Abstract
Description
〔2〕食後に胃に作用するように投与する手段が、食後に成分(A)と成分(B)の両者が有効血中濃度に達するように投与するものである〔1〕記載の食後期の胃運動亢進剤。
〔3〕糖尿病性胃麻痺、術後胃麻痺及び機能性ディスペプシアから選ばれる疾患における食後期の胃運動亢進剤である〔1〕又は〔2〕記載の食後期の胃運動亢進剤。
〔4〕(A)グレリン若しくはグレリンアゴニストと(B)モチリン若しくはモチリンアゴニストとの両者を食後に胃に作用するように投与することを特徴とする、食後期の胃運動亢進方法。
〔5〕食後に胃に作用するように投与する手段が、食後に成分(A)と成分(B)の両者が有効血中濃度に達するように投与するものである〔4〕記載の食後期の胃運動亢進方法。
〔6〕糖尿病性胃麻痺、術後胃麻痺及び機能性ディスペプシアから選ばれる疾患における食後期の胃運動亢進方法である〔4〕又は〔5〕記載の食後期の胃運動亢進方法。
〔7〕食後期の胃運動亢進に用いるための(A)グレリン若しくはグレリンアゴニストと(B)モチリン若しくはモチリンアゴニストとの組み合わせであって、成分(A)と成分(B)の両者が食後に胃に作用するように投与して用いられることを特徴とする、成分(A)と成分(B)の組み合わせ。

R8及びR9は、同一又は異なって、水素原子、炭素数1~6の直鎖状、分岐状若しくは環状のアルキル基、(ここでアルキル基にはハロゲン原子、水酸基、炭素数1~6のアルコキシ基、フェニル基、ベンジルオキシ基又はヒドロキシフェニル基が置換していてもよい)を示すか、あるいは、R8又はR9とR6又はR7が隣接する窒素原子と一緒になってピロリジン環若しくはピペリジン環(ここでピロリジン環若しくはピペリジン環には水酸基が置換してもよい)を形成してもよく;
R10及びR12は、同一又は異なって、水素原子又はメチル基を示し;

スンクスの食後期胃運動に対するグレリン、モチリン及びグレリンとモチリンの共投与の影響について検討した。
ア.薬物
スンクスモチリン(Scrum Inc,Tokyo,Japan)及びヒトグレリン(Asubio Pharma Co.,Ltd.,Hyogo,Japan)は、0.1%牛血清アルブミンを含むPBS(リン酸緩衝生理食塩液)に溶解した。
イ.動物
実験には、スンクス(オス、非近交系KAT株、8-9週齢、73-90g)を使用した。スンクスは、巣箱として空き缶を備えたプラスチックゲージで、23±2℃、照明時間8:00~20:00、商用の鱒の餌(SP,Nippon Formula Feed Manufacturing,Yokohama,Japan)の自由摂餌条件下で飼育した。
ウ.手術(フォーストランスデューサの縫着及び頸静脈カテーテルの装着)
3時間絶食したスンクスにペントバルビタールナトリウム(50mg/kg)を腹腔内投与し、麻酔した。正中切開後、胃を体外に露出し、胃体部の輪走筋方向にフォーストランスデューサを縫着した。フォーストランスデューサからのワイヤは、腹壁を通し、皮下を経由して首筋の後ろ側に露出した。
静脈内カテーテルを右頸静脈に挿入後、背部の体外に固定した。カテーテルは、凝固を防ぐために、ヘパリン化生食液(100units/mL)で満たした。
手術を施したスンクスは数日間、回復させた。
エ.胃運動の測定
投与及び胃運動の測定は、意識下、無麻酔及び無拘束の条件下で行った。
オ.投与
絶食したスンクス(8~10時間)に、餌2gを給餌し、10分間摂食させた。10分間摂食後に頸静脈カテーテルから、グレリン(50ng/kg)、モチリン(300ng/kg)又はグレリン及びモチリン(グレリン:50ng/kg、モチリン:100、200、300ng/kg)の混合液を急速静脈内投与した。
カ.胃運動の測定
増幅されたアナログ信号をアナログデジタル変換器(ADC-20,Pico Technology Ltd,St Neots,UK)により変換し、デジタル信号をコンピュータにより記録した。
意識下スンクスにおけるMMCのphaseIII収縮は、イヌ及びヒトのMMCのphaseIII収縮に基づき定義した(すなわち、8g以上の振幅を伴う収縮群で、5分以上続くこと)。
キ.データ解析
胃運動を運動係数(MI)により定量化した。薬物による運動係数(MI)は以下の式から算出した。

