Description
本發明係關於一種骨骼固定總成,可用於脊柱手術或創傷手術。具體而言,本發明係關於一種骨骼固定總成,包括至少二骨骼固定元件,有容納元件供容納桿,該桿係連接骨骼固定元件之用,其中桿是利用形式鎖定連接方式,固定於容納組件內。
US 5,207,678號揭示一種梗釘,包括螺釘,以及容納構件,把螺釘與桿連接。桿是螺紋桿,利用結合容納構件外側,且可在螺紋桿上調節之二螺帽,鎖定在容納構件內。為再調節桿相對於容納構件之位置,螺帽可在螺紋桿上轉動。此項程序費時,因為始終有二螺釘需要調節。
US 5,443,467號揭示一種骨骼固定總成,包括骨骼固定元件,有容納組件供容納桿,又有內螺釘和外鎖定螺帽,可將桿保固和固定於容納組件內。桿具有滑順表面。桿的固定是利用摩擦式鎖定達成。即使桿需要暫時鎖定,容許在稍後步驟中再調節,仍必須應用相當的力矩,才能達成充分的夾持力。
US 5,716,356號揭示一種固定構件,要與螺紋桿連接。固定構件有軸要固定於骨骼內,並有U形斷面的容納組件,以容納桿。利用凹部形成二開口股部,設有陽螺紋和陰螺紋。設有螺帽構件,要螺合於陽螺紋,而鎖定構件要螺合於股部之間。不論螺帽構件或鎖定構件或二者,具有凸部,與螺紋桿結合。固定構件又包括在U形凹部底之預載彈簧構件,作用於桿,以便一方面得固定構件和桿間之堅固連接,另方面可容易操作,準確調節固定構件和桿間之位置,最後鎖定位置。由於有彈簧構件存在,如果鎖定構件尚未完全鎖緊,桿仍可在縱向移動。又因彈簧構件的存在,桿和容納組件間之連接始終是摩擦式鎖定之功,需要應用大力矩,以供充分夾持。
US 5,961,517號揭示一種調節工具,以調節桿在如此固定構件內之位置。
本發明之目的,在於提供一種骨骼固定總成,可快速準確調節和再調節桿相對於固定構件之位置,不需用到大扭矩力道。
此目的係由申請專利範圍第1項之骨骼固定總成達成。進一步發展則列於申請專利範圍附屬項。
骨骼固定總成的優點是,較之桿經由摩擦式鎖定方式固定,經鎖定元件應用微小力矩,就可對桿提供大夾持力。此舉可在調節和再調節桿位置的過程中,對鎖定元件應用小力矩,即可方便桿的暫時固定。由於形式鎖定連接,暫時固定有充分強度,在對桿進行操作時防止鬆弛。再者,只要把內螺釘稍微回轉,就可將暫時固定很快鬆弛。所以,骨骼固定裝置適於應用在脊柱變形之矯正,諸如脊柱側彎症,在手術當中需要多次再調節桿的位置,直至最後鎖定為止。
若骨骼固定總成包括棘輪式桿,具有另一優點,即棘輪式桿需要較低的固定力矩,即可達成螺紋桿或光滑桿同樣的保持力。
由於應用比先前技術的裝置為低的力矩,達成桿之預固定,可減少容納組件的股部張開。
本發明其他特點和優點,由如下參照附圖詳述,即可最為明白。
第1至7圖所示固定裝置之第一具體例,包括骨骼固定元件1,有螺紋軸2和容納組件3。容納組件3實質上呈圓筒形,並具有U形凹部4,在螺紋軸側有底部5。利用U形凹部4,形成二自由股部6,7。在U形凹部4形成的溝道內,設備具有陰螺紋8之腔孔,形成與螺紋軸2的中心軸線共軸。陰螺紋8具有任何已知的螺紋形狀。然而,以平面螺紋或負角度螺紋為宜,因可防止股部6,7張開。
固定總成又包括縱向桿9,具有結構化表面。在圖示具體例中,桿具有圓形斷面。結構係棘輪式結構,包含複數沿周凹溝10,利用沿周峰部11分開。
