安定保存可能な酸素輸液剤 本明細書において引用 される種々 の文献は、 こ こでの引用 によ り 、 その全体が本明細書の一部をなすもの と して援用 さ れる。 技術分野
本発明は、 酸素輸液剤を長期保存する方法、 長期保存に適 した酸素輸液剤、 および該酸素輸液剤の製造方法に関する。 本発明の酸素輸液剤は医学および薬学での広い応用が可能 であ り 、 また輸血用血液と 同様、 そのまま或いは必要に応 じ 添加物を加えた状態で、 赤血球代替物と して臨床治療に利用 される。 背景技術
人血を血管内に注入する現行の輸血システムは、 血液型不 適合 ; 感染 (肝炎、 エイ ズヴィ ルスな ど) の可能性 ; 赤血球 の保存期限が短い (約 3 週間) 等、 種々 の問題に直面 してい る。 従って、 これらの問題を解決でき る代替物に対する要求 が大き く 、 その一つ と して、 電解質輸液、 膠質輸液等の輸液 製剤が従来広く 使用 されてき た。
しかし、 これらの輸液製剤は血液の最も重要な機能、 即ち 酸素を運搬する赤血球機能の代替機能を有 してないため、 こ の酸素運搬機能を も代替でき る酸素輸液剤 (人工赤血球) の
開発が極めて重要になっている。 このよ う な酸素輸液剤 と し て、 酸素溶解能の高いパーフ ルォロ カーボン誘導体の水性懸 濁液、 並びに可逆的な酸素結合能を有するへモグロ ビン ( ヒ トヘモ グ ロ ビン、 ゥシへモグロ ビン、 および組換えへモグ口 ビン) 、 分子内架橋ヘモ グ ロ ビン、 水溶性高分子結合へモグ ロ ビン 、 分子間架橋重合へモグロ ビンな どを用いた酸素輸液 剤の臨床試験が開発 され、 臨床試験が進行 している。 しか し その非細胞型構造に起因する各種の副作用が明 らかに されて いる
ヘモ グ ロ ビン ( H b ) が本来赤血球膜に包まれている理由 と して、 下記の事項が考え られる :
1 ) 1 2〜 1 5 g/dl濃度の濃厚 H b溶液によ る高い粘度およ び膠質浸透圧の影響を抑制する こ と ;
2 ) 生理活性の高いへモグロ ビンを膜内に封 じ込める こ と によ り 、 その逸脱を抑制する こ と ;
3 ) H b 機能を維持する ための各種 リ ン酸、 解糖 ■ 還元酵 素系を同一系内に保持する こ と ; および
4 ) 血球分散系は非ニ ュ ー ト ン性流体であ り 、 体内 (特に 末梢血管) 循環において特色ある流動形式によ り 生理作用を 示す利点がある こ と。
上記の よ う な赤血球構造の本来の役割を考慮すれば、 酸素 輸液剤 と しては、 へモグ口 ビンをカ プセル封入 した粒子の分 散系が適切である こ と は明 らかである。 一方、 生体成分であ る リ ン脂質が単独で小胞構造を形成する こ と が発見され、 Dj o rdj evi ch と Mi l l e r力 リ ン脂質 コ レス テ ロ ール/脂肪酸
から成る リ ポソームを利用 したへモグロ ビン小胞体の検討を 開始 して以来、 多く の機関でへモグロ ビン小胞体の研究が進 め られてき た。 ヘモ グ ロ ビン小胞体の使用には、 1 ) 天然の ヘモグロ ビンがそのま ま使える こ と 、 2 ) ヘモグロ ビン由来 の副作用を抑制でき る こ と 、 3 ) 粘度や膠質浸透圧、 酸素親 和度を任意の値に調節でき る こ と 、 4 ) 体内血中滞留時間を 延長でき る こ と 等の利点がある。
ヘモ グ ロ ビンの酸素結合部位であるヘム (プロ ト ポルフ ィ リ ン IX) は、 グロ ビンか ら逸脱する と 同時に酸素結合能を 失 う こ と は当業者には周知であ り 、 グロ ビン鎖の構築する立 体枠組みの役割 と疎水場の重要性は良 く 認識されていた。 そ こ で、 グロ ビンの機能を代替でき る系の開発に力が注がれて きた。 本発明者らは、 各種ポルフ ィ リ ン誘導体について検討 した結果、 水相系で可逆的に酸素を結合する能力を有する リ ピ ドヘム (ヘム結合脂質) : 5,10,15,20-tetrakis[α,α,α,α-o- ί2',2'-dimethyl-20'(2''-trlmethyammonloethy phosphonat- oxyeicosanamido}-phyenyl]porphynato iron ctびその ί也の 合成に成功 した。 こ の リ ピ ドヘムを リ ン脂質と 混合 して水相 に分散処理する こ と に よ り 構成 さ れる リ ピ ドヘム小胞体では リ ピ ドヘムが リ ン脂質膜疎水場に包埋 され分散配向 している 水相分散系にある粒径の揃つた リ ピ ドヘム小胞体では、 生理 条件下にある赤血球内へモグロ ビン と 同様に、 可逆的な酸素 配位が可能と なる こ と が観測 されている。 カゝく して、 血液と 同 じヘム濃度の赤い色を した こ の水相系は、 全合成に よ る酸 素輪液剤 と して誕生 した ( E.Hasesawa, et. al., Biochem.

