1324067 ⑴ 玖、發明說明 【發明所屬之技術領域】 本發明係關於經取代之二氫咪唑酮類,特別是〔1 -( 4 -氯苯基)-4 - ( 4 -嗎福啉基)-2,5 -二氫-]Η -咪唑-2 -酮〕 (AWD 131-138)或其生理上可接受之鹽類於處理狗內癲 癎及行爲不正常上之用途。
在人類及動物 > 特別是狗及貓中,癲癎發作性失調症 爲最普遍之顱內病(OLIVER 1 980, SCHWARTZ-PORSCHE 1 984,LOSCHER et al. 1 985,FREY 1 989 )。在狗及人類 中,癲癎發作失調症之病率據估計爲 0.5-1% ( US Department of Health, Education and Welfare 1 9 77, J AN Z 1 979,L0SCHER et a】· 1 98 5,KERANEN and RIEKKINGEN 1 9 8 8, FORRESTER et al. 1 9 8 9, SRENK et al. 1 994 )。癲 癎有不同之同義詞,但係用來描述腦皮質內之突然,過度 瞬間陣發性神經元放電(L0SCHER 1 993,JAGGY and STEFFEN 1 995 a)。以機轉爲基礎之瞭解已相當有進展, 抑制性(BURNHAM 1989,LOSCHER 1989)及激動性神 經遞質(MCNAMARA 1988,DINGLEDINE et al. 1990)間 之不平衡業已被述及(FENNER and HAAS 1 989 )。又, 改變之離子通道及神經遞質受體功能似乎在癲癎之致病性 上扮演一重大之角色(OWENS and KRIEGSTEIN 200 1 ) 癲癎發作被歸類爲部份性或全身性癲癎發作,它帶有 強迫性、陣攣性、或強迫性-陣攣性活性,造成或不造成 -5- (2)

意識之損失(SCHWARTZ-PORSCHE 1 9 84 )。癲癎被定義 爲特發性,當無基本之原因可被病理檢驗界定時( CUNNINGHAM 1971, DELAHUNTA 1 9 8 3, MONTGOMERY and LEE 1983, SCHWARTZ-PORSCHE 1 9 8 4 and CHRISM AN 1991 )。症狀性癲癎係被顱內損傷或外神經 性、代謝性擾亂所引起(JAGGY and STEFFEN 1995 b and c5 PODELL et al. 1 995, JAGGY and HEYNOLD 1 9 96 )。 以狗作實驗,獲知於患有特發性癲癎之狗中,經診斷出大 約45%案例帶有癲癎發作性失調症(JAGGY and STEFFEN 】99 5 a and c),且所有狗中有5.3-8.0%帶有神經系統之 疾病(SCHWARTZ-PORSCHE 1994, BERN ARDINI and JAGGY 1 99 8 )。於帶有特發性癲癎之大部份犬齒類動物 案例中,發現到全身性癲癎發作存在(80-90% )( SCHWARTZ-PORSCHE 1 984, LOSCHER et al. 1985, BRAUND 1 9 8 6, CENTER 1 9 8 6, JAGGY and STEFFEN, 1 99 5 c )。然而,部份性癲癎發作亦可能發生( BREITSCHWERDT et a 1 · 1 9 7 9 )。癲癎發作活力一般乃在 卜3 歲帶有特發性癲癎之狗內起動(CROFT 1 965, CUNNINGHAM 1971; DE LAHUNTA 1 98 3, FORRESTER et al. 1 989, OLIVER and LORENZ, 1 993 )。在某些動物種類 中,遺傳被證明存在(OLIVER 1 9 8 7 5 CHRISMAN 199], OLIVER and LORENZ, 1 993, JAGGY and STEFFEN, 1995 a ) ° -6- (3) (3)1324067 [先前技術】 在狗內,僅有數種抗癲癎藥物,例如苯巴比妥,嘧啶 酮及溴化鉀能夠成功地以生命長期治療方式加以使用( SCHWARTZ-PORSCHE 1 9 84: FREY and SCHWARTZ-PORSCHE 1 98 5, FREY 1 9 8 6, SCHWARTZ-PORSCHE and JURGENS 1991, LOSCHER 1 994 )。 然而,治療之結果在所有之案例中並非令人滿意。在 大約三分之一案例中,藥物抗性被觀察到(SCHWARTZ-PORSCHE et al. 1 9 8 2,FREY and SCHWARTZ-PORSCHE 1 9 8 5, LOSCHER et al. 1 9 8 5, LOSCHER and SCHWARTZ-PORSCHE 1986,HEYNOLD et al. 1 99 7 )。