(1)生体から分離された被検試料中に含まれる、miR-122、miR-144、let-7f、miR-128-3p及びmiR-107から成る群より選ばれる少なくとも1種のmiRNAの存在量を指標とするアルツハイマー病又は軽度認知症の検出を補助する方法であって、miR-122の存在量が健常者よりも多いこと、並びに/又はmiR-144、let-7f、miR-128-3p及びmiR-107から成る群より選ばれる少なくとも1種のmiRNAの存在量が健常者よりも少ないことが、アルツハイマー病又は軽度認知症を発症している可能性が大きいことを示す、方法(ただし、miR-144、let-7f又はmiR-107を単独で指標とする方法を除く)。
(2)生体から分離された被検試料中に含まれる、miR-122、miR-144及びlet-7fから成る群より選ばれる少なくとも1種のmiRNAの存在量を指標とするアルツハイマー病又は軽度認知症の検出を補助する(1)記載の方法。

第 1 節 臨床検体
第 1 項 使用した臨床検体
末梢血の採取は、広島大学ヒトゲノム・遺伝子解析研究倫理審査委員会から承認されたヒトゲノム遺伝子解析研究計画書に基づいて行われた。本実施例で解析に用いた末梢血の内訳を以下の表に示す。
1) EDTA-2K が添加されたベノジェクトII真空採血管に採血された 5 mL の全血を 15 mL チューブに移し、3500rpmで10分間、室温で遠心した。
2) 遠心により、上から血漿層、白血球層、赤血球層の 3 層に分離される。そのうち、血漿層のみを新しい 2 mL チューブ に移した。
3) 2)を 10000 rpm で 10 分間、室温で遠心し、採取した血漿に混入している血球成分を沈殿させた。
4) 血漿層のみを新しい1.5mLチューブに250μLずつ分注し、-80℃で凍結保存した。
血漿中 RNA の抽出は、miRNeasy Mini kit (QIAGEN) を用いて行った。
1) 凍結した血漿サンプルを融解し、10000 rpm で 5 分間、室温で遠心し、凝集タンパク質や血球成分を沈殿させた。
2) 上清 200 μL を新しい 1.5 mL チューブ に移した。
3) 1000 μL の QIAzol Lysis Reagent を加えて十分に混和し、タンパク質成分を変性した。
4) RNA 抽出のコントロール RNA として 0.05 nM cel-miR-39 を 10 μL 加え、ピペッティングで混和した後、室温で 5 分間静置した。
5) 水層と有機溶媒層の分離促進のため、200μLのクロロホルムを加え、十分に混和し、室温で 3 分間静置した。
6) 12000 xg、15分間、4℃で遠心し、上層の水層650μLを新しい2mLチューブ に移した。
7) RNA を析出させるため、975μLの 100% エタノールを加え、ピペッティングで混和した。
8) 650 μL の 7) を miRNeasy Mini spin column (以下、カラム) に移し、室温で 1 分間静置後、8000 xg、15 秒間、室温で遠心し、RNA をカラムのフィルターに吸着させた。カラムから通過した溶液は廃棄した。
9) 7) の溶液を全てカラムに通すまで 8) を繰り返し、全ての RNA をフィルターに吸着させた。
10) フィルターに付着した夾雑物の除去のため、650 μL の Buffer RWT をカラムに加え、 8000 xg、15 秒間、室温で遠心した。 カラムから通過した溶液は廃棄した。
11) フィルターに吸着した RNA の洗浄のため、500 μL のBuffer RPE をカラムに加え、8000 xg、15 秒間、室温で遠心した。カラムから通過した溶液は廃棄した。
12) フィルターに吸着した RNA の洗浄のため、500 μL のBuffer RPE をカラムに加え、8000 xg、2 分間、室温で遠心した。 カラムから通過した溶液は廃棄した。
13) フィルターに付着している溶液を完全に除去するため、カラムを新しい 2 mL collection チューブ に移し、10000 xg、1 分間、室温で遠心した。
14) カラムを 1.5 mL チューブ に移し、50 μL の RNase-free water を加え、室温で 1 分間静置した。
15) 8000 xg、1 分間、室温で遠心し、フィルターに吸着した RNA を溶出した。溶出した RNAは、そのまま次の実験に用い、残りは -80 ℃ で保存した。
第 1 項 次世代シーケンサーにおける網羅的解析
本実施例では、血漿中より抽出できる RNA 量が微量であり、濃度測定が困難であったため、 RNA 溶液の濃度調節は行わなかった。したがって、「同じ質量の RNA 中にどれだけの目的 miRNA が含まれていたか」ではなく、「同容量の血漿から抽出した RNA 溶液中にどれだけの目的 miRNA が含まれていたか」を比較したことになる。これは、この後の qRT-PCR 解析でも同様である。
第2節で抽出した 50μLの血漿由来 microRNA を 10μLに減圧濃縮する。
アダプターのハイブリダイゼーションのため、1)で濃縮したRNA溶液 4μL に 1μLの Ion Adoptor Mix v2 と 3μLのHybridization buffer を加える。