Commun. vol.105, 1416-1419, 1982) 。 動物投与 試験 も詳細にな され、 出血シ ョ ッ ク モデル犬の蘇生試験では ヘム濃度に応 じた酸素運搬力が確認された。 栄養油剤 (精製 大豆油、 ト リ グ リ セ リ ド) の油滴小球の周 り を リ ピ ドヘムで 覆った リ ピ ドヘム— ト リ グ リ セ リ ド小球体の酸素輸送能も確 認されている。 また、 ヘム誘導体である 2-[8-{N-(2- methvlimidazolvl; ) octanovloxymethylJ-5,10,15,20- tetrakisia^^^-o-pivalamidojphenyl-porphynato lronill)等 を ヒ ト血清アルブ ミ ンまたは組換え ヒ ト アルブ ミ ン の疎水ポケ ッ ト に吸着させた酸素輪液剤 (アルブ ミ ン-ヘム と称する) が合成 され、 その酸素運搬能が確認されている (E.Tsuchida, et al., Bioconjugate Chemistry, vol.8, 534-538, 1997) 。
こ の よ う な酸素輸液剤の現状において、 一つの大き な問題 は、 酸素輸液剤保存である。
酸素輸液剤の従来の保存方法には凍結保存、 凍結乾燥粉末 と しての保存がある。 しか し、 凍結体はこれを融解する手間 がかか り 、 また凍結乾燥粉末を溶解させる際には溶解までに 時間を必要とする上に、 気泡が発生 して これを除去する必要 があるな どの煩雑さ を伴 う 等の問題点が指摘されている。 従 つて、 凍結保存、 凍結乾燥粉末保存は好ま し く ない。
加えて、 酸素輸液剤はヘムタ ンパク質に特有の性質と して 経時的に劣化する ので、 安定に保存する こ と が困難である。 即ち、 ヘモ グ ロ ビン、 リ ピ ドヘム、 およびヘム誘導体は、 そ のヘム 中心元素である鉄が二価鉄 ( Fe2 + ) の と き には酸 素を可逆的に結合するのに対 して、 酸化型の三価鉄(Fe3 + )
である と き は酸素結合能がない。 また酸素を結合 した二価鉄 錯体でも、 スーパーォキサイ ドア二オン ( o2-) を遊離 して 次第に 自 動酸化 し、 三価鉄 (メ ト体) と なって酸素結合能を 失 う。 更に、 メ ト体からはヘムおよび鉄イ オンが遊離 し易 く なるので、 生体への悪影響が懸念される。
こ の酸化反応を抑制する方法と しては、 記述 したよ う に単 に冷蔵保存 して反応速度を低下させる こ と も可能であるが、 次第に三価鉄が増大する。 そこで、 赤血球に本来存在 してい る メ トヘモ グ ロ ビン還元酵素系や、 カ タ ラーゼおよびスーパ ーォキサイ ドディ ス ムターゼな どの活性酸素を消去する酵素 を添加する方法が知 られている。 また、 ヘモグロ ビンおよび ヘム誘導体に対 して酸素の 2 0 0倍の親和度を示す一酸化炭 素 (CO) を結合させて、 二価鉄の状態を保ち、 極めて長期 間に亘つて三価鉄への酸化を抑制する こ と もでき る。
しか し、 メ トヘモ グ ロ ビン還元酵素系や活性酸素消去酵素 を添加する方法では、 長期保存中に酵素活性が低下する。 ま た、 一酸化炭素雰囲気で冷蔵保存 した酸素輸液は、 大量の一 酸化炭素が有害であ り 、 また結合 した一酸化炭素を除去 しな い限 り 酸素結合能が無いので、 こ のま までの投与はでき なレ' しかも、 ォキシ型に変換 した後の冷蔵保存では、 三価鉄状態 への酸化が次第に進行 し、 酸素運搬能が低下する。 二価鉄状 態のへモグロ ビンの酸素分圧 と 酸化速度の相関は良 く 知 られ てお り 、 デォキシヘモ グ ロ ビンでは酸化反応が進行 しないこ と は実験的に確認されている(Sakai et al., Bull. Chem. Soc. Jpn., 1994, 1120-1125; Takeoka et al., Bioconjugate Chem.,
vol.8, 539-544, 1997)。
また、 へモグロ ビンお よ びヘム誘導体の酸化反応が抑制さ れた と して も、 酸素輪液剤の保存にはも う 一つ別の課題があ る。 即ち、 ヘモ グ ロ ビン小胞体、 リ ピ ドヘム小胞体、 リ ピ ド ヘム -ト リ グ リ セ リ ド小球体のよ う に、 ヘムの周囲環境を形 成 している脂質分子集合体構造は、 共有結合を介 さずに成分 分子間の相互作用力 (疎水性相互作用、 静電的相互作用、 水 素結合な ど) から構成されているため不安定な場合が多く 、 生理塩水に分散させて冷蔵保存する と 、 次第に凝集体を形成 し、 融合 して粒径が変化するので、 何らかの構造安定化が望 まれていた。 そのよ う な方法と して、 次のよ う な安定化技術 が知 られている。
へモグロ ビン小胞体ゃリ ピ ドヘム小胞体において、 重合性 の リ ン脂質を小胞体の構成成分と し、 γ 線や紫外線照射によ つて これを重合する こ と によ り 小胞体構造を極めて安定にす る こ と ができ る。 この場合、 分散液を液体窒素で急速冷凍 し て長期保存が可能と なる。 凍結融解を 1 0 回繰 り 返 して もへ モグロ ビンの漏出はな く 、 また粒径も変化せず、 酸素結合解 離曲線に も変動が無い ( Sat oh et al., ASAIO Jounal, vol.38, M580-M584, 1992) 。 また、 こ の系にマノレ ト ースゃス ク ロ ー スな どの糖を添加 し、 凍結乾燥 して得られる粉末は極めて安 定である。 例えばへモグ口 ビン小胞体の場合、 2 0 週間 4 °C で保存 した後、 純水を添加 して再溶解させた後の物性解析で は、 メ トヘモ グ ロ ビン含量の増大は認め られず、 へモグロ ビ ンの漏出 もな く 、 また小胞体の粒径が変化 してお らず、 凍結
