技術分野
本発明は、 インターフェロン作用物質の抗 C型肝炎ウィルス活性増強剤、 抗 C 型肝炎ウィルス用のィンターフェ口ン作用物質治療における副作用を軽減する副 作用軽減剤、 分岐鎖ァミノ酸とインターフェロン作用物質を組み合わせてなる医 薬、 及びそれらの医薬キットなどに関する。
背景技術
インターフェロン (以下、 I FNとも称する) は抗ウィルス作用、 細胞増殖抑 制作用、 免疫調節作用など多彩な生物活性を有するサイトカインであり、 臨床的 には I FN療法に、 具体的には抗ウィルス剤として B型慢性肝炎及び C型慢性肝 炎に、 抗腫瘍剤として慢性骨髄性白血病、 腎癌、 多発性骨髄腫、 ヘアリー細胞白 血病、 亜急性硬化性全脳炎、 HTLV— 1脊髄症、 皮膚悪性黒腫、 膠芽腫、 髄芽 腫、 星細胞腫などに使用されている。
C型肝炎ウィルス (以下、 HCVとも称する) の持続感染は、 慢性肝炎を引き 起こし、 20数年の経過の中で肝硬変となり、 そして最終的には肝細胞癌発症に 至る。 その HCVに対する唯一の抗 HCV剤による療法として行われてきた I F N療法の著効率 (本発明において、 著効率は I FN治療終了後 24週における血 中HCV— RNA陰性化率 (%) と定義する) は該療法を行った全症例に対し、 20 %程度、 H C Vが遺伝子型 I bかつ高ウィルス量の難治性 C型慢性肝炎に限 つては 1 0%未満と低い。 リバビリン (R i b a v i r i n, 以下 R i bとも称 す) は I FNと異なる作用機序で抗ウィルス活性を示す抗ウィルス剤である。 日 本肝臓学会編、 慢性肝炎の治療ガイド (文光堂) 、 p 30、 p 34 (2004 年) には、 R i b単独では HCVに対して作用効果を示さないが、 I FNとR i bとを併用すると、 全症例に対する著効率は 50 %程度、 難治性患者に対して 3 0%程度に改善することが記載されている。 しカゝしながら、 全ての治療患者に有
効な治療効果を示すまでにはいまだ至っていない。 よってさらに優れた効果を有 する I F Nの抗 C型肝炎ウィルス活性増強剤が必要とされているのが現状である。 また、 飯野四郎 編著、 慢性肝炎最新の治療 (中外医学社) p 1 5 4 ( 2 0 0 1年) などには、 I F Nの使用の際の、 発熱、 頭痛、 関節痛などの副作用が報告 されている。 これらの副作用は、 インターフェロン治療を受けている患者にとつ て大きな問題であり、 治療中の患者にも、 これから治療を受ける患者にも治療に 対する不安を引き起こしている。 よってこれらの副作用が軽減された治療剤が必 要とされている。
そして、 I F Nと R i bの併用療法時には、 I F Nによる副作用の他に R i b による副作用発現が加わることになる。 R i bによる副作用として最も特徴的な ものは溶血性貧血であるが、 その程度が重篤な場合は R i bの減量もしくは投与 中止が必要となる。 よって、 I F Nと R i bの併用療法において、 その抗ウィル ス効果を維持したまま R i bを減量、 あるいは完全に置き換えることが可能な薬 剤も必要とされている。
—方、 分岐鎖アミノ酸は I F N療法時に血中濃度が低下することが、 各務ら、 犬山シンポジウム 2 4 セッション 3、 p . 1 4 2 - 1 4 6 ( 2 0 0 3年) によ つて報告されているが、 分岐鎖アミノ酸が I F Nの効果を増強する作用や I F N の副作用を軽減する作用を有するという報告はない。
本発明が解決しょうとする課題は、 I F N療法時に、 I F N作用物質の抗 C型 fl干炎ウィルス活性を増強する優れた I F N作用物質の活性増強剤、 及び I F N作 用物質 (R i b作用物質併用時を含む) の副作用を低減する副作用軽減剤等を提 供することである。
発明の開示
本発明者らは、 上記課題を解決すべく鋭意研究を重ねた結果、 イソロイシン、 ロイシン、 及びバリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸が I F N作 用物質の抗 C型肝炎ウィルス活性を増強するとともに、 I F N作用物質における
副作用、 さらには R i b作用物質併用時における R i b作用物質の副作用を低減 することを見出し、 本発明を完成した。
すなわち、 本発明は以下の通りである。
