血液透析装置及び血液透析方法

[0001] 本発明は、患者の血液を体外循環させつつ血液透析処理及び除水を行うための 血液透析装置及び血液透析方法に関するものである。
背景技術
[0002] 一般に、透析治療においては、患者の血液を体外循環させるための血液回路と、 該血液回路の途中に接続されたダイァライザと、しごき型の血液ポンプと、ダイァライ ザとの間で透析液の導入又は導出を行って血液透析処理を行いつつ除水可能な透 析装置本体とを有した血液透析装置が用いられる。尚、血液回路は、先端に動脈側 穿刺針が取り付けられる動脈側血液回路と、先端に静脈側穿刺針が取り付けられる 静脈側血液回路とから主に構成されて 、る。
[0003] そして、動脈側及び静脈側の穿刺針を患者に穿刺した状態で血液ポンプを駆動さ せると、動脈側穿刺針から患者の血液が採取され、その血液が、動脈側血液回路、 ダイァライザ及び静脈側血液回路を経て、静脈側穿刺針力 患者の体内に戻るよう になっている。力かるダイァライザの内部には、血液浄化膜としての複数の中空糸が 配設されており、それぞれの中空糸の内部に血液を流通させるとともに、当該中空糸 の外部 (即ち、中空糸の外周面と筐体の内周面との間)に透析装置本体から供給さ れる所定濃度の透析液を流通させ、中空糸内部を通過する血液の老廃物等が血液 浄化膜を透過して透析液内に排出されるようになって!/ヽる。
[0004] これにより、老廃物等が排出されて清浄化された血液が静脈側血液回路を介して 患者の体内に戻るようになつている。一方、透析装置本体内には、患者の血液から 水分を取り除くための除水ポンプが配設されており、透析治療時に血液浄ィ匕膜を介 して除水が行われるように構成されて 、る。力かる除水ポンプによる除水時にお!、て 除去すべき水分量 (除水速度)は、除水ポンプの駆動を制御することにより行われる

(例えば循環血液量変化率 A BV)などの指標、透析治療前の患者の体重、或いは 透析治療時の体重減少量などを医師等の医療従事者が経験的に関連付けることに より算出されている。尚、力かる先行技術は、文献公知発明に係るものでないため、 記載すべき先行技術文献情報はな 、。
発明の開示
発明が解決しょうとする課題
[0006] し力しながら、上記従来の血液透析装置においては、医療従事者の経験的な関連 付けに基づ 、て算出されるドライウェイトに基づ 、て除水量の設定が行われて 、るの で、体格や血液指標等が元来異なる個々の患者に常に正確であるとは限らな 、と 、 う問題があった。即ち、患者の体重が正確なドライウェイトとなるまで除水すれば理想 的なのであるが、力かるドライウェイトの算出が医療従事者の知識や経験等によるとこ ろが多ぐ場合によっては不適切なドライウェイトを設定してしまう虞があるのである。
[0007] 本発明は、このような事情に鑑みてなされたもので、透析治療中における患者の所 定の血液指標と体重とを関連付けするとともに、患者のドライウェイトと相関して、多く の患者に適用できる標準化されたパラメータを算出し、適正な除水を行わせることが できる血液透析装置及び血液透析方法を提供することにある。
課題を解決するための手段

発明を実施するための最良の形態
[0034] 以下、本発明の実施形態について図面を参照しながら具体的に説明する。
本実施形態に係る血液透析装置は、患者の血液を体外循環させつつ血液透析処 理及び除水を行うためのもので、図 1に示すように、患者の血液を体外循環させるた めの血液回路 1と、該血液回路 1に接続されて血液透析処理を行うためのダイァライ ザ 2と、該ダイァライザ 2に接続されて透析液を供給しつつ除水する透析装置本体 6 とから主に構成されている。血液回路 1は、同図に示すように、可撓性チューブから 成る動脈側血液回路 la及び静脈側血液回路 lbから主に構成されており、これら動 脈側血液回路 laと静脈側血液回路 lbの間にダイァライザ 2が接続されている。
[0035] 動脈側血液回路 laには、その先端に動脈側穿刺針 aが接続されているとともに、途 中にしごき型の血液ポンプ 3、及びへマトクリットセンサ 5が配設されている。一方、静 脈側血液回路 lbには、その先端に静脈側穿刺針 bが接続されているとともに、途中 に除泡用のドリップチャンバ 4が接続されている。
