Abstract
본 발명은 다용도 지혈제 조성물 및 그 제조방법에 관한 것으로, 보다 상세하게는 개질된 음이온화 치환기를 가지는 제1성분 및 점착성을 가지는 고분자인 제2성분을 포함하여, 향상된 지혈능과 생체 적합성을 제공하여 수술, 복강경 및 다양한 외과적 시술에 적용할 수 있는 다용도 지혈제에 관한 기술이다.
Description
본 발명은 다용도 지혈제 조성물 및 그 제조방법에 관한 것으로, 보다 상세하게는 개질된 음이온화 치환기를 가지는 제1성분 및 점착성을 가지는 고분자인 제2성분을 포함하여, 향상된 지혈능과 생체 적합성을 제공하여 수술, 복강경 및 다양한 외과적 시술에 적용할 수 있는 다용도 지혈제에 관한 기술이다.
출혈이란, 혈액이 혈관 밖으로 나오는 것을 의미하며, 온 몸을 구성하는 혈관들이 심장의 압력에 의해 혈관 안의 혈액을 몸 전체로 순환을 시키는데, 일부 혈관에서 상처가 생기게 되면 압력이 빠져나갈 공간이 생기게 되어 혈액이 상처 밖으로 새는 것을 의미한다. 혈액이 혈관 밖으로 배출되는 출혈은 일상생활에서의 부상, 수술과 같은 의료 행위에서 일어날 수 있다.
지혈이란, 출혈을 멈추게 하는 과정으로 손상된 혈관 내의 혈액을 유지시키는 것을 말한다. 지혈 시, 지혈제, 붕대, 드레싱 등으로 상처 부위를 봉합하거나 압박하여 출혈 부위를 빠르게 지혈하는 것이 무엇보다 중요하다. 특히, 최근에는 인체 친화적인 생체적합성이 우수하면서 지혈능이 뛰어난 지혈제에 대한 수요가 증가하고 있다. 이에 대응하기 위하여, 무독성의 천연 고분자를 주재료로 하면서 향상된 지혈 효과와 시장의 다양한 요구를 충족하기 위한 지혈제들이 개발 중에 있다.
예를 들어서, 한국 공개특허 제10-2014-0074993호는 지혈에 사용하기 적합한 입상 형태의 생체적합성 중합체 및 반응기를 포함하는 하나의 친수성 중합체 성분을 포함하는 지혈 조성물을 개시하고 있다. 보다 자세하게는 하나의 친수성 중합체 성분과 입상 형태의 생체적합성 중합체의 조합은 개선된 지혈 성질을 갖고 개선된 조직부착성을 제공가능함을 언급하고 있다.
한국 등록특허 제10-1301276호는 카테콜기가 결합된 키토산 및 말단에 티올기가 결합된 플루로닉을 포함하는 접착성 하이드로젤 조성물에 관한 것으로, 보다 상세하게는 생체 내외에서의 안전성을 제공하면서, 온도 감응적이며, 지혈효과가 우수 하여 생체접착제로서 이용가능한 접착제 조성물 및 이를 포함하는 의료용 접착제, 유착 방지제 및 표면 흡착 방지제에 관한 것을 언급하고 있다.
한국 공개특허 제10-2017-0060054호는 생체 적합성 지혈 제품 또는 조직 실란트를 개시하고 있다. 보다 자세하게는 폴리옥시에틸렌 과립을 포함하고, 상기 폴리옥시에틸렌의 점도 평균 분자량이 100,000 돌턴 내지 7,000,000 돌턴이며, 상기 폴리옥시에틸렌 과립의 입경이 0.5㎛ 내지 2000㎛이고,흡수배율이 1 내지 500인 점을 특징으로 언급하고 있다. 이에 조직 기관 및 체강 내 조직 또는 기관의 유혈 상처표면에 대한 지혈, 블로킹 방지, 감염 방지, 조직 유합의 촉진 및 상처밀봉등의 효과를 제공할 수 있는 것이 특징이다.