乾燥前の状態と ほぼ同等である こ と が確認されている
( Wang et aし Polymer Adv. Technol., vol.4, 8-11, 1992) 。 ポ リ オキシエチ レンを結合 した脂質を、 リ ン脂質小胞体の 表面に導入する方法は良 く 知 られている。 しかし、 その 目 的 は、 血中滞留時間を延長 させて封入 した抗癌剤を腫瘍組織に 効率良 く 運搬する こ と であ り 、 既に臨床試験も最終段階と な り 、 その安全性は充分に認め られている。 また、 へモグロ ビ ン小胞体と血漿蛋白質と の相互作用を抑制 させる こ と を 目 的 と して、 ヘモグロ ビン小胞体の表面をポ リ オキシエチ レンで 修飾する こ と によ り 、 血流動態が改善される こ と も確認され ている { S akai et al . , Bioconjugate Chemistrv, vol.8, 23-30, 1997) 。 しかし、 酸素輸液剤の保存に関 して、 こ のポ リ オキ シエチ レ ン修飾法を利用する こ と は知 られていなかった。
発明の開示
本発明の 目 的は、 酸素輸液剤の長期室温棚置き保存を可能 にする技術手段を確立する こ と である。
本発明の一つの側面に よれば、 水性媒質中に分散された脂 質分子集合体と 、 該脂質分子集合体に含まれるヘムまたはへ ム誘導体を含有する水性分散液からなる酸素輸液剤の保存方 法であって :

本明細書において引用 される種々 の文献は、 こ こ こ での引 用によ り 、 その全体が本明細書の一部をなすもの と して援用 される。
本発明において。 「脂質分子集合体」 と は、 脂質および Z または リ ポタ ンパク 質に よ って、 共有結合を介 さずに、 水性
媒質中における分子間の相互作用 (疎水性相互作用、 静電気 的相互作用、 水素結合な ど) に よ り 構成された膜構造体を言 う 。 典型的には小胞体 ( リ ボソーム) および小球体 (マイ ク 口 ス フ ィ ァ) が挙げられるが、 広義には赤血球膜等の細胞膜 も含まれる。 更に、 ヘモグ ロ ビン小胞体、 リ ピ ドへム小胞体 リ ピ ドヘム ー ト リ グ リ セ リ ド小球体も また、 脂質分子集合体 からなる小胞体の典型例である。
本発明において、 「ヘムまたはその誘導体」 と は、 ヘムだ けでな く 、 ヘムのポルフ ィ リ ン環が置換基によ り 修飾された ヘム誘導体の う ち、 酸素 と の可逆的な結合能を有する もの全 てを包含する。
本発明における 「水性媒質」 には、 水および全ての生理学 的に許容可能な全ての水溶液、 例えば、 電解質水溶液、 緩衝 液、 タ ンパク 質水溶液、 脂質ェマルジ ヨ ン水溶液、 血漿、 血 漿増量剤 (デキス ト ラ ン、 ヒ ド ロ キシェチルス タ ーチ、 ゼ ラ チン等の コ ロ イ ド水溶液) およびこれらの組み合わせが含ま れる。
本発明における不活性ガス と は、 化学的に不活性なガス を 意味 し、 例えばヘ リ ウム、 アルゴン、 ネオンな どの希ガス に 加えて、 窒素が含まれる。 経済的な理由からは窒素ガスが好 ま しい。
次に、 本発明の構成を詳 し く 説明すれば次の通 り である。 既報に従って調製 したへモグロ ビン小球体( Sakai et al., Biotechnolosy Progres, vol. 12, 119-125, 1996)、 リ ピ ドへム 小胞体 (E. Hasegawa et al., Biochem. Biophys. Res C ommun . ,
vol. 105, 1416-1419, 1982)、 リ ピ ドヘム - ト リ グ リ セ リ ド小胞 体 (E. Tsuchiaa et al., Biochimica Biophysica Acta, vol. 1108(2), 253-256, 1992), およびァノレフ、' ミ ン -ヘム (E. Tsuchida et al., Bio conjugate Chemistry, vol. 8, 534-538, 1997) の生理塩水溶 液について、 ヘムが鉄二価の状態である こ と を確認 した後、 こ の分散液を所定の成分濃度 (例えばへモグロ ビン濃
10g/dL, ヘム濃度 6.2mM) に調節 し、 分散液から酸素を除去 する。 除去方法と しては、 酸素を含ま ない窒素またはその他 の不活性ガス (アルゴン、 ヘ リ ウムな ど) に曝 して、 溶解 し ている酸素を排気させる。 こ の操作をする こ と によ り ォキシ 型のヘムは酸素を結合 していないデォキシ型ヘムに変換させ る。 このためには、 上記酸素輸液剤を硝子瓶な どの酸素を透 過 しない密封性の容器に封入後、 容器内でこの気体の気泡を 発生させて通気する こ と で溶存酸素を排気する。