為使桿9固定於容納組件內,設有鎖定構件12。在此具體例中的鎖定構件為內螺釘或固定螺釘,要螺合於股部6,7間之陰螺紋8。內螺釘12包括環形凸部13,在朝向桿之側面,如第2至5圖所示。環形凸部13維度的選擇,是以桿的凹溝10維度為函數,使環形凸部13在螺合於桿上的位置,與凹溝10結合,如第3和4圖所示。凹溝10和相對應凸部13的形狀以圓形為佳。
如第3、4、6、7圖所示,容納組件3在U形凹部4的底部5具有結構,可與桿表面的結構結合。在圖示具體例中,結構包括複數凸部14,利用刻線15分開,按橫越凹部4所界定之溝道縱軸線L的方向沿周延伸。選擇凸部14的維度,容許與桿之凹溝10結合。凸部數視凹溝以及凹部的底部可用空間而定。
特別如第6和7圖所示,容納組件又有中心凹部16,在U形凹部4形成的溝道底部。利用凹部16,把凸部14分成二組,分別位於凹部16的右側和左側。此項配置較使用彎桿在溝道底部5全面設凸部的情形為佳。
第6和7圖所示結構的凸部14間之刻線15,又細又銳。在修飾例中,刻線15寬度可在凸部14間形成凹谷。凹谷可打圓或其他造型。同理,棘輪型桿的結構可有尖銳峰部11,或是圓滑或其他造型之峰部11。棘輪型桿和內螺釘的相對應環狀凸部13,以及容納組件底部等之結構,是按照所用材料和/或結構在結合時所需作用力,加以選擇。
同理,容納組件3底部5上之凸部數,是按照特別需要選擇。如第8和9圖所示,可在凹部16各側設有唯一單凸部14'。又,內螺釘12可有單一凸部以上,與桿結合。
製造骨骼固定總成組件之材料,是生理相容性材料,例如鈦、不銹鋼,或是骨釘可用之其他材料。材料可因不同元件而異。尤其是材料要夠硬,才能在桿和骨骼固定元件之間有實體形式鎖定連接。
操作時,首先把至少二骨骼固定元件螺入骨骼內,尤其是相鄰脊椎骨。然後,把桿9置入骨骼固定元件的容納組件之U形凹部4內,使其表面結構10,11與容納組件底部的結構14,15結合。然後,把內螺釘12旋入。一旦環形凸部13與桿表面的結構結合,桿即以形式鎖定方式,亦即積極連接,保持在容納組件內,防止桿在任一方向運動。由於形式鎖定或積極鎖定連接,應用低力矩即足以獲得充分預固定。
在以下階段進行微調,由此調節各固定元件在桿相對於桿縱軸線之結合位置。為了微調,稍微退轉內螺釘,以旋鬆桿結構與內螺釘結構之形式鎖定結合。由於以低力矩獲得預固定,可快速退轉固定螺釘旋鬆。此舉有利於處置和加速程序。找到固定元件相對於桿的最後位置後,旋緊內螺釘,將桿牢牢固定於容納組件內。
以上述固定總成,可以較僅靠摩擦式鎖定的光滑桿情況之預固定為低之扭矩,獲得桿之預固定。較之使用彈簧構件加以固定的結構化桿,亦是如此。再者,固定總成構造較具有彈簧元件夾持桿之構造更簡單。此有利於生產。
第一具體例之修飾如第10a、10b、11和12圖所示。與第一具體例不同的是,桿為螺紋桿9'。因此,凹溝10'和峰部11'有螺紋間距之螺旋結構。相對應地,容納組件底部的結構14"具有螺紋間距。螺紋桿9'較棘輪型桿更容易製造。
參考第13至15圖說明第二具體例。第二具體例和第一具體例不同,其中骨骼固定元件形成所謂多軸螺釘,因而容納桿之容納組件樞動連接於相關螺紋軸。此具體例與上述具體例相當之組件,使用同樣參考號碼。