[1] イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐 鎖アミノ酸またはその塩を含むことを特徴とする、 インターフェロン作用物質の 抗 C型肝炎ゥィルス活性増強剤。
[2] インターフェロン作用物質がインターフェロンである、 [1] に記載の活 性増強剤。
[3] イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐 鎖アミノ酸またはその塩を含むことを特徴とする、 抗 C型肝炎ウィルス用のイン ターフェロン作用物質治療における副作用軽減剤。
[4] インターフエ口ン作用物質治療がさらにリノ ビリン作用物質を組み合わせ てなるものである、 [3] に記載の副作用軽減剤。
[5] イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐 鎖アミノ酸またはその塩とインターフェロン作用物質を組み合わせてなる、 抗 C 型肝炎ウィルス用医薬。
[6] さらにリバビリン作用物質を含むことを特徴とする、 [5] に記載の医薬。
[7] インターフエ口ン作用物質の抗 C型肝炎ウィルス活性増強剤の製造のため の、 イソロイシン、 ロイシン、 及ぴバリンから選ばれる少なくとも 1種の分岐鎖 ァミノ酸またはその塩の使用。
[8] インターフェロン作用物質がインターフェロンである、 [7] 記載の使用。
[ 9 ] 抗 C型肝炎ウィルス用のインターフエ口ン作用物質治療における副作用軽 減剤の製造のための、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なく とも 1種の分岐鎖アミノ酸またはその塩の使用。
[ 1 0 ] インターフエ口ン作用物質治療がさらにリバビリン作用物質を組み合わ せてなるものである、 [9] 記載の使用。
[1 1] 抗 C型肝炎ウィルス用医薬の製造のための、 イソロイシン、 ロイシン、 及びバリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸またはその塩及びィン ターフェ口ン作用物質の使用。
[1 2] さらにリバビリン作用物質を含むことを特徴とする、 [1 1] 記載の使 用。
[1 3] 有効量の、 イソロイシン、 ロイシン、 及びバリンから選ばれる少なくと も 1種の分岐鎖アミノ酸またはその塩を投与することからなる、 インターフエ口 ン作用物質の抗 C型肝炎ゥィルス活性を増強させる方法。

[24] さらにリバビリン作用物質を含むことを特徴とする、 [23] 記載の医 薬組成物。
[25] イソロイシン、 ロイシン、 及びバリンから選ばれる少なくとも 1種の分 岐鎖アミノ酸またはその塩及びインターフェロン作用物質をそれぞれ単味剤とし て組み合わせたことを特徴とする、 抗 C型肝炎ウィルス用医薬キット。
[26] イソロイシン、 ロイシン、 及びバリンから選ばれる少なくとも 1種の分 岐鎖ァミノ酸またはその塩、 インターフエ口ン作用物質及びリバビリン作用物質 をそれぞれ単味剤として組み合わせたことを特徴とする、 抗 C型肝炎ウィルス用 医薬キット。

発明を実施するための最良の形態
<分岐鎖アミノ酸 >
本発明の有効成分 (分岐鎖アミノ酸) である、 イソロイシン、 ロイシンおょぴ パリンはそれぞれ L一体、 D—体、 DL—体いずれも使用可能であるが、 好まし くは L一体、 DL—体であり、 さらに好ましくは L一体である。 またイソ口イシ
ン、 ロイシンおよびパリンはそれぞれ、 遊離体のみならず、 塩の形態でも使用す ることができる。 塩の形態には酸付加塩や塩基との塩等を挙げることもでき、 ィ ソロイシン、 口ィシンおよびバリンの医薬品として許容される塩を選択すること が好ましい。 イソロイシン、 ロイシンおよびバリンにそれぞれ付加して医薬とし て許容される塩を形成する酸としては、 例えば、 塩化水素、 臭化水素、 硫酸、 リ ン酸塩等の無機塩、 酢酸、 乳酸、 クェン酸、 酒石酸、 マレイン酸、 フマル酸又は モノメチル硫酸等の有機塩が挙げられる。 イソロイシン、 ロイシンおよびバリン の医薬として許容される塩基の例としては、 例えば、 ナトリウム、 カリウム、 力 ルシゥム等の金属の水酸化物あるいは炭酸化物や、 ァンモユア等の無機の塩基と の塩、 エチレンジァミン、 プロピレンジァミン、 エタノーノレアミン、 モノアルキ ルエタノールァミン、 ジアルキルエタノールァミン、 ジエタノールァミン、 トリ エタノールァミン等の有機の塩基との塩が挙げられる。
くインターフエ口ン作用物質の抗 C型肝炎ゥィルス活性増強剤 >