10回ゆっくりピペッチィングをした後、スピンダウンをする。
サーマルサイクラーで 65℃ 10分、16℃ 5分 反応させる。
アダプターライゲーションのため、4)で反応させた溶液に 10μL の 2x Ligation buffer と 2μL のLigation Enzyme Mix を加える。
穏やかにピペッティングし、スピンダウンをする。
サーマルサイクラーで 16℃ 16時間反応させる。
アダプターをライゲーションした microRNA を逆転写するため、7)の溶液に 2μLの Nuclease-Free water, 4μLの10x RT buffer, 2μL の 2.5mM dNTP Mix, 8μLのIon RT Primer v2 を加える。
穏やかにピペッティングしスピンダウンをする。
サーマルサイクラーで 70℃ 10分 反応させる。
直ちに反応溶液を氷上に置き、4μLの10x SuperScript III Enzyme Mix を加える。
穏やかにピペッティングし、スピンダウンする。
サーマルサイクラーで42℃ 30分反応させる。
ビーズ精製により目的の cDNA と同じ長さを持つ核酸を抽出する。Nucleic Acid Binding Beads を穏やかにvortexする。
Plocessing Plate に 5μLの Nucleic Acid Binding Beads を加える。
15)に 250μLのBinding Solution Concentrate を加え、10回ピペッティングをする。
60μLの Nuclease-Free water を13)のcDNA溶液に加える。
17)の溶液を16)に加える。
P-1000のピペットマンを275μLに合わせ、100%エタノールでチップを3回リンスする。
19)のチップのまま、275μLの100%エタノールを17)に加え、10回ピペッティングをする。
5分間室温に置く。
Processing Plate を磁気スタンドに5分間置く。
上清は廃棄し、磁気スタンドに置いたまま2分間 Nucleic Acid Binding Beads を風乾させる。
磁気スタンドからProcessing Plate を外し、13μLの Nuclease-Free water を加え、10回ピペッティングをする。
1分間室温に置く。
磁気スタンドにProcessing Plate を1分間置き、溶液 13μLを回収する。
精製した cDNA を増幅させるため、0.2mLチューブに26)のcDNA溶液 を6μL分注する。
27)に 45μLの Platinum PCR SuperMix High Fidelity, 1μLの Ion Xpress RNA-Seq Barcode, 1μLの Ion Xpress RNA 3’ Barcode Primer を加える。
サーマルサイクラーで以下の条件で反応を行った。
94℃ 2分
(2 Cycles)94℃ 30秒、50℃ 30秒、68℃ 30秒
(19 Cycles)94℃ 30秒、62℃ 30秒、68℃ 30秒
68℃ 5分
アダプターダイマーを取り除くため、増幅したcDNAを電気泳動し、ゲル切り出しを行う。10% TBE アクリルアミドゲルを作成し、120V で泳動を行う。バッファーは 1x TBE を用いた。
タッパーに 20 mL の1x TBE と、30)の10% TBE アクリルアミドゲル を入れる。
2μLのSYBR gold を加え、5分間振とうさせる。
UV照射を行い、100-106 bp 付近のバンドを切り出す。
切り出したゲルを 1.5mLチューブに入れ、チップの先で適度に砕く。
500μL の TE buffer を加える。
37℃でオーバーナイトする。
3000 rpm で3分間遠心をする。
上清を新しい 1.5 mL チューブに移し、1μLの20 mg/mLグリコーゲンを共沈剤として加える。
50μLの 3M 酢酸ナトリウムを加える。
400μLのイソプロパノールを加え、vortex を行う。
-80℃ 30分静置する。
15,000 xg 30分間 4℃ で遠心をする。
上清を廃棄し、1 mL の70%エタノールを加える。
15,000 xg 5分間 4℃ で遠心をする。
上清を廃棄し、風乾させる。
15μLの Nuclease-Free water を加える。
Bioanalyzer で DNA High Sensitivity Kit を用いて濃度測定を行う。
cDNA を 20 pM に希釈し、エマルジョンPCR を行う。エマルジョンPCR はIon OneTouch 2(lifetechnologies) を用いた。
Ion OneTouch 2 に Recovery tube, Amplification Plate を装着する。
Reagent tube に Oil を 2分の1、別のReagent tube に Recovery Solution を 4分の1 入れ、Ion OneTouch 2 に装着する。
1.5 mL low-binding チューブに、48)で希釈した 25μLの cDNA, 25μLの Nuclease-Free water, 500μLの Reagent Mix, 300μLの PCR Reagent B, 50μLの Enzyme mix を混合する。
