Abstract

 縫合持針器は、シース部と、シース部の先端部に設けられた一対の把持部材と、一対の把持部材に接続されシース部に対して移動可能な操作ワイヤと、長手軸に沿って延びる軸状に形成され、シース部に対して長手軸に沿う方向及び長手軸周りに移動可能に接続された操作部本体と、操作ワイヤの基端部に固定されるとともに操作部本体に対して移動可能に取付けられ、操作部本体に対して操作ワイヤを長手軸に沿う方向に移動させる牽引部材とを備え、操作部本体の外周面には、長手軸に沿って延びる第1の溝部と、第1の溝部の基端部と自身の先端部とが連なり、基端側に向かうにしたがって長手軸周りに延びる第2の溝部とが形成され、シース部には、第1の溝部及び第2の溝部内を移動可能な凸部が設けられている。

Description

縫合持針器及び内視鏡システム
 本発明は、縫合針を用いるための縫合持針器、及びこの縫合持針器を備える内視鏡システムに関する。
 本願は、2014年07月03日に、日本に出願された特願2014-137886号に基づき優先権を主張し、その内容をここに援用する。
 従来、腹腔鏡下手術、あるいは軟性の内視鏡の処置チャンネル経由で臓器などを縫合する手段の一つに、縫合糸が取付けられた縫合針と縫合持針器とを組み合わせた縫縮手技がある。
 この種の縫合持針器として、例えば特許文献1に記載された縫合持針器が知られている。
 特許文献1の縫合持針器は、可撓性を有するコイルシース(シース部)と、コイルシースの先端に取付けられた処置部(把持部)と、処置部に接続されたワイヤ(操作ワイヤ)と、コイルシースの基端に接続された本体と、本体に接続された操作部とを備えて構成されている。
 処置部は、縫合針を把持するための第1、第2のジョー(把持部材)からなる。第2のジョーにはワイヤが接続されている。
 本体は、樹脂等で形成され、硬性を有する。
 操作部は、本体に取り付けられた第1ハンドル(操作部本体)と、第1ハンドルに対して一定範囲回動可能に取り付けられた第2ハンドル(牽引部材)とから構成されている。
 第1ハンドルの先端は、本体の基端に取り付けられている。本体に挿通されたワイヤの基端は、第1ハンドルの基端側に向かって延出している。
 第2ハンドルは、第1の端部が第1ハンドルに対して回動自在に固定されている。第2ハンドルは、第1ハンドルに取り付けられた第1付勢部材の弾性力によって、第1ハンドルから離間するように付勢されている。
 第2ハンドルには、第1の端部にワイヤの基端が接続されるリンク部材が、第1ハンドルと第2ハンドルとの間で回動できるように取り付けられている。リンク部材の第2の端部は、第2ハンドルに回動自在に取り付けられている。
 このように構成された縫合持針器は、両ハンドルを把持して接近させると、ワイヤが手元側に引かれ、処置部の第2のジョーが先端側に回動して処置部が閉じる。両ハンドルを放すと、第1付勢部材の付勢力によって第1ハンドルと第2ハンドルとが離間する。ワイヤが本体側に前進し、第2のジョーの先端が基端側に回動して処置部が開く。

 縫合持針器を用いた穿針操作を大きく分解すると、縫合針を把持する把持部をシース部の長手軸周りに回転させることによる運針(シース部の回転操作)と、縫合針の掴み直しという2つの操作に分解できる。
 この掴み直しの際には、把持部の角度を縫合針の状態に合わせて微調整させながら、縫合持針器全体を長手軸に沿う方向に進退させることにより把持部を縫合針に近づけた上で、牽引部材で把持部を操作して縫合針を把持する必要がある。
 このように縫合持針器での縫合では、シース部の回転操作、牽引部材による操作ワイヤの操作、及び縫合持針器の進退操作の3つを組合せる必要があり、操作性の悪さが手技の効率を低下させる要因となる。
 軟性の内視鏡では、内視鏡の操作を術者が、縫合持針器の操作を介助者が連携しながら処置を行うため、特にこの操作性の悪さが影響する。
 本発明は、このような問題点に鑑みてなされたものであって、牽引部材及びシース部の操作性を向上させた縫合持針器、及びこの縫合持針器を備える内視鏡システムを提供することを目的とする。
 上記課題を解決するために、この発明は以下の手段を提案している。
 本発明の第1の態様に係る縫合持針器は、縫合針と組み合わせて使用される縫合持針器であって、体内に挿入可能なシース部と、前記シース部の先端部に設けられ、互いに接近して閉じることで前記縫合針を把持可能な一対の把持部材と、前記一対の把持部材に接続され、前記シース部の長手軸に沿って前記シース部に対して相対的に移動することによって前記一対の把持部材を開閉動作させる操作ワイヤと、前記長手軸に沿って延びる軸状に形成され、前記シース部に対して前記長手軸に沿う方向及び前記長手軸周りに移動可能に接続された操作部本体と、を備え、前記操作部本体の外周面には、先端と基端を有し、前記長手軸に沿って延びる第1の溝部と、前記第1の溝部の基端と連なる一端と、該一端から前記長手軸周りの螺旋方向に離れた位置に他端を有し、該一端から該他端まで延びる第2の