本発明の I F N作用物質の抗 C型肝炎ウィルス活性増強剤は、 イソロイシン、 ロイシン、 バリンのいずれか 1種以上が分岐鎖アミノ酸として含有されていれば 良く、 イソロイシン、 ロイシン、 パリン全てが含有されていることが好ましい。 より好ましくは、 イソロイシン、 ロイシン、 バリンの重量比が 1 : 1〜3 : 0. 5〜2. 0である。 さらに好ましくは、 イソロイシン、 ロイシン、 バリンの重量 比が 1 : 1. 5〜2. 5 : 0. 8〜1. 5である。 最も好ましい重量比は 1 : 2 : 1. 2である。 本発明の活性増強剤は、 さらに、 後述の一般的な製剤用担体 を含有していてもよい。
本発明における I FN作用物質としては、 I FNのほか、 生体内で I FN様の 作用を奏する物質であれば特に限定されない。 I FNとしては、 例えば I FN— α、 I FN— j3及ぴ I FN— γ、 I F Νの化学修飾体などが挙げられる。 I FN 一 aには 20種類以上の亜型があるといわれており、 1 65〜1 66個のアミノ 酸からなるものと 1 72個のアミノ酸からなるものに大きく分けられる。 分子量 は、 1. 7〜2. 3万の間にあり、 前者は糖鎖を有するが後者は糖鎖を持たない。
I F N—ひには天然型と、 遺伝子組み換えによつて大腸菌で産生された遺伝子組 み換え型とがあり、 後者は糖鎖を持たない。 天然型 I FN— α製剤には、 ォーァ ィエフ (登録商標) 、 I FN—ひモチダ、 スミフエロン (登録商標) などがある (成人 1日あたり 250万〜 600万国際単位投与) 。 遺伝子組み換え型 I F N —ひ製剤としては、 アドパフェロン (登録商標) 、 口フエロン (登録商標) 、 キ ヤンフエロン (登録商標) A、 イントロン (登録商標) Aなどがある (成人 1日 あたり 300万〜 1800万国際単位投与) 。 I FN— は 166個のアミノ酸 からなり亜型は存在しない。 天然型 I FN— 製剤には、 フエロン (登録商標) 、 I F N ]3モチダ (成人 1日あたり 100万〜 300万国際単位投与) 、 遺伝子組 み換え型 I FN— 製剤には、 ベタフエロン (登録商標) などがある (成人 1日 あたり 800万国際単位投与) 。 天然型 I FN— γ製剤には、 オーガンマ (登録 商標) (成人 1日あたり 100万国際単位投与) 、 遺伝子組み換え型 I F Ν— V 製剤としては、 ビォガンマ (登録商標) 、 ィムノマックス (登録商標) 一 γなど がある (成人 1日あたり 200万〜 400万国際単位投与) 。
更に、 I F Ν作用物質は I F Ν様生物活性を惹起するものであれば良く、 必ず しも上記 I FNと同一のアミノ酸配列を有するものでなくても良い。 また抗体、 ペプチド、 低分子であってもよい。
更にまた、 I FNは上記のものが修飾されていてもよく、 かかるものとしては ポリエチレングリコール (PEG) にて修飾されたものが挙げられ、 例えば PE G—インターフェロン一 α 2 a (ぺガシス (登録商標) (成人 1日あたり 180 ^ g投与) ) や PEG—インターフェロン一a 2 b (PEG- I n t r o n (登 録商標) (成人 1日あたり 1. 投与) ) などの製剤がある。 PEGは I F Nの活性部位において I F Nと結合するものではないので、 I F Nの活性を低下 させず、 酵素による I FNの分解を抑制するものである。 従って、 本発明の活个生 増強剤、 副作用軽減剤は、 PEG修飾 I FN作用物質に対しても有効である。 本発明において I FN作用物質としては、 I FNが好ましく、 その中でも I F N— aおよび PEG修飾されたものが特に好ましい。 I FN— a及びその PEG
修飾体は R i bとの併用治療の有効性が確立されているという点で好ましい。 特 に、 PEG修飾された I FNは、 血中における半減期が長いため、 作用が長く持 続し、 投与回数が少なくてすむというメリットがある。
I FN作用物質の抗 C型肝炎ウィルス活性増強とは、 I FN作用物質治療の抗 C型肝炎ウィルス作用を増強することをいい、 その結果として得られる、 I FN 作用物質治療患者の著効率の改善、 治療期間の短縮、 I FN作用物質投与量の減 量なども含むものとする。