又,使用苯巴 比妥(P h e η o b a r b i t a 1 )或唆D定酮(p y r i m i d ο n e )會引起副 作用,例如過度催眠、運動失調、強迫性起搏、衰弱、貪 食、煩渴及多尿(SCHWARTZ-PORSCHE et a丨· 1 9 8 2 and LOSCHER 1 995 )。以溴化鉀治療之結果則導致易疲倦、 厭食、頑固性便秘、胃炎及皮膚損傷(L0SCHER 1 995 ) 〇 較新之抗癲_藥物,例如gabapentin或lamotrigine 由於半衰期不夠長之原因不能用來於狗內治療癲癎( LOSCHER 1 994 ),縱使這些藥物能夠成功地抑制由痙攣 毒性樣戊四唑(PTZ )所引起之癲癎發作。此類藥物之另 —例 abecarnil ( L0SCHER et al.s 1 990 ),它顯示能抑制 由PTZ所引起之癲癎發作,但不能用來治療癲癎。因此 ,現成之動物模式原則上可用來試驗是否個別藥物帶有可 -7 - 1324067 變成抗痙攣劑之潛力,然而,此類藥物卻不能預測藥物之 臨床效率。短半衰期之問題更因爲藥物通過腸道之時間, 在狗內較人體內爲快而變得加劇。
目前,有關試驗一種最古老之抗抽措劑,p h e n y t 〇 i η ,之抗癲癎活力的臨床硏究正進行中,於此試驗上, phenytoin係以一持續釋出調合物之方式被投服,於狗內 所作之進一步臨床硏究則用 vigabatrin來進行(Speciale et al.,1991 )。此化合物係因其所具之作用機制而予以選 擇。此作用機轉涉及7 -胺基丁酸之代謝途徑受不可逆抑 制。因此,由於不可逆之本性或機制,此藥物預期能展現 不受長半衰期之存在影響的活性。單一之高劑量已知會不 可逆性地阻斷7 -胺基丁酸之代謝性降解作用。然而,由 於此藥物在狗內會產生不能接受之副作用,此硏究乃告失 敗。
依據此資料,對於狗用之抗癲癎藥物的需求是顯而易 知。此類新藥物必須在患癲癎症之狗(即患者)內展現治 療癲癎活性。此類藥物亦必須特別在患有不能受現有藥物 治療之癲癎症的狗內,展現治療活性。此外,此類藥物必 須具有較佳之副作用輪廓,即,於治療時,必須產生較少 之副作用。 A W D〗3 1 -1 3 8 [】-(4 -氯苯基)-4 - ( 4 -嗎福啉基)-2,5·二氫-1H-咪唑-2-酮〕係一帶有抗抽搐及減輕焦慮效果 之新穎藥物(Rostock et al.; 1998 a-d)。此藥物亦被證 明能夠於狗及貓中,於靜脈注射戊四唑(PTZ )驟發試驗 -8- (5) (5)1324067 內,提高化學誘發之驟發閾。於狗內,20及40 mg/ kg 劑量口服之結果增加驟發閾分別達39及1 1 8% ( Bialer et a 1 _, 1 9 9 9 )。然如,如前所揭示,此模式並不能在患有癲 癎症之狗內,預測藥物之臨床活性。於此P T Z試驗中, 藥物係先予口服,然後於服藥後,於界定之時間點處輪注 PTZ,直至最先誘發癲癎發作樣強迫性顫搐爲止。誘發此 類顫搐所需之PTZ的劑量(按體重增減),其定義爲抽 搐性閾。藉比較受藥物處理之動物中的抽搐性閾與僅受賦 形劑處理之對照實驗中之閾作比較來評估藥物之效果。藥 物效果係以抽搐性閾之增加百分率來表示。固然上述者爲 證示對癲癎發作樣行爲上展現某些活性之模式,PTZ並不 在狗內誘發癲癎,由而限制了此類模式在有病動物上之可 預測性。再者,由於試驗藥物投服及P TZ輸注間之嚴格 的相互關係,資料並未指出是否試驗藥物可產.生足夠長久 之持續性血漿濃度以防護動物整日不受癲癎發作侵襲-假 若藥物投服次數每月不超過1至3曰。 AWD 13]-138之作用機轉迄今仍未完全明瞭。在一廣 泛之受體篩選作業中,人們發現到GAB Aa受體之鍵聯部 位對苯並二氮雜簞(benzodiazepine )之親和力很低弱。 使用不同之選殖之人類GABA受體錯合物進行之電生理硏 究指出:AWD在苯並二氮雜簞受體處充當低親和力部份 催動劑而無型選擇性,使用 AWD 13卜138而獲得之最大 刺激僅達d i a z e p a m效果之2 0 %。針對藥理活性,使用專 一性受體拮抗劑flumazenil來評估苯並二氮雜箪受體交互 -9- (6) (6)1324067 反應之促成作用。A WD 1 3 1 -1 3 8之抗抽搐活性可被部份拮 抗,而其減輕焦慮活性則於共同投服Humazenil後完全被 拮抗。於癲癎發作及減輕焦慮試驗中之拮抗程度可與 f] u m a z e n i 1對d i a z e p a m之抗抽搐及減輕焦慮活性的效果 相匹配。