Abstract

질병 부위의 실시간 및 비침습적 이미징은 중추신경계 또는 뇌종양의 치료에 매우 바람직하다. 예를 들어 질병 조직의 비침습적 추적 및 이미징은 일상적으로 수행되는 진단 절차로 조기 발견을 가능케 하여 질병의 적시 개입 및 환자의 전반적인 개선된 예후를 유도할 수 있다.
중추신경계 질환의 치료를 위한 치료제의 전달에 있어서 가장 큰 장벽이 되는 혈액뇌장벽(Blood-Brain Barrier)을 우회할 수 있는 새로운 전달 시스템으로 비강투여(intra-nasal delivery)가 대두되고 있다.
비강투여를 통한 뇌로의 전달 효율을 높이기 위해 다양한 물질들이 개발되고 있으며, 본 발명은 그 전달체로서 폴리머를 이용한다.
본 발명의 목적은 약물전달체로 사용할 수 있는 pH-감응형 생환원성 폴리머 및 이의 약제학적 용도를 제공하는 것이다.

본 발명은 또한 상기 약물전달체를 개체에 비강투여하는 단계를 포함하는 비강투여를 통한 뇌로의 약물 전달 방법을 제공한다.

본 발명의 화학식 1의 폴리머는 세포내 pH 범위(약 pH 6.0)에서 높은 흡수율을 통해 유입되며, 낮은 pH의 저산소성 종양 미세환경을 표적으로 하는 생분해성과 pH 민감성을 갖는 화합물로서, (i) 면역반응 회피성 부위(escapable portion from immune reactions), (ii) 전하성 부위(chargeable portion) 및 (iii) 이황화 결합을 포함하는 생환원성 부위(bioreducible portion)로 이루어진다.
보다 구체적으로, 상기 화학식 1의 폴리머는 일명 PP 또는 PPA일 수 있다.

우선, PEI-Arg를 합성하기 위해, EDC/NHS의 커플링 반응을 통해 아르기닌과 폴리에틸렌이민을 컨쥬게이션 시킨다.

또한, 본 발명의 pH-감응형 폴리머는 비강투여를 통해 혈액뇌장벽을 우회하여 뇌 조직으로 유입될 수 있어 약물 담체로 사용될 수 있다.
따라서, 본 발명의 또 다른 실시양태로서, 본 발명은 약물전달용 담체로서 상기 화학식 1로 표현되는 폴리머를 포함하는 비강투여를 통한 뇌로의 약물 전달용 조성물을 제공한다.
본 발명의 또 다른 실시양태로서, 본 발명은 상기 화학식 1로 표현되는 폴리머; 및 치료제 및 진단제로 이루어진 군에서 선택된 하나 이상의 약물을 포함하는 약물전달체를 제공한다.
본 발명의 PPA 폴리머는 약물 전달을 위한 담체로 사용될 수 있고, 종양의 미세환경을 표적으로 하거나, 표적 성분과의 결합을 통해 종양 세포 외에도 표적 질병 부위로 약물을 전달할 수 있으며, 광감각제와 결합하여 종양 세포로 전달되고, 700-900 nm 파장 범위의 근적외선 조사 시 광열요법을 통해 종양 세포를 사멸할 수 있다.
또한, 본 발명의 PPA 폴리머는 표면에 진단제와 결합하여 비강투여를 통해 뇌혈액장벽을 우회하여 뇌 조직으로 유입되어 뇌질환의 진단에 사용될 수 있다.
상기 약물전달체는 바람직하게는 비강투여용 약물전달체일 수 있다. 본 발명의 PPA 폴리머는 비강투여를 통해 뇌혈액장벽을 우회하여 뇌 조직으로 유입되어 효과적으로 약물을 전달할 수 있다.
본 발명의 약물전달체는 PPA 폴리머와 약물의 복합체 형태로 포함할 수 있다.
상기 치료제는 유전자 전달 시스템, 광감각제 및 약제학적 활성성분으로 이루어진 군에서 선택된 하나 이상일 수 있다.
상기 유전자 전달 시스템은 (i) 내이키드 (naked) 재조합 DNA 분자, (ii) 플라스미드, (iii) 바이러스 벡터 및 (iv) 상기 내이키드 재조합 DNA 분자 또는 플라스미드를 내포하는 리포좀 또는 니오좀의 형태일 수 있다.
통상적인 유전자 치료에 이용되는 모든 유전자 전달 시스템이 적용될 수 있으며, 바람직하게는 플라스미드; 아데노바이러스(Lockett LJ, et al., Clin. Cancer Res. 3:2075-2080(1997)), 아데노-관련 바이러스(Adeno-associated viruses: AAV, Lashford LS., et al., Gene Therapy Technologies, Applications and Regulations Ed. A. Sci USA 92:1411-1415(1995)), 백시니아 바이러스(Puhlmann M. et al., Human Gene Therapy 10:649-657(1999)), 리오바이러스, 폭스바이러스, 셈리키 포리스터 바이러스, 미즐즈 바이러스(Measles virus) 등의 바이러스 벡터; 상기 내이키드 재조합 DNA 분자 또는 플라스미드를 내포하는 리포좀 (Methods in Molecular Biology, Vol 199, S.C. Basu and M. Basu (Eds.), Human Press 2002) 또는 니오좀이 사용될 수 있다.
i. 아데노바이러스
아데노바이러스는 중간 정도의 지놈 크기, 조작의 편의성, 높은 타이터, 광범위한 타깃세포 및 우수한 감염성 때문에 유전자 전달 벡터로서 많이 이용되고 있다. 지놈의 양 말단은 100-200 bp의 ITR (inverted terminal repeat)를 포함하며, 이는 DNA 복제 및 패키징에 필수적인 시스 엘리먼트이다. 지놈의 E1 영역 (E1A 및 E1B)은 전사 및 숙주 세포 유전자의 전사를 조절하는 단백질을 코딩한다. E2 영역 (E2A 및 E2B)은 바이러스 DNA 복제에 관여하는 단백질을 코딩한다.
