Definitions
- the invention relates to a device for inserting deformable intraocular lenses, with which an intraocular lens in an elastically deformed state can be injected into an eye through a cannula by means of a plunger which can be moved translationally in the device.
- Devices for inserting deformable intraocular lenses are known. Their main purpose is generally to be able to keep the incision in the eye necessary for the insertion of an intraocular lens as small as possible.
- One difficulty with such devices is to bring the intraocular lens into an elastically deformed state such that it can be injected into an eye through a cannula in this state.
- the patent US4681102 shows such a device.
- a lens receptacle has a hinge which allows the lens receptacle to be brought from an open state in which the lens is inserted to a closed state in which the lens is folded up.
- the hinge in the middle of the lens holder initially hinders the deformation of the lens and it can even happen that the lens dodges at the beginning of the deformation process and is bent opposite to the intended direction of deformation.
- the device according to US5947975 brings an improvement in this regard in that the lens holder has two hinges. Both of the devices mentioned above have the disadvantage that it is cumbersome to place the intraocular lens in the lens holder.
- the invention is based on the objects of proposing a device of the type mentioned at the outset, in which the intraocular lens can be easily and safely brought into the deformed state and in this state into the device.
- the device is characterized in that it contains a lens holder that can be inserted into the device, that the lens holder contains an elastic base that is deformable in such a way that it starts from a relaxed, open position by bending into a tensioned one closed position is deformable, the intraocular lens in contact with the lens holder being subjected to an increasing curvature during the deformation.
- the plunger has at its free end a recess which is essentially transverse to the plunger cross section. This is designed to receive an edge of the intraocular lens so that it can be pushed securely into and through the cannula.
- the device has an externally open bearing part for the lens holder.
- the advantage of the bearing part is that it can accommodate the lens holder, but also other parts that serve to precisely position and guide the intraocular lens and the plunger.
- These further parts can include, for example, an alignment device for the plunger, which in turn can contain a guide element that bears against the plunger.
- This alignment device can be detachably or integrally connected to the bearing part.
- the cannula can also advantageously be connected in one piece to the bearing part. If, according to a further embodiment, the lens holder does not protrude from the bearing part, this prevents the lens holder, once inserted, from being removed from the device again. This is desirable for single use devices.
- the elastic base advantageously forms a channel in which the curved intraocular lens is located. According to one embodiment, this channel narrows towards one side of the device, as a result of which the intraocular lens is further compressed as it is advanced towards the cannula.
- the channel has a screw-like cross section at its end facing the cannula, larger intraocular lenses can be deformed in such a way that their opposite edges overlap. The intraocular lens is thus rolled up in the device.
- this screw-like cross section can also be present in the through opening of the bearing part.
- the elastic base has a taper on its side facing the plunger in order to form a guide for the plunger.
- This taper forms a ramp-like guide surface for the ram.
- a guide surface can also be present on the alignment device.
- connecting means are present on the lens holder in order to keep the lens holder in its closed position.
- Latching means can also be present in order to position and hold the lens holder in the device.
- FIG. 1 shows a section of a device for inserting deformable intraocular lenses with a lens holder 1 inserted in the device.
- the device has an elongated housing 2, in which an elongated opening 3 is provided, which is used to insert the lens holder 1
- the lens holder 1 with an intraocular lens received therein in the deformed state is inserted through the opening 3 into the device. Then the intraocular lens is pushed through a plunger 6 out of the lens holder 1 into a cannula 5. The cannula 5 is then inserted through a small incision in the eye of a patient and the intraocular lens is pushed out of the cannula 5 into the eye by the plunger 6.
- the lens holder 1 contains an elastic base 8, which is essentially flat in the relaxed, open position. A trough 12 arranged in the elastic base 8 can facilitate the placement of the intraocular lens.
- FIGS. 1 If an intraocular lens has been placed in the lens holder 1, it is deformed together with the intraocular lens resting on the elastic base 8 until it assumes the closed position shown in the figures. In this position, the lens receptacle 1 forms a channel 18 running in the longitudinal direction of the device, in which the vibrated intraocular lens is initially located and from which it is transported through the plunger 6 into a passage opening 28 present in the cannula 5. So that the lens holder 1 remains in the closed position until it is inserted into the device, connecting means are provided which, in the present example, consist of pins 15 which are present on an edge region 10 (FIG. 2) of the lens holder 1 and which engage in openings 16 which engage are arranged in the opposite edge region 9 (FIG. 2). FIGS.
- the lens holder 1 shows further details of embodiments of the lens holder 1.
- the edge regions 9 and 10 mentioned are arranged on both sides of the elastic base 8.
- the edge areas 9, 10 are thicker and therefore stiffer than the elastic base 8.
- Known intraocular lenses have two so-called haptics. These are small brackets that center the lens in the lens pocket in the eye.
- the lens holder 1 advantageously has in the area
- one of the undercuts 11 has an extension 19 (FIG. 2), which receives a haptic and prevents it from later getting caught on the lens receptacle 1 when the intraocular lens is moved by the plunger 6.
- the lens holder 1 is held in a bearing 4, the latter being able to be connected in one piece to the cannula 5 as shown.
- Latching means for example in the form of a rib 20 (FIGS. 2 and 3), are arranged on at least one side of the closed lens receptacle and are intended to precisely position and lock the lens receptacle 1 in the housing 2 or in the bearing part 4.
- An alignment device 7 ensures that the plunger 6 assumes a defined position within the device.
- the plunger 6 is received in a through opening 28 of the alignment device 7 and is held in the position shown in FIG. 1 by a guide element 27.
- the alignment device 7 is connected to the bearing part 4 by a kind of plug connection.
- the alignment device 7 can also be connected in one piece to the bearing part 4.
- a guide surface 14 has the purpose of guiding the plunger 6 in such a way that it hits the exact edge of the deformed intraocular lens when it is to be pushed out of the lens holder 1 into the passage opening 28 of the cannula 5.
- the guide surface 14 can be designed as a taper on one side of the elastic base 8.
- the guide surface can also be arranged on the bearing part 4.
- Means are also provided on the plunger 6 itself, which improve the accuracy of the detection of the deformed intraocular lens. These means have the shape of a recess 25 which extends over the end of the plunger 6.
- the depression 25 preferably runs in an arcuate manner, the radius of the arc corresponding to the bending radius of the deformed intraocular lens.
- FIG. 3 shows a special embodiment of the lens holder 1, which is provided specifically for injecting relatively large intraocular lenses. This differs from that according to FIG. 2 by an elevation 21 which is arranged in the edge region 9 adjacent to the flexible base 8. A corresponding recess is provided in the opposite edge region 10, in which the elevation 21 is accommodated in the closed position of the lens holder 1. It can be clearly seen in FIG. 3 that the channel 18 on the outlet side is given a helical cross section by the elevation 21. A gap 24 is present between the elevation 21 and a guide part 23, which allows a large intraocular lens to be deformed such that its opposite edges overlap, as a result of which the intraocular lens leaves the lens holder 1 m in a rolled-up state. Alternatively, the shape of the screw can also be. Through opening 28 may be formed; which extends from the bearing part 4> through the cannula 5. ⁇