本発明において I FN作用物質治療とは、 I FN作用物質によって抗 C型肝炎 ウィルス作用を発揮せしめ、 ひいてはウィルス性 C型肝炎を治療することを指す 力 I FN作用物質治療は、 さらにウィルス性肝炎に起因する肝硬変および肝癌 においても、 残存するウィルスに対して抗ウィルス活性を示し、 肝炎から肝硬変 および肝癌へ移行するのを遅延させ得る。 したがって、 本発明の I FN作用物質 の抗 C型肝炎ゥィルス活性増強剤は、 ゥィルス性肝炎に起因する肝硬変およぴ肝 癌の予防 ·進展抑制などの用途にも使用することができる。
また、 本発明における I FN作用物質治療には、 I FN作用物質とそれ以外の 薬剤とを併用する場合も含まれ、 例えば I FN作用物質と R i b作用物質の併用 治療が例示される。 本発明における R i b作用物質としては、 R i bのほか、 R i bの類縁体、 例 ば V i r am i d i n e (Wa t s o nら、 Cu r r e n t
O i n i o n i n i nv e s t i g a t i o n a l d r ug s 200 2, 3, (5) 680-683) など、 生体内でインターフェロンの抗ゥイノレス 効果を増強する R i b様作用を奏する物質であれば特に限定されない。
く副作用軽減剤〉
本発明の、 I FN作用物質治療における副作用軽減剤、 並びに I FN作用物質 と R i b作用物質との併用治療における副作用軽減剤は、 有効成分としてイソ口 イシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖アミノ酸ま たはその塩を含むことを特徴とするものである。
本発明の副作用軽減剤は、 イソロイシン、 ロイシン、 バリンのいずれか 1種以 上が分岐鎖アミノ酸として含有されていれば良く、 イソロイシン、 ロイシン、 バ リン全てが含有されていることが好ましい。 より好ましくは、 イソロイシン、 口 イシン、 バリンの重量比が 1 : 1〜3 : 0 , 5〜2., 0である。 さらに好ましく は、 イソロイシン、 ロイシン、 パリンの重量比が 1 : 1 . 5〜2 . 5 : 0 . 8〜 1 . 5である。 最も好ましい重量比は 1 : 2 : 1 . 2である。 本発明の副作用軽 減剤は、 さらに、 後述の一般的な製剤用担体を含有していてもよい。
C型ウィルス性肝炎に対する I F N作用物質治療における副作用軽減とは、 I F N作用物質投与に伴って発症する発熱、 頭痛、 関節痛などの副作用を軽減する 作用をいう。
本発明における I F N作用物質治療においては、 前述の通り、 R i b作用物質 を併用してもよいが、 R i b作用物質投与に伴う溶血性貧血、 全身倦怠感、 うつ、 不眠などの精神症状、 めまいなどの神経症状、 食欲低下、 吐き気などの消化器症 状、 筋肉痛、 湿疹、 搔痒感などが軽減される場合も、 本発明の副作用軽減に含ま れる。
<医薬>
本発明の医薬には、 イソロイシン、 ロイシン、 及ぴバリンから選ばれる少なく とも 1種の分岐鎖アミノ酸と I F N作用物質を併用するものであって、 分岐鎖ァ ミノ酸またはその塩と I F N作用物質とを含有する医薬、 あるいはイソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖アミノ酸またはその 塩と I F N作用物質が別個に製剤化された医薬のいずれもが含まれる。 本発明の 医薬には、 さらに R i b作用物質が、 同一製剤中に、 あるいは別個に製剤化され た配合剤として、 含まれていてもよい。
本発明の医薬は抗 C型ウィルス剤として、 C型ウィルス性肝炎、 C型ウィルス 性肝炎に起因する肝硬変および肝癌などの疾患に対して、 治療および予防の目的 で使用できる。 また、 本発明の医薬は、 肝炎ウィルスに感染することによって起 こされるウィルス血症の改善剤としても用いることができる。
<製剤 '組成物 >
本発明の医薬及び I F N作用物質の C型肝炎ウィルス活性増強剤、 副作用軽減 剤などの薬剤は、 例えば、 イソロイシン、 ロイシン、 及びパリンから選ばれる少 なくとも 1種の分岐鎖アミノ酸もしくはその塩または (および) I F N作用物質 を自体公知の方法に従って、 薬理学的に許容される担体と混合して医薬組成物 (例えば注射剤、 錠剤、 顆粒剤、 細粒剤、 散剤、 カプセノレ剤、 クリーム剤、 坐剤 等) として調製することができ、 経口又は非経口 (例、 局所、 直腸、 静脈投与 等) で投与することができる。