현재 개발된 아데노바이러스 벡터 중에서, E1 영역이 결여된 복제 불능 아데노바이러스가 많이 이용되고 있다. 한편, E3영역은 통상적인 아데노바이러스 벡터에서 제거되어 외래 유전자가 삽입되는 자리를 제공한다 (Thimmappaya, B. et al.,Cell, 31:543-551(1982); 및 Riordan, J. R. et al., Science, 245:1066-1073(1989)). 따라서, 세포내로 운반하고자 하는 목적 뉴클레오타이드 서열은 결실된 E1 영역 (E1A 영역 및/또는 E1B 영역, 바람직하게는 E1B 영역) 또는 E3 영역에 삽입되는 것이 바람직하고, 보다 바람직하게는 결실된 E1 영역에 삽입된다. 본 명세서에서 바이러스 지놈 서열과 관련하여 사용되는 용어, "결실"은 해당 서열이 완전히 결실된 것뿐만 아니라, 부분적으로 결실된 것도 포함하는 의미를 가진다.
아데노바이러스는 42개의 상이한 혈청형 및 A-F의 서브그룹을 갖는다. 이 중에서, 서브그룹 C에 속하는 아데노바이러스 타입 2 및 타입 5가 본 발명의 아데노바이러스 벡터를 얻기 위한 가장 바람직한 출발물질이다. 아데노바이러스 타입 2 및 타입 5에 대한 생화학적 및 유전적 정보는 잘 알려져 있다.
아데노바이러스에 의해 운반되는 외래 유전자는 에피좀과 동일한 방식으로 복제되며, 이에 숙주세포에 대해 유전적 독성이 매우 낮다. 따라서, 본 발명의 아데노바이러스 유전자 전달 시스템을 이용한 유전자 치료가 매우 안전할 것으로 판단된다.
ii. 레트로바이러스
레트로바이러스는 자신의 유전자를 숙주의 지놈으로 삽입시키고, 대량의 외래 유전 물질을 운반할 수 있으며, 감염시킬 수 있는 세포의 스펙트럼이 넓기 때문에 유전자 전달 벡터로서 많이 이용되고 있다.
레트로바이러스 벡터를 구축하기 위하여, 세포내로 운반하고자 하는 목적 뉴클레오타이드 서열은 레트로바이러스의 서열 대신에 레트로바이러스 지놈에 삽입되어 복제 불능의 바이러스를 생산한다. 바이리온을 생산하기 위하여, gag, pol 및 env 유전자를 포함하지만 LTR (long terminal repeat)와 Ψ서열은 없는 패키징 세포주를 구축한다 (Mann et al., Cell, 33:153-159(1983)). 운반하고자 하는 목적 뉴클레오타이드 서열, LTR 및 Ψ서열을 포함하는 재조합 플라스미드를 상기 세포주에 이입하면, Ψ서열은 재조합 플라스미드의 RNA 전사체의 생산을 가능하게 하며, 이 전사체는 바이러스로 패키징되고, 바이러스는 배지로 배출된다 (Nicolas and Rubinstein "Retroviral vectors," In: Vectors: A survey of molecular cloning vectors and their uses, Rodriguez and Denhardt (eds.), Stoneham: Butterworth, 494-513(1988)). 재조합 레트로바이러스를 함유하는 배지를 수집하고 농축하여 유전자 전달 시스템으로 이용한다.
2세대 레트로바이러스 벡터를 이용한 유전자 전달이 발표되었다. Kasahara et al. Science, 266:1373-1376(1994))은 몰로니 뮤라인 류케미아 바이러스 (MMLV)의 변이체를 제조하였고, 여기에서 EPO (erythropoietin) 서열을 엔벨로프 부위에 삽입하여 새로운 결합 특성을 갖는 키메릭 단백질을 생산하였다. 본 발명의 유전자 전달 시스템도 이와 같은 2세대 레트로바이러스 벡터의 구축 전략에 따라 제조할 수 있다.
iii. AAV 벡터
아데노-관련 바이러스 (AAV)는 비분열 세포을 감염시킬 수 있고, 다양한 종류의 세포에 감염할 수 있는 능력을 갖고 있기 때문에 본 발명의 유전자 전달 시스템으로 적합하다. AAV 벡터의 제조 및 용도에 대한 상세한 설명은 미국 특허 제5,139,941 호 및 제 4,797,368 호에 상세하게 개시되어 있다.
유전자 전달 시스템으로서의 AAV에 대한 연구는 LaFace et al, Viology, 162:483486(1988), Zhou et al., Exp. Hematol. (NY), 21:928-933(1993), Walsh et al, J. Clin. Invest., 94:1440-1448(1994) 및 Flotte et al., Gene Therapy , 2:29-37(1995)에 개시되어 있다.
전형적으로, AAV 바이러스는 두 개의 AAV 말단 리피트가 옆에 위치되어 있는 목적의 유전자 서열 (세포 내로 운반하고자 하는 목적 뉴클레오타이드 서열)을 포함하는 플라스미드 (McLaughlin et al., J. Virol., 62:1963-1973(1988); 및 Samulski et al., J. Virol., 63:3822-3828(1989)) 및 말단 리피트가 없는 야생형 AAV 코딩 서열을 포함하는 발현 플라스미드 (McCarty et al., J. Virol., 65:2936-2945( 1991))를 동시형질전환시켜 제조된다.
iv. 다른 바이러스 벡터
다른 바이러스 벡터들도 본 발명의 유전자 전달 시스템으로 이용할 수 있다. 백시니아 바이러스 (Puhlmann M. et al., Human Gene Therapy 10:649-657(1999); Ridgeway, "Mammalian expression vectors," In: Vectors: A survey of molecular cloning vectors and their uses. Stoneham: Butterworth, 467-492(1988); Baichwal and Sugden, "Vectors for gene transfer derived from animal DNA viruses: Transient and stable expression of transferred genes," In: Kucherlapati R, ed. Gene transfer. New York: Plenum Press, 117-148(1986) 및 Coupar et al., Gene, 68:1-10(1988)), 렌티바이러스 (Wang G. et al., J. Clin. Invest. 104(11):R55-62(1999)) 또는 헤르페스 심플렉스 바이러스 (Chamber R., et al., Proc. 이외에도, 바이러스 벡터로는 리오바이러스, 폭스바이러스, 셈리키 포리스터 바이러스 및 미즐즈 바이러스 (Measles virus) 등이 있다.
v. 리포좀
리포좀은 수상에 분산된 인지질에 의해 자동적으로 형성된다. 외래 DNA 분자를 리포좀으로 성공적으로 세포 내로 운반한 예는 Nicolau 및 Sene, Biochim. Biophys. Acta, 721:185-190(1982) 및 Nicolau et al., Methods Enzymol., 149:157-176(1987)에 개시되어 있다. 한편, 리포좀을 이용한 동물세포의 형질전환에 가장 많이 이용되는 시약으로는 리포펙타민(Lipofectamine, Gibco BRL)이 있다. 운반하고자 하는 목적 뉴클레오타이드 서열을 내포한 리포좀은 엔도사이토시스, 세포 표면에로의 흡착 또는 플라즈마 세포막과의 융합 등의 기전을 통해 세포와 상호작용하여 세포내로 목적 뉴클레오타이드 서열을 운반한다.
