의존적으로 억제하며, 지방의 분화를 유의하게 억제하고, 지방분화시 증가하는 PPAR Y , C/EBP α 및 C/ΕΒΡβ의 발현을 감소시킨다. 이러한 다각적인 실험적 증거에 의해, 본 발명자들은 본 발명의 조성물이 갑상선 안구병증의 효율적인 치료제로 유용하게 이용될 수 있음을 확인하였다. 본 발명의 조성물이 약제학적 조성물로 제조되는 경우, 본 발명의 약제학적 조성물은 약제학적으로 허용되는 담체를 포함한다. 본 발명의 약제학적 조성물에 포함되는 약제학적으로 허용되는 담체는 제제시에 통상적으로 이용되는 것으로서, 락토스, 덱스트로스, 수크로스, 솔비를, 만니를, 전분, 아카시아 고무, 인산 칼슘, 알기네이트, 젤라틴, 규산 칼슴, 미세결정성 셀를로스, 폴리비닐피를리돈, 샐를로스, 물, 시럽, 메틸 셀를로스 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 활석, 스테아르산 마그네슘 및 미네랄 오일 등을 포함하나, 이에 한정되는 것은 아니다. 본 발명의 약제학적 조성물은 상기 성분들 이외에 윤활제, 습윤제, 감미제, 향미제, 유화제, 현탁제, 보존제 등을 추가로 포함할 수 있다. 적합한 약제학적으로 허용되는 담체 및 제제는 Remington 's Pharmaceutical Sc/ences( l9t ed . , 1995)에 상세히 기재되어 있다. 본 발명의 약제학적 조성물은 경구 또는 비경구 투여할 수 있으며, 비경구 투여인 경우에는 정맥내 주입, 피하 주입, 근육 주입, 복강 주입, 경피 투여 등으로 투여할 수 있다.
본 명세서에서 용어 "투여" 또는 "투여하다" 는 본 발명의 조성물의 유효량을 대상체에 직접적으로 투여함으로써 대상체의 체내에서 동일한 양이 형성되도록 하는 것을 말한다.
본 명세서에서 용어 "대상체 " 는 제한없이 인간, 마우스 , 래트 , 기니아 피그, 개, 고양이 , 말, 소, 돼지, 원승이 , 침팬지, 비비 또는 붉은털 원승이를 포함한다. 바람직하게는, 본 발명의 대상체는 인간이다. 본 명세서에서 용어 "유효량" 은 대상체에게 투여했을 때 대상체가 고통받거나 고통받을 것이 예상되는 상태를 치료하거나, 최소한 이를 개선할 수 있는 있을 만큼 유효한 조성물의 양을 의미한다.
본 발명의 약제학적 조성물의 적합한 투여량은 제제화 방법, 투여 방식, 환자의 연령, 체중, 성, 병적 상태, 음식, 투여 시간, 투여 경로, 배설 속도 및 반응. 감웅성과 같은 요인들에 의해 다양하게 처방될 수 있다. 본 발명의 약제학적 조성물의 1일 투여량은 예컨대 0.001-100 mg/kg이다. 본 발명의 약제학적 조성물은 당해 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약제학적으로 허용되는 담체 및 /또는 부형제를 이용하여 제제화함으로써 단위 용량 형태로 제조되거나 또는 다용량 용기 내에 내입시켜 제조될 수 있다. 이때 제형은 오일 또는 수성 매질중의 용액, 현탁액, 시럽제 또는 유화액 형태이거나 액스제, 산제, 분말제, 과립제, 정제 또는 갑셀제 형태일 수도 있으며 , 분산제 또는 안정화제를 추가적으로 포함할 수 있다. 또한, 본 발명의 약제학적 조성물은 지속적으로 치료학적 유효량을 유지함으로써 약제의 복용횟수를 줄여 복약 순웅도를 높일 수 있도록 서방형. 제제로 제조될 수 있다. 서방형 제제는 본 발명의 유효성분 이외에 서방화 담체 및 기타 보조제를 포함하여 제제화 될 수 있다. 본 발명에서 이용되는 서방화 담체는 당업계에 공지된 다양한 서방화 담체를 이용할 수 있으나, 구체적으로는 친수성 폴리머 (polymer)이다. 본 발명의 다른 양태에 따르면, 본 발명은 본 발명의 바이구아니드 (biguanide) 유도체 또는 이의 약제학적으로 허용되는 염을 포함하는 약제학적 조성물을 대상체에 투여하는 단계를 포함하는 자가면역성 갑상선 질환의 예방 또는 치료방법을 제공한다.
