Abstract

发明领域
本发明涉及紧接下肢静脉手术后的一段时间中使用的压迫式矫治器领域。
发明背景
浅表静脉手术包括两大手术,两者都涉及手术除去患病的静脉。
第一种手术称为“剥离术”,它包括用金属线剥离出内隐静脉或外隐静脉。该手术在麻醉下进行,可以是全身麻醉、局部麻醉、硬膜外麻醉或安定镇痛。它对隐静脉的旁支引起严重损伤,导致出血,引起血肿,病人在手术后几小时和几天内感到疼痛和不舒服。
常用的另一种技术是门诊静脉切除术,它是一种比剥离创伤较小的手术行为(但也可在剥离术前后进行)。该技术涉及在门诊条件下完全抽出浅表静脉。引起的血肿比剥离手术小,但当病人起身时有立即出血的危险,这种危险是门诊手术固有的。
为了减少这两种手术后立即出血,和减轻其后果,外科医生对于手术的肢体施加非常强的压力,来限制手术后的炎症和出血。
在几乎所有情况下,这种压力目前是用干燥或粘性绷带施加的。
该技术有几个缺点:
·由于其性质,它依赖于实施该技术的人员的熟练程度;
·不可能检测施加的压力值和均匀程度:因此一些绷带可能有绑得太紧的危险,从而引起疼痛,或者绑得不够紧,失去其效果。
在实践中,在紧接手术后的一段时间内,病人通常会由于绷带放置不好而造成比手术本身大得多的痛苦。
其它血管手术的方法引起同样的不适,例如称为从血管机能不全的保守血液动力门诊治疗衍生而来的“保守外科”(在法语中以CHIVA为词首的)。
发明简述
本发明的一个目的是提供一种矫治器,它可以在手术后立即安放到位,起到对整条下肢提供受控和规则的压力的作用,而不依赖于安装矫治器的人的技术水平,而且压力足够限制手术后的炎症和出血。
目前,大量使用的绑带矫治器是最容易安放到位的,但如上所述,对施加压力来说又是不合适的。
当然,可以用一些其它现成类型的矫治器,特别是医用弹力长袜。然而这种长袜施加的压力递减,尤其是它们对大腿施加压力很少,而在手术后,特别需要在大腿上施加最强的压力。另外,这种长袜难于穿在由于全身麻醉无知觉的病人身上,还由于涂在腿上的敷料增加了它的体积。
本发明的矫治器克服了所有这些缺点。
因此,本发明提供了一种管状压迫式矫治器,用于在静脉手术,特别是门诊静脉切除术或静脉剥离术后压迫下肢,其中该矫治器包括针织或机织物的压力管状部分,包括与下肢形状相匹配的可变区段,其中选择的针织或机织物的结构随着下肢的周长而变,从而能沿着下肢长度方向施加基本不变的压力。
根据各种有利的次要特征:
·压力基本恒定在30毫米汞柱(mmHg)-40mmHg范围内;
·矫治器的形式是网眼长袜,没有针织的后根或足部;
·管状部分从脚部延伸到大腿根部;
·管状部分包括具有高度弹性的可伸展的内侧区域,沿两条母线与不能伸展或伸展性很弱的外侧区域相连,例如,接近沿直径相对的方向。
·管状部分的上端包括大腿套筒,套筒沿着母线开口,装有可调节的闭合装置;和
矫治器的上端和/或下端是用可切割,防脱散的材料制成的,因此当其安装好时可以切成所需的长度。
下文参照附图更详细的描述本发明。

具体实施方式
图1显示了本发明的矫治器,基本上是一种在足部开口的长袜。更确切说,矫治器10是一个具有可变区段的针织管,它与腿形相符,即向下逐渐变细,最好没有针织的足部或后跟。
该矫治器用于覆盖下肢从足部到大腿根,如图2和3所示。
它必须能对下肢从踝部到大腿施加恒定的压力(而常规弹力长袜上产生的压力从踝部开始减小),该压力应在约30mmHg-40mmHg(40.0百帕(hPa)-53.2hPa),因此能沿着整条下肢发挥恒定的止血作用。
管状矫治器10包括两个基本半圆柱体的部分12和14,它沿着矫治器整个长度延伸,并沿着位于直径20对端的母线16和18连接在一起。
位于腿内对侧半圆柱体部分12是由增强的弹性组织构成的,例如使用与连裤袜后档相同的组织结构。
位于腿外侧的半圆柱体部分14是用几乎不伸展的针织结构,即延伸小的组织针织而成,从而对外表面提供较大的压力。