ク.統計解析
One-way ANOVA後、Tukey法による多重比較検定を行った。0.05以下のp値を統計学的有意とした。
ア.グレリン単独の作用
50ng/kgのグレリン(G)は、胃体部収縮を誘発しなかった(図1)。
イ.モチリン単独の作用
300ng/kgのモチリン(M)は、胃体部収縮を誘発しなかった(図2)。
ウ.グレリン(G)及びモチリン(M)共投与の作用
G 50ng/kg+M200ng/kg:自発性phaseIII収縮よりも小さいphaseIII様収縮を誘発した(図3)。
(運動係数:約45%)
G 50ng/kg+M300ng/kg:phaseIII様収縮を誘発した(図4)。
(運動係数:約85%)
(1)材料及び方法
ア.薬物

イ.動物

3時間絶食したスンクスにペントバルビタールナトリウム(50mg/kg)を腹腔内投与し、麻酔した。正中切開後、胃を体外に露出し、胃体部の輪走筋方向にフォーストランスデューサを縫着した。フォーストランスデューサからのワイヤは、腹壁を通し、皮下を経由して首筋の後ろ側に露出した。その後、全迷走神経切除術を実施した。食道下部を露出し、迷走神経の両枝を単離後、肝枝及び腹腔枝の上で、約3mmの迷走神経を切除した。擬手術のスンクスでは、迷走神経を露出させたが、切断しなかった。静脈内カテーテルを右頸静脈に挿入後、背部の体外に固定した。カテーテルは、凝固を防ぐために、ヘパリン化生食液(100units/mL)で満たした。
手術の2日後から、胃運動を記録した。
エ.空腹及び食後期でのモチリン及びグレリンの投与
空腹期
モチリンの投与は、自発性phaseIIIが終了したl0分後に開始した。合成スンクスモチリンは、50ng/kg/minで10分間持続注入し、持続注入量は、50μL/100g BW/minとした。グレリンの投与は、自発性phaseIIが終了したl0分後に開始した。アシルグレリンは、0.1、0.3、1、3μg/kg/minで10分間持続注入した。
食後期
食後期収縮を観察するために、phaseIの間に1gの餌をスンクスに給餌した。摂食が終了した20分後にモチリンを50ng/kg/minで10分間持続注入した。
オ.胃運動の測定

ア.絶食状態での自発性収縮
絶食状態の偽手術(Sham)及び迷走神経切除(Vagotomy)スンクスの両方において、phaseI、II及びIIIから成る周期的な伝播性消化管収縮運動(MMC)が明らかに観察された(図5A及び5B)。しかし、迷走神経切除スンクスのphaseIIの期間(57±9分)は、偽手術スンクスのphaseIIの期間(139±16分)よりも有意に短く、そして迷走神経切除スンクスのphaseIの期間は増加した(238±76分 vs 60±6分)。MMCの全期間及びphaseIIIの期間は、偽手術と迷走神経切除スンクスでは有意な差はなかった(図5C)。迷走神経切除スンクスにおけるphaseIIの運動係数(MI)(13%±3%)は、偽手術スンクスのphaseIIのMI(29%±5%)より有意に低かった(図5D)。
イ.MMCのphaseIIにおけるグレリンの効果
phaseII収縮が開始した10分後に、生理食塩水またはグレリン(0.1、0.3、1、3又は10μg/kg/min)を10分間静脈内に注入した。偽手術スンクスでは、グレリンは自発性phaseII収縮の振幅を増幅した(図6A)。しかし、グレリンは、迷走神経切除スンクスの自発性phaseII収縮の振幅を変化させなかった(図6B)。
偽手術スンクスにおけるグレリン投与中のMIは、迷走神経切除スンクスのグレリン投与中のMIよりも高かった(図6C)。
ウ.MMCのphaseIにおけるモチリンの効果
自発性phaseIII収縮の10分後、モチリン(50ng/kg/min)を静脈内に10分間注入した。モチリン誘発性phaseIII様収縮が偽手術及び迷走神経切除スンクスの両方で観察された(図7A及び7B)。モチリン投与中の運動係数(MI)及びモチリンによる収縮時間は有意な差が認められなかった(図7C及び7D)。
エ.食後の胃運動及び食後状態でのモチリンの効果
摂食後、偽手術スンクスにおいて、MMCのphaseIは非周期的な食後期収縮に変わり、モチリン(50ng/kg/min)の10分間投与は、phaseIII様収縮を誘発しなかった(図8A)。対照的に、迷走神経切除スンクスにおいて、摂食はMMCのphaseIを中断せず、外因性モチリンは、絶食状態の時のモチリンの作用と同様に、強力なphaseIII収縮を誘導した(図8B)。摂食開始後10分間のMlは、偽手術スンクスよりも迷走神経切除スンクスで有意に低く、食後のモチリン誘発性収縮のMlは、偽手術スンクスよりも迷走神経切除スンクスで有意に高かった(図8C)。
これらの結果から、スンクスにおいて、グレリンは迷走神経を介してMMCのphaseIIを刺激すること、迷走神経は食後期収縮の開始に不可欠であること、モチリンにより誘発されるphaseIII収縮の阻害は迷走神経に依存していることが判明した。