전술한 바와 같이, 생체 적합성이 우수한 지혈제에 대한 다양한 연구 및 개발은 활발하게 이루어지고 있으며, 이러한 연구의 방안으로 본 발명은 지혈 효과를 제공하는 생체 고분자의 표면 개질시키는 방법을 통하여 지혈능 및 생체 적합성을 향상시키고 이를 수술, 복강경 및 다양한 외과적 시술에 적용할 수 있는 다용도 지혈제를 제공하기 위하여 완성하였다.

본 발명의 목적은 수술, 복강경 및 다양한 외과적 시술에 적용이 가능한 다용도 지혈제를 제공하는 것이다.
본 발명의 일 실시예를 따르면, 개질된 음이온화 치환기를 가지는 제1성분; 및 점착성을 가지는 고분자를 포함하는 제2성분;을 포함하는 다용도 지혈제 조성물이 제공된다.
본 발명의 일 실시예를 따르면, (a) 용액에 음이온화 치환기를 가지는 제1성분을 투입하고 교반하여, 개질된 음이온화 치환기를 가지는 제1성분을 제조하는 단계; (b) 원심분리를 이용하여 상충액을 분리하고 개질된 음이온화 치환기를 가지는 제1성분의 파우더를 수득하는 단계; (c) 상기 파우더를 세척하고 건조하는 단계; 및 (d) 상기 건조하여 개질된 음이온화 치환기를 가지는 제1성분의 파우더와 점착성을 가지는 고분자를 포함하는 제2성분을 혼합하는 단계;를 포함하는 다용도 지혈제 조성물의 제조방법이 제공된다.
본 발명에 따른 지혈제는 혈액 및 생체 적합성이 우수하면서 혈액의 흡수능을 극대화하여 지혈응고작용을 촉진 효과시켜 향상된 지혈작용을 제공할 수 있다.
본 발명에 따른 지혈제는 수술, 복강경 및 다양한 외과적 시술에 적용이 가능한 다용도로 제공될 수 있다.
본 발명에 따른 지혈제는 파우더 형태로 제공될 수 있고, 이에 보관 및 취급에 용이함을 제공할 수 있다.

본 발명의 일 실시예를 따르면, 개질된 음이온화 치환기를 가지는 제1성분 및 점착성을 가지는 고분자를 포함하는 제2성분을 포함하는 다용도 지혈제 조성물이 제공된다.
본 발명의 일 실시예를 따르면, 상기 개질된 음이온화 치환기를 가지는 제1성분은 음이온화된 치환기를 포함하는 생채 유래 고분자로서, 예를 들어서, 카복시메틸 전분, 카복시에틸 전분, 카복시메틸셀룰로오스, 카복시에틸 셀룰로오스, 카복시메틸 알지네이트, 폴리 아크릴산-전분 그래프트 공중합체, 전분-2-하이드록시프로필시트레이트, 전분-2-하이드록시프로필포스페이트 및 전분-2-하이드록시프로필설페이트에서 선택되는 적어도 어느 하나 이상을 포함하여 제공될 수 있고, 이에 생체 적합성, 생분해성 및 친수성을 제공할 수 있으며, 기존에 비하여 향상된 지혈능을 제공할 수 있다.
본 발명의 일 실시예를 따르면, 상기 개질은 염화칼슘, 탄산칼슘, 구연산 칼슘, 글루콘산 칼슘, 수산화칼슘, 옥살산칼슘에서 선택되는 적어도 어느 하나 이상을 포함하여 개질된 것을 특징으로 한다. 바람직하게는 염화칼슘을 제공하여, 음이온화 치환기를 개질할 수 있으며 음이온화 치환기가 칼슘에 의해 개질되면 친수성을 향상시켜, 유체 흡수성과 팽창성이 향상된다. 또한 칼슘이온은 지혈작용에 있어 도움을 주는 역할을 할 수 있으므로, 이에 지혈능을 현저하게 향상시킬 수 있고, 혈액응고기전에 도움줄 수 있다. 이는 후술할 실시예의 실험예의 결과를 통하여 확인이 가능하다.