본 발명의 일 구체예에서, 상기 유전자 전달 시스템은 재조합 아데노바이러스일 수 있다.
유전자 전달 벡터로서 재조합 아데노바이러스의 여러 가지 장점들이 부각되면서 암 유전자 치료에서 그 사용빈도는 꾸준히 증가하고 있는 추세이다. 특히, 유전자 치료제로 암을 치료하고자 할 경우, 장기적이고 지속적인 치료 유전자의 발현이 필요하지 않은 장점이 있다. 또한, 벡터로 사용되는 바이러스에 의해 유도되는 숙주의 면역반응이 크게 문제되지 않거나 오히려 이점으로 작용할 수 있기 때문에 재조합 아데노바이러스는 암 치료용 유전자 전달체로 각광을 받고 있다.
상기 재조합 아데노바이러스는 복제불능 아데노바이러스 또는 종양 살상 아데노바이러스일 수 있다.
복제불능 아데노바이러스는 아데노바이러스의 복제에 필수적인 E1 유전자(전부 또는 일부) 대신에 치료용 유전자를 삽입하여 재조합된 것으로, 아데노바이러스가 도입된 세포 내에서 복제되지 못하도록 만든 것이다.
종양 살상 아데노바이러스는 E1B 55kDa 유전자가 부분적으로 결손된 아데노바이러스로 p53이 기능적으로 비활성화된 세포에서만 증식이 가능하다. p53의 기능이 억제되어 있는 암세포에서는 바이러스의 증식이 활발하게 일어나게 되는 반면 정상세포에서는 바이러스의 증식이 억제된다. 따라서, 종양 살상 아데노바이러스는 정상세포에는 영향을 주지 않고, 암세포만 선택적으로 사멸시킬 수 있어 암 치료에 있어 특히 유리하다.
본 발명의 일 구체예에서, 재조합 아데노바이러스는 비활성화된 E1B 19kDa 유전자, E1B 55kDa 유전자 또는 E1B 19kDa/E1B 55kDa 유전자를 가지며, 바람직하게는 비활성화된 E1B 19kDa 및 E1B 55kDa 유전자를 갖는다.
본 명세서에서, 유전자와 관련하여 사용되는 용어 "비활성화"는 그 유전자의 전사 및/또는 해독이 정상적으로 이루어지지 아니하여, 그 유전자에 의해 코딩되는 정상적인 단백질의 기능이 나타나지 않는 것을 의미한다. 예를 들어, 비활성화 E1B 19kDa 유전자는 그 유전자에 변이 (치환, 부가, 부분적 결실 또는 전체적 결실)가 발생되어 활성의 E1B 19 kDa 단백질을 생성하지 못하는 유전자이다. E1B 19kDa 유전자가 결여되는 경우에는 세포고사능을 증가시킬 수 있고, E1B 55kDa 유전자가 결여된 경우에는 종양세포 특이성을 갖게 한다 (참조: 대한민국 특허출원 제 2002-0023760 호).
본 발명의 일 구체예에 따르면, 본 발명의 재조합 아데노바이러스는 활성의 E1A 유전자를 포함할 수 있다. E1A 유전자를 포함하는 재조합 아데노바이러스는 복제 가능한 특성을 갖게 된다. 본 발명의 보다 바람직한 구현예에 따르면, 본 발명의 재조합 아데노바이러스는 비활성화된 E1B 19kDa/E1B 55kDa 유전자 및 활성의 E1A 유전자를 포함한다.
본 발명의 일 구체예에서, 유전자전달시스템은 E1 부위가 결실된 복제-불능(replication-incompetent) 종양 선택적 살상 아데노바이러스일 수 있다.
또한, 상기 벡터는 선별마커를 추가로 포함하는 것이 바람직하다. 본 발명에서 용어 "선별마커(selection marker)"란 shRNA 발현 카세트가 도입되어 형질전환된 세포의 선별을 용이하게 하기 위한 것이다. 본 발명의 벡터에서 사용할 수 있는 선별마커로는 벡터의 도입 여부를 용이하게 검출 또는 측정할 수 있는 유전자라면, 특별히 한정되지 않으나, 대표적으로 약물 내성, 영양 요구성, 세포 독성제에 대한 내성 또는 표면 단백질의 발현과 같은 선택가능 표현형을 부여하는 마커들, 예를 들어 GFP(녹색 형광 단백질), 퓨로마이신(puromycin), 네오마이신(Neomycin: Neo), 하이그로마이신(hygromycin: Hyg), 히스티디놀 디하이드로게나제(histidinol dehydrogenase gene: hisD) 또는 구아닌 포스포리보실트랜스퍼라제(guanine phosphosribosyltransferase: Gpt) 등이 있다.
또한, 상기 광감각제는 포르피린계(phorphyrins) 화합물, 클로린계(chlorins) 화합물, 박테리오클로린계(bacteriochlorins) 화합물, 프탈로시아닌계(phthalocyanines) 화합물, 나프탈로시아닌계(naphthalocyanines) 화합물 및 5-아미노레불린 에스테르계(5-aminolevuline esters) 화합물로 이루어진 군에서 선택될 수 있다.
바람직하게는, 700-900 nm 파장 범위의 근적외선 하에서 광열 효과를 나타내는 IR-780, Cu-64-DOTA 등일 수 있다.
또한, 본 발명의 약제학적 활성성분의 종류는 특별히 한정되지 않으며, 예를 들면, 항암제, 항생제, 호르몬, 호르몬 길항제, 인터루킨, 인터페론, 성장 인자, 종양 괴사 인자, 엔도톡신, 림포톡시, 유로키나제, 스트렙토키나제, 조직 플라스미노겐 활성제, 프로테아제 저해제, 알킬포스포콜린, 방사선 동위원소 표지 성분, 계면활성제, 심혈관계 약물 및 신경계 약물로 이루어진 군으로부터 선택된 하나 이상을 들 수 있다.