這些資料指出:縱使親和性低,固有活性亦低, AWD 131-138與benzodiazepine受體間之交互作用可爲藥 理活性之主要機轉。然而,A WD 1 3 1 -1 3 8之精神藥理輪廓 則大不同於已知之苯並二氮雜簞催動劑。在一藥物區別硏 究中’猴子並不證明 AWD 131-138爲苯並二氮雜箄一樣 ’但卻證明 m i d a ζ 〇 ] a m及d i a z e p a m爲苯並二氣雜章一樣 。該缺乏苯並二氮雜簞樣精神藥理之事實亦在一自行投服 範本中被證明,於此範本中,A W D 1 3 1 - 1 3 8,不像完全之 苯並二氮雜罩催動劑,並不代替古柯鹼。該缺乏苯並二氮 雜簞樣精神藥理之事實可能與帶有低固有活性之部份催動 活性有關聯。A WD亦被發現具有微弱之鈣道阻斷效果。 此機轉可促成抗抽搐活性(Rostock et a丨.,1 998 a-d;

在以本發明爲基礎之硏究中,AWD 13卜138之效率係 於一臨床試驗,於患有癲癎症之狗內予以估計。更詳言之 ’將新近被診斷帶有特發性癲癎症而未受任何前處理之狗 以及帶有特發性癲癎症之狗加以處理,前述之二種樣式之 狗係對於傳統之抗癲癎藥療皆無反應者。又,以AWD I3I-138及其他抗癲癎藥物進行組合式處理。此外,AWD -10- (7) (7)1324067 ]3 ] - 1 3 8及其他抗癲癎藥物之副作用輪廓上比較亦加以估 計。令人驚奇的是’ AWD 1 3 1-138被發現在新近被診斷患 有癲癎症之物及對藥物具有抗性而患有癲癎症之物,在抑 制癲癎發作效果上’具有高效率,又,AWD 131-138即使 長期投服後仍具有良好之耐受性,且相較於傳統之抗癲癎 藥物,所觀察到之副作用較少》再者,A W D 1 3 1 -1 3 8有效 於治療狗內之行爲不正常’特別是與焦慮有關聯者,即恐 懼行爲’例如對人或環境之未預料性侵犯。 【發明內容】 首先’本發明之第一方法乃爲〔1-(4 —氯苯基)-4—( 4-嗎福啉基)-2,5 -二氫-1H -咪唑-2 -酮〕(AWD 131-]38) 或其生理上可接受之鹽類之用作活性成份以製造治療狗內 癲癎用之藥物。本發明之第二方法乃爲AWD 131-138之 用作活性成份以治療狗內之行爲不正常。 本發明之藥物可以任何適宜之途徑予以投服,例如腸 胃外 '經口、鼻內 '肺內投服。就實務目的而言,然而, 經口投服爲合宜者。 藥物每日可予投服一或數次,例如].5次/每日,每 曰投服1 -3次爲特別合宜者。活性成份之劑量爲治療有效 劑量,即,足以舒緩或消除癲癎症狀及/或行爲不正常之 劑Η。每日劑量宜爲1-2000 mg/kg,更宜爲5-100 mg/ kg °劑量可予改變以適應個別患者之需要。活性成份通常 以藥用組成物之方式予以投服,此組成物包含活性成份及 -11 - (8) (8)1324067 藥學上可接受之載體,稀釋劑及/或佐劑。 若需要時,活性成份亦可與至少一種另外活性成份共 同投服。該令外之活性成份或選自其他抗癲癎藥物,例如 ,苯巴比妥,嘧啶酮及溴化鉀。 本發明之藥物可用於處理任何類型之癲癎症,例如特 發性或症狀性癲癎。特別合宜者乃爲特發性癲癎之治療, 例如新近診斷出之特發性癲癎或業已建立之特發性癲癎等 之治療,特別是抗藥性癲癎(它不能被傳統抗癲癎藥物治 療)之治療。 本發明之藥物可用於處理行爲不正常,特別是焦慮。 處理上所帶來之一令人驚奇的優點是:引起不期望之 行爲副作用(例如催眠作用)的危險性乃降低。又,投服 藥物是不會誘發肝酵素活性,因此不會隱藏其他伴隨之病 症。 又,本發明將藉下列之圖表及實例加以說明。 【實施方式】 1 ·材料及方法 1. 1 .狗 於本硏究中,取患有特發性癲癎之.111隻狗加以檢驗 。於初步硏究中,取29隻狗,處理以AWD 131-138。於 新近診斷出患有特發性癲癎之12隻狗(新近診斷之狗) 內,使用 AWD 13卜138起動最初之抗抽搐處理。於患有 慢性癲癎且對傳統處理無回應之另外1 7隻狗內,加入 -12- (9) (9)1324067 AWD ] 3卜〗3 8進行處理(加上處理)。回想起來,吾人評 估8 2隻患有特發性癲癎之狗。7 0隻新近診斷之狗被處理 以已建立之抗癲癎藥物,苯巴比妥或嘧啶酮。在患有慢性 癲癎而於受該二藥物處理後仍無任何改進之殘餘的1 2隻 狗中,補服以溴化鉀(表1 )。 表1 :包括於本硏究中之狗數 處理 狗數 1 . 於這些狗之1 2隻中,特發性癲癎重新加以診斷。在 演出前,這些狗未受處理,但僅接收AWD 131-138之單 治療(表1 )。於受A WD 1 3卜1 3 8處理4個月後,這些狗 中之一展示癲癎發作頻數未見改進(減少),因此補服以 苯巴比妥(4 mg/ kg-體重,口服每日劑量)。 於演出前,所有之狗展示二次以上之全身性癲癎發作 。在所有之案例中,狗主觀察癲癎大發作,其中5隻狗發 展出聚集性癲癎發作。除了在3隻狗發現有全身性癲癎發 作以外,局灶性癲癎發作亦被觀察到(表1 2 )。在1 2隻 未受處理之狗內所展現之癲癎發作頻數乃由每月發作8次 至每8個月發作一次不等。