본 발명에서 이용되는 바이구아니드 (biguanide) 유도체 및 본 발명의 조성물로 예방 또는 치료될 수 있는 자가면역성 갑상선 질환에 대해서는 이미 상술하였으므로, 과도한 중복을 피하기 위해 그 기재를 생략한다.
【유리한 효과】
본 발명의 특징 및 이점을 요약하면 다음과 같다:
(a) 본 발명은 상기 화학식 1의 바이구아니드 (biguanide) 유도체, 보다 구체적으로는 메트포민 (met formin)을 유효성분으로 포함하는
자가면역성 갑상선 질환 (Autoimmune Thyroid Di seases)의 예방 또는 치료용
약제학적 조성물을제공한다.
(b) 본 발명의 조성물은 낮은 세포 독성을 가지면서도 히알루로난 (Hyaluronan)의 생성 및 지방의 분화를 유의하게 억제함으로써 갑상선 자가항체에 의해 발생하는 다양한 병리적 상태, 예를 들어 갑상선 안구병증을 효과적으로 치료 또는 개선할 수 있다.
【도면의 간단한 설명】
도 1은 본 발명의 조성물이 섬유모세포에 대해 독성을 가지지 않음올 확인한 MTT분석결과를 나타낸 그림이다.
도 2는 본 발명의 조성물이 갑상선안구병증의 병리기전 증 하나인 히알루로난 생성에 미치는 효능을 분석한 결과를 나타낸 그림이다. 섬유모세포에 메트포민을 24시간 동안 농도별 (10, 50, 100, 500, 1000 및 5000yg/ml)로 처리한 후 10ng/ml TNF- α를 처리하고 히알루론산의 생성을 ELISA kit로 분석한 결과, 메트포민은 ΙΟμΜ에서도 히알루로난 생성을 유의적으로 억제함을 확인하였다.
도 3은 본 발명의 조성물이 갑상선안구병증의 병리기전 중 하나인 지방분화에 미치는 효과를 농도별 (10, 100 및 1000 yg/ml)에서의 지방 형성을 특수 염색 없이 확인한 결과이다. 농도 증가에 따라 안와섬유모세포의 지방 형성이 억제되는 것을 확인할 수 있다.
도 4는 본 발명의 조성물이 갑상선안구병증의 병리기전 중 하나인 지'방분화에 미치는 효과를 농도별 (10, 100 및 1000 yg/ml)에서의 oil -red o 염색을 통해 확인한 결과를 나타낸 그림이다. 농도 증가에 따라 안와섬유모세포의 '지방 형성이 억제되는 것을 확인할 수 있다 (X40).
도 5는 도 4의 결과를 고배율 (X400)에서 확인한 결과이며 본 발명의 조성물이 갑상선안구병증의 병리기전 중 하나인 지방분화에 미치는 효과를 농도별 (10, 100 및 1000 yg/ml)에서의 oil—red o 염색을 통해 확인한 결과를 나타낸 그림이다.
도 6은 농도별 (10, 100 및 1000 yg/ml) 메트포민 하에서 지방분화시 증가하는 PPARg, C/EBP α 및 C/ΕΒΡβ의 발현이 감소됨을 웨스턴 블롯팅을 통해 관찰함으로써 본 발명의 조성물이 지방분화를 효율적으로 억제함을 확인한 결과를 나타낸 그림이다.