(1)材料及び方法
ア.薬物
スンクスモチリン(Scrum Inc.,Tokyo,Japan)及びヒトグレリン(Asubio Pharma Co.,Ltd.,Hyogo,Japan)は、0.1%牛血清アルブミンを含むPBS(以下0.1%BSA/PBS)に溶解した。
イ.動物
実験にはスンクス(オス、非近交系KAT株、7-14週齢、60-91g)を使用した。スンクスは、巣箱として空き缶を備えたプラスチックゲージで、23±2℃、照明時間8:00~20:00、商用の鱒の餌(SP,Nippon Formula Feed Manufacturing,Yokohama,Japan)の自由摂餌条件下で飼育した。
ウ.手術(頸静脈カテーテルの装着)
3時間絶食したスンクスにペントバルビタールナトリウム(50mg/kg)を腹腔内投与し、麻酔した。静脈内カテーテルを右頸静脈に挿入後、背部の体外に固定した。カテーテルは、凝固を防ぐために、ヘパリン化生食液(100 units/mL)で満たした。
手術を施したスンクスは数日間、回復させた。
エ.胃排出の測定
11~12時間絶食したphaseI期のスンクスに餌を1.5g与え、摂食開始10分後に溶媒である0.1%BSA/PBS溶液又はphaseIII様の強収縮を惹起するグレリン及びモチリン(グレリン:50ng/kg、モチリン:300ng/kg)の混合液を急速静脈内投与した。投与20分後にペントバルビタールナトリウムの腹腔内過剰投与によりスンクスを安楽死させ、幽門部及び噴門部を縫合糸で結紮し、胃を摘出した。続いて胃を大彎に沿って開き、胃内の食物をシャーレ内に集めた。胃内に残っていた食物は37℃に設定したインキュベーター内で乾燥させ、48時間後に電子天秤を用いて乾燥重量を測定した。胃内残存率(%)を(胃から取り出した餌の乾燥重量/食べた餌の重量)×100で算出し、評価した。
オ.統計解析
Un-paird Student’sのt検定で行った。0.05以下のp値を統計学的有意とした。
ア.0.1%BSA/PBS投与の作用
0.1%BSA/PBS:胃内残存率は、43.4%であった。
イ.グレリン(G)及びモチリン(M)共投与の作用
G50ng/kg+M300ng/kg:胃内残存率は、22.3%であった。
(1)材料及び方法
ア.薬物
スンクスモチリン(Scrum Inc.,Tokyo,Japan)は、0.1%牛血清アルブミンを含むPBSに溶解した。グレリンアゴニスト(WO2009/098901の実施例34(a)の化合物は生理食塩水に溶解した。
イ.動物
実験には、スンクス(オス、非近交系KAT株、8-10週齢、76-84g)を使用した。スンクスは、巣箱として空き缶を備えたプラスチックゲージで、23±2℃、照明時間8:00~20:00、商用の鱒の餌(SP,Nippon Formula Feed Manufacturing,Yokohama,Japan)の自由摂餌条件下で飼育した。
ウ.手術(フォーストランスデューサの縫着及び頸静脈カテーテルの装着)
3時間絶食したスンクスにペントバルビタールナトリウム(50mg/kg)を腹腔内投与し、麻酔した。正中切開後、胃を体外に露出し、胃体部の輪走筋方向にフォーストランスデューサを縫着した。フォーストランスデューサからのワイヤは、腹壁を通し、皮下を経由して首筋の後ろ側に露出した。
静脈内カテーテルを右頸静脈に挿入後、背部の体外に固定した。カテーテルは、凝固を防ぐために、ヘパリン化生食液(100units/mL)で満たした。
手術を施したスンクスは数日間、回復させた。
エ.胃運動の測定
投与及び胃運動の測定は、意識下、無麻酔及び無拘束の条件下で行った。
オ.投与
絶食したスンクス(8~10時間)に、餌2gを給餌し、10分間摂食させた。10分間摂食後に頸静脈カテーテルから、グレリンアゴニスト(10、30mg/kg)又はグレリンアゴニスト及びモチリン(グレリンアゴニスト:50、100μg/kg、モチリン:300ng/kg)の混合液を急速静脈内投与した。
カ.胃運動の測定
増幅されたアナログ信号をアナログデジタル変換器(ADC-20,Pico Technology Ltd,St Neots,UK)により変換し、デジタル信号をコンピュータにより記録した。
意識下スンクスにおけるMMCのphaseIII収縮は、イヌ及びヒトのMMCのphaseIII収縮に基づき定義した(すなわち、8g以上の振幅を伴う収縮群で、5分以上続くこと)。
キ.データ解析
胃運動を運動係数(ΔMI)により定量化した。薬物による運動係数(ΔMI)は以下の式から算出した。
ΔMI(%)=(A-C)/B×100
A:薬物を投与している10分間の収縮のAUC
B:薬物投与前に出現した自発性phaseIII収縮10分間のAUC
C:給餌後5~10分の5分間に出現した収縮のAUC×2
ア.グレリンアゴニスト単独の作用
10mg/kgのグレリンアゴニスト(Z)は、胃体部収縮を誘発しなかった。