본 발명의 일 실시예를 따르면, 상기 점착성을 가지는 고분자를 포함하는 제2성분은 알지네이트, 키토산, 키토산 유도체, 구아검, 잔탄검, 풀루란, 카라기난, 히아루론산, 폴리에틸렌 글리콜, 폴리 아크릴 산, 폴리 비닐 피롤리돈, 셀룰로오스 및 셀룰로오스 유도체에서 선택되는 적어도 어느 하나 이상인 것을 특징으로 한다.
예를 들어서, 점착성을 가지는 고분자를 포함하는 제2성분은 알지네이트가 제공될 수 있고, 알지네이트(alginate)는 알진산 또는 일긴산으로 불리는 다당류를 포함하는 천연의 고분자 응집체를 의미한다. 가지 사실이 없는 1,4 결합의 만누론산(β-d-mannuronic acid)과 글루론산(α-L-gluconic acid)이 무작위로 결합하여 이루어진 화합물이다. 알지네이트 젤의 물리적 성질은 M/G비, 즉 M과 G 사슬의 구성비가 추출한 해조류의 종류에 따라 달라진다. 알지네이트의 G함유량이 많을수록 단단한 다공성 젤의 형성과 젤 상태를 오랫동안 유지하는 경향이 강하며, 2가 양이온으로 가교되어 있는 동안 과도한 팽윤과 수축이 발생하지 않아 본래의 형태를 유지한다. 이에 반해, M의 함유량이 많게 되면 유연하고 기공이 적은 젤을 형성하게 된다. 이러한 알지네이트의 젤화는 G가 2가 양이온과 반응하여 V 모양의 구멍을 가지는 egg box모델의 3차원의 입체구조를 형성함에 따른 것이다. 따라서, 필요에 따라 M/G비를 조절하여 제공할 수 있다. 또한, 상기 알지네이트는 불용성이지만 알긴산염의 형태로 제공되어, 수용성을 나타낼 수 있다. 예를 들어서, 알긴산 나트륨이 제공될 수 있다. 상기 알긴산의 경우, 분자는 기본적으로 카복실기(-COOH)를 가지고 있고, 산 염기 반응 후 음이온의 비공유 전자쌍을 이용해 양이온과 결합하는 특성을 가진다. 따라서, 알긴산 나트륨은 알긴산 분자가 나트륨 양이온(Na+) 와 결합한 것인데, 이 경우 아주 유연한 고분자 사슬 구조를 가지게 된다. 이에 생분해성과 함께 점착성을 제공하고 수분과 접촉 시, 높은 팽윤성을 제공할 수 있다.
다른 예를 들어서, 점착성을 가지는 고분자를 포함하는 제2성분은 키토산이 제공될 수 있으며, 키토산은 유기산 수용액에 용해가 되며, 점도가 높은 특징이 있는데 이는 분자량, 탈아세틸화도, 이온강도, pH 등에 따라 차이가 있다. 더불어, 키토산은 생체적합성이 우수하며 금속 이온과 킬레이트를 형성하여 흡착성이 우수하므로 체액이나 침출액 등을 흡착하여 출혈 부위에서 효율적으로 지혈 효과를 제공할 수 있다. 특히, 마그네슘, 구리, 칼륨 등의 이온이 다량으로 존재하여도 킬레이트 형상에 크게 영향을 주지 않는 것이 특징이다. 또한, 천연 고분자로 고갈 가능성이 낮으며, 조직이나 체내에서 우수한 생체 적합성을 나타내고, 독성이 거의 없고 쉽게 생분해가 되는 장점이 있다.
나아가, 점착성을 제공하는 고분자로 상기 언급한 알지네이트와 키토산에 제한되는 것은 아니며, 유사한 역할을 제공할 수 있는 키토산 유도체, 구아검, 잔탄검, 풀루란, 카라기난, 히아루론산, 폴리에틸렌 글리콜, 폴리 아크릴 산, 폴리 비닐 피롤리돈, 셀룰로오스 및 셀룰로오스 유도체 등이 제공될 수 있으며, 이에 제한되는 것은 아니다.
본 발명의 일 실시예를 따르면, 상기 제1성분: 제2성분의 함량비는 99 내지 90: 1 내지 10인 것을 특징으로 한다. 상기 범위에서 혈액흡수능과 함께 점착성을 가지는 효과를 제공할 수 있다.