溶存酸素濃度は、 ク ラーク 式酸素電極を分散液に漬けて酸 素分圧をモニターする方法、 容器内の気相を採取 して これを ガス ク ロマ ト グラ フ ィ 一にて測定する方法、 または容器内の へモグロ ビンおよびヘムの特徴ある可視も し く は近赤外スぺ ク トルの測定から、 ォキシ型と デォキシ型の比率を算出する 方法によ り 知る こ と もでき る。 こ のよ う に して得 られたデォ キシ型の各酸素輸液剤は、 酸素を遮断 して保存する こ と によ り 、 ヘモグロ ビンおよびヘムの酸化、 また脂質な どその他の 成分の酸素酸化を抑制する こ と ができ る。
酸素除去操作後に、 溶液中に残存 している微量の酸素を更 に除去する こ と を 目 的 と して、 小胞体中、 或いは溶液中に適
量のチオール類 (ホモ シス ティ ン、 ァセチルシス ティ ン、 グ ノレタチオンな ど) 、 ァ ス コ ノレ ビン酸、 亜ニチオン酸な どのよ う な酸素 と反応する試薬を少量溶存させて も 良レ、 。
上記のよ う に して得られたデォキシ型の各酸素輸液剤は、 酸素を遮断して保存するために、 例えば硝子瓶に直接封入 し た り 、 アル ミ ニ ウ ム /ポ リ エチ レ ン層状バ ッ グ(aluminized polyethylene bag)や、 ポ リ 塩化ビニ リ デン系、 エチ レ ンー ビ ニルアルコ ール共重合体系な ど、 酸素透過性が極めて低い材 質から成る容器に入れて封入 した り 、 またはプラ スチ ッ ク製 バ ッ ク に封入 しこれを更に酸素を透過 しない容器に入れる。 保存温度は、 -20°C〜 60°Cの間で可能であるが、 好ま し く は 4 〜 25°Cの冷喑所保存をする。 以上の操作によ り 、 へモグロ ビンおよびヘムの酸化、 また脂質な どその他の成分の酸素酸 化を抑制する こ と ができ る。
上記の酸素除去に加えて、 ヘモ グ ロ ビン小胞体、 リ ピ ドへ ム小胞体、 およびリ ピ ドヘムー ト リ グ リ セ リ ド小球体の場合 は、 粒子表面に保存安定度向上のためにポ リ ォキシエチ レ ン を予め結合させるのが好ま しい。 このためには、 ポ リ オキシ エチ レ ンを結合 した脂質 (ポ リ オキシエチ レ ン脂質) の分散 液を 4〜 60°Cで添加する。 本脂質の疎水性部分は粒子表面の 脂質膜中に挿入され、 親水性のポ リ ォキシエチ レ ン鎖が水相 に伸びた状態で固定される ( Sakai et al., Bioconjugate
Chemistry, vol. 8, 23-30, 1997) 。 操作温度が高いほ ど導入速 度が速いが、 冷却 して行って も良い。 また小胞体の膜成分と して コ レステ ロ ールを多 く 含むため、 明確な相転移温度は無
いが、 成分 リ ン脂質の相転移温度以下でも十分に導入 さ れる t ポ リ ォキシエチ レン脂質のポ リ ォキシエチ レン鎖分子量は、 1 , 000〜 20 , 000 で良 く 、 導入量は、 粒子の外層に面 している 月旨質総量に対 して、 0 . 0 1〜3 m o /。、 好ま し く は 0 . 05〜
0 . 3 m o /。導入する。 ポ リ オキシエチ レ ン脂質の疎水性部分は エ タ ノ ールア ミ ン型 リ ン脂質、 コ レス テ ロ ール、 アルキル鎖 結合グルタ ミ ン酸、 アルキル鎖結合 リ ジンな どがある。 ポ リ ォキシエチ レ ン と脂質部分の結合様式は、 エス テル結合、 ゥ レ タ ン結合、 ア ミ ド結合、 エーテル結合な どがある。 ポ リ オ キシエチ レンを粒子表面に導入する こ と で保存中の凝集と融 合によ る粒径変化を抑え、 またヘモ グ ロ ビン小胞体の場合、 へモグロ ビンをは じめ とする 内包物の漏出を防ぐこ と ができ る。
上記の構成からなる本発明の作用は次の通 り である。 本発 明は、 酸素を除去する こ と で酸素輸液剤中のへモグロ ビンお よびヘム誘導体の酸化を抑制する。 こ の酸化抑制の効果によ り 、 保存中のス ーパ一ォキサイ ドア二オンや過酸化水素の発 生が防止 され、 従って、 ヘムおよびヘム誘導体を担持する脂 質分子集合体の酸化および変性が防止 される。 その結果、 脂 質分子集合体の物理的安定性が向上 し、 粒径変化および凝集 体の形成が防止 される ので、 脂質分子集合体か らなる酸素輸 液剤の保存寿命が向上する。
加えて、 分子集合体であるヘモ グ ロ ビン小胞体、 リ ピ ドへ ム小胞体、 および リ ピ ドヘム -ト リ グ リ セ リ ド小球体の表面 にポ リ オキシエチ レンを導入する こ と によ り 、 これら粒子の
脂質分子集合体を更に安定化させる こ と ができ る。 従って、 保存中の粒子の凝集と融合によ る粒径変化や、 へモグロ ビン をは じめ と する内包物の漏出等を効果的に防止 し、 酸素輸液 剤の保存安定性を更に向上させる こ と ができ る。