溝部と、が形成され、前記シース部の基端部には、前記シース部の径方向内側に突出した凸部が設けられ、前記凸部は、前記第1の溝部内に係合された状態で移動する第1の状態と、前記第1の溝部内に係合された状態で移動する第2の状態と、を有し、前記第1の状態では、前記操作部本体に対して前記操作ワイヤを牽引することによって、前記第1の溝部内の先端側から基端側に前記凸部を移動させるとともに、前記一対の把持部材が閉じて前記縫合針を把持し、前記第2の状態では、前記操作部本体に対して前記第1の状態の前記操作ワイヤをさらに牽引することによって、前記第2の溝部内の前記一端から前記他端に向かって前記凸部を移動させるとともに、前記一対の把持部材が前記縫合針を把持した状態で、前記一対の把持部材及び前記シース部を前記操作部本体に対して前記長手軸周りに回動させる。
 第2の態様に係る縫合持針器によれば、上記第1の態様において、前記操作部本体の前記外周面には、先端と基端を有し、該基端が前記第2の溝部の前記他端と連なり、該基端から該先端まで前記長手軸に沿って延びる第3の溝部と、前記第3の溝部の先端と前記第1の溝部の先端とを繋ぐように前記長手軸周り に延びる第4の溝部と、が形成され、前記凸部は、前記第3の溝部内に係合された状態で移動する第3の状態と、前記第4の溝部内に係合された状態で移動する第4の状態と、を有し、前記第3の状態は、前記第3の溝部内の基端側から先端側に向かって前記凸部を移動させるとともに、前記一対の把持部材を互いに離間して開いて前記縫合針を解放させ、前記第4の状態は、前記第4の溝部内の前記一端から前記他端に向かって前記凸部を移動させると、前記一対の把持部材を互いに離間して開いた状態で前記第2の状態における前記一対の把持部材及び前記シース部の回動の方向とは逆向きの方向に前記一対の把持部材及び前記シース部を回動させてもよい。
 第3の態様に係る縫合持針器によれば、上記第1の態様において、前記シース部と前記操作部本体とにそれぞれ接続された弾性部材をさらに備えてもよい。
 第4の態様に係る縫合持針器によれば、上記第1の態様において、前記操作ワイヤの基端部には、前記操作部本体に対して移動可能に取付けられ、前記操作部本体に対して前記操作ワイヤを前記長手軸に沿う方向に移動させるための牽引部材が設けられていてもよい。
 第5の態様に係る縫合持針器によれば、上記第4の態様において、前記第1の溝部内に前記凸部が配置されているときに、前記操作部本体に対する前記牽引部材の基端側への移動を許容し、かつ前記牽引部材の先端側への移動を規制し、前記第2の溝部内に前記凸部が配置されているときに、前記操作部本体に対する前記牽引部材の基端側及び先端側への移動を許容するラチェット機構をさらに備えてもよい。
 第6の態様に係る縫合持針器によれば、上記第1の態様において、前記第2の溝部の先端側の側面における前記第1の溝部側の端部には、先端側に向かって凹み、前記凸部の少なくとも一部を収容可能な没入部が形成されていてもよい。
 第7の態様に係る縫合持針器によれば、上記第6の態様において、前記第2の溝部の基端側の側面であって前記没入部に対向する部分には、先端側に向かって突出する突出部が形成されていてもよい。
 第8の態様に係る縫合持針器によれば、上記第1の態様において、前記第1の溝部の側面における前記第2の溝部側の端部には、第2の突出部が形成されていてもよい。
 第9の態様に係る内視鏡システムによれば、上記第1の態様の縫合持針器と、軟性の挿入部を有し、前記挿入部に前記縫合持針器が挿通可能なチャンネルが形成された内視鏡と、を備えていてもよい。
 本発明の縫合持針器及び内視鏡システムによれば、牽引部材及びシース部の操作性を向上させることができる。

 以下、本発明に係る内視鏡システムの一実施形態を、図1から図17を参照しながら説明する。
 図1に示すように、本内視鏡システム1は、本実施形態の縫合持針器2と、内視鏡3とを備えている。内視鏡3は、挿入部11を有している。挿入部11には、縫合持針器2が挿通可能なチャンネル12が形成されている。
 内視鏡3の構成は特に限定されない。例えば本実施形態では、内視鏡3は、口から胃の内部へ挿入される軟性の挿入部11を備えている。
 ここで言う軟性とは、術者等の操作者が撓ませられる程度の曲げ剛性(可撓性)を有することを意味する。
 縫合持針器2は、例えば曲針である縫合針D1と組み合わせて使用される。
縫合針D1には、縫合糸D2(図8参照)の一端部が取付けられている。
 図1及び2に示すように、縫合持針器2は、体内に挿入可能なシース部20と、シース部20の先端部に設けられた把持部30と、把持部30に接続され、シース部20の長手軸C1に沿ってシース部20に対して相対的に移動可能な操作ワイヤ40と、シース部20に接続された操作部50とを備えている。
 以下では、操作部50に対する把持部30側を先端側、把持部30に対する操作部50側を基端側とそれぞれ称する。
