Description

把桿保固於骨骼固定裝置的容納部內之鎖定總成及其骨骼固定裝置和操作工具
本發明係關於一種鎖定構件,可將桿構件保固於連接在脊椎或外傷手術用幹部之容納件。本發明又關於使用此類鎖定總成之骨骼固定裝置,以及與此類鎖定總成合作之工具。
US 6,224,598 B1揭示一種有螺紋的封塞,適用於把桿構件保固於骨釘植入體,該封塞包括塞件,有圓筒形的螺紋外表面,該塞件於使用中容納在醫學植入體之一對臂間,有中央共軸腔孔完全通過該塞件,該中央腔孔有陰螺紋表面,其形狀可容納固定螺釘。封塞和固定螺釘可單獨安裝,而固定螺釘旋緊時可合作提供捕捉和鎖定桿,以便保固桿,對抗相對於骨釘移動和轉動。
US 2003/01 008 96 A1揭示一種骨骼固定裝置,有幹部和相連接之容納件,供連接於桿。容納件具有凹部,呈U形截面,可容納桿,有二開口股部,以及開口股部上之陰螺紋。設有鎖定總成,包括有陽螺紋之螺帽構件,可與股部和固定螺釘之陰螺紋嚙合。螺帽構件一端有細縫,與螺釘工具結合。幹部有球形頭部,可樞動自如保持於容納件內,設有施壓元件,在螺帽構件旋緊時,對頭部施壓。旋緊固定螺釘時,桿即固定於容納件內。因此,桿和頭部可彼此單獨鎖定。陰螺紋和螺帽構件嚙合之陽螺紋,設計成平板螺紋。植入體呈精巧設計,因為不必要外環或螺帽來防止股部張開。
鎖定總成的外徑呈諸種層面,以所需旋緊扭矩和螺紋形式來決定。因而,骨骼固定裝置的上部總體維度,以鎖定總成的尺寸決定。
本發明之目的,在於提供一種鎖定總成,以及具有鎖定總成之骨骼固定裝置,與已知裝置同具可靠性,但上部維度大為減小。再者,本發明目的在於提供此類鎖定總成用之工具。
本發明鎖定總成的優點是,可設計比已知鎖定總成更小的外徑。所以,可以縮小骨骼固定裝置的尺寸。縮小尺寸之骨骼固定裝置特別適合應用在空間有限而需要精巧植入體之頸椎或其他區位。
本發明進一步特點和優點,參見附圖所示具體例詳述如下,即可清楚明白。
再者,鎖定總成可構成容許定置二或以上之鎖定元件。
以本發明工具可同時但單獨把鎖定總成之鎖定元件加以固定。
第1圖和第2圖表示多軸骨骼裝置1所用鎖定總成。骨骼固定裝置包括骨釘2,具有骨螺紋之幹部3,和球形頭部4。骨釘2容納在實質上圓筒形結構之容納件5,後者具有第一端6和第二端7。二端均垂直於縱軸線L。設有與縱軸線L共軸之腔孔8,從第一端6延伸到離第二端7預定距離處。在第二端7設有開口9,其直徑比腔孔8直徑小。共軸腔孔8朝開口9斜縮。在圖示具體例中,以球形截面10之形式斜縮。然而,截面10可有任何其他形狀,諸如錐形。
容納件5又有U形凹部11,在第一端6開始,按第二端7方向延伸,至離該第二端預定距離處。利用U形凹部,形成二自由股部12,13,尾端朝第一端2。容納件5鄰近第一端6包括在股部12,13內表面之陰螺紋。在圖示具體例中,陰螺紋14為平板螺紋,具有水平之上、下螺腹。
又設有施壓元件15,具備實質上圓筒形構造,其外徑尺寸容許施壓元件15引進入容納件之腔孔8內,並在軸向運動。施壓元件15在朝向第二端7的下側,包括球形凹部16,與頭部4之球形截面合作。施壓元件15在相反側具有U形凹部17,橫越縱軸線L延伸,藉此形成二自由股部18,19。選擇此U形凹部之側向直徑,使要容納於容納件5內的桿20,可插入凹部17內,在其內側向導引。此U形凹部17的深度,比桿20直徑大,在桿20插入時,使股部18,19在桿20表面上方延伸。