本発明において留意すべき こ と は、 ヘム鉄の二価から三価 への酸化 と脂質分子集合体構造の不安定性と の間には、 相互 に他を助長する関係が存在する こ と である。 即ち、 ヘム鉄の 酸化に伴って発生 したス ーパーォキサイ ドア二オン ( 02 - )や 過酸化水素、 およびそれによ つて生 じる フエ リ ルへモグロ ビ ンは、 リ ン脂質等の脂質分子集合体の構成成分を酸化 して、 脂質分子集合体の崩壊を助長する。 また、 脂質分子集合体の 崩壊は、 ヘム鉄の存在環境を悪化させて酸化を助長する可能 性がある。 本発明は こ の点に着 目 し、 ヘムおよびヘム誘導体 の酸化抑制 と 、 ヘムおよびヘム誘導体のキヤ リ ァである脂質 分子集合体の安定化と を同時に達成する こ と によ り 、 従来達 成され得なかっ た酸素輪液剤の室温棚置き保存を可能 と する なお、 アルブ ミ ン—ヘム水溶液を用いた酸素輪液剤は溶液 の状態で安定であるため、 脂質分子集合体構造は採用 されな レ、。 従って、 脂質分子集合体構造の安定化と い う 作用効果と は直接関係はないが、 酸素を除去 した状態で保存する こ と に よ り ヘムのメ ト化を防止 し、 保存性を顕著に向上でき る点に おいては同様である。
得られる脱酸素型の酸素輪液剤は長期保存が可能と なるの で、 医療機関の各部署、 救急車、 医療機関が無い遠隔地な ど に常備する こ と によ り 、 必要時に直ちに体内投与する こ と 力';
可能になる。 デォキシ型と した各酸素輸液剤は、 空気に曝す と 直ちに酸素を結合 してォキシ型と なる。 また、 デォキシ型 のま ま静脈内投与 して も、 先ず肺を通過する と き に直ちに酸 素と 結合 してォキシ型と な り 、 末梢にて酸素を放出する。
次に実施例を挙げて本発明を よ り 詳 し く 説明する。
実施例 1
無菌的雰囲気において、 ヒ ト赤血球から精製 して得た高純 度ス ト ロ マ フ リ ーヘモ グ ロ ビン溶液(40g/dL)に、 ピ リ ドキサ ル 5 '-リ ン酸を、 ヘモ グ ロ ビンに対 して 3倍モル と なる よ う に添加 した。 また、 ホモ シス ティ ンを濃度 5mM と なる よ う に添カロ し、 更に lM-Na2C03 によ り pH を 7.4 と した。
Remolino(Millipore社)を用レヽて孔径 0.22 / m の FM ミ ク ロ フ イ ノレタ ー(富士フ ィ ルム)で濾過 し、 仕込みへモグロ ビン溶液 を得た。 次いで、 混合脂質粉末 Presone PPG-I(ホス フ ァチジ ノレ コ リ ン Zコ レ ステ ロ —ノレ /ホス フ ァ チジノレグ リ セ ロ ーノレの 混合物)を脂質濃度が 4.5 wt。/。と なる よ う に少量ずつ添加 し、 4°Cで終夜撹拌 して、 ヘモ グ ロ ビ ン内包多重層小胞体を得た。 ェク ス ト ルージ ョ ン法によ り 、 これら小胞体の粒径及び被覆 総数の制御を行った。 最終的に、 F M ミ ク ロ フ ィ ルタ ー (孔 径 0.22 / m ) を 2 回透過 させた。 得られたヘモ グ ロ ビン小 胞体溶液を生理食塩水で希釈 し、 超遠心分離 ( 50,000g, 40 min) 後に上澄みへモグロ ビ ン溶液を吸引除去 し、 へモグ口 ビン小胞体を生理食塩水に再分散させた。
生理食塩水に溶解させたポ リ ォキシエチ レ ン結合脂質、 N-(monomethoxypolyoxyethvlene-
carbamyl)distearoylphosphatidyl-ethanolamine (ポ リ ォキシェ チ レ ン鎖の分子量は 5300) を、 小胞体外表面の脂質の 0.3mo /。分に対応する量で滴下 した。 これを 2 5 °Cで 2 時間 撹拌 した後、 4 °Cで終夜撹拌 して、 へモグロ ビン小胞体の表 面をポ リ ォキシエチ レ ンで修飾 した。
円筒型フ ラ ス コ にへモグロ ビン小胞体分散液 ( 0.5g/dL, 200mL) を入れ、 これを 口 一タ リ ーエバポレータ に装填 して 回転( 56 rpm)させた。 これによ つて形成された液膜に、 ノ、口 ゲンラ ンプ(500W)を用いて酸素通気下(IL/min)で 3 分間可視 光を照射 し、 一酸化炭素結合へモグロ ビン(HbCO)からォキ シへモグ口 ビン(Hb02)への配位子交換を行った。 該分散液 を超遠心分離(50,000g, 60 min)してへモグロ ビン小胞体粒子 を沈降させ、 外水相生理食塩水を除去 した後、 分散媒と して リ ン酸緩衝生理食塩水を加えて再分散させた。 へモグロ ビン 濃度を 10g/dL と し、 Dismic-25: 0.45 μ m フ ィ ノレター
(ADVANTEC)で濾過 して、 ポ リ オキシエチ レ ン修飾へモグロ ビン小胞体を得た。
上記で得たポ リ ォキシエチ レ ン修飾へモグロ ビン小胞体分 散液 30mL を、 lOOmL容量のバイ アル瓶に入れて密封 した。 滅菌ディ ス ク フ ィ ルターを通 し更に水蒸気飽和 させた窒素ガ スを瓶内に導き 、 小胞体分散液中でバブ リ ングさせて、 溶解 している酸素を除去 した。 系内の酸素分圧をク ラーク型酸素 電極 (酸素分圧測定装置, =Po2- 100, Inter Medical) によ って モニター した と こ ろ、 酸素分圧が 1 Torr まで低下 した。 こ の操作によ り 、 ォキ シへモ グ ロ ビンはでォキ シへモ グ ロ ビン
に変換された と判断した。
こ う して得られた本発明の酸素輸液剤について、 保存試験 を行った。 保存条件と しては冷蔵保存 ( 4 °C ) 、 室温保存
( 2 3 °C ) 、 恒温槽保存 ( 4 0 °C ) を設定 し、 それぞれの試 料について 1 年後まで、 以下の測定を行って保存前の試料と 比較 した。