[0036] へマトクリットセンサ 5は、例えば LED等の発光素子及びフォトダイオード等の受光 素子を備え、発光素子から血液に所定波長の光を照射するとともに、その透過した 光或いは反射した光を受光素子にて受光することにより、血液回路 1内を流れる患者 の血液の濃度を示すへマトクリット値を測定するものである。即ち、へマトクリット値と は、血液の濃度を示す指標であり、具体的には、全血に占める赤血球の容積率で表 されるちのである。
[0037] そして、動脈側穿刺針 a及び静脈側穿刺針 bを患者に穿刺した状態で、血液ポンプ 3を駆動させると、患者の血液は、動脈側血液回路 laを通ってダイァライザ 2に至り、
該ダイァライザ 2によって血液浄ィ匕が施され、ドリップチャンバ 4で除泡がなされつつ 静脈側血液回路 lbを通って患者の体内に戻る。即ち、患者の血液を血液回路 1に て体外循環させつつダイァライザ 2にて浄ィ匕するのである。
[0038] ダイァライザ 2は、その筐体部に、血液導入ポート 2a、血液導出ポート 2b、透析液 導入ポート 2c及び透析液導出ポート 2dが形成されており、このうち血液導入ポート 2 aには動脈側血液回路 laの基端が、血液導出ポート 2bには静脈側血液回路 lbの基 端がそれぞれ接続されている。また、透析液導入ポート 2c及び透析液導出ポート 2d は、透析装置本体 6から延設された透析液導入ライン L1及び透析液排出ライン L2と それぞれ接続されている。
[0039] ダイァライザ 2内には、複数の中空糸が収容されており、該中空糸内部が血液の流 路とされるとともに、中空糸外周面と筐体部の内周面との間が透析液の流路とされて いる。中空糸には、その外周面と内周面とを貫通した微少な孔 (ポア)が多数形成さ れて中空糸膜を形成しており、該膜を介して血液中の不純物等が透析液内に透過し 得るよう構成されている。
[0040] 一方、透析装置本体 6は、図 2に示すように、透析液導入ライン L1及び透析液排出 ライン L2に跨って形成された複式ポンプ Pと、透析液排出ライン L2にお 、て複式ポ ンプ Pを迂回して接続されたバイノ スライン L3と、該バイノ スライン L3に接続された 除水ポンプ 8とから主に構成されている。そして、透析液導入ライン L1の一端がダイ ァライザ 2 (透析液導入ポート 2c)に接続されるとともに、他端が所定濃度の透析液を 調製する透析液供給装置 7に接続されて ヽる。
[0041] また、透析液排出ライン L2の一端は、ダイァライザ 2 (透析液導出ポート 2d)に接続 されるととも〖こ、他端が図示しない廃液手段と接続されており、透析液供給装置 7から 供給された透析液が透析液導入ライン L1を通ってダイァライザ 2に至った後、透析 液排出ライン L2及びバイパスライン L3を通って廃液手段に送られるようになって ヽる
[0042] 除水ポンプ 8は、ダイァライザ 2中を流れる患者の血液力 水分を除去するためのも のである。即ち、力かる除水ポンプ 8を駆動させると、複式ポンプ Pが定量型であるた め、透析液導入ライン L1から導入される透析液量よりも透析液排出ライン L2から排
出される液体の容量が多くなり、その多い容量分だけ血液中から水分が除去される のである。尚、力かる除水ポンプ 8以外の手段 (例えば所謂バランシングチャンバ等 を利用するもの)にて患者の血液力も水分を除去するようにしてもよい。尚、複式ボン プ P及び除水ポンプ 9は、体外循環する患者の血液に対し血液透析処理及び除水を 行うための透析処理手段を構成する。
[0043] 一方、透析装置本体 6には、図 3に示すように、透析治療前の患者の体重を入力す るための入力手段 9と、測定手段 10と、算出手段 11と、透析装置本体 6が具備する ディスプレイ等から成る表示手段 12と、音声出力可能なスピーカ等から成る報知手 段 13と、記憶手段 14と、制御手段 15と、除水ポンプ 8の駆動量に基づき除水量を計 測するための除水量計測手段 16とが配設されている。
[0044] 測定手段 10は、透析治療中の患者の体重変化率、及び所定の血液指標としての 循環血漿量変化率をそれぞれ測定するためのもので、入力手段 9、ポンプ駆動制御 手段 16及びへマトクリットセンサ 5と電気的に接続されている。具体的には、入力手 段 9により入力された透析治療前の患者の体重 (BW1)と、除水ポンプ 8の駆動量に 基づき除水量計測手段 16にて把握される除水量 (測定時点での除水積算値: UFV )とを用い、体重変化率 A BW%が以下の式 1にて求められる。

