Abstract
Ein Computer-Tomograph besteht aus einem Gehäuse (1) mit einer Patientenaufnahmeöffnung (2), in der rechnergesteuerte Abtasteinrichtungen zur Erzeugung von Computer-Tomogrammen angeordnet sind und in die von der Vorderseite des Gehäuses (1) her eine motorisch angetriebene und ebenfalls rechnergesteuerte Patientenliege (3) einfahrbar ist. Damit bei der Behandlung von Patienten zwischen Diagnose und Eingriff möglichst wenig Information verloren geht und der Eingriff bzw. die Behandlung am Patienten örtlich und zeitlich möglichst nahe an der Diagnose liegen, ist die Patientenliege (3) zugleich als Operationstisch ausgebildet. An der Vorderseite des Gehäuses (1) sind mehrere mit dem Rechner verbundene Informations-Displays, insbesondere in Form von Bildschirmen (9), angeordnet. Vor dem Gehäuse (1) ist ein Operations- und Behandlungsplatz für den Patienten vorgesehen, dem weitere Diagnose- und/oder Eingriffs- und/oder Therapiehilfsmittel zugeordnet sind.
Description
Computer-Tomograph
Die Erfindung betrifft einen Computer-Tomographen, be¬ stehend aus einem Gehäuse mit einer Patientenaufnah- meöffnung, in der rechnergesteuerte Abtasteinheiten zur Erzeugung von Computer-Tomogrammen angeordnet sind und in die von der Vorderseite des Gehäuses her eine moto¬ risch angetriebene und ebenfalls rechnergesteuerte Pa¬ tientenliege einfahrbar ist.
Die nach dem Stande der Technik üblichen Computer-Tomo¬ graphen werden ausschließlich zu Diagnosezwecken ver¬ wendet. Sie erzeugen in verschiedenen Schnittebenen angeordnete Schnittbilder durch den Patientenkörper. Die üblichen Abtaεteinheiten von Computer-Tomographen arbeiten mit Röntgenstrahlen oder auf der Basis einer Messung und Auswertung der magnetischen Kernspinreso¬ nanz in dem zu untersuchenden Körpergewebe des Patien¬ ten. Eine Besonderheit der Computer-Tomographen besteht darin, daß die Tomogramme nicht sofort, sondern erst mit einigem Zeitverzug erstellt werden können, weil der Rechenalgorhithmus für die Auswertung der Abtast-Me߬ werte sehr umfangreiche Rechenoperationen erfordert. Aufgrund der hohen Leistungsfähigkeit moderner Datenver¬ arbeitungsanlagen beträgt dieser Zeitverzug allerdings nur noch wenige Sekunden.
Bei der herkömmlichen Verwendung von Computer-Tomograp¬ hen ist nachteilig, daß die Diagnose einerseits und der chirurgische Eingriff bzw. die gezielte Behandlung des Patienten andererseits räumlich und zeitlich voneinan¬ der getrennt sind, so daß die Gefahr besteht, daß zwischen dem Erstellen der Diagnose und dem Eingriff am Patienten Information verloren geht, deren Kenntnis Voraussetzung für den Erfolg des Eingriffes oder der Behandlung ist. Besondere Schwierigkeiten entstehen dadurch, daß der Patient auf dem Operationstisch, auf welchem die eigentliche Operation vorgenommen wird, meistens eine völlig andere Körperstellung als bei der Diagnose im Computer-Tomographen einnimmt, wodurch es schwierig wird, bei der Operationsplanung und bei der nachfolgenden Operation die zu behandelnden Organe und Körperteile genau zu lokalisieren.
Es ist deshalb Aufgabe der Erfindung, den Co puter-To-. mographen der eingangs genannten Art dahingehend wei¬ terzubilden, daß zwischen Diagnose und Eingriff mög¬ lichst wenig Information verloren geht und der Eingriff bzw. die Behandlung am Patienten örtlich und zeitlich möglichst nahe an der Diagnose liegt, um eine bessere und genauere Planung und Durchführung der Operation oder Behandlung zu ermöglichen.