【발명의 실시를 위한 형태】
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 당업계에서 통상의 지식을 가진 자에 있어서 자명할 것이다. 실시예 갑상선안병증의 안와지방결체조직에서 안와 섬유모세포 분리 배양 갑상선안병증 환자에서 안와감압술 시행후 안와 지방결체 조직 으로부터 안와 섬유모세포를 일차 배양하였다. 비교실험을 위해 갑상선 안병증이 없는 정상인을 대상으로 시행한 위눈꺼풀성형술 또는 안와수술 등을 통해 안와 지방결체조직을 얻어, 정상 안와 섬유모세포도 일차 배양하였다. 배양방법은, 조직을 웨스코트 가위 (wescott scissors)로 잘게 절단 후, 콜라게나아제 (collagenase) n(3mg/ml in HBSS, 1.5% BSA)을 1시간 동안 37°C에서 흔든 다음, 300g에 5분간 원심분리함으로써 부유지방세포와 지방구 (fat globule)를 제거하였다. 2차례 PBS로 세척한 후 10 % FBS( fetal bovine serum)를 첨가한 DMEM(Dulbecco' s Modified Eagle' s Medium) 배지용액에 넣은 후 C02배양기 (5% C02,37°C)내에서 배양하였다. 2-3일 간격으로 배양배지를 교체하면서 융합된 세포들을 트립신 -EDTA용액으로 박리하여 계대배양하였다. 지방분화 (adipogenesis)
6-웰 플레이트에 안와 섬유모세포를 분주한 후, 세포가 컨플루언스 (confluence)에 도달하면 지방전구세포의 지방분화를 유도하였다. 유도제로는 기본 세포 배지용액인 10 % FBS를 첨가한 DMEM에 PPARy 아고니스트, 로지글리타존 (10 mM, Cayman, Ann Arbor , MI, USA), 33 mM 비오틴, 17 mM 판토테닉산, 10 mg/ml 트랜스페린, 0.2nM T3, ImM
인슐린 (Boehringerᅳ Mannheim, Mannheim, Germany) 및 0.2mM 카바프로스타글란딘 (cPGI2; Calbiochem, La Jolla, CA, USA), ImM 텍사메타손 및 O.lmM 이소부틸메틸잔틴을 추가하여 사용하였다. 지방 분화는 10일 동안 지속하며, 2-3일 간격으로 배지와 약물을 교체하였다. 세포 독성 테스트 메트포민의 세포 독성 테스트는 MTT(3-(4,5-Dimethylthiazol-2-yl)- 2,5-di phenyl t et razo 1 i urn bromide) 분석기 (CellTiter 96 Aqueous nonreactive cell proliferative assay, Promega)를 이용하여 측정하였다. ΜΊΤ 분석은 각 웰에 안와 섬유모세포를 배양하고, 본 발명의 메트포민을 농도별 (10-5000 μ,Μ)로 24시간 동안 처리함으로써 수행하였다. 이 후 48 시간 배양한 뒤 ΜΊΤ 용액 20μ1를 투여하고 4시간 경과 후 ELISA 플레이트: 리더에서 490nm 파장의 UV로 광밀도 (0D)를 측정하여 세포성장 억제를 비교하였다. 대사가 왕성한 세포에서 발견되는 디하이드로게네이즈
(dehydrogenas) 효소에 의해서 MTS는 포르마잔으로 바뀌며, 490 nm 자외선으로 측정되는 흡광도의 정도가 배양 시 살아있는 세포의 수와 직접 비례한다. 실험은 3번 반복하여 그 평균치를 비교하였고, 그 결과 메트포민은 안와 섬유모세포에 독성이 없음을 확인하였다 (도 1). 히알루로난 (Hyaluronan) ELISA