Ion Sphere Particles を vortex し、スピンダウンした後 100μLを51)に加える。
Vortex し、スピンダウンする。
Reaction Filter Assembly に反応液を充填し、Ion OneTouch 2 に装着し、エマルジョンPCR を開始する。
エマルジョンPCR サンプルを回収し、Ion One Touch ES(lifetechnologies) を用いてビーズ精製を行う。
サンプルを回収し、全量を 0.2 mL に移す。
5μLの Control Ion Sphere を加える。
15,500 xg で2分間遠心する。
チューブに 15μL 残して上清を廃棄する。
12μL の Sequencing Primer を加える。
サーマルサイクラーで 95℃ 2分、37℃ 2分 反応させる。反応後は室温に戻す。
サンプルを 318 Chip にローディングする。
Ion PGM に装着し、解析を行う。
解析は CLC bio Genomics Workbench を用いて行った。各 microRNA のリード数は、各サンプルのリード数を 1,000,000 とした時の数に補正する。アルツハイマー群と健常人群で各 micro RNA で t検定を行い、P<0.05 となり、且つ少なくとも片方の群の平均リード数が50以上となる microRNAをピックアップし、次の qRT-PCR でさらに解析を行った。
第 1 項 血漿中 miRNA の網羅的解析は、miRCURY LNATM Universal RT microRNA PCR, Universal cDNA synthesis kit II, LightCycler480 multi-well plate (Exiqon) を用いて行った。
血漿中 miRNA の逆転写は Universal cDNA Synthesis Kit II (EXIQON) を用いて行った。
1) 0.2 mL チューブ に、2μL の 5x Reaction buffer,5μL の Nuclease-Free water,1μL の Enzyme mix, 2μLの血漿由来 RNA 溶液 を分注する。
2) タッピングで混和後、スピンダウンする。
3) サーマルサイクラーで 42℃ 60分、95℃ 5分反応を行った。
4) 合成した cDNA は 1.5 mL チューブ に移し、-80 ℃ で保存した。
real-time PCR 反応は LightCycler(商品名) 480 (Roche)、miRCURY LNATM Universal RT microRNA PCR (EXIQON)、384-well plate は LightCycler(商品名)480 Multiwell Plate 384, white (Roche) を用いて行った。PCR reaction mix、希釈済み cDNA の384-well plate への分注は、Bravo Automated Liquid Handling Platform (Agilent Technologies) を用いた。
2) 0.65 mL チューブ に、次のように PCR reaction mix を調製した (表示はレプリケート数 1 のときの 1 サンプルあたりの量)。
PCR Master mix 5μL, PCR primer mix 1μL
3) PCR reaction mix を 384-well plate に 6μLずつ分注した。
4) 1) で調製した希釈 cDNA を 384-well plate に 4μLずつ分注し、ピペッティングで十分に混和した。
5) サンプルの蒸発防止のため、384-well plate にシールを貼り、1500 xg、1 分間、室温で遠心した。
6) LightCycler(商品名) 480 (Roche) を用い、以下の条件で real-time PCR を行った。
95℃ 10分
(45 Cycles) 95℃ 10秒、60℃ 1分
Ct 値の算出には、蛍光の増幅曲線の変化幅が最大となる点を Ct 値とする二次導関数法を用い、解析には、検量線を作成せず、相対的に miRNA 量を比較する ΔΔCt 法を用いた。また、サンプル同士を比較するために、miRNA 量の補正を行う必要があるが、この補正には、第 2 節の 4) で添加した外部コントロール cel-miR-39 を用いた。補正値 (ΔCt 値) の算出方法を下記に示す。

ROC (受信者操作特性) 曲線を描き AUC を算出して比較する方法は、診断マーカーの精度評価の方法である。ROC は横軸に「1-特異度」(偽陽性率) を、縦軸に「感度」をプロットし、陽性/陰性を判断するカットオフ値を媒介変数として変化させたときに得られる曲線である。AUC (曲線下面積) はこの ROC 曲線下の面積のことであり、0.5~1 の値をとる。ある診断マーカーを用いてROC 曲線を作成し、AUC を算出した際、その値が 1 に近づけば近づく程精度の良いマーカーと評価される。一般にAUC ≧ 0.7 であれば診断マーカーとして精度がよいとされる。
第 1 節 アルツハイマー患者特異的な変動を示す血漿中 microRNA の同定