 図2及び3に示すように、シース部20は、軟性のシース21と、シース21の基端部に接続された回転子22とを有している。シース21の基端部の外周面には、全周にわたりフランジ21aが形成されている。フランジ21aの外周面におけるシース部20の周方向の一部には、フランジ21aの外周面からシース部20の径方向外側に突出するキー突部21bが形成されている。
 シース21は、コイルシース等の可撓性を有する材料で形成されている。フランジ21a及びキー突部21bは、ステンレス鋼やチタン等の金属で一体に形成され、シース21の基端部に溶接等により固定されている。
 回転子22は、円筒状に形成されている回転子22の先端側に形成された小径部22aの内径よりも、回転子22の基端側に形成された大径部22bの内径の方が大きい。
 小径部22aの内周面には、全周にわたり係合溝22cが形成されている。
 係合溝22cの底面における周方向の一部には、係合溝22cの底面から径方向外側に延びるキー溝22dが形成されている。
 図2及び4に示すように、大径部22bの基端部は、支持部22eを挟んだ径方向の両側がきり欠かれて切欠き(符号省略)が形成されることで、この支持部22eが基端側に向かって延びる棒状に形成されている。支持部22eの基端部には、径方向内側に突出した凸部22gが設けられている。凸部22gは、例えば円柱状に形成されている。
 大径部22bの基端部の内周面における凸部22gに対向する位置には、係合部22hが設けられている。係合部22hは、例えば周方向に延びるリブ状に形成されている。
 小径部22a、大径部22b、支持部22e、凸部22g、及び係合部22hは、ステンレス鋼やチタン等の金属で一体に形成されている。
 シース21は回転子22に挿通され、回転子22の係合溝22cにシース21のフランジ21aが係合し、回転子22のキー溝22dにシース21のキー突部21bが係合している。係合溝22cとフランジ21a、キー溝22dとキー突部21bは、図示しない接着剤や溶接等により固定されている。
 このように構成されることにより、シース21及び回転子22は一体となって、長手軸C1に沿う方向に移動したり、長手軸C1周りに回転したりする。
 図2に示すように、シース21及び回転子22には、操作ワイヤ40が挿通されている。回転子22の大径部22b内には、コイルバネ(弾性部材)24が配置されている。コイルバネ24の先端部は、回転子22の小径部22aと大径部22bとの間に形成された段部(符号省略)に接続されている。
 コイルバネ24内には、操作ワイヤ40が挿通されている。
 図1に示すように、把持部30は、把持部材31及び把持部材32を有している。把持部材31は支持部材33を介してシース21の先端部に固定されている。把持部材32は、支持部材33に取付けられたピン34周りに回転可能に支持されている。把持部材32の基端部には、図示しないリンク部材を介して操作ワイヤ40の先端部が接続されている。
 図1では、シース21に対して操作ワイヤ40を先端側に移動させて、把持部材31と把持部材32の先端部とが互いに離間して開いた開状態を示している。一方で、シース21に対して操作ワイヤ40を基端側に移動させると、把持部材32は位置Q1に移動し、把持部材31と把持部材32の先端部とが互いに接近して閉じた閉状態になる。この閉状態のときに、把持部材31と把持部材32との間に縫合針D1を把持することができる。つまり、操作ワイヤ40は、把持部材31と把持部材32に接続されており、シース21の長手軸に沿ってシース21に対して相対的に移動することによって、把持部材31と把持部材32と間で開閉するように動作させることができる。
 操作ワイヤ40は、単線や撚り線等で形成されている。操作ワイヤ40の基端部には、図2に示す筒状の固定部材41がロウ付け等により取付けられている。
 操作部50は、図2及び5に示すように、長手軸C1に沿って延びる軸状に形成され、シース部20の回転子22に接続された操作部本体51と、操作部本体51に対して長手軸C1に沿ってスライド自在に設けられたスライダ(牽引部材)52とを有している。この例では、操作部50はいわゆるスライダ方式のハンドルである。
 操作部本体51は術者に把持される。操作部本体51の長手軸C1に平行な側面51aには、長手軸C1に沿って複数の歯部54が形成されている。それぞれの歯部54には、図2中の拡大図に示すように、長手軸C1に直交する直交面54aと、基端側に向かうにしたがって長手軸C1から離間する傾斜面54bとが形成されている。
 操作部本体51の基端部には、指掛けリング55が取付けられている(図1参照)。
 図2及び4に示すように、操作部本体51の先端部には、基端側よりも外径が小さな小径部56が形成されている。小径部56の長手軸C1に直交する平面による断面は、円筒状に形成されている。
 小径部56の先端部の外周面には、全周にわたりフランジ56aが形成されている。操作部本体51の小径部56は、回転子22の大径部22b内に挿通されている。操作部本体51のフランジ56aと回転子22の係合部22hとが係合し、操作部本体51の小径部56と回転子22とが摺動する。これにより、シース部20に対して操作部50の操作部本体51が、長手軸C1に沿う方向及び長手軸C1周りに移動することができる。