상기 중 항암제는 베바시주맙(bevacizumab), 테모졸로마이드(temozolomide), 나이트로조유레아(nitrosourea), 시스플라틴(cisplatin), PCV(procarbazine+CCNU+vincristine), 빈블라스틴(vinblastine), 빈크리스틴(vincristine), 빈플루닌(vinflunine), 빈데신(vindesine), 비노렐빈(vinorelbine), 카바지탁셀(cabazitaxel), 도세탁셀(docetaxel), 라로탁셀(larotaxel), 오르타탁셀(ortataxel), 파클리탁셀(paclitaxel), 테세탁셀(tesetaxel), 이사베필론(ixabepilone), 허셉틴(herceptin), 어비툭스(erbitux), 사이클로포스파마이드(Cyclophosphamide), 독소루비신(Doxorubicin), 에토포사이드(Etoposide), 토포테칸(Topotecan), 카르보플라틴(carboplatin), 프로카르바진(procarbazine), 메클로레타민(mechlorethamine), 이포스파미드(ifosfamide), 멜팔란(melphalan), 클로라부실(chlorambucil), 비술판(bisulfan), 디악티노마이신(dactinomycin), 다우노루비신(daunorubicin), 블레오마이신(bleomycin), 플리코마이신(plicomycin), 미토마이신(mitomycin), 탁목시펜(tamoxifen), 트랜스플라티눔(transplatinum), 5-플루오로우라실(5-fluorouracil) 또는 메토트렉세이트(methotrexate) 등을 들 수 있다.
본 발명의 약물전달체는 PPA5 폴리머의 종양 표적성을 통해 뇌종양의 예방 또는 치료를 위해 사용될 수 있으며, 이외 표적 성분과의 결합을 통해 표적 질병 부위로 약물을 전달할 수 있으므로 중추신경계 질환 또는 신경퇴행성 질환의 예방 또는 치료를 위해 사용될 수 있다.
상기 중추신경계 질환은 인지장애, 지적장애, 소뇌증, 뇌전증, 신경발달장애, 치매, 자폐스펙트럼장애, 다운증후군, 레트증후군, 취약X증후군 등을 들 수 있다.

상기 뇌종양(brain tumor 또는 encephaloma)은 뇌질과 뇌막에 생기는 종양을 말한다. 상기 뇌종양은 신경교종(glioma), 희돌기세포교종(oligodendroglioma), 교모세포종(glioblastoma), 교질낭종(colloid cyst), 유표피종(epidermoid cyst) 수막종(meningioma), 혈관모세포종(hemangioblastoma), 림프종(lymphoma), 뇌하수체선종(pituitary adenoma), 전이암(metastatic tumor), 또는 이들의 조합일 수 있다. 예를 들어, 교종은 다형성 교모세포종(glioblastoma multiforme: GBM)이다. 상기 뇌종양은 원발성 뇌종양 또는 다른 암으로부터 전이된 종양일 수 있다.
또한, 본 발명의 PPA 폴리머는 진단제(진단용 표지물질)이 표지될 수 있고, 표지물질에 의해 표지된 폴리머는 생체 내에서 추적될 수 있어 중추신경계 질환, 신경퇴행성 질환 또는 뇌종양의 광학적 검출 및 이미징이 가능하다.
상기 진단제는 표적세포를 탐지해내어 인식가능하게 할 수 있는 물질이라면 제한 없이 사용할 수 있다. 예컨대, 생체를 투과할 수 있는 근적외선 계열의 형광물질, 예컨대, 시아닌, 알로피코시아닌(allophycocyanin), 플루오레신(fluorescein), 테트라메틸로드아민(tetramethylrhodamine), 보디피(BODIPY) 또는 알렉사(Alexa) 등; Calcium-47, Carbon-11, Carbon-14, Chromium-51, Cobalt-57, Cobalt-58, Erbium-169, Fluorine-18, Gallium-67, Gallium-68, Hydrogen-3, Indium-111, Iodine-123, Iodine-131, Technetium-99m와 같은 방사선의약품; 또는 MRI 조영제 등을 들 수 있다.
본 발명의 약물전달체는 진단 대상에서 분리한 조직 또는 세포에 투여하여 PPA 폴리머 및/또는 진단제가 신호를 감지하여 영상을 수득하는데 이용될 수 있다. 이러한 신호를 감지하기 위해서는 자기공명영상장치 또는 광학영상장치를 이용할 수 있다. 상기 자기공명영상장치는 강력한 자기장 속에 생체를 넣고 특정 주파수의 전파를 조사하여 생체조직에 있는 수소 등의 원자핵에 에너지를 흡수시켜 에너지가 높은 상태로 만든 후, 상기 전파를 중단하여 상기 수소 등의 원자핵 에너지가 방출되게 하고 이 에너지를 신호로 변환하여 컴퓨터로 처리하여 영상화한 장치이다. 자기 또는 전파는 골에 방해를 받지 않기 때문에 단단한 골 주위 또는 뇌나 골수의 종양에 대하여 종단, 횡단, 임의의 각도에서 선명한 입체적인 단층상을 얻을 수 있다. 특히 상기 자기공명영상장치는 T2 스핀-스핀 이완 자기공명영상장치인 것이 바람직하다.
본 발명의 약물전달체는 약학적으로 허용가능한 담체를 추가로 포함할 수 있으며, 담체와 함께 제제화될 수 있다. 본 발명에서 용어, "약학적으로 허용가능한 담체"란 생물체를 자극하지 않고 투여 화합물의 생물학적 활성 및 특성을 저해하지 않는 담체 또는 희석제를 말한다. 액상 용액으로 제제화되는 약물전달체에 있어서 허용되는 약제학적 담체로는, 멸균 및 생체에 적합한 것으로서, 식염수, 멸균수, 링거액, 완충 식염수, 알부민 주사용액, 덱스트로즈 용액, 말토 덱스트린 용액, 글리세롤, 에탄올 및 이들 성분 중 1 성분 이상을 혼합하여 사용할 수 있으며, 필요에 따라 항산화제, 완충액, 정균제 등 다른 통상의 첨가제를 첨가할 수 있다. 또한 희석제, 분산제, 계면활성제, 결합제 및 윤활제를 부가적으로 첨가하여 수용액, 현탁액, 유탁액 등과 같은 주사용 제형, 환약, 캡슐, 과립 또는 정제로 제제화할 수 있다.