骨骼固定元件101包括螺紋軸102和頭部102a,後者有球形截面形部,其凹部102b可供旋入工具結合。容納組件103實質上呈圓筒形,包括共軸腔孔,有一段103a可容納頭部102a,故頭部可以樞動方式保持於容納組件內。容納組件103又有U形凹部104,可供容納桿。利用U形凹部104,形成二股部106,107,包括陰螺紋108。
設有施壓構件120,實質上呈圓筒形,其外徑得在容納組件內前後移動。施壓構件在朝向頭部之側面,有(最好是球形)凹部121,與頭部102a之至少一部份結合。在對面側,施壓構件具有圓筒形凹部122,圓筒軸線橫越螺釘軸線延伸。圓筒形凹部之半徑,適於容納桿9。
圓筒形凹部122的表面包括凸部114,以刻線115分開,按沿周方向橫越桿軸線延伸,與桿9之凹溝10結合,與第一具體例的容納組件3之底部5結構相似。施壓元件120又有共軸腔孔123,容許旋入工具進出。施壓元件的尺寸使得在組合狀態下,其圓筒形凹部122之底部,突出容納組件的U形凹部之底部105上方,如第14和15圖所示。螺釘、容納組件和施壓元件可以預組合。
桿9和內螺釘12,與第一具體例一致。
操作時,把至少二骨骼固定元件置入二骨碎片或二相鄰脊椎骨。然後,將桿插入容納組件的U形凹部內。由於容納組件可相對於螺釘樞動,容納組件相對於螺紋軸的角度位置即可調節。在內螺釘旋入時,對桿施壓,桿對施壓元件施壓,而壓住容納組件之段103a。內螺釘以低力矩旋緊時,即可達成暫時固定或預固定。桿之暫時固定,是一方面利用內螺釘和桿,另方面藉桿和施壓元件間之形式鎖定連接所造成。微調是以第一具體例之同樣方式進行。
本發明可構想其他修飾。桿表面的結構可以變化,例如可具有波浪螺旋式渦形。例如,亦可有尖銳峰部形成鈍角。若具有如此尖銳峰部,可達成更微細層級,因為形成之齒在桿的縱向較小。然而,以圓滑形狀,可以更可靠進行微調。容納組件底部和鎖定元件所設凸部之結構,亦可變化。
可以只在一部份具有此結構,可分別選在鎖定構件、容納組件或施壓元件。
在進一步修改中,U形凹部不向容納組件頂部,而是向側面開口。
在第二具體例進一步修飾中,頭部和桿可單獨固定。在此情況下,施壓元件的圓筒形凹部可改為U形凹部,藉此形成股部,在桿上方延伸。鎖定構件包括第一螺釘,只壓住施壓元件,並有螺紋共軸腔孔,內設固定螺釘,與桿結合。
本發明不限於螺釘做為骨骼固定元件,但可以骨鉤或任何其他骨骼固定元件實施。

Claims (6)
- 一種骨骼固定總成,可用於脊柱手術或創傷手術,包括:固定元件(1;101),有軸(2;102)和容納組件(3;103),呈實質上U形斷面,基部連接於該軸,而二自由股部(6,7;106,107)形成溝道(4;104),可容納桿;桿(9),有至少部份結構化之表面(10,11;10',11');鎖定構件(12),與容納組件合作,把該桿鎖定於溝道內;其中在鎖定構件(12)上,設有結合部(13),供結合桿之該結構化表面(10,11;10',11');其中容納件(103)和軸(102)係樞動連接,而軸包括在一端之頭部(102a),保持於容納組件(103)內,並設有施壓元件(120),可對頭部施壓,把頭部鎖定於容納組件內,其特徵為,在施壓元件(120)上設有結合部(14,15;114,115),供結合桿之結構化表面(10,11;10',11'),鎖定構件、桿和容納組件合作,其方式為一旦結合部與桿之結構化表面結合,桿即以形式鎖定方式,保持在容納組件內,以防止桿運動者。
