNA 발현 수준이 단순 지방간 환자보다 1.5배 이상인 경우 비알코올 지방간염으로 판별하는 단계를 포함하는, 정보제공방법.
- 단순 지방간과 비알코올 지방간염 감별진단이 필요한 환자의 생물학적 시료로부터 miRNA-4449과miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA 발현 수준을 측정하는 단계를 포함하는, 단순 지방간과 비알코올 지방간염을 감별하여 진단하기 위한 진단방법.
- 제10항에 있어서,상기 단계 이후에 측정된 miRNA 발현 수준과 단순 지방간 환자에서 상기 측정한 miRNA와 상응하는 miRNA의 발현 수준을 비교하는 단계를 포함하는, 진단방법.
- miRNA-4449의 발현을 억제하는 제제 및miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현을 억제하는 제제를 추가로 포함하는, 비알코올 지방간염의 예방 또는 치료용 약학적 조성물.
- 제12항에 있어서,상기 제제는 siRNA, 앱타머(aptamer), 안티센스 올리고뉴클레오티드, 및 화합물로 이루어진 군으로부터 선택되는 1종 이상인 것을 특징으로 하는, 약학적 조성물.
- miRNA-4449의 발현을 억제하는 제제 및miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA의 발현을 억제하는 제제를 추가로 개체에 투여하는 단계를 포함하는, 비알코올 지방간염 예방 또는 치료방법.
- 하기 단계를 포함하는, 비알코올 지방간염의 예방 또는 치료용 약제의 스크리닝 방법:(1) 간 세포에 후보물질을 처리하는 단계;(2) 상기 세포의 miRNA-4449 발현 수준과miRNA-15, miRNA-21, miRNA-29, miRNA-126, miRNA-151, miRNA-183, 및 miRNA-192으로 이루어진 군으로부터 선택되는 어느 하나 이상의 miRNA 발현 수준을 측정하는 단계; 및(3) 상기 (2) 단계에서 측정한 miRNA 발현 수준이 후보물질 처리 전보다 감소한 경우 상기 후보물질을 비알코올 지방간염의 예방 또는 치료용 약제로 선정하는 단계.