본 발명의 약물전달체를 유효성분으로 포함하는 비강투여용 제형으로는, 주사용 형태, 또는 호흡기를 통하여 흡입이 가능하도록 하는 에어로졸제 등 스프레이용으로 제제화할 수 있다. 주사용 제형으로 제제화하기 위해서는 본 발명의 약물전달체를 안정제 또는 완충제와 함께 물에서 혼합하여 용액 또는 현탁액으로 제조하고, 이를 앰플 또는 바이알의 단위 투여용으로 제제화할 수 있다. 에어로졸제 등의 스프레이용으로 제형화하는 경우, 수분산된 농축물 또는 습윤 분말이 분산되도록 추진제 등이 첨가제와 함께 배합될 수 있다.
본 발명의 약물전달체는 비강-뇌 전달용 약물전달장치를 통해 비강-뇌 투여 경로로 분사될 수 있다.
상기 비강-뇌 전달용 약물전달장치는 공지의 네뷸라이저 형태를 사용할 수 있다.
본 발명의 약물전달체는 단독의 요법으로 이용될 수 있으나, 다른 통상적인 화학 요법 또는 방사선 요법과 함께 이용될 수도 있으며, 이러한 병행 요법을 실시하는 경우에는 보다 효과적으로 질환 치료가 가능하다.
따라서, 본 발명의 또 다른 실시양태로서, 본 발명은 상기 약물전달체를 개체에 비강투여하는 단계를 포함하는 비강투여를 통한 뇌로의 약물 전달 방법을 제공한다.
본 발명의 또 다른 실시양태로서, 본 발명은 상기 약물전달체를 필요로 하는 개체에 약학적으로 유효한 양으로 비강투여하는 단계를 포함하는, 중추신경계 질환, 신경퇴행성 질환 또는 뇌종양 중 어느 하나를 치료하는 방법을 제공한다.
본 발명의 용어 "약학적으로 유효한 양"은 의학적 치료에 적용가능한 합리적인 수혜/위험 비율로 질환을 치료하기에 충분한 양을 의미한다. 본 발명의 약물전달체의 적합한 투여량은 제제화 방법, 투여 방식, 환자의 연령, 체중, 성, 질병 증상의 정도, 음식, 투여 시간, 배설 속도 및 반응 감응성과 같은 요인들에 의해 다양하며, 보통으로 숙련된 의사는 목적하는 치료에 효과적인 투여량을 용이하게 결정 및 처방할 수 있다. 일반적으로, 본 발명의 약물전달체는 1Х10

본 발명의 용어 "개체"는 본 발명에 따른 약물전달체의 투여에 의해 증상이 호전될 수 있는 질병을 가진 말, 양, 돼지, 염소, 낙타, 영양, 개 등의 동물 또는 인간을 포함한다. 본 발명에 따른 약물전달체를 개체에게 투여함으로써, 질병을 효과적으로 예방 및 치료할 수 있다. 본 발명에 따른 상기 치료방법은 인간을 제외한 동물을 치료하는 방법일 수 있으나, 이에 제한되지 않는다. 즉, 인간의 경우 본 발명에 따른 약물전달체의 투여에 의해 증상이 호전될 수 있는 질병을 가지는 것을 고려할 때, 인간의 치료에 있어서도 충분히 사용될 수 있다.
이하, 본 발명의 실험예를 통해 상세히 설명한다. 하기 실험예는 본 발명을 예시하는 것일 뿐 본 발명의 범위가 하기 실험예에 한정되는 것은 아니다. 본 실험예들은 본 발명의 개시가 완전하도록 하고, 본 발명이 속하는 기술 분야에서 통상의 지식이 가진 자에게 발명의 범주를 완전하게 알려주기 위해 제공되는 것이며, 본 발명은 청구항의 범주에 의해 정의될 뿐이다.
<실시예 1> PPCBA-PEI-Arg의 합성
(1) 폴리(에틸렌이민)-아르기닌(PEI-Arg)의 합성
아르기닌을 폴리에틸렌이민에 참고문헌에 따라 컨쥬게이션 하였다(Journal of Colloid and Interface Science 348 (2010) 360-368, Biomaterials 31(2010) 8759-8769). 아르기닌 아미노산(350 mg, 2.0 mmol)의 카르복실산기를 PBS(phosphate saline buffer, pH= 7.4 (PBS, 3.0 mL)에서 EDC/NHS (EDC, 384 mg, 2.0 mmol 및 NHS = 230 mg 2.0 mmol) 시약과 4℃에서 4시간 동안 커플링시킴으로써 활성화시켰다. 그리고 나서, 폴리에틸렌이민(polyethylenimine(PEI)(360 mg, 0.2 mmol))을 첨가하여 아르기닌을 활성화시키고, 반응은 실온에서 18시간 동안 수행되었다. 반응 산물을 2차 증류수에 대해 1일 동안 투석하여(MWCO 1.0 kDa) 미반응 화합물을 제거하고 동결건조하여, 백색 물질로 PEI-Arg를 얻었다. 화학 구조는

(2) PPCBA-PEI-Arg(PPA)의 합성
pH-감응형 및 생환원성 폴리머를 생성하기 위해, mPEG-acrylate(125 mg, 0.025 mM(PEG = 5.0 kDa 또는 2.0 kDa; Laysan Bio, Inc)) 및 N, N'-cystamine bisacrylamide(71.5 mg, 0.275 mM)(PolySciences Inc., Warrington, PA)를 메탄올(5.0 mL)에 녹였다. 이어서, 피페라진(19.5 mg)(Sigma-Aldrich)을 첨가하고, 반응 혼합물은 100% 질소 대기 하에서 50℃-60℃에서 48시간 동안 암 조건에서 교반하고, 실온에서 냉각 후 PEI-Arg(12 mg)를 첨가하였다. 결과적인 반응 혼합물을 실온에서 하루 동안 교반하고, 최종 폴리머는 이차 증류수에 대해 하루 동안 투석(MWCO 3.5 kDa, Pro Spectrum, dialysis membrane) 시킨 후 동결건조하여 백색 물질로서 PPCBA-PEI-Arg를 얻었다. (3) 세포주 및 세포배양
U87MG-Fluc 세포주를 American Type Culture Collection(ATCC, Manassas, VA)로부터 구입하였고, 10% FBS(Gibco BRL) 및 HEPES(Gibco BRL)를 포함하고 있는 DMEM(Gibco BRL, Grand Island, NY) 배지에서 37℃, 5% CO
2 조건으로 배양하였다.