Zur Lösung dieser Aufgabe schlägt die Erfindung ausge¬ hend vom Computer-Tomographen der eingangs genannten Art vor, daß die Patientenliege zugleich als Operations¬ tisch ausgebildet ist, daß an der Vorderseite des Gehäuses mehrere mit dem Rechner verbundene Informa¬ tions-Displays, insbesondere in Form von Bildschirmen angeordnet sind und daß vor der Vorderseite des Gehäu¬ ses ein Operations- und Behandlungsplatz für den Patien¬ ten angeordnet ist, dem weitere Diagnose- und/oder Ein¬ griffs- und/oder Therapiehilfsmittel zugeordnet sind.
Der Computer-Tomograph gemäß der Erfindung gestattet es, den Patienten unmittelbar vor dem Computer-Tomo¬ graphen, d. h. unmittelbar nach der Diagnose mit Hilfe von Computer-Tomogrammen und ohne den Patienten auf einen gesonderten Operationstisch umbetten zu müssen, zu operieren oder sonstwie zu behandeln. Für den Ein¬ griff oder - die Behandlung können auch besondere Ein¬ griffs- und/oder Therapiehilfsmittel verwendet werden, wie z. B. ein Laser-Skalpell, ein Lithotripter oder Geräte für die Strahlentherapie etc. Dabei stehen dem behandelnden Arzt auf den Bildschirmen an der Frontsei¬ te des Gehäuses des Computer-Tomographen jeweils ak¬ tuelle Informationen aus der soeben gestellten Diagnose zur Verfügung. Außerdem können auf die Informations- Displays auch weitere Patientendaten eingespielt werden oder weitere Bilder dargestellt werden, die für den Arzt bei der Behandlung oder dem Eingriff hilfreich sind.
Zweckmäßig ist an der Vorderseite des Gehäuses oberhalb der Patientenliege eine Röntgenröhre und oberhalb der Patientenliege ein Röntgenbildverstärker zur Erzeugung eines Röntgenbildes des auf der Liege liegenden Patien¬ ten angeordnet, welches auf einen Bildschirm des Infor¬ mations-Displays darstellbar ist. Dieses zusätzliche Röntgenbildgerät gestattet es, dem behandelnden Arzt vor dem Eingriff oder vor der Behandlung noc • einmal ein aktuelles Röntgenbild des auf der Patientenliege liegenden Patienten zu geben, und zwar ein Röntgenbild, welches sich in einer anderen Ebene erstreckt, als die zuvor erstellten Computer-To ogramme.
Weiterhin sind die Röntgenröhre einerseits und der Röntgenbildverstärker andererseits allseitig verstell¬ bar an der Vorderseite des Gehäuses des Computer-Tomo¬ graphen befestigt. Hierdurch ist es möglich, Röntgen- bilder von allen möglichen Details und in allen denkba¬ ren Ebenen herzustellen.
Außerdem ist an der Vorderseite des Gehäuses an einem allseitig beweglichen und rechnergesteuerten Arm ein Positionsanzeiger angeordnet, der mindestens einen auf die Eingriffs- oder Behandlungsstelle am Patientenkör¬ per gerichteten Lichtstrahl erzeugt. Dieser Positions¬ anzeiger wird vom zentralen Rechher der Gesamtanlage gesteuert und kann zu seiner Steuerung Informationen aus den Computer-Tomogrammen oder den anderen im Rech¬ ner gespeicherten Patientendaten verwenden.
Zweckmäßig erzeugt der Positionsanzeiger mehrere Licht¬ strahle, die überkreuz oder im Winkel zueinander ver¬ laufen und dem Operateur die Eingriffsstelle und z. B. die Eingriffsrichtung anzeigen.
