Abstract

L'invention concerne un formulaire comprenant une carte pliable pouvant être produite à partir de ce formulaire et un substrat plan (10) sur lequel est disposé un matériau multicouche (20, 30). Un procédé de découpe (11, 12, 13, 14, 15) réalisé à partir de la partie antérieure du formulaire est utilisé pour produire une ligne de pliage (15), des parties centrales (A, B) de la carte pliable dans une première zone (10.1) recouverte par le matériau multicouche (20), ainsi que des parties plastifiées transparentes (E, F) de la carte pliable dans une deuxième zone (10.2) recouverte par le matériau multicouche (30), de l'autre côté de ladite ligne de pliage (15), ainsi que des parties de recouvrement (CD) de même forme pour lesdites parties plastifiées transparentes. Les surfaces des parties centrales du formulaire qui sont orientées vers l'extérieur après l'assemblage de la carte pliable sont exposées sur la face antérieure du formulaire. Les surfaces des parties centrales du formulaire qui sont orientées vers l'intérieur après l'assemblage de la carte pliable sont pourvues d'une première couche d'adhésif. Les surfaces des parties plastifiées du formulaire qui sont orientées vers l'intérieur après l'assemblage de la carte pliable sont pourvues d'une deuxième couche d'adhésif. Les parties de recouvrement du formulaire sont placées sur la deuxième couche d'adhésif, sur la face antérieure du formulaire, et peuvent être retirées dudit formulaire par décollement desdites parties de recouvrement de ladite deuxième couche d'adhésif. Les parties plastifiées peuvent recouvrir les parties centrales du formulaire et être collées sur ces parties de recouvrement au moyen de la deuxième couche d'adhésif, par pliage du formulaire au niveau de la ligne de pliage lorsque les parties de recouvrement ont été retirées. Les parties centrales peuvent être retirées du formulaire par séparation de leur première couche d'adhésif d'une couche adjacente à cette première couche d'adhésif, et les surfaces des parties de recouvrement comportant la première couche d'adhésif peuvent être collées ensemble par pliage. Cette invention est caractérisée en ce qu'un premier (20) et un deuxième matériau multicouche (30) sont disposés sur le substrat (10), en ce que la ligne de pliage (15) est découpée uniquement au niveau du substrat (10), en ce que les parties plastifiées (E, F) du formulaire sont maintenues uniquement par l'intermédiaire de perforations en pointillés, et en ce que les surfaces des parties plastifiées (E, F) qui sont orientées vers l'extérieur après l'assemblage de la carte pliable sont exposées sur la face postérieure du formulaire.