本発明における医薬及び I F N作用物質の C型肝炎ウィルス活性増強剤、 副作 用軽減剤は、 薬理学的に許容される担体や添加剤を用いて自体公知の方法により 各種製剤として製剤化することができる。 製剤としては、 例えば、 注射剤 (筋注 用、 静注用) 、 経管液剤などの液剤、 散剤、 細粒剤、 顆粒剤、 錠剤、 カプセル剤 などの固形剤などが挙げられる。
用いられる製剤用担体としては、 例えば、 乳糖、 ブドウ糖、 D—マンニトール、 澱粉、 結晶セルロース、 炭酸カルシウム、 カオリン、 デンプン、 ゼラチン、 ヒ ド 口キシプ口ピノレセズレロース、 ヒ ドロキシプロピノレメチノレセ /レロース、 ポリビュノレ ピロリ ドン、 エタノール、 カルポキシメチルセルロース、 力/レポキシメチルセル ロースカノレシゥム塩、 ステアリン酸マグネシウム、 タノレク、 ァセチノレセルロース、 白糖、 酸化チタン、 安息香酸、 パラォキシ安息香酸エステル、 デヒドロ齚酸ナト リウム、 アラビアゴム、 トラガント、 メチルセノレロース、 卵黄、 界面活性剤、 白 糖、 単シロップ、 クェン酸、 蒸留水、 エタノール、 グリセリン、 プロピレンダリ コール、 マク口ゴール、 リン酸一水素ナトリウム、 リン酸ニ水素ナトリウム、 リ ン酸ナトリウム、 ブドウ糖、 塩化ナトリウム、 フエノール、 チメロサール、 パラ ォキシ安息香酸エステル、 亜硫酸水素ナトリウム等があり、 製剤の形に応じて、 本発明の医薬又は薬剤に含有して使用される。
<投与方法 ·投与量 >
本発明の I FN作用物質の C型肝炎ウィルス活性増強剤、 副作用軽減剤、 医薬 および医薬組成物において、 分岐鎖アミノ酸の投与量 (摂取量) は、 患者の病態、 年齢、 投与方法などによって異なるが、 通常、 1人あたり 1日、 イソロイシン 0. 5— 30. 0 g、 ロイシン 1. 0〜60. O g、 ノ リン 0. 5〜30. O gであ る。 一般の成人の場合、 好ましくは 1人あたり 1日、 イソロイシン 2. 0〜10. 0 g、 ロイシン 3. 0〜20. 0 g、 ノ リン 2. 0~ 10. 0 g、 より好ましく は、 イソロイシン 2. 5〜3. 5 g、 ロイシン 5. 0〜7. 0 g、 ノ リン 3. 0 〜4. O gである。 イソロイシン、 ロイシン、 バリン全てが含有されている場合 は、 成人 1日あたりの投与量が 3種の分岐鎖アミノ酸の合計量として、 通常 2. 0-50. 0 g程度が好ましく、 これを必要に応じて 1〜6回、 好ましくは 1〜 3回に分割して投与する。
I FN作用物質の投与量は、 患者の病態、 年齢、 投与方法などによって異なる 力 成人 1日量として 100万〜 2000万国際単位程度、 あるいは成人 1日量 として 20~400 μ g程度とする。 また R i b作用物質の投与量は、 成人 1日 量として 200〜200 Omg程度とする。
本発明において、 分岐鎖アミノ酸および I FN作用物質は、 それぞれ別々の製 剤として、 同一または異なる投与形態で投与してもよい。 あるいは、 分岐鎖アミ ノ酸と I FN作用物質とを同時に含有する医薬である配合剤として投与してもよ レ、。
1回投与当たりの分岐鎖アミノ酸の配合量は、 通常 0. 5〜50. 0 g程度、 好ましくは 1. 0〜20. 0 g程度、 より好ましくは 2. 0〜6. 0 g程度とす る。
1回投与当たりの I F N作用物質の配合量は、 それぞれ薬剤ごとに設定する。 上記配合剤中の分岐鎖アミノ酸と I FN作用物質の比率は分岐鎖アミノ酸 1 g の場合、 I FN作用物質が 2万〜 20000万国際単位あるいは I FN作用物質 が 0. 4〜4000 μ gであり、 好ましくは、 分岐鎖アミノ酸 1 gの場合、 I F N作用物質が 2. 5万〜 800万国際単位あるいは I FN作用物質が 0. 5〜1
であり、 より好ましくは、 分岐鎖アミノ酸 1 gの場合、 I FN作用物質 が 3万〜 700万国際単位あるいは I FN作用物質が 0. 7〜1 30 / gである。 さらに本発明においては、 分岐鎖アミノ酸及ぴ I FN作用物質に加えて、 R i b作用物質を含有する配合剤の提供も可能である。 この場合、 分岐鎖アミノ酸、 I FN作用物質及び R i b作用物質は、 それぞれ別々の製剤として、 若しくは任 意の 2種と残る 1種の組み合わせで、 同一または異なる投与形態で投与してもよ い。 あるいは、 分岐鎖アミノ酸、 I FN作用物質、 及ぴ R i b作用物質を同時に 含有する医薬である配合剤として投与してもよい。 この場合、 1回投与当たりの R i b作用物質の配合量は、 それぞれ薬剤ごとに設定する。