Zweckmäßig ist der Lichtstrahl ein Laserstrahl, der gegebenenfalls auch zur Behandlung der Körperoberfläche verwendet werden kann (Lasertherapie) .
Eine vorteilhafte Weiterbildung sieht vor, daß dem Ope¬ rations- und Behandlungsplatz ein Ultraschall-Bildgerät zugeordnet ist, dessen Bilder ebenfalls auf einen Bild¬ schirm des Informations-Displays an der Vorderseite des Gehäuses des Computer-Tomographen einspielbar sind. Ein solches Ultraschall-Bildgerät kann dem behandelnden Arzt weitere wertvolle Informationen liefern.
Ebenso kann dem Operations- und Behandlungsplatz auch ein Endoskop zugeordnet sein, dessen endoskopische Bil¬ der ebenfalls auf einem Bildschirm des Informations- Displays darstellbar sind. Auch hierdurch werden dem Arzt für seinen Eingriff weitere wertvolle Informatio¬ nen geboten.
Die Bewegungen der Patientenliege sind zweckmäßig sprachgesteuert. Hierdurch ist es möglich, die notwen¬ digen Verschiebungen oder Bewegungen der Patientenliege vorzunehmen, ohne irgendwelche Schalter oder derglei¬ chen zu berühren.
Besondere Vorteile ergeben sich, wenn weitere wesentli¬ che Fμnktionen des Computer-Tomographen und der Diagno¬ se- und/oder Eingriffs- und/oder Therapiehilfsmittel sprachgesteuert sind. Hierdurch ergeben sich im Prinzip die gleichen Vorteile, wie bei der Sprachsteuerung der Patientenliege.
Alternativ können bestimmte Funktionen des Computer- Tomographen und der Diagnose- und/oder Eingriffs- und/ oder Therapiehilfsmittel auch körperbewegungsge¬ steuert sein. Auch diese Art der Steuerung der Funktio¬ nen erspart im Einzelfall die Betätigung von Schaltern oder dergleichen.
Um den Computer-Tomographen gegebenenfalls für ver¬ schiedene Operationen bzw. Behandlungen umrüsten zu können, sind zweckmäßig alle dem Computer-Tomographen zugeordneten Diagnose- und/oder Eingriffs- und/oder Therapiehilfsmittel modular aufgebaut.
Ein Ausführungsbeispiel der Erfindung wird im folgenden anhand der Zeichnung näher erläutert. Es zeigen:

Fig. 2 eine Vorderansicht des Computer- Tomographen.
In der Zeichnung ist das Gehäuse des Computer-Tomograp¬ hen mit dem Bezugszeichen 1 bezeichnet. In dem Gehäuse 1 befindet sich eine im Querschnitt runde Patientenauf- nahmeöffnung 2, in welche eine Patientenliege 3 ein¬ schiebbar ist. Der Patient ist in der Zeichnung mit dem Bezugszeichen 4 bezeichnet. Die Patientenliege 3 mit dem darauf liegenden Patienten 4 ist mittels eines nicht näher dargestellten motorischen Antriebes rech¬ nergesteuert in die Patientenaufnahmeöffnung 2 einfahr¬ bar und aus dieser Öffnung 2 herausfahrbar, wie durch, den Doppelpfeil 5 angedeutet ist. Alle Funktionen des Computer-Tomographen werden von einem in der Zeichnung nicht dargestellten Rechner gesteuert. Insoweit ent¬ spricht der in der Zeichnung dargestellte Computer-To¬ mograph dem Stand der Technik.
Erfindungsgemäß ist die Patientenliege 3 zugleich als Operationstisch ausgebildet und hat dementsprechend die bei Operationstischen üblichen Einrichtungen zur Fixie¬ rung des Patienten in einer bestimmten Lage. Weiterhin befindet sich erfindungsgemäß vor der Patientenaufnah¬ meöffnung 2 des Gehäuses 1 ein Operations- und Behand- lungsplatz für den Patienten 4. Diesem Operations- und Behandlungsplatz sind Eingriffs- und/oder Therapie¬ hilfsmittel zugeordnet, die in der Zeichnung stark vereinfacht dargestellt sind und zusammenfassend mit dem Bezugszeichen 6 bezeichnet sind.