(4) 아데노바이러스(Ad) 준비
E1 부위에서 CMV 프로모터 조절하에 GFP(green fluorescent protein)를 발현하는 비복제성 Ad(dE1/GFP) 및 종양살상 Ad(DWP418, RdB/IL-12/Decorin 또는 HE5cT-Rd19-k35/Decorin; oAd)를 사용하였으며, 기본적으로 본 발명자들의 기존 연구에 기재된 바와 같은 방법을 사용하였다(Kim, E. et al. Hum. Gene Ther. 2003, 14, 14151428; Kim, J. H. et al. J. Natl. Cancer Inst. 2006, 98, 14821493; Choi, J. W. et al.Gene Ther. 2013, 20, 880892; Kim, P. H. et al. Biomaterials 2011, 32, 93289342). 모든 Ad를 HEK293 세포에서 번식시킨 다음, CsCl(Sigma, St Louis, MI) 농도 구배 정제를 하였다. 바이러스 입자(VP)의 수를 OD
260 측정에서 흡광도 1을 10
12 VP/㎖와 동등한 것으로 하여 계산하였다. HEK293 세포에 대한 한계희석검정법을 이용하여 바이러스 역가(Infectious titers, PFU/mL)를 결정하였다. dE1/GFP 및 DWP418의 바이러스 파티클/PFU 비율은 각각 29:1 및 81:1이었다. MOI를 바이러스역가로부터 계산하였다.
(5) Ad/PPA 복합체 준비
Ad/PPA 복합체를 제작하기 위하여, Ad 입자(2×10
10 VP/PBS, pH 7.4)를 다양한 농도의 PPA 폴리머와 혼합하였다. 그 결과 폴리머 당 Ad 입자의 몰 비율은 1Х10
5 및 1Х10
6이 되었다. 사용하기 전에 용액을 실온에서 30분 동안 배양하였다.
(6) 통계 분석
데이터를 평균±표준편차(SD)로 나타내었다. 통계분석을 양측 Student t 검정(SPSS 13.0 software; SPSS, Chicago, IL)으로 수행하였으며, P 값이 0.05 미만인 경우에 통계적으로 유의한 것으로 간주하였다.
<실험예 1> Ad/PPA-IR780의 생체 내 형광 이미지
Ad/PPA-IR780 복합체의 생체 내 분포를 분석하기 위해, 복합체를 비강 경로로 주사하고, 투여 후 5분, 6, 12, 18, 24, 48 h에 NIR 이미징을 수행하였다.
도 1은 주사 후 5분 내지 18h까지 신호가 유사하게 남아 있고, 이후 48h까지 시간 의존적 방식으로 점차 감소함을 보여준다. Ad/PPA-IR780 복합체가 종양 조직으로 유입됨에 따라 종양 보유 뇌 영역에 많은 형광 신호가 유지되었다: 종양 영역에서의 최대 형광 강도는 조사 후 5분에 달성되었다. 이 결과는 Ad/PPA-IR780 복합체가 비강 경로를 통해 뇌종양을 표적으로 하는 적당한 치료제임을 시사한다.
<실험예 2> Ad/PPA-IR780 복합체의 강력한 항암 효과
뇌종양 모델을 구축하기 위해, 반딧불이 루시퍼레이즈 유전자를 안정하게 발현하는 인간 교모세포종 세포주인 U87MG-Fluc를 해밀턴 주사기를 이용하여 뇌에 정위적으로 주사하였다. 비침습적으로 종양 성장을 모니터링하기 위해 6일 간격으로 생체발광 이미징을 수행하였다. 21일째에 Ad/PPA5-IR780 복합체를 비강 주사하고 나서, 투여 후 6시간째에 뇌종양에 대해 레이저 조사를 수행하였다.
도 2에 도시된 바와 같이, 종양 세포 주사 후 30일째에, Ad/PPA5-IR780(7.26Х10
6 p/s) 처리된 마우스의 동소 뇌종양에서 얻은 루시퍼레이즈 신호는 비히클 처리군에서 관찰되는 것보다 유의적으로 낮았고, PBS(9.48Х10
8 p/s)보다 131배 낮은 종양 형광 강도를 보였다. 이들 결과는 Ad/PPA5-IR780 복합체가 고도의 응집성 동소 교모세포종의 종양 성장 억제를 유도할 수 있음을 보여준다.
<실험예 3> 종양 표적화 및 항종양 효과 모니터링을 위한 엑스 비보 이미징
엑스 비보 생체발광 이미징을 수행하여 동소 종양의 형광 강도를 평가하였다.
도 3에 도시된 바와 같이, 형광 강도는 PBS-처리군(1.26Х10
9 p/s)에 비해 Ad/PPA5-IR780-처리군(7.64Х10
6 p/s)에서 165배 더 낮았다. 이 결과는 비강투여된 Ad/PPA5-IR780 복합체의 강력한 항종양 효과를 확인시켜준다.
또한, 엑스 비보 NIR 이미징 및 광학 사진 비교를 통해 Ad/PPA5-IR780 복합체의 강력한 항종양 효능을 재확인하였다.
<실험예 4> Ad/PPA-IR780의 항종양 효능 및 생체 내 분포
U87MG-Fluc 동소 교모세포종 모델을 사용하여 Ad 및 다양한 유형의 IR780-결합된 폴리머를 이용한 나노복합체의 다양한 비강투여의 치료적 효능 및 생체 내 분포 프로파일을 비교하였다. 모든 처리는 세포 주사 후 6일째에 비강 경로를 통해 투여되었다. 모든 종양에 대해 처리의 비강투여 후 6시간째에 레이저 조사를 수행하였다.
도 4에 도시된 바와 같이, 종양 세포 주사 후 13일째에, Ad/PPA5-IR780, Ad/PPA2-IR780, Ad/PP-IR780 투여된 마우스의 동소 종양에서 얻은 루시퍼레이즈 신호는 PP에 의해 감쇄되었다.
생체 내에서 다양한 Ad/폴리머 복합체의 생체 내 분포 프로파일을 평가하기 위해, 종양 세포 주사 후 6일째에 치료제의 1차 비강투여 후 24시간 마다 NIR 이미징을 실시하였다.