本文使用的术语“弹力”的含义是纺织业常用的,即表示经伸展的织物材料一旦除去了变形力,回到原来的形状和尺寸的能力;这种弹力表达成相对于在不受力的状态下1米材料延伸的百分比。
当最大延伸小于70%(特别见C.Gardon-Mol lard和A.-A.Ramelet于“LaContention Médicale”[Medical compression],Masson,Paris出版,1999,pp.51-54)时,就称为“短的”(对于几乎不伸展的织物,例如部分14所用的),当最大延伸在70%-140%范围内时,叫做“中等”(“弹性”织物,例如部分12使用的)。
矫治器最好制造几种尺寸,通常是三种或四种不同直径,但长度是单一的,通过在两端割去超过的长度使矫治器与病人的腿长相符。然后用防脱散织物至少两端缝合矫治器,从而避免割到所需长度后松开。
织物可以是用于常规弹力长袜,如Innothéra Topic生产的Varisma(注册商标)长袜的相同类型的织物。它可以是纬线、平纹、压细或跳花的微孔小网眼等类型,针织技术领域的专家都知道这些织物结构。
用于针织矫治器的纱线可以是用棉和聚酰胺包覆的弹性纤维,用聚酰胺包覆,而不用棉包覆的弹性纤维,或还有弹性纤维和二烯类弹性纤维(合成橡胶乳液)的混合物。然而,材料最好含有大比例的棉,从而能够更有效的吸收浆液和出血;选择它还由于可洗涤和可任选重新使用。
最好用WO-A-99/44558(Innothéra Topic International)所述的辅助工具来安装本发明的矫治器,这种安装是能简单、快速准确的套上和调节矫治器样品,即使在厚敷料和在仍麻醉的肢体上也是如此。
自然可想像出各种变化形式。
在图4所示的第一种变化形式中,外侧的半圆柱体部分14的周长比弹性的内侧12的大,即连接两个部分的母线6和18都在直径20的相同侧。
另一个变化形式不是针织的矫治器,而是用两种机织物制成的具有如上所述相同的变形特征,即两种矫治器织物都有延伸很少的外侧面14织物,和弹力的内侧面12的织物。为了避免与组织结构有关的缺点,都使用织物热封或高频率的方法,将两个半圆柱部分连结起来。
图5-7所示的改进在于在矫治器顶部提供了一种由例如机织物制成的大腿套,它不是弹性的,但是可调节的。
为了容易穿上,该套沿图5所示的母线打开。在矫治器穿到腿上后,医师将套22按在大腿根部,通过调节松紧程度,将其自由的两边相对移动,从而对下肢的该区域提供所需量的压力。然后在所需的位置闭合大腿套22,例如通过自闭合搭扣条24,使得在需要的地方很容易调节松紧程度。
图8显示了用本发明的矫治器获得的压力情况,即压力不变的曲线A,与之相比,用常规弹力长袜获得的曲线B呈逐渐下降。虚线是理想的曲线,而实线是真实的测量值。这些测量值同时考虑了局部阻碍施加规则压力的膝盖形状,和考虑了由于大腿根部区域直径大,难于维持压力的情况(Laplace’s Law)。
Claims (6)
1.一种用于在静脉手术、门诊静脉切除术或静脉剥离术后压迫下肢的管状压力矫治器,其特征在于,该矫治器是一个针织或机织物的压迫管状部分(10),从足部延伸到大腿根部,该管状部分具有与下肢形状相匹配的可变区段,其中选择的针织物或机织物的结构随下肢的周长而变,从而能沿下肢方向施加不变的压力,所述压力在30mmHg-40mmHg范围内。
2.如权利要求1所述的矫治器,其特征在于,所述管状部分(10)包括沿两条母线(16,18)与不能伸展或伸展性很弱的外侧区域(14)连接的高弹性的可伸展内侧区域(12)。
3.如权利要求2所述的矫治器,其特征在于,使内侧区域和外侧区域互相连接的两条母线(16,18)沿直径方向相对。
4.如权利要求1所述的矫治器,其特征在于,它是一种网眼长袜的形状,但没有针织的后跟或足部。
5.如权利要求1所述的矫治器,其特征在于,管状部分顶端具有一个大腿套(22),它沿着一条母线打开,并装有可调节的闭合装置(24)。