第 1 項
本節では、健常人とアルツハイマー患者の血漿中 miRNA プロファイルを網羅的に解析し、比較することで、アルツハイマー患者で変動する miRNA の同定を行った。
(第1のスクリーニング)
健常人群、アルツハイマー患者群、各 62 名の血漿中における miRNA 量を次世代シーケンサー Ion PGM (lifetechnologies)を用いて網羅的に解析し、各群における miRNA 量を比較した。各群毎に miRNA 量の平均値を算出し、各群間での比較を行った(図1-A及び図1-B)。
これら 10 種類の miRNA をアルツハイマー マーカー 候補 miRNA として、qRT-PCR による解析を行った。以下にその miRNA を記す。
健常人に比べ、アルツハイマー病患者で血漿中 miRNA 量増加:miR-122-5p, miR-185-5p
健常人に比べ、アルツハイマー病患者で血漿中 miRNA 量減少:let-7f-5p, let-7g-5p, miR-15b-5p,miR-30b-5p,miR-484-5p,miR-660-5p, miR-144-5p
(第2のスクリーニング)
第1のスクリーニング で示された健常人とアルツハイマー病患者における miRNA の血漿中量の違いは、実験に用いた検体の個人差の影響を少なからず受けていると考えられる。そこで、マーカー 候補 miRNAの血漿中量から個人差の影響を排除するため、健常人群とアルツハイマー患者群の各群をそれぞれ 62 検体に増やし、第2のスクリーニング として各検体における個々の候補 miRNA の血漿中量を qRT-PCR で測定した。第2のスクリーニング での マーカー 候補 miRNA 量の測定は、SYBR Green を用いた qRT-PCR 法で行った。
第 3 項でアルツハイマー診断 miRNA 候補として同定した miR-122, let-7f, mir-144 について、同様に qRT-PCR を行った。MCI 患者 42名におけるΔCt値を算出し、健常人群とt検定にて比較したところ、P<0.05 で有意に差がみられた。MCI 患者においても、miR-122 は上昇傾向、let-7f と miR-144 は減少傾向と、アルツハイマー患者と同様の傾向がみられた(図5-A、図5-B及び図5-C)。
2種類のmiRNAを組合せた場合の精度を検討した。具体的には以下の試験を行った。第3項及び第6項で使用した検体においてqRT-PCRを行い、ΔCt値を算出した。そして、2種類のmiRNAのΔCt値の差、ΔCt(let-7f) - (miR-122)、ΔCt(miR-144) - (miR-122)、ΔCt(miR-128-3p) - (miR-122-5p)及びΔCt(miR-107) - (miR-122-5p)を算出した。結果をそれぞれ図6-Aから図6-Dに示す。
上記した網羅的なqRT-PCRにより、アルツハイマー病で減少するマイクロRNAとして、miR-128-3p及びmiR-107が同定された。結果をそれぞれ図7及び図8に示す。また、検体情報を図9に示す。
健常人とアルツハイマー病患者におけるlet-7gを測定した。結果を図10に示す。let-7gは変動が少なく内部標準として用いることができ、診断の精度を上げることができる優位性があることがわかった。なお、let-7gの塩基配列は、ugagguaguaguuuguacaguu(配列番号6)である。
第5項と同様にして、MCI患者42名におけるΔCt(miR-128) - (miR-122)を算出した。結果を図12に示す。
Claims (6)
- 生体から分離された被検試料中に含まれる、miR-122、miR-144、let-7f、miR-128-3p及びmiR-107から成る群より選ばれる少なくとも1種のmiRNAの存在量を指標とするアルツハイマー病又は軽度認知症の検出を補助する方法であって、miR-122の存在量が健常者よりも多いこと、並びに/又はmiR-144、let-7f、miR-128-3p及びmiR-107から成る群より選ばれる少なくとも1種のmiRNAの存在量が健常者よりも少ないことが、アルツハイマー病又は軽度認知症を発症している可能性が大きいことを示す、方法(ただし、miR-144、let-7f又はmiR-107を単独で指標とする方法を除く)。
- 生体から分離された被検試料中に含まれる、miR-122、miR-144及びlet-7fから成る群より選ばれる少なくとも1種のmiRNAの存在量を指標とするアルツハイマー病又は軽度認知症の検出を補助する請求項1記載の方法。
- (1)miR-122とmiR-144、
(2) miR-122とlet-7f、
(3) miR-122とmiR-128-3p、及び
(4) miR-122とmiR-107
から成る群より選ばれる少なくとも1種の組合せの存在量を指標とする請求項1記載の方法。 - (3) miR-122とmiR-128-3pの組合せの存在量を指標とする請求項3記載の方法。
- 前記被検試料が血清又は血漿である請求項1~4のいずれか1項に記載の方法。
- let-7g-5p、miR425-3p及びmiR425-5pから成る群より選ばれる少なくとも1種のマイクロRNAを内部標準として用いる請求項5記載の方法。