본 발명의 일 실시예를 따르면, 상기 제1성분 및 제2성분은 파우더 형태인 것을 특징으로 한다. 이에 우수한 혈액 흡수능을 기대할 수 있고, 사용의 편의성, 보관 및 취급에 용이함을 제공할 수 있다.
본 발명의 일 실시예를 따르면, 상기 제1성분의 파우더 형태는 1 내지 500㎛인 직경을 가지는 입자가 과립(granule)화되어 있는 것을 특징으로 한다. 이에, 제1성분 파우더는 다공질구조가 되어 습윤성이 좋아지고, 동시에 수중에서의 분산, 침강 속도를 증가시키는 효과를 제공할 수 있다.
본 발명의 일 실시예를 따르면, 상기 제2성분 파우더의 직경은 1 내지 500㎛으로 제공된다. 파우더 직경이 상기 범위에서 적절한 비표면적을 제공하여, 우수한 지혈능과 혈액흡수성을 제공할 수 있다.
한편, 본 발명의 따른 다용도 지혈제 조성물의 제조방법이 제공된다. 이하에서는 전술한 다용도 지혈제 조성물과 동일한 내용이 적용될 수 있고, 중복되는 범위 내에서 설명은 생략하도록 한다.
본 발명의 일 실시예를 따르면, (a) 용액에 음이온화 치환기를 가지는 제1성분을 투입하고 교반하여, 개질된 음이온화 치환기를 가지는 제1성분을 제조하는 단계; (b) 원심분리를 이용하여 상충액을 분리하고 개질된 음이온화 치환기를 가지는 제1성분의 파우더를 수득하는 단계; (c) 상기 파우더를 세척하고 건조하는 단계; 및 (d) 상기 건조하여 개질된 음이온화 치환기를 가지는 제1성분의 파우더와 점착성을 가지는 고분자를 포함하는 제2성분을 혼합하는 단계;를 포함하는 다용도 지혈제 조성물의 제조방법이 제공된다.
본 발명의 일 실시예를 따르면, (a) 용액에 음이온화 치환기를 가지는 제1성분을 투입하고 교반하여, 개질된 음이온화 치환기를 가지는 제1성분을 제조하는 단계가 제공된다. 이 경우, 상기 용액은 수용액에 염화칼슘, 탄산칼슘, 구연산 칼슘, 글루콘산 칼슘, 수산화칼슘 및 옥살산칼슘에서 선택되는 적어도 어느 하나 이상을 포함하는 것을 특징으로 한다. 예를 들어서, 수용액에 염화칼슘이 용해되어 칼슘이온이 제공되고, 제공되는 칼슘이온에 의하여 음이온화 치환기를 가지는 제1성분의 표면이 개질될 수 있다. 음이온화 치환기가 칼슘에 의해 개질되면, 친수성을 향상시킬 수 있고, 더불어 유체 흡수성과 팽창성이 향상된다. 이에 지혈능을 현저하게 향상시킬 수 있고, 혈액응고기전의 도움을 주는 역할을 제공할 수 있다.
본 발명의 일 실시예를 따르면, 상기 (a) 단계의 용액의 농도는 5 내지 75 퍼센트 농도인 것을 특징으로 한다. 5 퍼센트 농도 미만인 경우에는 칼슘이온의 부족으로 지혈능이 저하되고, 75 퍼센트 농도를 초과하는 경우에는 CaCl2가 과량이므로 용액상에서 포화상태인 고체 상태로 존재하는 문제가 있다. 또한, 칼슘이온이 과량 존재하면, 혈액 내 트롬빈의 활성도를 저하시켜 지혈작용에 역효과를 유발하므로 5 내지 75 퍼센트 농도로 제공되는 것이 바람직하다.