Im Bereich des Operations- und Behandlungsplatzes, d. h. an der Vorderseite des Gehäuses 1, sind als weitere Diagnose-Hilfsmittel eine Röntgenröhre 7 und ein Rönt- genbildverstärker 8 zur Erzeugung eines Röntgenbildes des auf der Patientenliege 3 liegenden Patienten 4 angeordnet. Die Röntgenröhre 7 ist allseitig verstell¬ bar ausgebildet und entlang einem Halbkreis um die Pa- tieήtenaufnahmeöffnung 3 herum verfahrbar. Auch der Röntgenbildverstärker 8 ist allseitig verstellbar, um im Zusammenwirken mit der Röntgenröhre 7 verschiedene Röntgenbilder des Patienten 4 herstellen zu können.
An der Vorderseite des Gehäuses 1 sind weiterhin mehre¬ re Informations-Displays in Form von Bildschirmen 9 angeordnet, auf denen die Computer-Tomogramme des Com¬ puter-Tomographen, das Röntgenbild und gegebenenfalls weitere für den behandelnden Arzt wichtige Patientenda¬ ten darstellbar sind.
Weiterhin ist an der Vorderseite des Gehäuses 1 ober¬ halb des Operations- und Behandlungsplatzes ein allsei¬ tig beweglicher und rechnergesteuerter Arm 10 eines Positionsanzeigers 11 angeordnet. Dieser Positionsan¬ zeiger 11 erzeugt einen oder mehrere auf die Eingriff- εoder Behandlungsstelle am Patienten 4 gerichtete Lichtstrahle, die überkreuz oder im Winkel zueinander verlaufen und dem Operateur die Eingriffsstelle und gegebenenfalls die Eingriffsrichtung am Körper des Pa¬ tienten 4 anzeigen. Die Lichtstrahle können gegebenenfalls auch Laserstrahle sein, die für Ein¬ griffsund/oder Therapiezwecke verwendbar sind. Dem Ope¬ rations- und Behandlungsplatz kann weiterhin ein Ultra¬ schallsichtgerät 12 zugeordnet sein, dessen Bilder auf einen Bildschirm 9 des Informations-Displays einspiel¬ bar sind.
Alle Bestandteile des oben beschriebenen Systems sind modular aufgebaut, so daß der Operations- und Behand¬ lungsplatz auf einfache Weise für die verschiedensten Operationen und Behandlungen hergerichtet werden kann.
Weiterhin sind alle oben erwähnten Einrichtungen an den zentralen Rechner des Computer-Tomographen angeschlos¬ sen. Dieser Rechner kann alle Bewegungen steuern und die verschiedenen Informations-Funktionen aufrufen. Dieser in der Zeichnung nicht näher dargestellte Rech¬ ner kann herkömmlich durch die üblichen Tastaturen angesteuert werden, kann aber für die Steuerung be¬ stimmter Funktionen auch sprachgesteuert oder körperbe¬ wegungsgesteuert ausgeführt sein. Die Sprachsteuerung ist insbesondere für die Bewegungen der zugleich als Operationstisch dienenden Patientenliege 3 vorteilhaft. Sie ermöglicht es nämlich dem operierenden Arzt, sich voll auf die Operation zu konzentrieren und etwa not¬ wendige Bewegungen der Patientenliege durch sprachliche Befehle zu bewirken.
