В но бре 1973 г. установлен диагноз: двухсторонний мегауретерогидронефроз 2-3 стадии, обусловленный первичным пузырно-мочеточниковолоханочным рефлюксом, осложненный вторичным пиелонефритом. Больному выполн етс антирефлюксна пластика предложенным способом лечени пузырно-мочеточниково-лоханочного рефлюкса 3 феврал 1977 г. Под интратрахеальным наркозом с миорелаксантами, разрезом над лобком внебрюшинно обнажают переднюю и левую боковую стенки мочевого пузыр . Переходную складку брюшины сдвигают кверху . Выдел ют и берут на держалку левый мочеточник в нижней трети. Мочеточник И1у1еет утолщенные гипертрофированные стенки, его диаметр в юкставезикальном сегменте равен 12 мм, он активно сокращаетс . Вальдейерово влагалище отсутствует . Фиксируют отсутствие подслизистого сегмента мочеточника,т. е. отсутствие антирефлюксной защиты. Отдел ют от мышц слизистую мочевого пузыр и нижней трети мочеточника в области юкста везикального угла гидравлической препаровкой 0,25%-ного раствора новокаина. Делают разрез мышечной стенки мочевого пузыр от юкставезикального угла, по направлению , к его верхушке длиной 2,0 см до слизистой . Разрез продолжают на мочеточник на такую же длину. Полученную рану развод т . Дном раны вл ютс слизистые мочевого пузыр и мочеточника, его юкставезикального сегмента. Одноименные кра раны мочевого пузыр и мочеточника сшивают отдельными капроновыми щвами так, что слизистые мочеточника и мочевого пузыр накладываютс друг на друга, образу дубликатуру. Нити швов противоположных сторон св зывают над мочеточником. Выще щвов мочеточник фиксируетс к стенке мочевого пузыр отдельными швами. Операци выполн етс на полиэтиленовом перфорированном дренаже, введенном через устье мочеточника на высоту 12-14 см. Раны мочевого пузыр и передней брюшинной стенки послойно зашивают наглухо. Изменение просвета мочеточника на прот жении антирефлюксного клапана в щелевидно-полулунный позвол ет примен ть этот метод также и при диаметре мочеточника более 1 см.in 8 children.