본 발명의 일 실시예를 따르면, 상기 (a) 단계의 음이온화 치환기를 가지는 제1성분은 카복시메틸 전분, 카복시에틸 전분,카복시메틸셀룰로오스, 카복시에틸 셀룰로오스, 카복시메틸 알지네이트, 폴리 아크릴산-전분 그래프트 공중합체, 전분-2-하이드록시프로필시트레이트, 전분-2-하이드록시프로필포스페이트 및 전분-2-하이드록시프로필설페이트에서 선택되는 적어도 어느 하나 이상인 것을 특징으로 한다. 이에 생체적합성, 생분해성 및 친수성을 제공할 수 있으며, 기존에 비하여 향상된 지혈능을 제공할 수 있다.
본 발명의 일 실시예를 따르면, 상기 (a) 단계의 교반 시간은 30분 내지 12시간으로 제공될 수 있다. 교반 시간이 30분 미만이면 충분히 반응이 진행되지 않고, 12시간을 초과하면 필요이상의 교반시간이므로, 상기 범위가 바람직하다.
본 발명의 일 실시예를 따르면, (b) 원심분리를 이용하여 상충액을 분리하고 개질된 음이온화 치환기를 가지는 제1성분의 파우더를 수득하는 단계가 제공된다. 이 경우, 수득된 제1성분의 파우더는 1 내지 500㎛인 직경을 가지는 입자가 과립화되어 있는 것을 특징으로 한다.
상기 (a) 단계의 이후, 용액에 용해된 제1성분을 분획하기 위하여, 원심분리를 이용하여 상충액을 분리하여 개질된 음이온화 치환기를 가지는 제1성분의 파우더를 수득할 수 있다. 이 경우, 상충액을 분리하는 과정에서 감압필터를 사용할 수 있다. 감압필터는 여과기 아래쪽 플라스크 내부의 압력을 감소시켜 발생하는 여과액에 대한 흡인력을 이용하여 여과하는 장치를 의미한다. 감압필터로 상충액을 제거하면서 제1성분을 분리하여 수득가능하다. 감압필터를 제공하여 빠른 시간 안에 많은 양의 물질을 여과할 수 있고, 여과의 효율 또한 향상시킬 수 있다.
본 발명의 일 실시예를 따르면, (c) 상기 파우더를 세척하고 건조하는 단계가 제공된다. 상기 세척은 에탄올을 이용하여, 2 내지 5회 상온에서 세척하여, 이물질을 완전히 제거하는 것이 바람직하다. 또한, 횟수는 필요에 따라 조절이 가능함은 물론이다. 상기 건조는 동결건조 및 온열건조에서 선택되는 적어도 어느 하나인 것을 특징으로 한다. 바람직하게는 온열 건조가 제공될 수 있다.
예를 들어서, 상기 동결 건조의 경우, 바람직한 방법은 0℃ 내지 -70℃에서 마이크로-비드를신속하게 동결시켜 용매를 진공 조건에서 제거하는 것이다. 바람직하게는 -30℃ 내지 -40℃가 제공될 수 있고, 1시간 내지 48시간에 제공될 수 있으며, 바람직하게는 2 내지 24시간이 제공될 수 있다.
상기 온열 건조의 경우, 70 내지 100℃가 제공될 수 있고, 바람직하게는 75 내지85℃의 오븐에서 건조가 가능하다. 이 경우, 건조 시간은 10분 내지 1시간이 제공될 수 있고, 바람직하게는 10분 내지 30분이 제공된다.
본 발명의 일 실시예를 따르면, (d) 상기 건조하여 개질된 음이온화 치환기를 가지는 제1성분의 파우더와 점착성을 가지는 고분자를 포함하는 제2성분을 혼합하는 단계가 제공된다. 이에, 최종적으로 다용도 지혈제 조성물을 제조된다.
본 발명의 일 실시예를 따르면, 상기 (d)단계의 혼합은 제1성분의 파우더와 점착성을 가지는 고분자를 포함하는 제2성분을 99 내지 90: 1 내지 10으로 혼합하는 것을 특징으로 한다. 상기 범위에서 혈액흡수능과 함께 점착성을 가지는 효과를 제공할 수 있다.
또한, 이 경우 제공되는 1성분 파우더는 1 내지 500㎛인 직경을 가지는 입자가 과립화된 것을 특징으로 하고, 상기 제2성분 파우더의 직경은 1 내지 500㎛으로 제공된다. 파우더 직경이 상기 범위에서 바람직한 비표면적을 제공하여, 우수한 지혈능과 혈액응고, 혈액 흡수 효과를 제공할 수 있다.