 図4及び6に示すように、小径部56の外周面56bには、第1の溝部56cとた第2の溝部56dが形成されている。第1の溝部56cは、先端と基端を有し、先端から基端まで長手軸C1に沿って延びている。第2の溝部56dは、第1の溝部56cの基端と連なる一端と、その一端から長手軸C1周りの螺旋方向に離れた位置に他端を有し、一端から他端まで延びて、螺旋状に形成されている。さらに、小径部56の外周面56bには、第3の溝部56eと第4の溝部56fとが形成されている。第3の溝部56eは、先端と基端を有し、基端が第2の溝部の他端と連なり、基端から先端まで長手軸C1に沿って延びている。第4の溝部56fは、第3の溝部の先端と第1の溝部の先端とを繋ぐように長手軸C1周りに延びており、本実施形態では螺旋状に形成された第4の溝部56fを例示している。
 溝部56c、56eは直線状に形成されている。第1の溝部56cの長手軸C1の長さは、複数の歯部54全体の長手軸C1の長さ(歯部54のピッチ×歯部54の数)以下である。
 第2の溝部56dは、基端側に向かうにしたがって長手軸C1周りの第一側(第一方向)X1に向かって延びている。第4の溝部56fは、先端側に向かうにしたがって長手軸C1周りの第二側(第二方向)X2に向かって延びている。第4の溝部56fの先端は、第1の溝部56cの先端と連なっている。溝部56d、56fは、長手軸C1を螺旋の軸とする螺旋状に形成されている。第4の溝部56fのリード角α4は、第2の溝部56dのリード角α2よりも小さい。
 溝部56c、56d、56e、56fの幅は、回転子22の凸部22gの外径よりもわずかに大きい。
 回転子22の凸部22gは、溝部56c、56d、56e、56fに係合し、溝部56c、56d、56e、56fに沿って移動(摺動)可能である。
 第1の溝部56c内に凸部22gが係合されて移動する第1の状態では、シース21(操作部本体)に対して操作ワイヤ40を牽引することによって、第1の溝部56c内の先端側から基端側に凸部22gが移動するとともに、把持部材31、32が開状態から閉状態になり、縫合針D1を把持する。
 第2の溝部56d内に凸部22gが係合されて移動する第2の状態では、第1の状態の操作ワイヤ40をさらに牽引することによって、第2の溝部56d内の一端から他端に向かって凸部22gが移動するとともに、凸部22gとともに回転子22、シース21、及び把持部30が一体となって、第2の溝部56dに沿って長手軸C1周りに回転する。つまり、把持部材31、32により縫合針D1を把持した状態で、把持部30は、シース21と共に、操作部本体51に対して長手軸C1周りに回動され、縫合針D1を組織に穿刺する。
 第3の溝部56e内に凸部22gが係合されて移動する第3の状態では、第3の溝部56e内の基端側から先端側に向かって凸部22gが移動するとともに、シース21に対して操作ワイヤ40が先端側に移動し、把持部材31、32が閉状態から開状態になり、縫合針D1を解放する。
 第4の溝部56f内に凸部22gが係合されて移動する第4の状態では、第4の溝部56f内の一端から他端に向かって凸部22gが移動するとともに、凸部22gとともに回転子22、シース21、及び把持部30が一体となって、第4の溝部56fに沿って長手軸C1周りに回転する。つまり、把持部材31、32を互いに離間して開いた状態で、把持部30は、シース21と共に、第2の状態における回転の方向とは逆向きの方向に回転する。
 図2に示すように、操作部本体51の小径部56の先端面には、コイルバネ24の基端部が接続されている。
 操作部本体51の長手軸C1上には、スリット51bが形成されている。このスリット51bには、操作ワイヤ40が挿通されている。
 スライダ52における長手軸C1上には、凹部52aが形成されている。この凹部52a内には固定部材41が配置され、固定部材41が凹部52aに係合することで、スライダ52に操作ワイヤ40の基端部に固定されている。
 スライダ52には、図5に示す長手軸C1に沿って延びるスリット52fが形成されている。このスリット52fに操作部本体51が挿通されることで、操作部本体51に対してスライダ52が長手軸C1に沿ってスライドできる。
 スライダ52の基端側には、図2に示すように、スライダ52の外面から長手軸C1に交差する方向に延びる凹部52bが形成されている。スライダ52の外面には、長手軸C1周りに指掛け凹部52cが形成されている。
 凹部52bにおける外部に連通する開口52dには、ボタン57が挿通されている。
 スライダ52の凹部52b内における長手軸C1を挟んで開口52dとは反対側には、連結板58が設けられている。連結板58は、ボタン57に接続されている。連結板58の長手軸C1側の面には、歯部59が形成されている。図2中の拡大図に示すように、歯部59には、長手軸C1に直交する直交面59aと、基端側に向かうにしたがって長手軸C1から離間する傾斜面59bとが形成されている。