-14 - 1 1 .〗.2 .患有慢.性癲癎之狗 於演出前,將1 7隻患有慢性癲癎之狗處理以苯巴比 妥或嘧啶酮,該等17隻狗呈未反應該傳統之藥療。處理 時期範圍乃由3個月至5年(中間].5年,平均及標準偏 差].6±1.3年)。於引導硏究期間,這些狗接收苯巴比妥 或嘧啶酮及A WD 1 3〗-1 3 8之組合治療(表])。 2 這些狗中有11隻乃受苯巴比妥之處理,其每日劑量 爲由6至23 mg/kg體重口服(中間値10.7,平均及標準 偏差12_9 ±6.6 mg/ kg )。苯巴比妥之血淸濃度測知乃 (11) 1324067 介於1 9.5至5 8.9以g / ηι 1之間(中間値2 6.5,平均及標 準偏差 3 2.0 ± 1 3 · 6 y g / m 1 ;參考値 1 5 - 4 5 // g / m 1,此値 係被FARNBACH 1984所建立者)。 將殘餘之六隻狗處理以嘧啶酮,其每日投服劑量爲由 25至53 mg / kg體重,口服(中間値45.5,平均及標準 偏差42.8 ±8.9 mg/ kg)。在這些案例中,苯巴比妥之 血淸濃度爲由23.2至27.4 # g/ ml間(中間値23.7,平
均及標準偏差24.8 ±1.8 //g/ml)。 於所有之案例中觀察癲癎大發作,其中有]5隻狗發 展出聚集性發作,而8隻狗則呈現存活或急性狀態癲癎。 除了全身性癲癎發作以外,在5隻狗內觀察到局灶性癲癎 發作,在2隻狗內觀察到複雜部份性癲癎發作.(表2 )。 在這些17隻狗內之癲癎發作頻數乃介於每月6次癲癎發 作至每6個月癲癎發作一次。

這些狗中有70隻狗新近被診斷出患有特發性癲癎, 且於演出前不受處理。所有之狗於受處理前帶有二次以上 癲癎發作。將這些狗中之44隻狗處理(投服)以苯巴比 妥’其每日劑量介於4至】3 mg/ kg體重,口服(中間値 5.0’平均及標準偏差6.0 ±2.4 mg/kg)。血淸濃度介 於 4.6-33.2 eg/ml(中間値 17.2,平均及標準偏差 18.4 ±7.2 II g/ ml)。 將26隻狗處理以嘧啶酮,其接收劑量爲由24至7〇 mg/kg體重’口服(中間値60.0,平均及標準偏差51.0 ±13.4mg/kg)。苯巴比妥之血淸濃度介於59〜375/Zg /ml間(中間値18.3’平均及標準偏差7 ± 1 0.2 μ g /ml) 〇
於受處理前’所有之狗至少有二次以上之癲癎發作。 在大部份狗(n = 61)中’觀察到全身性癲癎發作(癲癎大 發作)。1 9隻狗在其癲癎發作歷史中,患有聚集性發作 ’且有7隻狗於狀態性癲癎之急性期中或於痊癒後被提及 。】3隻狗患有局灶性癲癎,且在3隻狗中,複雜部份性 癲癎發作被描述(表3)。 患有慢性癲癎症之]2隻狗於演出前已被處理以苯巴 比妥或嘧啶酮3月至3年(中間値〇 · 5年,平均及標準偏 差1.7 ±0.9年)’但對此樂療未有反應。於焉,這些狗 -16- (13) 1324067 接收額外之藥物,溴化鉀。 這些狗中之8隻狗乃受苯巴比妥之處理,其每日劑量 由6至1 7 mg / kg體重,口服(中間値9.5,平均及標準 偏差】0.0 ±3.2 mg/ kg),具血淸濃度經測量得知介於 18.7至41 #g / ml (中間値24.6,平均及標準偏差27.2 ±8.4 y g / m 1 )。
剩餘之4隻狗乃受嘧啶酮之處理,其每日劑量爲由 50至70 mg/kg體重,口服,(中間値60.0,平均及標 準偏差60.0 ±7.0 m g / k g )。在這些案例中,血淸濃度 乃介於由24.5至36.2 # g/ ml (中間値30.4,平均及標準 偏差 30.4 ±5.9 μ g/ ml )。 將溴化鉀以40-60 mg/ kg體重,口服之每日劑量( 中間値41,平均及標準偏差42.6 ±5.4 mg/ kg)予以給 藥,溴化物濃度介於由0.6至1.4 mg/ ml (中間値0.9, 平均及標準偏差1.0 ±0.3 mg/ml;治療範圍1.0-2.0 mg