이하, 실시예를 통해 본 발명을 설명하고자 하며, 이로써 본 발명을 제한하는 것은 아니다.
<실시예 1 >
카복시메틸 전분(carboxymethyl starch) 염화칼슘(CaCl2) 수용액에 넣어 1시간동안 교반한다. 교반 후 원심분리기를 이용하여 상충액을 제거하고 파우더를 수득하여 감압필터 한다. 감압필터 시 에탄올을 이용하여 5회 세척하고, 세척이 완료된 파우더를 건조오븐에서 건조하여 제1성분을 수득한다.
점착성을 가지는 고분자를 포함하는 제2성분을 상기 제1성분과 혼합하여 다용도 지혈제 조성물을 제조한다. 이 경우, 제1성분과 제2성분의 혼합비는 99: 1이다.
<비교예 1>
상용화 제품인 Arista-AH를 준비하였다.
<비교예 2>
상용화 제품인 Perclot을 준비하였다.
<대조군>
필요에 따라 Control(대조군)은 지혈제를 처방하지 않고 실험 효과를 확인하기 위하여 진행하였다.
<실험예 1: 세포독성평가>
실시예 및 비교예에 따른 다용도 지혈제의 세포독성을 평가하기 위하여, 세포독성 시험은 ISO 10993-5: Biological evaluation of medical devices, Part 5 : Tests for in vitro cytotoxicity 시험법과 식품의약품안전처가 제공하는 의료기기의 생물학적 안전에 관한 공통기준규격에 준하여 시험을 진행하였다. 이에, 시험물질이 세포의 배양 환경상에서 L-929(마우스의 섬유아세포)세포에 세포독성을 야기하는지 평가하기 위하여 실시하였다. 이에 대한 결과는 도 2에 기재하였다.
<실험예 2: 생분해 평가>
실시예 및 비교예에 따른 다용도 지혈제의 생분해성 평가를 위하여, 생분해 실험을 실시하였다. 시험은 Simulated Body Fluid(SBF)용액에 체내에서도 존재하는 amylase를 녹여 생분해 용액을 제조하여 시료를 투입한다. 이후 시간에 따라 무게변화를 관찰하여 생분해 평가를 진행하였다. 이에 대한 결과는 도 3 및 표 1에 기재하였다.

<실험예 3: in vitro 지혈능 실험>
실시예 및 비교예에 따른 다용도 지혈제의 지혈능을 평가 확인하기 위하여 다음 순서로 in vitro 실험을 Lee white method에 따라 진행하였다. 이에 대한 결과는 도 4에 나타내었다.
① 유리바이알에 혈액 3ml 투입한다.
② 37℃ 항온수조에 유리바이알을 올려준다.
③ 유리바이알에 실시예 또는 비교예에 따른 지혈제 시료를 투여한 후 가볍게 흔들어 준다.
④ 0.025M 염화칼슘(CaCl2)수용액을 0.3ml 혈액 위에 떨어뜨려준 후, 유리바이알을 흔들어 섞어준다.
⑤ 혈액이 응고되는 시간을 측정한다.
<실험예 4: in vivo 지혈능 실험>
실시예 및 비교예에 따른 다용도 지혈제의 지혈능을 평가 확인하기 위하여, 다음 순서로 in vivo 실험을 진행하였다. 참고를 위하여, 실험 순서에 대한 사진 및 이에 대한 설명을 도 5에 나타내었다. 더불어 실험예 4에 따른 in vivo 실험 결과를 도 6와 표 2에 나타내었다.
① Rat의 간을 꺼내 실험 준비를 한다. 시험 동물은 8주령 Rat으로 11마리 제공하였다.
② 메스를 이용하여 +자 형태로 1×1cm 절단한 뒤 거즈로 출혈 부위를 막아준다.
③ 거즈 제거 후 실시예 또는 비교예에 따른 지혈제 시료 0.1g을 출혈부위에 적용한다.