[0050] 上記の式(1)及び(2)により、測定手段 10にて患者の体重変化率( ABW%)、及 び循環血漿量変化率( Δ CPV%)が測定されることとなり、これら測定値が算出手段 11に送られ、パラメータ (PWI) (後述する)を求めるための所定の演算が行われるこ ととなる。
[0051] 力かる算出手段 11は、患者のドライウェイトと相関するとともに、測定手段 10で得ら れた体重変化率と循環血漿量変化率 (血液指標の変化率)とを関連付けするパラメ ータを透析治療中に連続的に算出し得るもので、算出すべきパラメータとして、例え ば患者に対する除水による体重変化 (減少)が血液濃度に対してどの程度影響する かの指数である PWIを用いることができる。ここで、算出手段 11による PWIの算出は 、透析治療中に連続的に行われるとしているが、この「連続的」とは、透析治療開始 力 終了まで複数回行われることを意味し、所定間隔を有しつつ算出するものも含む 。尚、算出手段 11による PWIの算出は、透析治療中において 1回のみ行われるもの とすることもできる。例えば透析終了時点のみ 1回 PWIを算出し、透析治療によってド ライウェイトに到達した力否かをリアルタイムに確認することが可能となる。
[0052] 一方、 PWIは、循環血漿量変化率( Δ CPV%)を患者の体重変化率( Δ BW%)で 除する演算式 (PWI= ACPV%ZABW%)にて算出されるもので、透析治療によ つて患者がドライウェイトに到達したときには、適正範囲内にあることが検証の結果明 らカとされている。然るに、 PWIが大きいと除水による体重減少に対する血液濃縮率
が大きいことを示し、除水により血液中力 水分を奪っている割に血管外への間質液 の補充がなされていないことが認識できる一方、 PWIが小さいと、血液から水分を奪 つても間質液の補充に余裕があることが認識できる。尚、 PWIの適正値は、透析時間 経過とともに変化する可能性があり、或いは透析終了時の PWIの適正値も透析条件 によって変化し得るものである。
[0053] 表示手段 12は、例えば透析装置本体 6が具備するディスプレイ (不図示)などから 成り、算出手段 11にて算出されたパラメータ (PWI)を表示するとともに、その経時的 変化をグラフ (例えば折れ線グラフ等)として表示するものである。これにより、除水が 適正に行われている力否かをリアルタイムに医師等の医療従事者に認識させること ができ、透析治療中における除水の適正化を良好に行わせることができる。

[0055] また更に、透析治療に先立ち、目標除水量 (UFV)の設定、入力手段 9に対する透 析治療前の体重入力、透析治療終了時のパラメータ (PWI)の適正範囲(2〜5)の設 定を行うと、血液指標の測定開始により Htlが測定された時点で、治療終了時点に おける Ht2を逆算して予想することができる。これにより、血液指標変化の観点から、 患者に無理のない透析治療が可能力否力を事前に把握することができ、より適正な 透析治療を行うことができる。
[0056] 報知手段 13は、算出手段 11にて算出されたパラメータ (PWI)が適正範囲を逸脱 した場合、周囲の医療従事者等に報知するためのものであり、例えば音声等を出力 し得るスピーカや光を照射し得る光源 (LEDなど)力ら成るものである。当該適正範 囲は、例えば透析装置本体 6の入力部力 予め入力して設定されるべきものであり、 透析治療の過程において、目標ドライウェイトに対する各測定時点での理想的なパラ メータ(PWI)とされる範囲である。
[0057] 制御手段 15は、算出手段 11にて算出されたパラメータ (PWI)が上述の如き適正
範囲を逸脱した場合、当該適正範囲内となるよう透析処理手段 (本実施形態におい ては、除水ポンプ 8)を制御するものである。より具体的には、算出されたパラメータ( PWI)が適正範囲を逸脱した際、当該パラメータ (PWI)が適正範囲内に到達して透 析治療が終了するように、除水ポンプ 8を制御して除水速度を調整するよう構成され ているのである。