上記配合剤中の分岐鎖アミノ酸、 I FN作用物質及び R i b作用物質の比率は 分岐鎖ァミノ酸 1 gに対して、 I F N作用物質が 2万〜 20000万国際単位あ るいは I FN作用物質が 0. 4〜4000 μ §であり、 R i b作用物質が 4〜 2 000 Omgであり、 好ましくは、 分岐鎖アミノ酸 1 gに対して、 I FN作用物 質が 2. 5万〜 800万国際単位あるいは I FN作用物質が 0. 5〜 1 60 /x g であり、 R i b作用物質が 5〜 800 m gであり、 より好ましくは、 分岐鎖ァミ ノ酸 1 gに対して、 I FN作用物質が 3万〜 700万国際単位あるいは I FN作 用物質が 0. 7〜1 30 ;u g、 R i b作用物質が 7〜70 Omgである。
分岐鎖アミノ酸および I FN作用物質がそれぞれ別々の製剤である場合、 分岐 鎖アミノ酸と I FN作用物質とは両者が体内において同時に存在するのであれば 投与形態は特に限定されず、 例えば、 分岐鎖アミノ酸を、 I FN作用物質と同時 又は I FN作用物質投与の前後のいずれかに投与してもよい。 併用時の投与方法 及び投与量は、 併用する薬剤の種類や効果によつて適宜決定する。
分岐鎖アミノ酸及び I FN作用物質を、 時間差をおいて投与する場合、 時間差 は投与する有効成分、 剤形、 投与方法により異なるが、 例えば、 分岐鎖アミノ酸 を先に投与する場合、 分岐鎖アミノ酸を投与した後 5分〜 14日以内、 好ましく は 1 0分〜 7日以内に I FN作用物質を投与する方法が挙げられる。 I FN作用
物質を先に投与する場合、 I FN作用物質を投与した後、 5分〜 120時間以内、 好ましくは 10分〜 80時間以内に分岐鎖アミノ酸を投与する方法が挙げられる。 上記本発明で使用する有効成分である分岐鎖アミノ酸の投与量 (摂取量) につ いて算出する際、 本発明で目的とする疾患異常の治療、 予防等の目的で使用され る薬剤の有効成分として前記の算定範囲が決められているので、 これとは別目的 で、 例えば通常の食生活の必要から、 或いは別の疾患の治療目的で、 摂取又は投 与される分岐鎖ァミノ酸についてはこれを前記算定に含める必要はない。
例えば、 通常の食生活から摂取される一日あたりの分岐鎖アミノ酸の量を前記 本発明における有効成分の一日あたりの投与量から控除して算定する必要はなレ、。 <医薬キット〉
また、 本発明の I FN作用物質の C型肝炎ウィルス活性増強剤又は副作用軽減 剤を、 例えば有効量の I FN作用物質製剤、 又は I FN作用物質製剤と R i b作 用物質製剤の両方などとそれぞれ単味剤として組み合わせて、 一体包装されるよ うな医薬用キットとすることができる。
<商業用パッケージ >
また本発明は、 抗 C型肝炎ウィルス用医薬、 及び当該抗 C型肝炎ウィルス用医 薬が抗 C型肝炎ウィルス作用を増強させるのに使用することができること又は使 用すべきであることを記載した、 当該抗 c型肝炎ウィルス用医薬に関する説明を 記載した記載物を含む商業用パッケージを提供する。
実施例
次に、 実施例により本発明をさらに詳細に述べる。 なお、 以下の実施例は、 本 発明を説明するものであつて、 本発明をこれに限定するものではない。
(実施例 1 )
C型慢性肝炎 (CHC) 患者 23例を対象に、 同意を取得したうえで、 I FN α- 2 b (イントロン) とリバビリン (レべトール) の併用治療に、 イソ口イシ ン、 ロイシン、 バリンの重量比が 1 : 2 : 1. 2 (イソロイシン: 0. 952 g、 ロイシン: 1. 904 g、 ノ リン: 1, 144 g) である分岐鎖アミノ酸 (BC
AA) 製剤リーバタト (登録商標) 顆粒 (味の素株式会社) を併用する群 (n = 9) と対照群 (n=14) に割付をおこなった。 I FNa— 2 b 6MUを最初 の 2週は週 6回、 その後は 22週間、 週 3回筋肉内注射した。 リバビリンは体重 により、 毎 060 Omgまたは 80 Omgを朝夕 2回に分けて 24週間経口投与 した。 BCAA製剤併用群は、 BCAAとして 1回 4 gを 1日食後 3回 24週間 経口投与した。 投与 4週、 8週、 12週、 24週目に Am p i i c o r法で HC V— RNAを定性的に測定すると共に、 投与 8週目に全身症状 (発熱) 、 神経- 筋肉症状 (頭痛、 関節痛) の有無を問診により確認した。