④ 상기 시료 도입 후, 시료 상부에 거즈를 올린 뒤, 50g 추를 이용하여 동일한 압력을 유지해준다.
⑤ 1분동안 거즈로 눌러준 후, 15초마다 지혈여부를 확인한다.
⑥ 지혈 후 생리식염수로 씻어내고 재출혈여부를 확인한다.

상기 ③ 이후, 블라인드 테스트 (blind test)를 진행하며 Rat의 간 4곳을 절제하여, 실시예 및 비교예의 지혈제 시료를 순서 변경하여 진행하도록 한다.
실험 결과를 살펴보면, 먼저 도 1에서 실시예에 따라 제조된 다용도 지혈제 조성물의 SEM 사진을 통해서 일정한 직경을 가지는 파우더가 과립화되어 뭉쳐있는 형태를 확인할 수 있다.
실험예 1의 세포독성 결과인 도 2를 살펴보면, 실시예 1의 경우, 대조군(control)과 비교하여 약 98%의 세포 생존율을 보이는 것으로 확인하였다. 대조군(control)은 Polystyrene cell culture dish를 이용하였고, 실시예 1의 시료를 용출하여 제작된 배지를 이용하여 세포의 생존율을 관찰하였다. 도 2에서 표현한 수치는 대조군(control)의 세포 생존율을 100%로 두었을 때, 실시예 1의 시료를 용출하여 제작된 배지의 환경에서 98%의 세포생존율을 알 수 있다. 이는 ISO 10993-5, Determination of cytotoxicity에 따른 세포용해도의 평가 기준과 의료기기 생물학적 안전성 시험기준에 부합함을 의미하며, 실험예 1에 따른 지혈제가 생체적합함를 의미한다.
실험예 2의 생분해 평가 결과인 도 3및 표 1을 살펴보면, 비슷한 분해 기간 동안 실시예의 분해도가 우수함을 확인하였다.
실험예 3의 in vitro 실험 결과인 도 4의 결과를 살펴보면, 비교예에 비하여 실시예는 혈액응고시간이 단축됨을 확인할 수 있다.
실험예 4의 in vivo 실험 결과인 도 6의 결과를 살펴보면, 비교예에 비하여 지혈시간이 절반 이하로 현저하게 단축됨을 확인할 수 있었다. 따라서, 지혈제의 필수 조건인 혈액 응고시간 및 지혈능이 매우 우수함을 확인 가능하다.
이에, 본 발명에 따른 다용도 지혈제 조성물은은 지혈을 즉각적으로 유도함으로써 출혈 관리를 할 수 있고, 특히, 외과적 시술 시 의사들의 시야를 확보하여 빠른 수술을 가능하게 하고, 과다 출혈을 신속하게 지혈할 수 있으므로 효과적임을 확인할 수 있다. 또한, 파우더 형태로 제공될 수 있어, 복강경 및 다양한 시술에 쉽고 간편하게 적용이 가능하다.
이상, 첨부된 도면을 참조하여 본 발명의 실시예를 설명하였지만, 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자는 본 발명이 그 기술적 사상이나 필수적인 특징을 변경하지 않고서 다른 구체적인 형태로 실시될 수 있다는 것을 이해할 수 있을 것이다. 그러므로 이상에서 기술한 실시예에는 모든 면에서 예시적인 것이며 한정적이 아닌 것으로 이해해야만 한다.
따라서, 본 발명의 사상은 상기 설명된 실시예에 국한되어 정해져서는 아니 되며, 후술하는 청구범위뿐만 아니라 이 청구범위와 균등하게 또는 등가적으로 변형된 모든 것들은 본 발명의 사상의 범주에 속한다고 할 것이다.
Claims (17)
- 개질된 음이온화 치환기를 가지는 제1성분; 및점착성을 가지는 고분자를 포함하는 제2성분;을 포함하는 다용도 지혈제 조성물.
- 제1항에 있어서,상기 개질은 염화칼슘, 탄산칼슘, 구연산 칼슘, 글루콘산 칼슘, 수산화칼슘 및 옥살산칼슘에서 선택되는 적어도 어느 하나 이상을 포함하여 개질된 것을 특징으로 하는 다용도 지혈제 조성물.