①試料 30 L を生理食塩水で 100倍に希釈 し、 室温にお いて 1 mmセルを用いて 300〜 900nm まで紫外可視吸収スぺ ク ト ル測定を行った。 保存前の試料と 比較 して、 新たな吸収 極大の出現の有無や、 Q帯ピー ク の出現波長のずれについて 検討した。
②試料の沈澱形成の有無を 目視によ り 確認した。 また試料 30 μ L を生理食塩水で 10倍に希釈 し、 室温において 1 mmセ ノレを用いて 900n mの吸光度を測定した。 生理食塩水の 900nm の吸光度を リ フ ァ レ ンス と して差 し引 き、 試料の濁度 と した。
③試料約 0.2mL を り ん酸緩衝生理食塩水 ( P B S ) で 200 倍に希釈 し、 超遠心分離(100,000g, 15min)を行った後、 上澄 み液のヘモ グ ロ ビン定量を行って溶血の有無を確認 した。
④粒径分布は、 2 5 °Cにおいて、 Sub-micron Particle Analyzr Model N4-SD (し oulter し orporate Communications; ¾r 用いて動的光散乱法によ り 測定 した。
⑤ Hemox- Analyze" TCS Medical Prducts Co.)を用レヽて酸素 結合解離曲線を測定 し、 その解析から酸素親和度(P50)、 酸 素運搬効率(OTE)、 Hill係数を算出 した。
⑥脂質の分解を検討するため、 試料約 0.2mL を凍結乾燥 させた後、 CHC13 によ り 脂質を抽出 し、 展開溶媒と してク ロ ロ ホルム /メ タ ノ ール /28%ァ ンモニァ =13/7 /1(容量比) お よ び、 ク ロ 口 ホルム /アセ ト ン/メ タ ノ ール /酢酸/水 = 10/4/2 /2/1(容量比)を用いて二次元薄層ク ロ マ ト グラ フ ィ ー(シ リ 力 ゲルプレー ト )を測定 した。
⑦試料約 0.2mL を凍結乾燥させた後で、 約 lmL の CDC13 によ り 膜成分を抽出 し、 フ ィ ルタ ー濾過 した後、 iH-NMRス ぺク ト ノレ(JNM-LA500、 日 本電子)を測定 した。 また外水相に 遊離 したポ リ オキシエチ レ ン鎖を除去するため、 試料約 0.2 mL を PBS で約 200倍に希釈 し、 超遠心分離( 100, 000g, 15min)を行って上澄み液を除去 した。 沈澱を P B S で再分散 させた後で凍結乾燥させ、 約 1 m L の CDC13 によ り 膜成分 を抽出 し、 フ ィ スレタ ー濾過を した後に、 'H-NMRスぺク トノレ を測定 した。 ポ リ ォキシエチ レン脂質のポ リ ォキシエチ レン 鎖メ チ レン基プロ ト ンの ピーク ( B) は、 δ :3.63ppm に、 ホ ス フ ァ チジノレコ リ ンの コ リ ン メ チノレプ ロ ト ンの ピー ク (A) は δ :3.39ppm に出現する。 各々 のプロ ト ン数の比が積分比 B/A に等 しい と して、 ポ リ オキシエチ レン鎖の導入率は次式 に従って算出 した。
(水相除去後の積分比 B/A) ÷ (仕込みの積分比) X 100 図 1 に、 保存中におけるへモグロ ビン小胞体分散液の諸物 性値の推移を示す。 いずれの試料について も、 1 年の保存期 間中に、 紫外可視吸収スぺク ト ルにおける メ ト体由来の
630nm の新たな ピーク の出現、 Q 帯および Soret 帯の吸光度 の変化、 および波長のずれはいずれも確認されなかった。 ま た溶血は認め られず、 二次元薄層ク ロマ ト グラ フ ィ 一におい て遊離の脂肪酸は観察されなかった。 いずれの試料において も、 保存 6 力月 後では、 凝集に よ る沈澱は観測 されず、 粒径 や濁度も殆ど変化 しなかった。 また、 40°C · 6 力月 後のポ リ ォキシエチ レ ン鎖の導入率は、 保存前と 比較 して 7%程度の 低下に留ま った。 P50 の減少傾向は、 40°C · 6 力月 後でも保 存前と 比較 して 5.5 Torr の減少に過ぎず、 こ の程度の減少で はへモグ口 ビン小胞体の酸素運搬機能に問題は無い と判断さ れた。 しカゝ し 40°C · 1 年後では、 脂質の分解と 、 P50 力 43 Torr にまで増大する現象が観測された。 保存後の初期メ ト化率はいずれの試料において も低下 し、 保存 1 力月 後まで には 1 %未満と なる傾向が認め られた。 これはホモシスティ ンによ り 酸化型メ トへモグロ ビンが還元されたためである。 以上から、 窒素雰囲気下ではポ リ ォキシエチ レン鎖で表面修 飾 したへモグ口 ビン小胞体は、 4 0 °Cでは 6 力月 間、 2 3 °C では 1 年間は棚置き保存が可能である こ と が判明 した。
実施例 2
ポ リ ォキシエチ レン修飾を していないへモグロ ビン小胞体 分散液を実施例 1 と 同様に調製 し、 バイ アル瓶に入れて密封 し、 滅菌ディ ス ク フ イ ノレターを通 した窒素をバブ リ ングさせ て通気 し、 溶解 している酸素を完全に除去 した。 系内の酸素 分圧を酸素分圧測定装置(Po2- 100, Inter Medicals社) でモニ ター し、 2 Torr にまで低下 した こ と を確認 した。 この操作に
よ り 、 ォキシヘモグロ ビンはデォキシへモグロ ビンにほぼ変 換された。
