[概略構成]
図1は、本発明の一実施の形態に係るステント(ステント11)の概略構成例を、模式的に斜視図で表したものである。ステント11は、例えば体内の管状器官としての消化管に適用される器具であり、後述するように、腫瘍によって狭窄した消化管の内腔を押し開けるために使用されるものである。具体的には、ステント11は、治療対象の部位(例えば大腸等の消化管内)に留置されるようになっている。なお、このステント11は、本発明における「ステント」の一具体例に対応している。
続いて、図2を参照して、図1に示したステント11の詳細構成例(上記した網目状構造等の構成例)について説明する。図2は、ステント11における要部(前述した端部領域Aea,Aeb付近)の詳細構成例を、模式的に展開図で表したものであり、図1に示した軸方向Zおよび周方向Rの各方向に沿って表している。
(A.基本動作)
このステント11は、患者における消化管付近の腫瘍等の治療の際に、その治療対象の部位(例えば大腸等の消化管内)に留置されることで、腫瘍によって狭窄した消化管の内腔を押し開けることが可能となる。
次いで、図1,図2に加えて図3を参照して、ステント11における作用および効果について、比較例と比較しつつ、詳細に説明する。
図3は、比較例に係るステント(ステント100)における要部(端部領域Aea,Aeb付近)の構成例を、模式的に展開図で表したものであり、軸方向Zおよび周方向Rの各方向に沿って表している。
端部領域(端部領域Aeaまたは端部領域Aeb)内における1列目(最端列目)が、デリバリーシース内から展開して拡径する際に、この1列目が突出し始めても、端部領域における各単位構造U1の一部が、デリバリーシース内に収納されている状態となる。このような状態では、端部領域における1列目は、十分には拡径できないことになる。
(B)1列目の全領域が突出し、2列目はまだ収納されているとき
端部領域内における1列目の全領域が、デリバリーシース内から外部へと突出し、2列目はまだデリバリーシース内に収納されている場合、以下のようになる。すなわち、この場合、1列目における遠位端は、拡径して元の径に戻る一方、1列目における近位端は、2列目における遠位端の径と同じになるため、元の径には戻らない(1列目の全領域が突出した段階では、1列目における近位端の径は、デリバリーシースの内径と同一となる)。これは、1列目における近位側の屈曲部b1と、2列目における遠位側の屈曲部b1とが、前述したようにして連結している(掛け合っている)ためである。するとこの場合、1列目における近位端は十分には拡径せず、1列目が斜めに拡径する状態(テーパ状態)となってしまう。
一般に、治療対象となる狭窄部に対してステントを留置する場合には、その狭窄部を跨ぐようにして、ステントが留置される。具体的には、例えば狭窄部の両側にそれぞれ、所定の長さ(例えば2cm以上)のステントが留置されているようにする。ここで、例えば、留置時のもたつきなどにより、ステント100の留置位置がずれている場合や、ステント100の留置後に位置ずれ(マイグレーション)が生じた場合、以下のようになる。すなわち、デリバリーシースが狭窄部に変更されただけで、例えば、1列目の一部が腫瘍によって押されて縮径した場合、上記問題点(A)と同様の状況となってしまい、例えば、2列目の一部が腫瘍によって押されて縮径した場合、上記問題点(B)と同様の状況となってしまう。
これに対して本実施の形態のステント11では、図2に示したように、非端部領域Amにおける各単位構造U1の軸方向長L1と比べ、端部領域Aea,Aebにおける各単位構造U2の軸方向長L2が、相対的に短くなっている(軸方向長L2<軸方向長L1)。これにより、例えば上記したように、ステント11が所定のデリバリーシース内に縮径された状態で挿入されて、このデリバリーシースが消化管内の患部付近まで運ばれた後、ステント11がデリバリーシース内から展開されて拡径する際に、以下のようになる。
続いて、上記実施の形態の変形例(変形例1~4)について説明する。なお、これらの変形例1~4において、実施の形態等における構成要素と同一のものには同一の符号を付し、適宜説明を省略する。
図4は、変形例1に係るステント(ステント11A)における要部(端部領域Aea,Aeb付近)の構成例を、模式的に展開図で表したものであり、軸方向Zおよび周方向Rの各方向に沿って表している。なお、この変形例1のステント11Aも、本発明における「ステント」の一具体例に対応している。
図5は、変形例2に係るステント(ステント11B)における要部(端部領域Aea,Aeb付近)の構成例を、模式的に展開図で表したものであり、軸方向Zおよび周方向Rの各方向に沿って表している。なお、この変形例2のステント11Bも、本発明における「ステント」の一具体例に対応している。
図6は、変形例3に係るステント(ステント11C)における要部(端部領域Aea,Aeb付近)の構成例を、模式的に展開図で表したものであり、軸方向Zおよび周方向Rの各方向に沿って表している。なお、この変形例3のステント11Cも、本発明における「ステント」の一具体例に対応している。
ここで図6を参照して、本変形例の網目状構造体111Cにおける、非端部領域Amでの編み組みパターンの構成例について、詳細に説明する。
このような構成の本変形例のステント11Cにおいても、基本的には、実施の形態のステント11と同様の作用により、同様の効果を得ることが可能である。
図7は、変形例4に係るステント(ステント11D)における要部(端部領域Aea,Aeb付近)の構成例を、模式的に展開図で表したものであり、軸方向Zおよび周方向Rの各方向に沿って表している。なお、この変形例4のステント11Dも、本発明における「ステント」の一具体例に対応している。
続いて、上記した実施の形態および変形例1~4に係るステント(ステント11,11A~11D)の、医療機器への適用例について説明する。
筒状部材12は、図8に示したように筒状(円筒状)の形状を有しており、ステント11(11A~11D)の少なくとも一部分を覆う(被覆する)ように配置されている。具体的には、この例では、筒状部材12がステント11(11A~11D)の外周側を覆うように配置されている。
以上、実施の形態、変形例および適用例を挙げて本発明を説明したが、本発明はこれらの実施の形態等に限定されず、種々の変形が可能である。

1または複数の線材を用いて形成され、前記ステントの軸方向に沿って延在する筒状構造を有する網目状構造体を備え、
前記網目状構造体では、前記軸方向に沿った双方の端部領域および非端部領域の各々において、複数の単位構造が並んで配置されており、
前記非端部領域における前記単位構造の前記軸方向に沿った長さである第1軸方向長と比べて、前記双方の端部領域のうちの少なくとも一方の端部領域における、前記単位構造の前記軸方向に沿った長さである第2軸方向長が、相対的に短くなっている