 スライダ52の凹部52bの底面と連結板58との間には、コイルバネ60が配置されている。コイルバネ60は、連結板58を長手軸C1側に付勢している。
 なお、複数の歯部54、ボタン57、連結板58、歯部59、及びコイルバネ60により、ラチェット機構61が構成されている。
 操作部本体51の複数の歯部54の直交面54aに連結板58の歯部59の直交面59aが接触したときに、歯部54に対して歯部59が先端側に移動するのが規制される。このとき、歯部59が形成された連結板58が凹部52bの内周面に係止することで、操作部本体51に対してスライダ52が先端側に移動するのが規制される。
 複数の歯部54の傾斜面54b上を歯部59の傾斜面59bが摺動することで、操作部本体51の歯部54に連結板58の歯部59が接触していても、操作部本体51に対してスライダ52を基端側に移動させることができる。
 一方で、コイルバネ60を弾性的に変形させつつ、ボタン57を長手軸C1側に位置Q2まで移動させて押込む。このとき、歯部54が位置Q3に移動することで、操作部本体51の歯部54に連結板58の歯部59が接触しなくなり、操作部本体51に対してスライダ52を先端側及び基端側のいずれにも移動させることができる。
 第1の溝部56c内の基端部に凸部22gが配置されているときに、複数の歯部54のうち最も基端側に位置する歯部54Aと歯部59とが係合する。すなわち、第1の溝部56c内に凸部22gが配置されているときに歯部54と歯部59とが係合するが、第2の溝部56d内に凸部22gが配置されているときに歯部54と歯部59とは係合しない。
 このように、ボタン57を押込まない状態では、第1の溝部56c内に凸部22gが配置されているときには、操作部本体51に対してスライダ52が先端側に移動するのが規制され、基端側に移動するのが許容される。第2の溝部56d内に凸部22gが配置されているときには、操作部本体51に対してスライダ52が先端側及び基端側のいずれに移動するのも許容される。
 スライダ52を操作して操作部本体51に対してスライダ52を基端側に移動させる(引き戻す)と、操作部本体51及びシース21に対して操作ワイヤ40が基端側に移動し、把持部材31、32が閉状態になる。
 一方で、操作部本体51に対してスライダ52を先端側に移動させる(押込む)と、操作部本体51及びシース21に対して操作ワイヤ40が先端側に移動し、把持部材31、32が開状態になる。
 次に、以上のように構成された本内視鏡システム1の作用について説明する。以下では、患者の胃の内部に形成された処置対象部位である組織を縫合する手技を例にとって説明する。
 患者の体外において、第3の溝部56eの基端部内に凸部22gを配置し、把持部材31、32を閉状態にしておく。患者の口等の自然開口から内視鏡3の挿入部11を挿入し、処置対象部位である組織に挿入部11の先端面を対向させた状態で保持する。
 内視鏡3のチャンネル12の基端部から縫合持針器2のシース部20を挿通し、チャンネル12の先端部から把持部30を突出させる。
 術者は、操作部本体51の指掛けリング55に一方の手の親指を通し、スライダ52の指掛け凹部52cに人差指と中指を掛けることで操作部50を把持する。
 スライダ52を押込んで、第4の溝部56fの先端部、すなわち、第1の溝部56cの先端部に凸部22gを配置すると、把持部材31、32が開状態になり、長手軸C1周りの第二側(第二方向)X2に回る。
 図示しない搬送手段により胃の内部に搬送した縫合針D1を、把持部材31と把持部材32との間に配置する。第1の溝部56cの先端部に凸部22gが配置されている状態からスライダ52を引き戻す。これにより、図7及び8に示すように第1の溝部56c内を凸部22gが基端側に移動するとともにシース部20に対して操作ワイヤ40が基端側に移動し、把持部材31、32が閉状態になり縫合針D1を把持する。
 このとき、コイルバネ24が自然長である状態よりも長手軸C1に沿う方向に圧縮される。ラチェット機構61の複数の歯部54と歯部59とが係合しているため、スライダ52を引き戻せるが、一度引き戻したスライダ52を押込むことはできない。
 術者は、縫合持針器2を押込んだり、内視鏡3の挿入部11の図示しない湾曲部が湾曲する向きを調節したりすることで、組織P1と縫合針D1との位置関係を調節する。
 スライダ52をさらに引き戻すと、図9から図11に示すように第2の溝部56d内を凸部22gが基端側に移動するとともに、把持部材31、32、及びシース部20が操作部本体51に対して長手軸C1周りの第一側(第一方向)X1に回動する。このとき、コイルバネ24が長手軸C1に沿う方向にさらに圧縮され、組織P1を縫合針D1の先端部で穿刺する。
コイルバネ24の基端側が、長手軸C1周りの第一側(第一方向)X1に捩じられる。