こ う して得られた比較例酸素輪液剤について保存試験を行 つた。 保存条件と して冷蔵保存(4°C)、 室温保存(23°C)、 恒温 槽内保存(40°C)を設定 し、 それぞれの試料について 6 力月 後 まで以下の測定を行い、 保存前の試料と 比較 した。 試料の沈 澱形成の有無を 目視によ り 確認 した。 また試料 30 L を生 理食塩水で 10倍に希釈 し、 室温において 1 m mセ ルを用い て 900nm の吸光度を測定 した。 生理食塩水の 900nm の吸光 度を リ フ ァ レ ンス と して差 し引 き、 試料の濁度と した。 粒径 分布測定は、 25°Cにおいて、 Sub-micron Particle Analyzer Model N4 SD (Coulter Corporate Communications) ¾*用レヽて動 的散乱法によ り 測定した。
メ トヘモグロ ビン含量の増大は全く 認め られず、 保存 1 力 月 後はほぼ一定に推移 した。 粒径の増大についは、 保存 1 週 間後には 8%程度増大 して凝集によ る沈澱も若干観察された が、 未だ使用可能な状態であった。 これに対 して、 酸素除去 を行わなかった場合には、 保存 1 週間後には最早使用不能な 状態にまで沈殿を生 じるか ら、 酸素除去がへモグロ ビ ン小胞 体の安定化に も寄与 した こ と が分かる。
しか し、 実施例 1 の結果と の比較においては、 いずれの試 料も保存に伴って粒径が急激に増大 している。 この結果は、 へモグロ ビン小胞体の表面をポ リ ォキシエチ レンで修飾する こ と と 、 無酸素の条件下で保存する こ と と が相乗的に作用 し て、 更に顕著な保存安定性が得られた こ と を示 している。
実施例 3
実施例 1 と 同様に して調製 したポ リ ォキシエチ レ ン修飾へ モグ口 ビン小胞体分散液 (ポ リ ォキシエチ レ ンの分子量は 2 0 0 0 ) を実施例 1 記載の方法と 同様に調製 した。 こ のデォ キシ体を窒素雰囲気下でアルミ ニ ゥムバ ッ グ(Aluminized polyethylene bag, ジーエルサイ エ ンス社製)の中に移 して酸 素を遮断 し、 冷蔵保存(4°C)、 室温保存(23°C:)、 恒温槽内保存 (40°C )を行った。 それぞれの試料について 1 年後まで実施例 1 と 同様の測定を行い、 同様の結果を得た。
実施例 4
実施例 1 の調製法に記載のホモシスティ ンをグルタチオン に換え、 またポ リ ォキシエチ レン脂質のポ リ ォキシエチレン 鎖分子量を 10,000 と し、 同様に してポ リ オキシエチ レン修 飾へモグ口 ビン小胞体分散液(50mL)を調製 した。 これを円 筒型フ ラ ス コ(2L)に入れ、 ロ ータ リ ーエバポ レータ に装填 し て回転(60rpm)させる こ と によ り 生 じるへモグロ ビン小胞体 分散液の液膜に窒素を通気( 1. OL/min)し、 酸素を除去 した。 近赤外非侵襲酸素モニ タ (OM-200型、 島津)に よ って、 全へ モグ口 ビンの 98%以上がデォキシへモグロ ビンである こ と を確認した。 これを凍結保存用パ ッ ク Cryocyte(Baxter)に密 封 し、 更にこれをアルミ 缶に封入 して酸素の透過を遮断し、 保存試験を行った。 保存条件と しては、 冷蔵保存(4°C)、 室 温保存(23°C)、 恒温槽内保存(40°C)を設定 し、 それぞれの試 料について 1 年後まで実施例 1 と 同様の測定を行い、 同様の 結果を得た。
phenyl]porphynato iron(II) ( リ ピ 卜、ヘム ) Z l- stearylimidazole (モル比 1/2.5) に、 大豆油 ( [大豆油 ] [へ ム ] =2〜 4 重量比) を添加 し、 更に 2 %グ リ セ リ ン水溶液を 加えた後、 窒素雰囲気下において水浴下で超音波撹拌する こ と に よ り 行った。 こ の分散液に、 平均分子量 2000 のポ リ オ キシェチ レ ン結合脂質(N-
(monomethoxvpolyoxyethylenecarbamyl)dipalmitoylphosphatidyl ethanolamine)を、 リ ピ ドヘムに対 して 0.02mol%添カ卩 してボ リ オキシエチ レン修飾 した。 分散液 180mL を 200mL の硝子 容器に小過剰のァス コ ル ビン酸と共に封入 し、 実施例 1 記載 の方法と 同様に して窒素バブル して酸素分圧を 2Torr にまで 低下させ、 デォキシ型の リ ピ ドヘム - ト リ グ リ セ リ ド小球体 を得た。 これを室温にて 4 力月 間保存 した後の分析では、 鉄 三価のヘムの増大は認め られず、 また粒子径は保存前の 85 ± 25nm に対 して、 保存後 86 ± 28 nm であ り 、 殆ど変化は見 ら れなかった。
実施例 7
アルブ ミ ン-ヘムの調製は、 ヘム誘導体 (2-[8— {N-(2- methylimydazolyl) } octanoyloxymethyl]-5,10,15,20- tetrakisfa,a, ,a-o-pivalamido)phenylporphynato iron(II)) と ヒ ト血清アルブ ミ ンを も と に、 既報に従って行った ( E.