도 5에 도시된 바와 같이, Ad/PPA2-IR780 또는 Ad/PPA5-IR780 처리된 마우스는 Ad/PAMAM(G2)-IR780 처리된 마우스에 비해 뇌 조직에서 더 높은 형광 강도를 보였다; Ad/PAMAM(G2)-IR780를 투여한 후 뇌 영역에서는 최소 형광 내지는 형광이 검출되지 않았다. 이 결과는 비강 주사를 통한 효율적인 뇌 전달이 PPA-기반 폴리머의 일반적인 기능임을 시사한다.

다양한 Ad/폴리머 복합체의 항종양 효과를 비교하기 위해, 3일 간격으로 총 3회 Ad/PAMAM-PEG, Ad/PPSA(PPSA는 KR10-2016-0103078에 개시됨), Ad/APP 또는 Ad/PPA5-IR780를 마우스에 비강투여하였다. 일군의 마우스는 정맥 경로를 통해 Ad/PPA5-IR780를 받았다(Ad/PPA5-IR780(I.V.) 군으로 명명). 모든 종양에 대해 처리 후 6h째에 레이저를 조사하였다.
도 7에 도시된 바와 같이, 종양 세포 주사 후 26일째에, Ad/PPA5-IR780(I.N. 주사) 처리된 마우스의 동소 종양에서 얻은 루시퍼레이즈 신호는 다른 군에 비해 현저하게 낮았고, Ad/PAMAM-PEG, Ad/PPSA, Ad/APP 또는 Ad/PPA5-IR780(I.V. 주사) 처리된 군에 비해 각각 30.5-, 29.5-, 9.66-, or 27.9-배 더 낮은 종양 형광을 보여, PPA5-IR780-매개된 비강 전달은 정맥으로 전달되는 것에 비해 훨씬 더 좋은 치료적 효능을 유도함을 보여준다.

<실험예 8> PPA-IR780-PTX의 비강투여 후 뇌조직에서 PTX의 분포 프로파일
PPA-IR780 폴리머에 결합된 PTX가 뇌 조직에 효율적으로 전달될 수 있는지를 측정하기 위해, PTX-결합된 폴리머 변이체(PPA5-PTX-IR780)를 정맥 또는 비강 경로 중 어느 하나를 통해 투여하였다. 주사 후 0.5, 1, 2 및 6h째에, 뇌 조직을 수확하고, 뇌 조직의 다양한 해부 영역에서의 PTX 양을 LC-MS/MS에 의해 측정하였다.
도 11에 도시된 바와 같이, 뇌 종양 보유 마우스 모델에서, 뇌의 하위 영역에서 PTX 양은 0.5h 내에 불규칙하게 상승하고, 비강 주사 후 2h째에 정상에 도달하였다. 비강 주사 2h 후 종양 내 PTX 농도는 뇌의 하위 영역 중 가장 높았다.
정상 마우스 모델에서, PPA5-PTX-IR780의 비강 주사는 도 11에서 종양 보유 마우스에서 관찰된 것과 유사하게 PTX를 뇌 조직으로 효율적인 유입을 유도한다(도 12); 유사한 생체 내 분포 패턴이 두 마우스에 관찰되나, 종양 보유 마우스보다 정상 마우스에서 PTX의 총량이 약간 더 낮았다. 이들 결과는 종양의 존재가 PPA5-PTX-IR780의 효율적인 뇌 전달에 필수적인 것은 아님을 시사한다. 중요한 것은, 이들 결과는 PPA5-IR780 폴리머 백본이 중추신경계 질환, 신경퇴행성 질환 또는 뇌종양의 치료를 위한 약물전달체로 이용될 수 있음을 시사한다.
PPA5-PTX-IR780의 비강 전달 후 PTX의 DTP(코-뇌로의 직접 전달) 효율은 도 11 및 도 12에서 얻은 값을 가지고 DTP(nose-to-brain direct transport) 및 DTE(drug targeting efficiency)를 계산하여 분석하였다. DTP%는 후각 경로를 통해 뇌로 직접적으로 전달되는 약물의 비율을 나타낸다. PPA5-PTX-IR780는 가장 높은 DTE% (종양 모델에서 1620.95 +/- 38.81 및 정상 모델에서 1019.17 +/- 28.39) 및 DTP% (종양 모델에서 93.83 +/- 0.97 및 정상 모델에서 90.18 +/- 0.96)를 보여 비강 경로를 통한 PPA5-PTX-IR780 투여는 비강의 후각 영역을 통한 효율적인 전달로 인해 정맥 주사보다는 뇌로의 치료제 전달을 더 좋게 함을 보여준다.
도 13에 도시된 바와 같이, 비강 경로를 통해 뇌로 전달된 PTX의 양은 종양 모델 및 정상 모델을 비교하였을 때 종양 모델에서 더 높았다.
<실험예 9> siRNA-Cy5.5/PPA-IR780의 비강투여 후 siRNA의 뇌 분포
siRNA가 PPA5-IR780 폴리머에 의해 비강으로 전달될 수 있는지를 평가하기 위해, Cy5.5-labeled siRNA(siRNA-Cy5.5)를 비강투여를 위해 PPA5-IR780와 복합체를 형성하였다. 0.5, 1, 2 및 6h 주사 후, 형광 리더를 사용하여 뇌 조직에서 siRNA-Cy5.5 존재를 시험하였다.
도 14에 도시된 바와 같이, 비강투여된 Cy5.5-labeled siRNA의 형광 강도는 2h 조사 후 동소 뇌종양에서 가장 높은 수준에 도달하였다; 종양 영역에서 이 시간의 강도는 뇌의 다른 모든 영역에서 관찰된 것보다 상위에 있었다. 이들 결과는 siRNA 역시 전달체로 PPA-IR780를 사용하여 비강 경로를 통해 종양에 효율적으로 전달될 수 있음을 보여준다.
<실험예 10> pDNA-FITC/PPA-IR780의 비강투여 후 pDNA의 뇌 분포
pDNA가 비강을 통해 뇌로 전달되는지 측정하고, 그것의 전달 효율을 평가하기 위해, FITC-labeled pDNA(pDNA-FITC)를 비강 전달을 위해 PPA5-IR780과 복합체를 형성하였다. pDNA-FITC/PPA5-IR780 복합체를 정맥 및 비강 경로를 통해 투여하였다. 주사 후 0.5, 1, 2 및 6h에, 형광 리더를 사용하여 pDNA의 존재를 뇌 조직에서 시험하였다.
