- Surgical superficial veins include two main procedures, both of which perform surgical removal of the malignant vein (vein removal).
- the first intervention is the "stripping", which is a tearing of the varicose axis of the internal or external saphenous wire.
- This intervention is performed under general, loco-regional, epidural or neuroleptanalgesia anesthesia. It is a source of significant dilapidation of the collateral branches of the saphenous vein which, therefore, will bleed causing hematomas, sources of pain and discomfort for patients in the hours and days following surgery.
- the other commonly used technique is ambulatory phlebectomy, which is a less traumatic surgical procedure than stripping (but which can also be performed as part of a stripping). This technique performs a careful removal of the superficial veins under local anesthesia on an outpatient basis.
- Hematomas are less important than in the case of stripping, but the risks of immediate bleeding exist when the patient stands up - risks linked to the ambulatory method. In both cases, to reduce bleeding and reduce its consequences, the surgeon immediately applies very strong compression on the operated limb after the operation, to limit bleeding and post-operative inflammation. In almost all cases, this compression is currently carried out with dry or adhesive tapes.
- One of the aims of the invention is to propose an orthosis which, put in place immediately after the end of the operation, provides controlled, regular compression over the entire lower limb, not dependent on the operator and sufficiently strong to limit bleeding and post-operative inflammation.
- Bandages, currently the most used, are the easiest orthosis to use but, as we have seen, the least suitable for the purposes pursued by compression.
- orthosis Another existing type of orthosis could be used instead, namely medical compression stockings. But these stockings give a degressive, ineffective pressure to the thigh whereas, in post-operative, it is precisely on this area that a strong pressure must apply. In addition, these stockings are very difficult to put on an unconscious patient because of general anesthesia and also because of the bandages placed on the leg, which increase the volume.
- the orthosis of the invention overcomes all of these drawbacks.
- the invention provides a tubular compressive orthosis for the compression of a lower limb after a venous surgery intervention, in particular after venous or ambulatory phlebectomy, which includes a compressive tubular part, in mesh or fabric, with variable section conformed to the profile of the lower limb, and the structure of the mesh or fabric of which is chosen, as a function of the perimeter dimensions of the lower limb, so as to apply thereto a substantially constant compression pressure over the height of the limb.
- - the substantially constant compression pressure is between 30 and 40 mm Hg;
- the orthosis is in the form of an open stocking, without a knitted foot and heel;
- the tubular part extends from the foot to the root of the thigh;
- the tubular part comprises an extensible internal region with high elasticity, connected along two generatrices, for example approximately diametrically opposite, to an external region which is weakly extensible or inextensible;
- the tubular part comprises, in the upper part, a thigh sleeve open along a generator and provided with adjustable closing means;
- the upper and / or lower ends of the orthosis are made of a breakable and non-scalable material, so as to allow adjustment by cutting the length of the orthosis at the time of its installation.

- Figure 1 shows the orthosis of the invention, which essentially has the shape of a bottom open to the foot. More specifically, the orthosis 10 is in the form of a knitted tube of variable section adapted to the morphology of the leg, that is to say decreasing from top to bottom, and before- conspicuously devoid of knitted foot and heel.
- This orthosis is intended to cover the lower limb, as illustrated in Figures 2 and 3, from the foot to the root of the thigh. It must be able to exert a constant compression pressure from the ankle to the thigh on the lower limb (unlike traditional compression stockings, which produce a decreasing pressure from the ankle), of the order of 30 to 40 mm Hg (40.0 to 53.2 hPa), allowing to exert a regular and constant hemostatic effect throughout the lower limb.
- the tubular orthosis 10 consists of two substantially semi-cylindrical parts 12, 14 extending over the entire length of the orthosis, and connected along two generatrices 16, 18 located on either side of a diameter 20. On the semi-cylindrical part 12 situated on the internal side of the leg, the tubular orthosis consists of a reinforced elastic mesh, for example of the same type as that produced on the rear face of a multi-mesh panty with tights.
- the semi-cylindrical part 14 located on the external side of the leg, for its part, is knitted with a slightly stretchy mesh, that is to say with short elongation, so as to apply a greater compression on this external face.
- the orthosis is manufactured in several sizes, typically three or four sizes more or less wide, but in a single length, the orthosis being adjusted to the length of the patient's leg by cutting to the desired length of each of the two ends.
- the orthosis is then, at least at these two ends, knitted with a structure of the non-ratcheting type to avoid fraying after cutting.
- the mesh can be of the same type as that of conventional compression stockings, for example Varisma stockings (registered trademark) from Innothéra Topic. It can be of the woven, jersey, pinched or float micromesh type, etc., all meshes known in themselves to those skilled in the art of knitting techniques.
- the yarn chosen for the weaving of this orthosis may be a cotton and polyamide gimped elastane, a cotton-free polyamide gimped elastane, or a mixture of elastane and elastodiene (rubber latex synthetic).
- the material advantageously incorporates a high proportion of cotton, to allow easier absorption of serum and bleeding; it is also chosen to be washable and possibly reusable.
- the fitting of the orthosis of the invention is advantageously done by means of a threading accessory such as that described in WO-A- 99/44558 (Innothéra Topic International), which allows threading and adjustment simple, fast and precise, even in the presence of thick dressings and on a limb still anesthetized.

- the external semi-cylindrical part 14 has a greater peripheral extent than the elastic internal part 12, that is to say that the two connecting generators 16, 18 are on the same side of the diameter 20.
- Another variant consists in making the orthosis instead of knitting it, by using for this purpose two kinds of fabric having the same deformation characteristics as those exposed above, namely a fabric with low elongation on the external face. 14 and an elastic fabric on the internal face 12. To avoid the drawbacks associated with the seams, it is possible to use the two semi-cylindrical parts to join processes of hot or high frequency textile welding, in themselves known.
- FIGS. 5 to 7 An improvement, illustrated in FIGS. 5 to 7, consists in providing in the upper part of the orthosis a non-elastic and adjustable thigh sleeve, for example made of fabric.
- this sleeve is an open sleeve along a generator, as illustrated in Figure 5.
- the practitioner places the sleeve 22 on the upper part of the thigh and brings the two ends together free by adjusting the tightening to apply the desired compression on this region of the lower limb.
- the thigh sleeve 22 is then closed in the desired position, for example by means of hook-and-loop fasteners 24 which allow the tightening to be easily readjusted, if necessary.
- FIG. 8 illustrates the compression pressure profile - substantially constant - provided by the orthosis of the invention (in A), compared to that - declining - provided by a conventional compression stocking (in B).
- the dashed profiles correspond to the ideal profiles, while the solid lines illustrate the actual readings. The latter take into account, on the one hand, the morphology of the knee which locally prevents the application of regular pressure and, on the other hand, the fact that it is difficult to maintain the pressure at the top of the thigh due to the large diameter in this region (Laplace law).