Tsuchida et al., Bioconjueate Chemistry, vol. 8, 534-538, 1997 : この文献は、 こ こ での引用に よ り 本明細書の一部をな すもの と して援用する) 。 ヘムが二価鉄の状態で酸素を結合 してレ、る こ と を確認 した後、 アルブ ミ ン -ヘム溶液を硝子容
器に封入 し、 実施例 1 に記載の方法で窒素を通気する こ と に よ り 酸素分圧を 3 To rr にまで低下させて、 デォキシ型のアル ブ ミ ンヘムを得た。 これを 20 °Cにて 5 力月 間保存 した後の 分析では、 三価鉄のヘムの増大は認め られず、 不溶物の増大 は認め られなかった。
比較例 1
実施例 1 と 同様に して調製 したポ リ オキシエチ レン修飾へ モグ口 ビン小胞体分散液の酸素分圧を、 無菌的雰囲気下にお いて、 大気 と 同 じ 1 49 To rr と した。 これをバイ アル瓶に密封 し、 酸素除去を行 う こ と な く 、 ォキシヘモ グ ロ ビンのままで 恒温槽内保存(40 °C )を行った。 保存 1 , 4,24 時間後にメ ト化率 を紫外可視吸収スぺク トルか ら測定した。 保存に伴ってメ ト 化率は上昇 し、 保存前の 2 . 7 %か ら保存 1 時間後 には 5 % に、 保存 4 時間後には 1 2 %に、 保存 2 4 時間後には 3 6 %に增 大 した。
こ の結果は、 ポ リ ォキシエチ レ ン修飾 したへモ グ ロ ビン小 胞体分散液でも 、 酸素を除去 しない場合にはメ トへモグ口 ビ ンの増大が著 し く 、 本発明 と 同様の保存安定性が得られない こ と を示 してレヽる。
Claims
1 . 水性媒質中に分散された脂質分子集合体と 、 該脂質分 子集合体に含まれるヘムまたはヘム誘導体を含有する水性分 散液か らなる酸素輸液剤の保存方法であって :
前記水性分散液から酸素を除去 し、 前記ヘムまたはへ ム誘導体をデォキシ型にする こ と を特徴とする酸素輸液剤の 保存方法。
2 . 請求項 1 に記載の方法であって、 更に、 酸素を除去さ れた後の前記水性分散液を不活性ガス雰囲気中で保存する こ と を特徴とする酸素輸液剤の保存方法。
3 . 請求項 1 に記載の方法であって、 更に、 前記脂質分子 集合体表面をポ リ ォキシエチ レンで修飾する こ と を特徴とす る酸素輸液剤の保存方法。
4 . 請求項 1 に記載の方法であって、 前記脂質分子集合体 が、 へモグロ ビン小胞体、 リ ピ ドヘム小胞体、 およびリ ピ ド ヘム 一 ト リ グ リ セ リ ド小球体から なる群から選択される酸素 輪液剤の保存方法。
5 . 請求項 1 に記載の方法であって、 前記酸素の除去は、 不活性ガス と のガス交換に よ り 行われる こ と を特徴とする酸 素輸液剤の保存方法。
6 . アルブ ミ ン ヘムを含有する水溶液から なる酸素輸液 剤の保存方法であって :
前記水溶液か ら酸素を除去 し、 前記ヘムまたはヘム誘 導体をデォキシ型にする こ と を特徴とする酸素輸液剤の保存 方法。
7 . 請求項 6 に記載の方法であって、 前記酸素の除去は、 不活性ガス と のガス交換によ り 行われる こ と を特徴とする酸 素輸液剤の保存方法。
8 . 請求項 6 に記載の方法であって、 更に、 酸素を除去さ れた後の前記水溶液を不活性ガス雰囲気中で保存する こ と を 特徴と する酸素輸液剤の保存方法。
9 . 水性媒質中に分散された脂質分子集合体と 、 該脂質分 子集合体に含まれるヘムまたはヘム誘導体を含有する酸素輸 液剤であって :
前記脂質分子集合体表面がポ リ オキシエチ レ ンで修飾 されてレ、る こ と と ;
前記ヘムまたはヘム誘導体がデォキシ型である こ と と ;
前記酸素輪液剤が、 不活性ガス を充填 した酸素不透過 性の容器内に収容されている こ と を特徴とする酸素輸液剤。
1 0 . 請求項 9 に記載の酸素輸液剤であって、 更に、 生理 学的に許容可能な還元剤を含有する こ と を特徴とする酸素輸 液剤。
1 1 . アルブ ミ ン 一ヘムを含有する水溶液か らなる酸素輸 液剤であって :
前記ヘムまたはヘム誘導体がデォキシ型である こ と と ;
前記酸素輸液剤が、 不活性ガス を充填 した酸素不透過 性の容器内に収容 さ れている こ と と を特徴 と する酸素輸液剤 c

1 3 . 水性媒質中に分散された脂質分子構造体と 、 該脂質 分子集合体に含まれるヘムまたはヘム誘導体を含有する水性 分散液からなる酸素輸液剤の製造方法であって :
表面がポ リ オキシエチ レ ンで修飾され、 且つ前記ヘム またはヘム誘導体を含んだ前記脂質分子集合体の水性分散液 を調製する工程と ;
該水性分散液から酸素を除去する こ と によ り 、 前記へ ムまたはヘム誘導体をデォキシ型にする工程と ;
前記ヘムまたはヘム誘導体をデォキシ型に した水性分 散液を酸素不透過性の容器に収容する と と も に、 該容器内に 不活性ガス を充填する工程を具備 したこ と を特徴とする酸素 輸液剤の製造方法。
1 4 . 請求項 1 3 に記載の方法であって、 前記酸素の除去 は、 不活性ガス と のガス交換によ り 行われる こ と を特徴とす る酸素輸液剤の製造方法。
1 5 . アルブ ミ ン一ヘムを含有する水溶液からなる酸素輸 液剤の製造方法であって :
アルブミ ンーヘムを含有する水溶液を調製する工程 と ;
該水溶液から酸素を除去する こ と によ り 、 前記ヘムま たはヘム誘導体をデォキシ型にする工程と ;
前記ヘムまたはヘム誘導体をデォキシ型に した水溶液 を酸素不透過性の容器に収容する と と もに、 該容器内に不活
性ガス を充填する工程を具備 した こ と を特徴とする酸素輸液 剤の製造方法。
1 6 . 請求項 1 5 に記載の方法であって、 前記酸素の除去 は、 不活性ガス と のガス交換によ り 行われる こ と を特徴とす る酸素輸液剤の製造方法。