도 15에 도시된 바와 같이, 정맥 투여와 비교하여 뇌의 다양한 부분에서 현저히 더 높은 수준의 비강투여된 FITC-labeled pDNA이 관찰되었다. 중요한 것은, pDNA의 가장 높은 양은 2 내지 6h 주사 후 종양 조직에서 검출되어, PPA5-IR780이 종양으로의 pDNA의 우호적인 유입을 촉진함을 입증한다.
정상 마우스 모델에서, pDNA-FITC/PPA5-IR780의 비강투여는 도 15의 종양 보유 마우스에서 관찰된 것과 유사하게 뇌 조직으로의 pDNA의 효율적인 유입을 유도하였다(도 16); 유사한 생체 내 분포 패턴이 두 마우스에서 관찰되나, 종양 보유 마우스보다 정상 마우스에서 pDNA의 총량이 약간 더 낮았다(도 17). 이들 결과는 종양의 존재가 PPA5-IR780에 의해 pDNA의 효율적인 뇌 전달에 필수적인 것은 아님을 시사한다.
PPA5-IR780 폴리머의 어떤 성분이 비강 경로를 통한 효율적인 뇌 유입에 중요한지를 평가하기 위해, pH-반응성 또는 IR780 부분 중 어느 하나가 없는 PPA5-IR780의 몇몇 폴리머 유도체를 생산하였다. pH-반응성이 없는 유도체의 생산을 위해 pH-반응 모이어티인 피페라진을 pH-반응성이 없는 디티오쓰레이톨(DTT) 모이어티로 치환하여 P(DTT)PA5-IR780를 생산하였다. 또한, PPA5-IR780 및 P(DTT)PA5-IR780 둘 다에 대해 광감각제가 없는 대조군 폴리머를 생산하여 결과적으로 각각 PPA5 및 P(DTT)PA5 폴리머를 생산하였다. PPA5-IR780과 하나 또는 두 개의 기능성 모이어티가 없는 이의 유도체들의 뇌 전달 효율을 비교하기 위해, pDNA-FITC를 P(DTT)PA5, P(DTT)PA5-IR780, PPA5, 또는 PPA5-IR780 폴리머와 복합체를 형성하고 나서 비강 경로로 투여하였다. 주사 후 0.5, 1, 2 및 6h에, 형광 리더를 사용하여 뇌 조직의 다양한 영역에서 pDNA의 존재를 시험하였다.
도 18에 도시된 바와 같이, FITC 강도는 비-pH 반응성 폴리머와 복합체를 형성한 pDNA-FITC가 처리된 군(그룹 2 및 3)과 비교하여 pH 반응성 폴리머와 복합체를 형성한 pDNA-FITC가 처리된 마우스(그룹 4 및 5)에서 더 높았다. 또한, 이들 결과는 비강투여된 pDNA/폴리머 복합체의 뇌 유입 효율은 그룹 2 및 4가 뇌 조직의 다양한 영역에서 그룹 3 및 5와 유사한 축적을 보여주는 것과 같이, 광감작성 IR780 모이어티에 의존하지 않음을 나타냈다. 더불어, 이들 결과는 pH 민감도가 비강 경로를 통한 치료제의 유효한 폴리머-매개 전달에 중요함을 시사한다.
<실험예 11> 비강투여에 의한 oAd/PPA-Cu-64-DOTA-허셉틴 복합체의 효율적인 전달: PET/CT 이미징
PET/CT 이미징을 통해 비강투여, 복강 투여 및 정맥 투여를 통한 oAd/PPA-Cu-64-DOTA-허셉틴 복합체의 뇌로의 전달 효율을 측정하였다.
도 19에 도시된 바와 같이, 주사 후 1h째에, 비강 전달은 다른 전달 방법과 비교하여 뇌 영역에서 가장 강한 신호를 나타냈다.
<실험예 12> 정상 마우스에서 PPA-Erbitux 전달
LC-MS/MS 실험을 이용하여 정맥투여와 비강투여시의 치료 항체가 결합된 PPA의 뇌로의 전달효율을 비교하였다.
도 20에 도시된 바와 같이, 치료 항체와 PPA5의 결합을 통해 비교군인 PBS나 정맥투여(IV)를 통한 전달보다 비강투여한 실험군에서 효과적으로 뇌로 전달될 수 있었다. 또한, 뇌 조직에서 시간에 따른 투여효율을 비교한 결과, 투여 후 2시간 후에 초기 투여량의 20%로 가장 높은 비율로 검출되었다.
<실험예 13> 비강투여 후 PPA-IR780-PTX의 생체이용률
LC-MS/MS 실험을 이용하여 정맥투여와 비강투여시의 치료 약물인 PTX가 결합된 PPA-IR780의 뇌로의 전달 효율을 비교하였다.
도 21에 도시된 바와 같이, PTX는 비강 전달에 의해 뇌의 다양한 영역으로 성공적으로 전달되었다. 또한, 뇌 조직에서 초기 투여량의 12%까지 높은 비율로 검출되었다.
<실험예 14> PPA-PTX의 강력한 항종양 효능: MR 이미징
도 22에 도시된 바와 같이, MR 이미징을 통해, PPA5-PTX의 비강 전달은 PTX I.V. 전달 및 PTX I.N. 전달보다 훨씬 더 나은 항종양 효능을 나타냈으며, PPA5-PTX(I.N.)은 뇌종양의 완전한 퇴화를 유도하였음을 보였다.
또한, U87MG/Fluc 동소 뇌종양 모델에서 주사 후 13일째에 H&E 염색 결과, PTX 농도(I.N. 전달)의 1/25 투여량은 뇌종양의 완전한 퇴화를 유도하였다(도 23).
또한, 도 24에 도시된 바와 같이, U87MG/Fluc 동소 뇌종양 모델에서 PPA5-PTX의 비강 전달은 PTX I.V. 전달 및 PTX I.N. 전달보다 훨씬 더 나은 항종양 효능을 나타냈으며, PTX 농도(I.N. 전달)의 1/25 투여량은 정맥 전달보다 훨씬 더 나은 효능을 나타냈다.
본 발명은 중추신경계 질환, 신경퇴행성 질환 또는 뇌종양의 진단 및 치료 분야에서 사용할 수 있다.