経週的な HCV— RNA陰性ィヒ率を表 1に示す。

I F Nひ一 2 b /リバビリン併用治療により H C V— R N A陰性ィ匕率は経週的 に高くなり、 治療終了時の 24週目で 50%の HCV— RNA陰性化率に達した。 I F N α— 2 b /リバピリン併用治療に B C A A製剤を併用すると対照群に比し、 すべての時点で高ぃHCV— RNA陰性化を示し、 24週目で 83%の HCV— RNA陰性化率に達した。
投与 8週目に観察した自他覚症状の頻度を表 2に示す。 表 2, 自他^ ey
表 2により明らかなように、 BCAA製剤を併用することによって発熱、 頭痛 と関節痛の副作用頻度が低下した。
以上の結果より、 C H C患者に対する I F N療法に B C A A製剤を併用するこ とで、 インターフェロンの抗ウィルス効果を増強すると共に副作用を軽減するこ とが判明した。
産業上の利用可能性
本発明の I F N作用物質の抗 C型肝炎ウィルス活性増強剤は、 I F N作用物質 の当該活性を顕著に向上させるものである。 これによつて I F N作用物質の投与 量を減じることも可能である。 また、 本発明における、 副作用軽減剤は、 抗 C型 肝炎ウィルス用としてヒトを含む動物に投与した場合に I F N作用物質による副 作用を顕著に軽減することができる。 これによつて、 I F N作用物質の投与量を 増大することも可能である。
以上のことから、 本発明の活性増強剤及び副作用軽減剤の投与量、 投与スケジ ユールを患者によつて適宜調整することにより、 患者に対してより効果的な抗 C 型肝炎ウィルス効果を発揮させることができる。 即ち、 当該増強剤、 副作用軽減 剤により、 I F N作用物質の投与量を減量し、 あるいは投与期間を短縮すること も可能であり、 副作用などのために適応できなかった患者にも適応を拡大するこ とができる。 また、 副作用の軽減によって、 逆に投与量を増量あるいは投与期間 を延長するような投与形態の選択も可能となるため、 治療方針の幅を広げること も可能である。
また、 I F N作用物質の R i b作用物質による併用治療の場合にも、 本発明の I F N作用物質の抗 C型肝炎ウィルス活性増強剤又は副作用軽減剤を使用するこ とにより、 I F N作用物質ばかりでなく R i b作用物質の副作用をも軽減するこ とができる。 そのため、 R i b作用物質の副作用のために治療を断念又は中断せ ざるを得なかった患者にも適応を広げることができる。
I F N作用物質や R i b作用物質がウィルス性肝炎に代表されるウィルス性疾 患の重要な治療剤でありながら、 副作用や著効率の点で必ずしも満足のいくもの ではなかった現状において、 本発明は I F N作用物質治療成績をさらに向上させ る上で本発明は極めて有用である。
以上、 本発明の具体的な態様のいくつかを詳細に説明したが、 当業者であれば 示された特定の態様には、 本発明の教示と利点から実質的に逸脱しない範囲で様 々な修正と変更をなすことは可能である。 従って、 そのような修正及び変更も、 すべて後記の請求の範囲で請求される本発明の精神と範囲内に含まれるものであ る。
本出願は、 日本で出願された特願 2 0 0 5— 2 1 7 8 3 3 (出願日 : 2 0 0 5 年 7月 2 7日) を基礎としており、 その内容は本明細書に全て包含されるもので める。
Claims
1 . イソロイシン、 ロイシン、 及びバリンから選ばれる少なくとも 1種の分岐鎖 ァミノ酸またはその塩を含むことを特徴とする、 インターフエ口ン作用物質の抗 c型肝炎ウィルス活性増強剤。
2 . インターフェロン作用物質がインターフェロンである、 請求項 1に記載の活 性増強剤。
3 . イソロイシン、 ロイシン、 及ぴバリンから選ばれる少なくとも 1種の分岐鎮 ァミノ酸またはその塩を含むことを特徴とする、 抗 C型肝炎ウィルス用のィンタ 一フエ口ン作用物質治療における副作用軽減剤。
4 , インターフエ口ン作用物質治療がさらにリバビリン作用物質を組み合わせて なるものである、 請求項 3に記載の副作用軽減剤。
5 . イソロイシン、 ロイシン、 及びバリンから選ばれる少なくとも 1種の分岐鎖 アミノ酸またはその塩とインターフェロン作用物質を組み合わせてなる、 抗 C型 肝炎ウィルス用医薬。