骨骼固定裝置包括鎖定總成30。特別參見第2至5圖,鎖定總成30包含第一鎖定元件31和第二鎖定元件32。第一鎖定元件有第一端33和第二端34,第一端和第二端之間為實質上圓筒形,外表面有陽螺紋35,在圖示具體例中為平板螺紋,與容納件5之陰螺紋14相配。再者,第一鎖定元件包括共軸腔孔36,從第二端34按第一端33方向延伸。共軸腔孔36包括陰螺紋,在圖示具體例內為公制螺紋。第一鎖定元件31又包括共軸凹部37,從第一端33開始,延伸至離第二端34預定距離。凹部37的平均直徑,比共軸腔孔36直徑大。特別如第3圖所示,藉凹部37形成實質上環形壁。形成複數縱向凹溝38,從第一端33沿壁延伸至凹部37的底部39。本具體例所示凹溝38具有大致半圓形截面。等距分佈於凹部37周緣方向。最好形成二凹溝。凹部37壁鄰接第一端33有傾斜表面40,以便利工具進入。選擇凹部37的深度,使腔孔36的長度仍然足以與第二鎖定元件32合作,以供良好固定。另方面,凹部37的深度使具有足夠與工具結合之面積。
第二鎖定元件32狀如固定螺釘,有陽螺紋與共軸腔孔36的陰螺紋嚙合。第二鎖定元件32的軸向長度,是在第二鎖定元件32完全旋入第一鎖定元件31內時,會從第一鎖定元件的第二端34稍微突出。尤其在第4圖內可見,第二鎖定元件32包括共軸凹部43,有凹溝在縱向延伸,與第一鎖定元件之凹部37和凹溝38類似。凹部43和凹溝44用於與工具以形式套合方式合作,詳後。
與鎖定總成合作用之工具,如第6至9圖所示。工具50包括管51和可在管51內滑動之棒52。與鎖定總成合作之管51有末段53,和具有六角形外部形狀的握部58之第二端。特別如第7至9圖所示,末段53有縮小之外徑,相當於第一鎖定元件的凹部37之內徑。末段53包括複數凸部54,其數目少於或等於第一鎖定元件之凹溝38數目。凸部54的構造和設計,是與第一鎖定元件之凹部38結合,在工具末段53和第一鎖定元件的凹部37間提供形式套合方式之連接。末段53之軸向長度,最好等於或大於凹部37之深度。
棒52包括末段55,其構造和設計在於與第二鎖定元件32之凹部43合作。為此目的,末段55具有複數凸部56,數目等於或少於第二鎖定元件之凹溝44數目,其構造和設計是與凹溝44結合。棒52在其相反端具有握部57,得以握棒52,並與管51分開轉動。選擇棒52的長度,在管的末段53與第一鎖定元件結合時,第二鎖定元件32可利用棒的末段55單獨結合,並螺入第一鎖定元件內。
操作時,首先把往往預先組合之至少二骨骼固定裝置(包括骨釘2、容納件5和施壓元件15),旋入骨骼內。然後,把桿20插入容納件5之U形凹部11內。再將鎖定總成30(包括最好是預組成之第一鎖定元件31和第二鎖定元件32),旋入容納件5的股部12,13之間。運用工具50旋緊第一鎖定元件,使管的末段53與第一鎖定元件之凹部37和凹溝38結合,形成形式套合之連接。以此方式,由第一鎖定元件之下側對施壓元件之自由股部18,19施壓,壓緊骨釘2之頭部4,把頭部鎖定在相對於容納件之轉動位置。
然後,應用工具把第二鎖定元件32旋緊,即棒之末段56與第二鎖定元件之凹部43結合,施以扭矩。如此一來,桿20相對於容納件之位置即被固定。
容納件5相對於骨釘2和桿20相對於容納件之位置,可藉旋鬆第一鎖定元件31或第二鎖定元件32,進行微調。
第9圖表示鎖定總成30之透視圖,工具之末段53和56與鎖定元件結合,工具以斷面表示。純為圖示起見,工具之其餘部份略而不示。由第9圖可見,工具之末段和第一、第二鎖定元件之凹部,形成形式套合連接,可供應用扭矩,把鎖定元件旋入。第一鎖定元件之陽螺紋35呈連續形式,無凹部或間斷。所以,鎖定元件之維度可縮小。如此可保證第一鎖定元件之安全鎖定。工具與第一鎖定元件結合所需面積,位在凹部37內。如此得以設計縮小直徑之第一鎖定元件31。若凹溝38數目增加,凹溝深度可縮小。因此,第一鎖定元件尺寸可減小,相當於凹溝數量增加。
第一鎖定元件31的外部維度,決定骨骼固定裝置的容納件和其他元件的尺寸。此外,由於第一鎖定元件的陽螺紋全長無缺,故鎖定元件的高度可以縮減。
第10圖表示第一鎖定元件31具有修飾形狀的凹溝。凹溝38'呈三角形截面。第11圖表示第一鎖定元件31具有另外修飾形狀之凹溝。凹溝38"呈方形截面。然而,本發明不限於此等修飾,凹溝截面亦可有其他任何形狀。