於以AWD 13 1-138進行處理後,緊接著進行首次追 蹤檢驗,接著每隔6或8星期或視癲癎發作之各別出現而 作檢驗,進行包括血液作業之一臨床或神經檢驗。於硏究 期間’所有之狗主手中持有一筆記本,詳細記述出現之癩 癎發作’包括頻數,持續時間及嚴重度,行爲改變,其他 藥物治療及可能觀察到之副作用。在這些時間點時,將 A W D 1 3 1 -1 3 8及具代謝性之血漿濃度加以測量。 於以AWD 131-138進行處理之第二個月後,狗主乃 針對癲癎發作發展及副作用:於服用A W D後出現催眠、 貪食症 '多尿及煩渇 '嘔吐、下瀉、厭食、態度改變不 安靜’ Π且嚼加增、對狗主或其他狗出現侵犯性,以及步態 不正常。 -20- (17) 1324067 1.2.1 .1 · AWD 13 1-138之血漿濃度之測量
於開始處理時,於2隻狗內,以AWD 13]-138單一 治療進行藥物動力學硏究,且於4隻內,以A WD 1 3 1 - I 3 8 及苯巴比妥或嘧啶酮之組合治療進行藥物動力學硏究。所 有之狗接收5mg/kg體重AWD 131-]38。每隔2時取出 血液共三次。使用HP LC/質譜分析法來測量AWD 13卜 1 3 8及其代謝物之血漿濃度’於緊隨之檢驗期間,使用相 合之方法作爲一致性對照組。於口服A WD 1 3 1 -1 3 8後,2 及1 2小時,取出血液。 1.2.2.回顧性硏究
將於該硏究部份內獲得之資料作爲對照組。在所有受 傳統藥療之8 2個案例(見].丨· 2 .)中,將癲癎發作之歷 史’嚴重度及持續時間,癲癎發作初起及抗癲癎處理時之 年齡等記錄於各案例中。依據狗主之觀察及視頻監視來分 類癲癎發作。 所有之狗皆受一標準化之身體及神經學檢驗(J A G G Y and TIPLD 1999)。於所有狗中,進行血液作業,它包括 血液學及血液化學(見〗·2·1·)。藉標準方法分析苯巴比 妥(ALOMED,Radolfzell)及漠化紳(Gesellschaft fiir Epilepsieforschung E.V,,Bielefeld)之血淸濃度》其他特 別之檢驗包括頭顱之電腦體層成像,腦脊髓液,EEG及胸 部之X-射線等之檢驗(表6 )。 -21 - (18) 1324067 表6 :於8 2隻狗內進行之特別檢驗(回顧性硏究) 狗數 特別檢驗 新近診斷者_;[曼性癲_ 零腦體層成像法 22/70 5/12 腦脊髓液 22/70 5/12 EEG 27/70 9/12 X-射線胸部 3 6/70 8/12 假若所有進行之檢驗皆在正常限度內’則分別診斷爲 疑似特發性靡摘。狗主人乃被問及下列之事項·有關處理 前及後之癲癎發作頻數、持續時間及嚴重度等之臨床觀察 及治療結果,觀察期間爲】個月及9個月間。 ].2.3 ·統計
使用統計軟體封裝 WinSTAT® for EXCEL®來計算各 組中之敘述性參數,例如年齡之平均値、中間値及標準偏 差,癲癎發作初起時之年齡,處理開始時之年齡,苯巴比 妥或嘧啶酮之劑量,包括其血淸濃度。處理前及處理期間 所呈現之癲癎發作頻數間的差異之有效値乃藉Wilcox on Signed Rank test’使用Stat®而予以計算,爲精確起見, 使用成對複製(雙重份),將有效値之水平選定爲p = 〇.05 。藉 ANOVA( 3 組)或 Fisher’s exact test (2 組)進行 處理組間之比較。 -22- (19) (19)1324067 2.結果 2 _ 1 .癲癎發作頻數 2.1.1·初步硏究:A WD 1 3 1 - ] 3 8處理 2-1.1.1.新近被診斷之狗 於演出前’癲癎發作頻數乃介於由每個月發作8次至 每Μ固月發作一次(中間値1 .6 )。於以A WD ] 3 1 -1 3 8進 行單一治療期間,每個月之癲癎發作頻數乃由癲癎發作之 受完全控制至每個月發作9次(中間値〇. 71 )(表7 )。 在這些]2隻狗中之.9隻中,觀察到有癲癎發作減少之情 形。計算這些9隻狗中之數値,並排除未反應者,得知於 處理前每月之中間發作頻數爲1.7,而於AWD 131-138藥 療期間此中間値爲0 · 5 5。於這些狗中癲癎發作頻數之改進 在統計上顯得有重大意義(p<〇.05)。於反應者內之癲 癎發作減少的百分率爲49.8%,此値係以平均値加以表示 (表 7 )。 有一隻狗(它於受處理前,癲癎發作5次)於9個月 之觀察期間保持未癲癎發作(依照狗主之新資料,此狗發 癲癎發作已有17個月之久)。12隻狗中有4隻狗(33% )之癲癎發作頻數的減少率値已達5 0 %以上(表7 ) 。3 隻狗(25% )被認爲未反應者,未反應者之定義是:於受 處理期間,既不展示發作頻數減少,亦不展示發作頻數增 加之狗。這些3隻狗中之一隻狗乃於接收30mg/kg劑量 之AWD 131-138首次藥療後(它於接收藥療後2小時, 其AWD 131-138在血漿中之濃度爲3997.5ng/m丨),死 -23-