Claims
1. Computer-Tomograph, bestehend aus ei¬ nem Gehäuse mit einer Patientenaufnahmeöffnung, in der . rechnergesteuerte Abtasteinrichtungen zur Erzeugung von Computer-Tomogrammen angeordnet sind und in die von der Vorderseite des Gehäuses her eine motorisch angetriebe¬ ne und ebenfalls rechnergesteuerte Patientenliege ein¬ fahrbar ist, d a d u r c h g e k e n n z e i c h n e t , daß die Patientenliege (3) zugleich als Operationstisch ausgebildet ist, daß an der Vorderseite des Gehäuses (1) mehrere mit dem Rechner verbundene Info mations- Displays insbesondere in Form von Bildschirmen (9) angeordnet sind und daß vor der Vorderseite des Gehäu¬ ses (1) ein Operations- und Behandlungsplatz für den Pa¬ tienten (4) angeordnet ist, dem weitere Diagnose und/ oder Eingriffs- und/oder Therapiehilfsmittel zugeordnet sind.

Computer-Tomograph nach Anspruch 1, dadurch gekennzeichnet, daß an der Vorderseite des Gehäuses (1) oberhalb der Patientenliege (3) eine Rönt¬ genröhre (7) und unterhalb der Patientenliege ein Rönt- genbildverstärker (8) zur Erzeugung eines Röntgenbildes des auf der Liege befindlichen Patienten (4) angeordnet ist, welches auf einem Bildschirm (9) des Informations- Displays darstellbar ist.
3. Computer-Tomograph nach Anspruch 2, dadurch gekennzeichnet, daß die Röntgenröhre (7) und der Rönt- genbildverstärker (8) rechnergesteuert allseitig ver¬ stellbar sind.
4. Computer-Tomograph nach einem der Ansprüche
1 bis 3, dadurch gekennzeichnet, daß an der Vorderseite des Gehäuses (1) an einem allseitig beweglichen und rechnergesteuerten Arm (10) ein Positionsanzeiger (11) . angeordnet ist, der mindestens einen auf die Eingriffs¬ oder Behandlungsstelle am Patientenkörper gerichteten Lichtstrahl erzeugt.
5. Computer-Tomograph nach Anspruch 4, dadurch gekennzeichnet, daß der Lichtstrahl ein Laserstrahl ist.
6. Computer-Tomograph nach den Ansprüchen 4 oder 5, dadurch gekennzeichnet, daß der Positionsanzei¬ ger (11) mehrere Lichtstrahle erzeugt, die überkreuz oder im Winkel zueinander verlaufen.
7. Computer-Tomograph nach einem der Ansprüche
1 bis 6, dadurch gekennzeichnet, daß dem Operations- und Behandlungsplatz ein Ultraschallsichtgerät zugeordnet ist, dessen Bilder auf einem Bildschirm (9) des Infor- mations-Displays darstellbar sind.
8. Computer-Tomograph nach einem der Ansprüche
1 bis 7, dadurch gekennzeichnet, daß dem Operations- und Behandlungsplatz ein Endoskop zugeordnet ist, dessen endoskopische Bilder auf einem Bildschirm (9) des In¬ formations-Displays darstellbar sind.
9. Computer-Tomograph nach Anspruch 1, dadurch gekennzeichnet, daß die Bewegungen der Patientenliege (3) sprachgesteuert sind.
10. Computer-Tomograph nach einem oder mehreren der Ansprüche 1 bis 9, dadurch gekennzeichnet, daß einige Funktionen des Computer-Tomographen und der Diagnose- und/oder Eingriffs- und/oder Therapiehilfsmittel sprachgesteuert sind.
11. Computer-Tomograph nach einem oder mehreren der Ansprüche 1 bis 9, dadurch gekennzeichnet, daß ei¬ nige Funktionen des Computer-Tomographen und der Diag¬ nose- und/oder Eingriffs- und/oder Therapiehilfsmittel körperbewegungsgesteuert sind.
12. Computer-Tomograph nach einem der Ansprüche
1 bis 11, gekennzeichnet durch eine Modulbauweise aller Baugruppen der Diagnose- und/oder Eingriffs- und/oder Therapiehilfsmittel.