第12圖表示鎖定總成之第二具體例。鎖定總成300包括三個鎖定元件。第一鎖定元件301狀似第一具體例之鎖定元件31。第二鎖定元件302與第一具體例的第二鎖定元件30不同,在於具有共軸螺紋腔孔,一如第一鎖定元件,容納第三鎖定元件303。亦可設計鎖定總成具有三個以上的置入鎖定元件,使各鎖定元件具有共軸螺紋腔孔,以容納又一鎖定元件。以此方式,對某些用途而言,例如用於複雜的最少侵入性手術程序,可達成改進固定。
又一具體例之鎖定總成,包含單一鎖定元件400,具有陽螺紋401,和共軸腔孔402,從第一端延伸貫穿整個鎖定元件,至第二端404。共軸環形凹部405從第一端403,按第二端方向延伸。凹部壁包括複數縱向凹溝406,可與工具結合。利用凹部405和共軸腔孔402,在鎖定元件內形成共軸空心圓筒形段407。
與鎖定元件結合之工具,適於與環形凹部405結合。第16圖表示與鎖定元件400合作之工具500例。末段501包括環形凸部502,適於以形式套合方式與凹部405結合。末段501亦包括中央凸部503,有扣簧504與共軸腔孔402結合,方便鎖定元件之對準和處置。
鎖定元件可用於必須經腔孔402引進儀器或金屬絲之用途,例如最少侵入性手術之情況。
可以有進一步變化,陽螺紋和陰螺紋可以有任何螺紋形狀,例如公制螺紋。然而,對第一鎖定元件的陽螺紋和容納件之合作陰螺紋,使用平板螺紋、鋸齒螺紋或負角度螺紋,其優點是防止容納件的股部張開。所以,不需外環或螺帽來防止張開。使用本發明鎖定總成連同平板螺紋做為陽螺紋,可進一步把植入體降低尺寸。
凹溝數量和凹溝形狀可以變化。
第一鎖定元件亦可設計成凹部具有四角形、六角形或其他多角形截面,有無凹溝均可。在此情況下,工具有末段具有相配合形狀。如此亦可在工具和第一鎖定元件之間提供形式套合之連接,而第一鎖定元件之陽螺紋保留完整無缺。
就第二具體例的鎖定總成300而言,第二鎖定元件32或第三鎖定元件303,或就具有複數鎖定元件之鎖定總成而言,最內的鎖定元件不需有具體例所示帶凹溝之凹部。最內的鎖定元件具有可與旋動工具結合之凹部(諸如六角形凹部)即夠。工具之相對應末段即適於此形狀。
本發明不限於第一具體例所示多軸骨骼固定裝置。亦可用於單軸骨骼固定裝置,其中容納件係固定連接於骨釘之幹部。此外,多軸骨骼固定裝置可有不同構造,容納件可設計成把螺釘從容納件底部以代替從頂部插入。
鎖定總成亦可用於如此骨骼固定裝置,即把容納件設計並構造成把桿偏離骨釘中央軸線,在側向固定。

Claims (16)

  1. 一種把一桿(20)保固於一骨骼固定裝置(1)的一容納部內之鎖定總成,可用於脊椎或外傷手術,該鎖定總成(30;300)包含:一第一鎖定元件(31;301),包含一第一端(33)和一第二端(34),一縱向轉動軸線,一圓筒形外表面,具有一陽螺紋(35),一共軸腔孔(36)完全通過該第一鎖定元件(31;301),以及一陰螺紋(14),設於該腔孔;一第二鎖定元件(32;302),包含一縱向轉動軸線,和一圓筒形外表面,具有一陽螺紋(42)與該第一鎖定元件之該陰螺紋(14)嚙合;其中該第一鎖定元件(31;301)具有一凹部(37),從該第一端(33)朝該第二端(34)的方向延伸,該凹部界定一沿周封閉壁部,其中該壁部內面有一縱向延伸結構(38),可與一工具結合;其中該凹部(37)的平均直徑,比該共軸腔孔直徑大,得以在該凹部(37)和該共軸腔孔(36)之間形成底部表面(39),而該第二鎖定元件(32)可單獨結合並螺入該第一鎖定元件內者。

  8. 如申請專利範圍第1或2項之鎖定總成,其中該第二鎖定元件包括一共軸腔孔,具有一陰螺紋,以容納一第三鎖定元件(303),後者具有一陽螺紋,與該陰螺紋嚙合者。