 このように、術者がスライダ52を引き戻すだけで、把持部材31、32が閉状態になって縫合針D1を把持する動作と、組織P1を縫合針D1で穿刺する動作とが連続して行われる。凸部22gが第2の溝部56d内に配置されているときには把持部材31、32が閉状態であるため、縫合針D1を確実に把持した状態で、組織P1を縫合針D1で穿刺することができる。
 これら縫合針D1の把持、及び組織P1の穿刺の動作が終了したとき、凸部22gは第2の溝部56dの基端部に配置されている。
 なお、第2の溝部56d内に凸部22gが配置されているときには、ラチェット機構61の複数の歯部54と歯部59とが係合していないため、スライダ52を押込んで把持部30を長手軸C1周りの第二側(第二方向)X2に回動させることで、組織P1から縫合針D1を抜くことができる。このとき、ラチェット機構61により凸部22gを第1の溝部56c内まで移動させることはできない。
 組織P1を縫合針D1で穿刺する場所を調節し、スライダ52を引き戻して組織P1を縫合針D1で穿刺する。
 凸部22gが第2の溝部56dの基端部に配置された状態から術者がスライダ52を引き戻す力を緩めると、コイルバネ24の弾性力により、図12に示すように第3の溝部56e内を凸部22gが先端側に移動し、把持部材31、32が開状態になって縫合針D1を放す。第4の溝部56fのリード角α4は、第2の溝部56dのリード角α2よりも小さいため、縫合針D1を放すときの把持部材31、32の位置がより先端側になる。
 スライダ52を引き戻す力をさらに緩めると、コイルバネ24の弾性力により凸部22gは第4の溝部56f内を先端側に移動する。このとき、把持部材31、32、及びシース部20が操作部本体51に対して長手軸C1周りの第二側(第二方向)X2に回動する。すなわち、把持部材31、32の長手軸C1周りの向きが、把持部材31、32が縫合針D1を把持したときの向きに戻る。
 この後で、必要に応じてスライダ52を引き戻し、図13に示すように把持部材31、32を閉状態にして縫合針D1の先端部を把持し直す、いわゆる掴み直しをする。
 スライダ52をさらに引き戻すと、把持部材31、32等が長手軸C1周りの第一側(第一方向)X1に回動し、組織P1を縫合針D1が貫通し、組織P1に縫合糸D2が通る。
 このように、術者がスライダ52を押込むだけで、把持部材31、32が開状態になることで縫合針D1を放す動作と、把持部30の向きが戻る動作とが連続して行われる。スライダ52を引き戻したり押込んだりする動作を繰り返して、組織P1を縫合していく。
 以上説明したように、本実施形態の縫合持針器2及び内視鏡システム1によれば、スライダ52を引き戻すと、把持部材31、32が閉状態になって縫合針D1を把持する動作と、シース部20を長手軸C1周りの第一側(第一方向)X1に回動させて組織P1を縫合針D1で穿刺する動作とが連続して行われる。したがって、スライダ52を引き戻すだけで、シース部20の回転動作も行うことができ、スライダ52及びシース部20の操作性を向上させることができる。
 第2の溝部56d内に凸部22gが配置されているとき(把持部30を長手軸C1周りに回転させているとき)にスライダ52を引き戻す力を緩めて開放しても、凸部22gは第1の溝部56c内に移動しない。したがって、意図せずに縫合針D1を開放することがない。
 操作部本体51の外周面56bには、溝部56e、56fが形成されている。スライダ52を押込むと、縫合針D1を放す動作と、シース部20を長手軸C1周りの第二側(第二方向)X2に回動させる動作とが連続して行われる。したがって、スライダ52を押込むだけでシース部20の回転動作も行うことができ、スライダ52及びシース部20の操作性をさらに向上させることができる。
 縫合持針器2がコイルバネ24を備えることで、スライダ52を引き戻した後でスライダ52を押込まなくても、コイルバネ24の弾性力によりスライダ52を先端側に移動させることができる。
 縫合持針器2がラチェット機構61を備えることで、一度把持部材31、32で把持した縫合針D1を放さないようにするとともに、縫合針D1による組織P1の穿刺位置の調節を容易に行うことができる。
 複数の歯部54に歯部59が係止されなくなったときに、凸部22gが第1の溝部56cから第2の溝部56dに移る。したがって、複数の歯部54と歯部59との係止により、把持部材31、32の開閉状態を感知することができ、把持部材31、32の開閉操作を容易に行うことができる。
 本実施形態の縫合持針器2は、以下に説明するようにその構成を様々に変形させることができる。
 例えば、図14に示す縫合持針器2Aのように、第2の溝部56dの先端側の側面65aにおける第1の溝部56c側の端部に没入部65bを形成してもよい。没入部65bは、先端側に向かって凹んでいて、凸部22gの少なくとも一部を収容可能である。
 コイルバネ24により、凸部22gには矢印B1のような凸部22gを先端側に移動させようとする弾性力が作用する。