6 . さらにリバビリン作用物質を含むことを特徴とする、 請求項 5に記載の医薬。
7 . インターフェロン作用物質の抗 C型肝炎ウィルス活性増強剤の製造のための、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖ァミ ノ酸またはその塩の使用。
8 . インターフェロン作用物質がインターフェロンである、 請求項 7記載の使用。
9 . 抗 C型肝炎ウィルス用のインターフェロン作用物質治療における副作用軽減 剤の製造のための、 イソロイシン、 ロイシン、 及ぴバリンから選ばれる少なくと も 1種の分岐鎖ァミノ酸またはその塩の使用。
1 0 . インターフエ口ン作用物質治療がさらにリバピリン作用物質を組み合わせ てなるものである、 請求項 9記載の使用。
1 1 . 抗 C型肝炎ウィルス用医薬の製造のための、 イソロイシン、 ロイシン、 及 びバリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸またはその塩及ぴィンタ 一フエ口ン作用物質の使用。
1 2. さらにリバビリン作用物質を含むことを特徴とする、 請求項 1 1記載の使 用。
1 3. 有効量の、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖アミノ酸またはその塩を投与することからなる、 インターフェロン 作用物質の抗 C型肝炎ウィルス活性を増強させる方法。
14. インターフェロン作用物質がインターフェロンである、 請求項 1 3記載の 方法。
1 5. 有効量の、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸またはその塩を投与することからなる、 抗 C型肝炎ウィル ス用のィンターフェロン作用物質治療における副作用を軽減させる方法。
1 6. インターフェロン作用物質治療がさらにリバビリン作用物質を組み合わせ てなるものである、 請求項 1 5記載の方法。
1 7. 有効量の、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸またはその塩とィンターフェ口ン作用物質を投与すること からなる、 C型肝炎ウィルス感染を治療する方法。
18. さらにリバビリン作用物質を含むことを特徴とする、 請求項 1 7記載の方 法。
1 9. 有効量の、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸またはその塩および薬理学上許容される担体を含むことを 特徴とする、 インターフェロン作用物質の抗 C型肝炎ウィルス活性増強用組成物。
20. インターフェロン作用物質がインターフェロンである、 請求項 1 9記載の 組成物。
2 1. 有効量の、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸またはその塩および薬理学上許容される担体を含むことを 特徴とする、 抗 C型肝炎ウィルス用のインターフェロン作用物質治療における副 作用軽減用組成物。
2 2 . インターフエ口ン作用物質治療がさらにリノ ビリン作用物質を組み合わせ てなるものである、 請求項 2 1記載の組成物。
2 3 . 有効量の、 イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐鎖ァミノ酸またはその塩とインターフェロン作用物質を組み合わせて なる、 抗 C型肝炎ウィルス用医薬組成物。
2 4 . さらにリバビリン作用物質を含むことを特徴とする、 請求項 2 3記載の医 薬組成物。
2 5 . イソロイシン、 ロイシン、 及ぴバリンから選ばれる少なくとも 1種の分岐 鎖アミノ酸またはその塩及ぴインターフェロン作用物質をそれぞれ単味剤として 組み合わせたことを特徴とする、 抗 C型肝炎ウィルス用医薬キット。
2 6 . イソロイシン、 ロイシン、 及びパリンから選ばれる少なくとも 1種の分岐 鎖アミノ酸またはその塩、 インターフェロン作用物質及びリバピリン作用物質を それぞれ単味剤として組み合わせたことを特徴とする、 抗 C型肝炎ウィルス用医 薬キット。
2 7 . 請求項 5または 6に記載の抗 C型肝炎ウィルス用医薬、 及び当該抗 C型肝 炎ウィルス用医薬が抗 C型肝炎ウィルス作用を増強させるのに使用することがで きること又は使用すべきであることを記載した、 当該抗 C型肝炎ウィルス用医薬 に関する説明を記載した記載物を含む商業用パッケージ。