- 제1항에 있어서,상기 음이온화 치환기를 가지는 제1성분은 카복시메틸 전분, 카복시에틸 전분, 카복시메틸셀룰로오스, 카복시에틸 셀룰로오스, 카복시메틸 알지네이트, 폴리 아크릴산-전분 그래프트 공중합체, 전분-2-하이드록시프로필시트레이트, 전분-2-하이드록시프로필포스페이트 및 전분-2-하이드록시프로필설페이트에서 선택되는 적어도 어느 하나 이상을 포함하는 것을 특징으로 하는 다용도 지혈제 조성물.
- 제1항에 있어서,상기 점착성을 가지는 고분자를 포함하는 제2성분은 알지네이트, 키토산, 키토산 유도체, 구아검, 잔탄검, 풀루란, 카라기난, 히아루론산, 폴리에틸렌 글리콜, 폴리 아크릴 산, 폴리 비닐 피롤리돈, 셀룰로오스 및 셀룰로오스 유도체에서 선택되는 적어도 어느 하나 이상을 포함하는 것을 특징으로 하는 다용도 지혈제 조성물.
- 제1항에 있어서,상기 제1성분: 제2성분의 함량비는 99 내지 90: 1 내지 10인 것을 특징으로 하는 다용도 지혈제 조성물.
- 제1항에 있어서,상기 제1성분 및 제2성분은 파우더 형태인 것을 특징으로 하는 다용도 지혈제 조성물.
- 제6항에 있어서,상기 제1성분의 파우더 형태는 1 내지 500㎛인 직경을 가지는 입자가 과립화되어 있는 것을 특징으로 하는 다용도 지혈제 조성물.
- 제6항에 있어서,상기 제2성분의 파우더 형태는 직경이 1 내지 500㎛인 것을 특징으로 하는 다용도 지혈제 조성물.
- (a) 용액에 음이온화 치환기를 가지는 제1성분을 투입하고 교반하여, 개질된 음이온화 치환기를 가지는 제1성분을 제조하는 단계;(b) 원심분리를 이용하여 상충액을 분리하고 개질된 음이온화 치환기를 가지는 제1성분의 파우더를 수득하는 단계;(c) 상기 파우더를 세척하고 건조하는 단계; 및(d) 상기 건조하여 개질된 음이온화 치환기를 가지는 제1성분의 파우더와 점착성을 가지는 고분자를 포함하는 제2성분을 혼합하는 단계;를 포함하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (a)단계의 용액은 염화칼슘, 탄산칼슘, 구연산 칼슘, 글루콘산 칼슘, 수산화칼슘 및 옥살산칼슘에서 선택되는 적어도 어느 하나 이상을 포함하는 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (a)단계의 용액의 농도는 5 내지 75 퍼센트 농도인 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (a)단계의 음이온화 치환기를 가지는 제1성분은 카복시메틸 전분, 카복시에틸 전분,카복시메틸셀룰로오스, 카복시에틸 셀룰로오스, 카복시메틸 알지네이트, 폴리 아크릴산-전분 그래프트 공중합체, 전분-2-하이드록시프로필시트레이트, 전분-2-하이드록시프로필포스페이트 및 전분-2-하이드록시프로필설페이트에서 선택되는 적어도 어느 하나 이상을 포함하는 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (a) 단계의 교반 시간은 30분 내지 12시간인 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (b)단계의 개질된 음이온화 치환기를 가지는 제1성분의 파우더는 1 내지 500㎛인 직경을 가지는 입자가 과립화 되어 있는 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (c)단계의 건조는 동결건조 및 온열건조에서 선택되는 어느 하나인 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (d)단계의 점착성을 가지는 고분자를 포함하는 제2성분은 직경이 1 내지 500㎛인 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
- 제9항에 있어서,상기 (d)단계의 혼합은 제1성분의 파우더와 점착성을 가지는 고분자를 포함하는 제2성분을 99 내지 90: 1 내지 10으로 혼합하는 것을 특징으로 하는 다용도 지혈제 조성물의 제조방법.