 没入部65b内に凸部22gが収容されると、凸部22gに矢印B1のような弾性力が作用しても、凸部22gが没入部65b内から出にくい。したがって、凸部22gが第2の溝部56d内から第1の溝部56c内に戻ることをより確実に防止することができる。
 なお、図15に示す縫合持針器2Bのように、上記変形例の縫合持針器2Aの各構成に加えて、第2の溝部56dの基端側の側面65dに突出部65eを形成してもよい。突出部65eは、側面65dの没入部65bに対向する部分に先端側に向かって突出するように形成されている。これら突出部65e及び没入部65bで、第2の溝部56dが蛇行した蛇行部65fが形成される。蛇行部65fの幅は、凸部22gが通れるように設定される。
 第2の溝部56dに突出部65e及び没入部65bを形成することで、一度没入部65b内に入った凸部22gが没入部65bから出にくくなる。
 図16に示す縫合持針器2Cのように、第1の溝部56cの長手軸C1周りの第一側(第一方向)X1の側面66aにおける第2の溝部56d側の端部に没入部66bを形成するとともに、第1の溝部56cの長手軸C1周りの第二側(第二方向)X2の側面66cに突出部66dを形成してもよい。これら没入部66b及び突出部66dで、第1の溝部56cが蛇行した蛇行部66eが形成される。
 凸部22gが第1の溝部56cから第2の溝部56dに一度配置されると、コイルバネ24の基端側が長手軸C1周りの第一側(第一方向)X1に捩じられることで、コイルバネ24により凸部22gを長手軸C1周りの第二側(第二方向)X2に移動させようとする弾性力が作用する。
 このため、術者がスライダ52を引き戻す力を緩めると、第1の溝部56cの側面66cにおける突出部66dの基端側に隣接して形成される窪み66f内に凸部22gが収容される。突出部66dが第1の溝部56cの側面66cから突出する長さは、ある程度以上長いことが好ましい。
 第1の溝部56cの第2の溝部56d側の端部に蛇行部66eを形成することで、窪み66f内に凸部22gが収容され、凸部22gが第2の溝部56d内から第1の溝部56c内に戻ることをより確実に防止することができる。
 突出部66dだけでなく没入部66bが形成されることで、第1の溝部56cを凸部22gが基端側に通過しやすくなる。
 図17に示す縫合持針器2Dのように、第1の溝部56cの側面66cにおける第2の溝部56d側の端部に第2の突出部66gを形成してもよい。第2の突出部66gは、側面66cから半円状に突出している。側面66aと第2の突出部66gとの距離Lは、凸部22gの外径よりもわずかに小さい。
 側面66aと第2の突出部66gとの間を通して凸部22gを基端側に移動させるときには、術者がスライダ52を引き戻す力により通す。一方で、コイルバネ24のバネ定数を小さくすることで、コイルバネ24の弾性力では側面66aと第2の突出部66gとの間を凸部22gが先端側に移動できないようにする。
 第1の溝部56cの側面66cにおける第2の溝部56d側の端部に第2の突出部66gを形成しても、前記変形例の縫合持針器2Aと同様の効果を奏することができる。
 以上、本発明の一実施形態について図面を参照して詳述したが、具体的な構成はこの実施形態に限られるものではなく、本発明の要旨を逸脱しない範囲の構成の変更、組み合わせ、削除等も含まれる。
 例えば、前記実施形態では、操作部本体51の外周面56bに溝部56e、56fが形成されていなくてもよい。操作部本体51の外周面56bに溝部56c、56dが形成されていれば、縫合針D1を把持して組織P1を縫合針D1で穿刺するという動作を一度行うことができるからである。
 この場合、図7における係合溝22c及びキー溝22dと大径部22bとの間の領域R1、R2の壁部は回転子22に形成されていなくてもよい。すなわち、回転子22に対してシース21は、長手軸C1周りの第一側(第一方向)X1及び第二側(第二方向)X2、そして先端側に係止されていればよく、基端側に係止されていなくてもよい。
 弾性部材はコイルバネ24であるとしたが、弾性部材はゴム等でもよい。
 また、縫合持針器2はコイルバネ24を備えなくてもよい。術者がスライダ52を押込んで操作すればよいからである。
 シース21が回転子22に挿通された状態で回転子22に固定されているとしたが、回転子がシースに挿通された状態でシースに固定されるように構成してもよい。
 内視鏡3は軟性の挿入部11を備えているとした。しかし、内視鏡が、軟性よりも曲げ剛性の大きな硬性の挿入部を備えているとしてもよい。
 前記実施形態では、操作部50はスライダ方式のハンドルであるとした。しかし、操作部は、操作部本体に対して牽引部材の一端部が回動可能に取付けられ、この牽引部材の他端部に操作ワイヤ40の基端部が接続された、いわゆるインライン方式のハンドルであってもよい。この操作部の牽引部材を操作することでも、シース部20に対して操作ワイヤ40を長手軸C1に沿う方向に移動させることができるからである。