L (20) 1324067 於狀態性癲癎症下》—隻狗病狀惡化,但經以苯巴比妥補 療後,其病狀即獲得改善。 單一治療處理性(2.2·].-2.2.2.)之比較並未顯示 A W D 1 3 1 - 1 3 8,苯巴比妥或嘧啶酮之抗癲癎效率間有任何 重大之差異,只證示治療均等而已。 2.1.1.2.患有慢性癲癎症之狗以及以AWD 131-138進行
之加上治療
於以苯巴比妥或嘧啶酮進行之未成功的處理期間,每 月之癲癎發作頻數乃由每個月發作8次至每6個月發作1 至4次不等(中間値).。於以A WD 1 3卜1 3 8進行之加上 處理期間,每月之癲癇發作頻數則由0至發作9次不等( 中間値2.0)(表8)。於這些]7隻狗中之]0隻中,觀 察到癲癎發作減少。計算這些1 0隻狗中之數値,然後排 除反應者,即得知:於處理前,每月之中間癲癎發作頻數 爲2.4,而於以AWD 131-138進行處理期間,平均値則爲 1.1。這些狗中之癲癎發作頻數的改良在統計上有重大意 義(ρ<0·05)。在反應者內之癎發作降低率爲47.2% ,此値係以中間値之方式表示(表8)。 6隻狗顯示其癲癎發作減低百分率達50%。一隻狗( 6% )完全不患有癲癎。此狗係於8 1 /2歲時初起癲癎發作 ,每月呈現1至4次聚集性癲癎發作。於以AWD 131-]38 進行組合治療3個月後,此狗因患白血病而被處以安樂死 。其他二隻狗在處理開始之際仍呈現二次發作,但在剩餘 -24- (21) (21)1324067 之觀察期間則不再呈現癲癎發作。剩餘之Π隻狗中有4 隻狗展現發作減低百分率在5 0 %以下,7隻則被認爲無反 應者。此群組中之9隻患有癲癎之狗並未死亡或未反應狗 主之要求’於狗呈現狀態性癲癎之情況下予以安裝死。該 9隻狗中之2隻乃依組織病理學予以檢驗。結果,未驗測 到有神經外或神經f貝傷。另一隻狗乃於c 〇 u m a r i η中毒後 死亡。於這些狗中,處理觀察期乃降低至2至8個月。 2.1.2.回顧性硏究,傳統處理 2·1·2.1·新近診斷之狗,苯巴比妥單—治療 於處理前’癲癎發作乃以由每個月發作7次至每6個 月發作一次之頻數(中間値1 .6 )呈現(表7 )。於以苯 巴比妥進行治療期間,每月之發作頻數乃由〇至1 〇次發 作不等(中間値0.59)。於這些44隻狗中之32隻狗中, 癲癎發作次數之減低被觀察出來。計算這些32隻狗內之 數値,並排除未反應者,發現到:於處理前,每月之中間 發作頻數爲1 .6 8,而於以苯巴比妥進行藥療期間,此中間 發作頻數爲0.42。使用相同之計算,顯示:在這些狗內之 發作頻數的改善,在統計上顯出有重大的意義。在反應者 內之發作減低的百分率乃爲7 2.4 %,此値係以平均値表示 (表 7 )。 這些44隻狗中之9隻狗(2 0%)於受處理期間無癲 癎發作。於2 8隻狗(64 % )內,觀察到發作減低率達 5 〇 %以上’ 1 2隻狗(2 7 % )被認爲未反應者。這些組群中 -25- (22) 1324067 之]〇隻狗乃死亡,或於狗主之要求下,於狀態性癲癎症 之情況下予以安樂死。3隻額外之狗則因患有除了癲癎以 外之病症而予以安樂死。 2.1 .2.2.新近診斷之狗,嘧啶酮單一治療

< 2 n g / m L 2.3.1.於新近被診斷之狗內,AWD 131-138的血漿濃度 爲欲控制狗主之一致性,乃於服藥2及1 2小時後開 始進行處理’於開始處理3星期後,首先測量AWD 131-13 8血漿濃度。在1丨案例中,於口服2小時後,濃度範 圍乃介於53.28及8619.4 ng/mL(中間値2585.0,平均 及標準誤差3356.3 ±3290.3 ng/mL (圖1及2)之間。 AWD 131-138劑量在8隻狗內乃l〇mg/kg體重(於8隻 狗內)’】5 mg/kg體重(於2隻狗內)以及20mg/kg 體重(每次二次,於—隻狗內)間不等。在該於口服12 小時後之時間點處,血漿濃度在所有之1 2隻狗內乃介於 -36- (33) (33)1324067 5.4及 H39.2 ng/mL(中間値218.1,平均及標準誤差 377.5 ±406.0 ng/mL (圖])間。於不同之時間處,於 各狗內執行進一步之對照檢驗。於口服2小時後,血漿濃 度乃介於53.28及10737.41 ng/mL間不等(圖1及3) 。血漿濃度及癲癎發作頻數間並無關聯(圖2及3 )。 2.3.2.於患有慢性癲癎之狗內,A W D 1 3 1 -1 3 8之血漿濃 度 患有慢性癲癎症且接受加上處理之1 7隻狗,於服藥 2小時’其血淸濃度介於279.6至10613.7 ng/mL間(中 間値2992.4,平均及標準偏差3896.1 ±3339.2 ng/mL) (圖 2 ) 。AWD 13 1 -1 38 劑量乃由 10mg / k.g 體重(於 1 5 隻狗內)及]5 mg/ kg體重(於二隻狗內)間,每曰服用, 二次,不等。