[0058] 記憶手段 14は、算出手段 11にて算出されたパラメータ (PWI)を記憶するためのも ので、例えば透析装置本体 6内に配設されたメモリ等力 成る。かかる記憶手段 14で 記憶されたパラメータ (PWI)又はその経時的変化は、他の透析治療中(同一患者の 次回以降の透析治療時、又は他の患者の透析治療時)又はその透析治療前後に表 示させ得るようになっており、例えば、同一患者の次回及びそれ以降の透析治療中 に表示させれば、特定の患者の中 ·長期に亘る治療傾向や現況を把握することがで き、他の患者の透析治療中に表示させれば、患者間での比較を行うことができる。ま た、他の透析治療前後 (例えば穿刺針の穿刺前、患者が透析装置近傍 (透析治療の 際に横になるベッドなど)で待機しているときや、穿刺針の抜針後、患者が透析装置 近傍 (透析治療の際に横になつていたベッドなど)を離れるまで)に表示させれば、当 該時点において過去の経過とそのときの治療結果との比較を示して医師等医療従事 者が患者に説明することができる。
[0059] また、同一患者の透析治療中にぉ 、て、記憶手段 14にて記憶されたパラメータ (P WI)と現在算出手段 11にて算出されたパラメータ (PWI)とを表示手段 12に併せて 表示すれば、パラメータ (PWI)の適正範囲に対する現在の状況を医療従事者が把 握することができ、透析治療をより確実に行わせることができる。
[0060] 更に、把握された現在の状況に基づき、理想的な治療方針を案内するガイダンス 機能を具備してもよい。この場合、今回の透析治療終了時に、パラメータ (PWI)が適 正範囲より低い場合、次回の透析治療中における除水量を増力 tlさせるようなガイダン スを行わせるようにするのが好ましい。また更に、記憶手段 14にて記憶されたデータ を、例えばネットワーク等を介して透析装置本体 6の外部端末に送信し、外部端末で も記憶、表示、比較できるようにしてもよぐこの場合、患者のデータを容易に共有ィ匕 することができ、共有ィ匕した患者のデータをデータベース化して一括管理することが
できる。
[0061] 上記実施形態によれば、患者のドライウェイトと相関するとともに、体重変化率と血 液指標の変化率とを関連付けするパラメータ (具体的には、 PWI)を、透析治療中に リアルタイムに算出するので、力かるパラメータを手計算等で算出させることが不要で ある。また、算出されたパラメータ(PWI)をリアルタイムに表示させているので、透析 治療中の患者の状態や治療条件の適否の把握、推移から以降の状態の予測、及び 治療結果 (効果)の確認が即座に可能となる。
[0062] また、パラメータ (PWI)を算出するための指標が全て既存の血液透析装置に配設 された入力手段 9やセンサ(へマトクリットセンサ等)により得られるとともに、そのパラメ ータ (PWI)を表示させる表示手段等も既存の血液透析装置に配設されたディスプレ ィ等を利用できるので、既存の血液透析装置への適用が容易であり、製造コストを抑 ff¾することができる。
[0063] 以上、本実施形態について説明した力 本発明はこれに限定されるものではなぐ 例えば患者のドライウェイトと相関するとともに、体重変化率と血液指標の変化率 (循 環血漿量変化率には限らない)とを関連付けするパラメータを透析治療中に算出す るよう構成するものであれば、 PWIとは異なる他のパラメータを算出するようにしてもよ い。
[0064] また、血液指標の変化率として循環血漿量変化率を用いてパラメータを算出するに あたり、へマトクリット値に代えて他の血液指標(例えばヘモグロビン濃度、血清総蛋 白濃度など)を用いるようにしてもよい。ここで、ヘモグロビンは、赤血球中の色素のこ とをいうので、ヘモグロビン濃度はへマトクリット値と相関性がある。また、透析におい て蛋白質がダイァライザの透析液側に一部漏出する場合もある力 その漏出量は測 定誤差範囲内にあると考えられており、血液中の血清総蛋白濃度を用いても血液指 標としての循環血漿量変化率を測定することができる。尚、これらヘモグロビン濃度 ゃ蛋白濃度は、光学的或いは超音波等を利用して測定可能である。
[0065] また、体重変化率や血液指標の変化率に影響を与える外乱要素 (例えば、透析液 濃度の変更、血液温度変更、血流量変更、補液 '高ナトリウム液'その他の薬剤投与 などの血液側への影響要素、飲食、排泄、補液などの体重側への影響要素)に関し
て、外乱要素があったことの入力又は記憶、外乱要素の検知を報知、或いはパラメ ータ算出時の中断や補正などを行うようにしてもよ!、。
産業上の利用可能性