12-웰 플레이트에 안와 섬유모세포를 분주한 후, 메트포민을 10-5000 Ug/ml 농도에 따라 24시간 동안 처리한 후, TNF-α를 이용하여 세포에 자극을 주었다. 이 세포 배양액을 수집하여 히알루로난 ELISA 킷 (Echelon, Salt Lake City, UT, USA)을 이용하여 하알루로난 농도를 측정하엿다. 흡광도는 405 nm에서 측정하고 농도를 계산하였다. ELISA 분석 결과, 메트포민은 낮은 농도 (10μΜ)에서도 히알루로난 생성을 효율적으로 억제함을 확인하였다 (도 2). 분화된지방세포의오일 레드 0 염색
0.5% (w/v)'의 오일 레드 0(oil red 0) 용액을 준비하고, 이를 3:2의 비율로 멸균된 증류수와 희석하여 지방세포의 염색용 시약으로 사용하였다. 6-웰 플레이트에 안와 섬유모세포를 분주한 후, 세포가 컨플루언스에 도달하면 지방전구세포의 지방분화를 10일 간 유도하였다. 지방분화가 모두 끝나면, 배지를 모두 제거하고 PBS를 이용하여 2번 세척한 후, 3/7% (w/v) 포르말린을 이용하여 세포를 1시간 동안 고정하였다. 고정 후, 오일 레드 0 워킹 솔루션을 1시간 동안 처리하고, 증류수를 이용하여 2번 세척한 뒤 이를 현미경 (Olympus, Melville, NY, USA)을 이용하여 확인하였다. 관찰 결과, 메트포민은 농도 의존적으로 지방의 분화를 억제함을 확인하였다 (도 3 내지 5) 웨스턴 블롯팅 항원 에피토프와 반웅하는 항체를 이용하여 단백질 흔합물 증에서 원하는 단백질 (antigen)만을 찾는 방법으로 갑상선안병증의 안와지방 결체조직에서 메트포민의 지방분화 유도 중 지방분화 억제 효과를 살펴보았다. 안와지방결체조직 세포에 연자방 추출물을 50, 100yg/ml를 지방분화 유도 10일 동안 처리하였다. 세포용해물 (lysate)을 얻은 후 세포용해 완충액에 단백질 분해효소 억제제와 포스파타제 억제제를 첨가한 후 20분 동안 얼음에서 보관하였다. 12,000 rpm에서 원심분리하여 상층액을 얻은 후; 단백질 정량을 하여 각 실험군마다 동일한 단백질 양을 만들었다. 10% SDS-PAGE를 이용하여 단백질을 분리하고, 분리된 단백질을 PVDF 막 (Millipore, MA, USA)에 옮겨주었다. 5% 탈지유를 이용하여 막에 있는 비특이적 결합부위를 모두 블로킹하였다. 일차항체인 PPARY ,
C/EBP α , C/ΕΒΡβ , ρ-Ρ65, Ρ65, H0-1 및 β-actin을 24시간 동안 4°C에서 보관한 후 이차항체를 1시간 동안 특수발색 기질 (chromogenic substrate)을 이용하여 처리하였다. 메트포민을 10, 100 및 1000 μΜ 처리했을 때, 지방분화 관련인자인 PPARy , C/EBP α , C/EBP β의 발현을 감소시킴을 확인할 수 있었다 (도 6).

상기 화학식에서, 내지 는 각각 독립적으로 수소 또는 알킬이다.

제 1 항에 있어서, 상기 화학식 1의 및 ¾는 수소이고, 및 ¾는 각각 독립적으로 d-C3 알킬인 것을 특징으로 하는 조성물.

제 2 항에 있어서, 상기 화학식 1의 및 R4는 메틸인 것을 특징으로 하는 조성물.

제 1 항에 있어서, 상기 자가면역성 갑상선 질환은 갑상선 안구병증 (thyroid-associated ophthalmopathy) , 그레이브스 병 (Graves' disease), 하시모토 갑상선염 (Hashimoto' s thyroiditis), 위축성 갑상선염 (atrophic thyroiditis), 무통성 갑상선염 (painless thyroiditis) 및 산후갑상선염 (postpartum thyroidit is)으로 구성된 군으로부터 선택되는 것올 특징으로 하는 조성물.