於服藥1 2小時後,血漿濃度乃介於7 · 5 7及 5873.04 ng/mL 間(中間値 179.3,平均値 644.0)。於 不同之時間點處,對此組群執行進一步之對照檢驗。於□ 服二小時後,血漿濃度乃介於1 5 6.4 6及2 6 7 1 0.5 8 ng / 間不等(圖1 )。然而,直到此時間點處,AWD 1 3 1 -1 3 8 之治療範圍才爲人所知。血漿濃度及癲癎發作頻數並無關 聯(圖2及3 )。 2.4.1 .問卷之評估:AWD 13卜138之服用於新近被診斷 爲患有特發性癲癎症之]2隻狗內 狗受 AWD ]3]-138之處理例外地展現良好的耐受性 -37- (34) (34)1324067 。於受傳統之抗癲癎藥處理且受AWD 131-138補療之17 隻狗內,被狗主觀察到的主要副作用是貪食症,此症發生 於1 0個(59% )實例中,其中,7隻狗僅處在硏究之開端 而已。受組合之苯巴比妥處理,且帶有高巴比妥血淸濃度 水平( 56.6-58.9eg/mL)之二隻狗則於投服AWD ]31· 138(劑量:40mg/kg AWD 13 卜]38 體重 / 每日)2-4 小 時後,展示其後肢運動失調及淡漠,且於服用2小時後., 展示其血發濃度介於由5563.26至10858.45 ng/mL。於 這些狗中之一隻狗中觀察到:於A WD服用後有咀嚼加增 的情形。進一步之副作用則未見到。 2-4.3.跟進:AWD 131-138之投服於新近被診斷爲患有 特發性癲癎之1 2隻狗內 於正規之跟進檢驗期間,臨床及神經學檢驗仍維持正 常。因有患有腸感染,一隻狗乃透過指示之獸醫服用 enrofloxacin,且於24小時後開始癲癎發作。俟移開此藥 療後,不再有癲癎發作被觀察到。在血液學(紅血球、白 血球及血球分類計數_)內,無不正常情況被發現到。 2.4.4.跟進:AWD 131-138之服用之患有慢性癲癎之17 隻狗內 於正.規之跟進檢驗內,於]5隻狗內,臨床及神經學 檢驗仍維持正常,於受組合之苯巴比妥處理之狗中之二隻 狗中,業已被狗主觀察到之運動失調整可被證實。該二Λ -38- (35) (35)1324067 狗於投服A W D 1 3 1 - 1 3 8僅2 - 4小時後在其所有之4條腿 中,帶有些微本體感受之短絀。於此時間點後,此二隻狗 在臨床上顯現正常》—隻狗因肺部受感染,乃透過指示之 獸醫接收青黴素-鏈黴素’並於藥療後變成淡漠。移開該 額外之藥療即導致驟然之改善。 於血液學(紅.血球、白血球、及血球分類計數)上未 發現有不正常現象。血漿化學顯示不正常現象業已在硏究 開始時及於加上處理期間存在。AP之上升發生於6隻狗 中,ALT之上升發生於一隻狗中,GLDH之上升則發生於 3隻狗中’所有之其他被檢驗之參數仍保持在正常範圍內 3.討論 A WD 13卜138,一種新抗癲癎及焦慮減輕劑,乃在新 近被診斷爲患有慢性特發性癲癎之狗內,於此臨床初步硏 究中被評估,以試驗此物質之抗抽搐效果。類似於人類癲 癎’動物可被挑選成患有抗藥物性癲癎發作之狗及患有藥 物感受性癲癎發作之狗(L0SCHER,1997) 。 於本硏究中,獲自受 AWD 131-138處理之狗的資料 乃與來自受傳統抗癲癎藥療處理之結果作比較。爲了進行 硏究’所有之狗必須符合下列二項主要標準:正常之臨床 及神經學檢驗,在特別檢驗中不展示不正常狀況以及於開 始處理前,出現二次以上之癲癎發作。於患有慢性癲癎之 狗內’苯巴比妥之血淸濃度必須在治療範圍內。爲了使狗 -39- (36) 1324067 之來源寬廣,狗之挑選並不受品種、年齡、癲癎發作型類 以及癲癎發作頻數等因素的影響。因此不同的品種及混種 狗皆被包括在內。然而大種狗,例如德國狼狗或獵狗( G e r m a n S h e p h e r d 〇 r R e t r i e v e 1· s )則代表性過高,它反映 下列之已知事實:大種狗(>]5kg )受特發性癲癎之影響 程度遠大於小型狗(PODELL et al. ]995)。