[0066] 患者のドライウェイトと相関するとともに、体重変化率と血液指標の変化率とを関連 付けするパラメータを透析治療中に算出する血液透析装置及び血液透析方法であ れば、別途の機能が付加されたもの不随機能がないもの等、他の形態の血液透析 装置及び血液透析方法に適用することができる。

Claims
[1] 体外循環する患者の血液に対して血液透析処理及び除水して透析治療を行う透 析処理手段と、
該透析処理手段による透析治療中の患者の体重変化率、及び所定の血液指標の 変化率を測定する測定手段と、
患者のドライウェイトと相関するとともに、前記測定手段で得られた体重変化率と血 液指標の変化率とを関連付けするパラメータを透析治療中に算出する算出手段と、 を具備したことを特徴とする血液透析装置。
[2] 前記パラメータは、透析治療前の患者の体重及び除水量に基づいて得られる体重 変化率と、体外循環する血液の濃度に基づいて得られる血液指標の変化率とから算 出されることを特徴とする請求項 1記載の血液透析装置。
[3] 前記血液の濃度から体外循環する血液の循環血漿量変化率を算出し、該循環血 漿量変化率を前記血液指標の変化率とすることを特徴とする請求項 2記載の血液透 析装置。
[4] 前記算出手段で算出されたパラメータを表示する表示手段を具備したことを特徴と する請求項 1〜請求項 3の何れか 1つに記載の血液透析装置。
[5] 前記表示手段は、前記算出手段で算出されたパラメータの経時的変化をグラフとし て表示することを特徴とする請求項 4記載の血液透析装置。
[6] 前記パラメータの適正範囲を設定するとともに、前記算出手段にて算出されたパラ メータが適正範囲を逸脱した場合、報知する報知手段を具備したことを特徴とする請 求項 1〜請求項 5の何れか 1つに記載の血液透析装置。
[7] 前記パラメータの適正範囲を設定するとともに、前記算出手段にて算出されたパラ メータが適正範囲を逸脱した場合、当該適正範囲内となるよう前記透析処理手段を 制御するための制御手段を具備したことを特徴とする請求項 1〜請求項 6の何れ力 1 つに記載の血液透析装置。
[8] 前記算出手段にて算出されたパラメータを記憶する記憶手段を具備するとともに、 該記憶手段で記憶されたパラメータ又はその経時的変化を他の透析治療中又はそ の透析治療前後に表示させ得ることを特徴とする請求項 1〜請求項 7の何れか 1つに
記載の血液透析装置。
[9] 前記記憶手段は、外部端末と接続されて成り、前記算出手段にて算出されたパラメ ータを当該外部端末に送信し得ることを特徴とする請求項 8記載の血液透析装置。
[10] 体外循環する患者の血液に対して血液透析処理及び除水を行って透析治療しつ つ、透析治療中の患者の体重変化率、及び所定の血液指標の変化率を測定した後 、患者のドライウェイトと相関するとともに、測定された前記体重変化率と血液指標の 変化率とを関連付けするパラメータを透析治療中に算出することを特徴とする血液透 析方法。
[11] 前記パラメータは、透析治療前の患者の体重及び除水量に基づいて得られる体重 変化率と、体外循環する血液の濃度に基づいて得られる血液指標の変化率とから算 出されることを特徴とする請求項 10記載の血液透析方法。
[12] 前記血液の濃度から体外循環する血液の循環血漿量変化率を算出し、該循環血 漿量変化率を前記血液指標の変化率とすることを特徴とする請求項 11記載の血液 透析方法。
[13] 前記算出されたパラメータを表示することを特徴とする請求項 10〜請求項 12の何 れか 1つに記載の血液透析方法。
[14] 前記パラメータの経時的変化をグラフとして表示することを特徴とする請求項 13記 載の血液透析方法。
[15] 前記パラメータの適正範囲を設定するとともに、算出されたパラメータが適正範囲を 逸脱した場合、報知することを特徴とする請求項 10〜請求項 14の何れか 1つに記載 の血液透析方法。
[16] 前記パラメータの適正範囲を設定するとともに、算出されたパラメータが適正範囲を 逸脱した場合、当該適正範囲内となるよう透析処理手段又は除水の制御を行うことを 特徴とする請求項 10〜請求項 15の何れか 1つに記載の血液透析方法。
[17] 前記算出されたパラメータを記憶するとともに、該記憶されたパラメータ又はその経 時的変化を他の透析治療中又はその透析治療前後に表示させ得ることを特徴とする 請求項 10〜請求項 16の何れか 1つに記載の血液透析方法。
[18] 前記記憶されたパラメータを外部端末に送信し得ることを特徴とする請求項 17記載
の血液透析方法。