30mg/kgのグレリンアゴニスト(Z)は、胃体部収縮を誘発しなかった。
(運動係数:約3.7%)
イ.グレリンアゴニスト(Z)及びモチリン(M)共投与の作用
Z 50μg/kg+M300ng/kg:phaseIII様収縮を誘発した。
(運動係数:約65.9%)
Z 100μg/kg+M300ng/kg:phaseIII様収縮を誘発した。
(運動係数:約59.1%)
Claims (12)
- (A)グレリン若しくはグレリンアゴニストと(B)モチリン若しくはモチリンアゴニストとの両者が食後に胃に作用するように投与して用いられることを特徴とする、成分(A)と成分(B)を有効成分とする食後期の胃運動亢進剤。
- 食後に胃に作用するように投与する手段が、食後に成分(A)と成分(B)の両者が有効血中濃度に達するように投与するものである請求項1記載の食後期の胃運動亢進剤。
- 糖尿病性胃麻痺、術後胃麻痺及び機能性ディスペプシアから選ばれる疾患における食後期の胃運動亢進剤である請求項1又は2記載の食後期の胃運動亢進剤。
- (A)グレリン若しくはグレリンアゴニストと(B)モチリン若しくはモチリンアゴニストとの両者を食後に胃に作用するように投与することを特徴とする、食後期の胃運動亢進方法。
- 食後に胃に作用するように投与する手段が、食後に成分(A)と成分(B)の両者が有効血中濃度に達するように投与するものである請求項4記載の食後期の胃運動亢進方法。
- 糖尿病性胃麻痺、術後胃麻痺及び機能性ディスペプシアから選ばれる疾患における食後期の胃運動亢進方法である請求項4又は5記載の食後期の胃運動亢進方法。
- 食後期の胃運動亢進に用いるための(A)グレリン若しくはグレリンアゴニストと(B)モチリン若しくはモチリンアゴニストとの組み合わせであって、成分(A)と成分(B)の両者が食後に胃に作用するように投与して用いられることを特徴とする、成分(A)と成分(B)の組み合わせ。
- 食後に胃に作用するように投与する手段が、食後に成分(A)と成分(B)の両者が有効血中濃度に達するように投与するものである請求項7記載の成分(A)と成分(B)の組み合わせ。
- 糖尿病性胃麻痺、術後胃麻痺及び機能性ディスペプシアから選ばれる疾患における食後期の胃運動亢進剤に用いるためのものである請求項7又は8記載の成分(A)と成分(B)の組み合わせ。
- (A)グレリン若しくはグレリンアゴニストと(B)モチリン若しくはモチリンアゴニストとの両者が食後に胃に作用するように投与して用いられることを特徴とする、成分(A)と成分(B)の組み合わせの食後期の胃運動亢進剤製造のための使用。
- 食後に胃に作用するように投与する手段が、食後に成分(A)と成分(B)の両者が有効血中濃度に達するように投与するものである請求項10記載の使用。
- 糖尿病性胃麻痺、術後胃麻痺及び機能性ディスペプシアから選ばれる疾患における食後期の胃運動亢進剤の製造のための使用である請求項10又は11記載の使用。