 以上、本発明の好ましい実施形態を説明したが、本発明はこれら実施形態及びその変形例に限定されることはない。本発明の趣旨を逸脱しない範囲で、構成の付加、省略、置換、およびその他の変更が可能である。
 また、本発明は前述した説明によって限定されることはなく、添付のクレームの範囲によってのみ限定される。
上記一実施態様(変形例を含む)の縫合持針器及び内視鏡システムは、牽引部材及びシース部の操作性を向上させることができる。

  1.  縫合針と組み合わせて使用される縫合持針器であって、
     体内に挿入可能なシース部と、
     前記シース部の先端部に設けられ、互いに接近して閉じることで前記縫合針を把持可能な一対の把持部材と、
     前記一対の把持部材に接続され、前記シース部の長手軸に沿って前記シース部に対して相対的に移動することによって前記一対の把持部材を開閉動作させる操作ワイヤと、
     前記長手軸に沿って延びる軸状に形成され、前記シース部に対して前記長手軸に沿う方向及び前記長手軸周りに移動可能に接続された操作部本体と、
     を備え、
     前記操作部本体の外周面には、
      先端と基端を有し、前記長手軸に沿って延びる第1の溝部と、
      前記第1の溝部の基端と連なる一端と、該一端から前記長手軸周りの螺旋方向に離れた位置に他端を有し、該一端から該他端まで延びる第2の溝部と、が形成され、
     前記シース部の基端部には、
     前記シース部の径方向内側に突出した凸部が設けられ、
     前記凸部は、前記第1の溝部内に係合された状態で移動する第1の状態と、前記第1の溝部内に係合された状態で移動する第2の状態と、を有し、
     前記第1の状態では、前記操作部本体に対して前記操作ワイヤを牽引することによって、前記第1の溝部内の先端側から基端側に前記凸部を移動させるとともに、前記一対の把持部材が閉じて前記縫合針を把持し、
     前記第2の状態では、前記操作部本体に対して前記第1の状態の前記操作ワイヤをさらに牽引することによって、前記第2の溝部内の前記一端から前記他端に向かって前記凸部を移動させるとともに、前記一対の把持部材が前記縫合針を把持した状態で、前記一対の把持部材及び前記シース部を前記操作部本体に対して前記長手軸周りに回動させる
    縫合持針器。
  2.  前記操作部本体の前記外周面には、
      先端と基端を有し、該基端が前記第2の溝部の前記他端と連なり、該基端から該先端まで前記長手軸に沿って延びる第3の溝部と、
      前記第3の溝部の先端と前記第1の溝部の先端とを繋ぐように前記長手軸周りに延びる第4の溝部と、
     が形成され、
     前記凸部は、前記第3の溝部内に係合された状態で移動する第3の状態と、前記第4の溝部内に係合された状態で移動する第4の状態と、を有し、
     前記第3の状態は、前記第3の溝部内の基端側から先端側に向かって前記凸部を移動させるとともに、前記一対の把持部材を互いに離間して開いて前記縫合針を解放させ、
     前記第4の状態は、前記第4の溝部内の前記一端から前記他端に向かって前記凸部を移動させると、前記一対の把持部材を互いに離間して開いた状態で前記第2の状態における前記一対の把持部材及び前記シース部の回動の方向とは逆向きの方向に前記一対の把持部材及び前記シース部を回動させる
    請求項1に記載の縫合持針器。
  3.  前記シース部と前記操作部本体とにそれぞれ接続された弾性部材をさらに備える請求項1に記載の縫合持針器。
  4.  前記操作ワイヤの基端部には、前記操作部本体に対して移動可能に取付けられ、前記操作部本体に対して前記操作ワイヤを前記長手軸に沿う方向に移動させるための牽引部材が設けられている
    請求項1に記載の縫合持針器。
  5.  前記第1の溝部内に前記凸部が配置されているときに、前記操作部本体に対する前記牽引部材の基端側への移動を許容し、かつ前記牽引部材の先端側への移動を規制し、
     前記第2の溝部内に前記凸部が配置されているときに、前記操作部本体に対する前記牽引部材の基端側及び先端側への移動を許容するラチェット機構をさらに備える
    請求項4に記載の縫合持針器。
  6.  前記第2の溝部の先端側の側面における前記第1の溝部側の端部には、先端側に向かって凹み、前記凸部の少なくとも一部を収容可能な没入部が形成されている
    請求項1に記載の縫合持針器。
  7.  前記第2の溝部の基端側の側面であって前記没入部に対向する部分には、先端側に向かって突出する突出部が形成されている
    請求項6に記載の縫合持針器。
  8.  前記第1の溝部の側面における前記第2の溝部側の端部には、第2の突出部が形成されている
    請求項1に記載の縫合持針器。
  9.  請求項1に記載の縫合持針器と、
     軟性の挿入部を有し、前記挿入部に前記縫合持針器が挿通可能なチャンネルが形成された内視鏡と、
     を備える内視鏡システム。