包括於本硏究於AWD 131-138處理內及於傳統藥療 之回顧性評估內之硏究之所有部份內之大部份狗乃於其生 命之第一及第三年間首次癲滴發作。幾位作者述及特發性 癲癎大部份皆在此期間內開始(OLIVER 1987,OLIVER and LORENZ 1 9 93,CHRISMAN 1991,DE LAHUNTA 1 98 3, MARTINEK et al. 1970,CROFT 1965 and 1971, CENTER 1 9 8 6, CUNNINGHAM 19 7 1, SCHWARTZ-PORSCHE 1984 and FORRESTER et a]. 1989)。於本硏究中,—部份狗開 始癲癎發作時之年齡年青或老於1至3歲。在近乎所有之 群組中’帶有下列狀況之一隻老狗乃參與本硏究:此狗經 臨床診斷爲患有特發性癲癎’但帶有正常之臨床及特別檢 驗結果。即使因爲年老的關係而使診斷結果令人起疑,但 it些狗符合本硏究本發明之標準,且將資訊加於臨床初步 硏究之廣泛方位上。 爲欲接收有關存在於天然犬科特發性癲癎內之AWD 1 3 1 - i 3 8的理想劑量的資訊,開始硏究時,使所有之狗服 用5mg/ kg體重’口服’每日二次,持續—星期。於第 二星期後,於每隻狗內加倍此劑量。於未反應之狗內,將 -40- (37) 1324067 劑量增加至30m g/ kg體重,口服,每日二次。在 速增加之單一案例中,無副作用被觀察到,且高劑 可在處理開始時即使用。依照處理反應而修正之不 亦可予使用於受傳統藥療之組群內。因此,不同之 劃爲可供比較者。 爲了下列之目的,AWD 131-138之血漿濃度乃 量:於患有癲癎之狗口服單一劑量之新物質後,控 動力學,以及於硏究期間,控制狗主之一效性。藥 學展示血漿濃度之高度變異,此高度變異可能因物 同之組織內展現不同之分佈而造成。於以 AWD 1 進行藥療後3星期以及於不同之時間點處,血漿濃 同可變性亦發生。令人驚奇的發現的這樣的:在患 癲癎,且受苯巴比妥處理及 AWD 131-138補療之 最低値乃被發現。進一步之硏究必須予以執行以評 使苯巴比妥及AWD 131-138間之交互反應發生而 血漿濃度的話。血漿濃度與癲癎發作並無關聯。然 用 AWD 1S1-138劑量徐徐增加之狗中,癲癎發作 會發生,但僅是癲癎發作延緩會一段時間而已。因 作用被觀察到,更積極之處理規劃可予引入未來之 且增強患有特發性癲癎之狗的治療效果。 於新近被診斷爲患有特發性癲癎之狗內使用 13卜138所促成之癲癎發作頻數的減少可與於狗內 比妥或嘧啶進行處理所促成之癲癎發作相比較。苯 及嘧啶酮處理結果之本回顧性評估結果乃與先前描 劑量快 量似乎 同劑量 處理規 加以測 制藥物 物動力 質在不 31-138 度之相 有慢性 狗內, 估-假 導成低 而在服 之減少 爲無副 實驗, I AWD 以苯巴 巴比妥 述之硏 -41 - (38) (38)1324067 究(SCHWARTZ-PORSCHE et al. 1 9 8 5 )相一致。三處理 組並未在三種藥物之抗癲癎效率上顯現任何具有重大意義 之差異。計算癲癎發作頻數之減少(把未反應者排除在外 )顯示於處理前及於處理期間,三處理組之數値間存在著 具有重大意義之不同。在受 AWD 131-138處理組內,癲 癎發作減少之總百分率或多或少低於其他組內者。然而, 在預期性初步硏究組內之病狗數低於回顧性組內之病狗數 ,因此可能影響作用。在處理規劃開始之際,投服較高劑 量之AWD 13卜138可進一步改進新物質之抗抽搐效果ε 在患有慢性癲癎而受 AWD 1 3 1 - 1 3 8或溴化鉀加上治 療之狗內,補療以其他物質會影響癲癎發作頻數。然而, 在該二組中,狗病情獲得改善之程度卻類似。計算癲癎發 作頻數(於未反應者排除在外)之結果顯示在該二組內, 於處理前及處理期間,數値間並無重大之差異。未反應者 之百分率高於新近被診斷預期患有癲癎之狗內者。 除了癲癎發作頻數外,於處理前及處理期間之癲癎發 作持續時間及嚴重性亦加以評估。於A W D 1 3 1 -1 3 8藥療 期間,在1 /3以上案例內,癲癎大發作縮短。此現象在受 苯巴比妥處理之狗內鮮少發現(約1 /1 0狗)。相反地, 在受苯巴比妥處理之狗內,於藥療期間,發作持續時間之 中間値乃增加。然而,在所有受檢驗之組群內,發作後期 則縮短。除了發作及發作後期縮短以外,於受A W D 1 3 1 -1 3 8處理期間’單一癲癎發作事件之嚴重度亦減少。癲癎 大發作乃改變成局灶性癲癎大發作,聚集性癲癎大發作之 -42- (39)1324067

苯巴比妥或嘧啶酮。當與傳統之抗癲癎藥作比較時長期服 用 A WD 13 1-138時,耐受性良好,且觀察到之副作用較 低。這些正性結果支持A W D 1 3 1 -1 3 8之發展成爲一有效 之抗癲癎藥以供處理狗兒。意料之外的是,A WD ] 3 1 -1 3 8 對苯並二氮雜箪之受體展現非常低之親和力及部份激動活 力,此現象乃轉譯成:在患者體內展現抗抽搐活力,但造 成副作用之潛力則減少》使用較高數目之受處理動物及於 處理開始之際使用較高劑量之AWD 131-138之結果可能 導致更佳之臨床效率而不伴隨因劑量限制所造成之副作用 。再者,來自臨床輪廓之觀察指出:除此之外,藥物可能 在狗中展現減輕焦慮之效果。 【圖式簡單說明】
