Предлагаемое изобретение относится к медицине, в частности к хирургии, гастроэнтерологии, и касается хирургического лечения рефлюкс-эзофагита (РЭ). The present invention relates to medicine, in particular to surgery, gastroenterology, and for the surgical treatment of reflux esophagitis (ER).
Известен способ хирургического лечения рефлюкс-эзофагита и скользящих грыж пищеводного отверстия диафрагмы, предложенный в 1956 г. R.Nissen. Автор после мобилизации абдоминального отдела пищевода и малой кривизны желудка, в области кардии, сшивал переднюю и заднюю стенки фундального отдела впереди пищевода, т. е. дном желудка, как манжеткой, по всему периметру окутывал пищевод ( Шалимов А.А., Саенко В.Ф. Хирургия пищеварительного тракта. Киев: Здоровья, 1987, с. 60-61). Недостатками данного способа являются: вынужденная, широкая мобилизация абдоминального отдела пищевода и кардио-фундального отдела желудка (рассечение верхней половины малого сальника и желудочно-селезеночной связки), что технически сложно и сопряжено с интраоперационными осложнениями (кровотечение, перфорация органа) особенно у тучных больных, при рубцово-склеротической деформации связочного аппарата кардии, выраженном периэзофагите и перигастрите ( Черноусов А.Ф., Крчак А.М., Степанкин С.Н. и др. Повторные операции после фундопликации по Ниссену. Хирургия, 1985, N 8, с. 5-10). Формирование известным способом фундопликационной манжеты приводит к нарушению естественной оси пищевода, выраженной деформации кардио-фундального отдела желудка, гиперфункции манжетки, что в послеоперационном периоде может проявиться дисфагией, синдромом порочной фундопликации. Частичное пересечение или сдавление блуждающих нервов осложняется гастростазом, парезом кишечника. При тракции дна желудка возможно ятрогенноое повреждение сосудов селезенки с последующим кровотечением. Прорезывание швов вследствие постоянного натяжения осложняется развертыванием фундопликационной манжетки или развитием "феномена телескопа" ( Уткин В.В., Амбалов Г.А., Демченко Ю.М. Опасности и осложнения фундопликации по Ниссену. //Хирургия, 1986, N 3, с. 88-91). Способ осуществляется следующим образом. По выполнении верхней срединной лапаротомиии, под интубационным наркозом, в желудок больного вводится толстый зонд. Доступ к кардии и абдоминальному участку пищевода облегчает мобилизация левой доли печени. Желудок оттягивают в каудальном направлении и ножницами надрезают брюшину над абдоминальным участком пищевода и осуществляют его мобилизацию. Обойдя пищевод, продевают вокруг него резиновую трубочку и, оттягивая ее в вентрокаудальном направлении, облегчают дальнейшую конструкцию фундопликации. Не прибегая к мобилизации фундального отдела желудка и рассечению желудочно-селезеночной связки, манжету образуют, проводя заднюю стенку дна желудка позади пищевода, и фиксируют в нижней точке к медиальной поверхности пищевода одним-двумя антравматическими серомускулярными швами, выше на 2 см от анатомической кардии. Таким способом препятствуют соскальзыванию манжеты. Затем выше этой точки на протяжении не менее 4 см переднюю стенку дна желудка сшивают с проведенной позади пищевода задней стенкой дна желудка при этом в шов захватывают правую ножку диафрагмы. Пищевод в накладываемые швы не захватывают. Для формирования манжеты обычно бывает достаточно наложить 4-5 швов. Лапаротомная рана ушивается. После завершения операции анестезиолог удаляет из пищевода зонд. The method is as follows. Without resorting to mobilization of the fundus of the stomach and dissection of the gastro-splenic ligament, the cuff is formed by conducting the posterior wall of the bottom of the stomach behind the esophagus, and is fixed at the lower point to the medial surface of the esophagus with one or two anthraumatic seromuscular sutures, 2 cm higher from the anatomical cardia. In this way, the cuff is prevented from slipping. Then, above this point for at least 4 cm, the anterior wall of the fundus of the stomach is sutured with the posterior wall of the fundus of the stomach, held behind the esophagus, while the right leg of the diaphragm is taken into the suture. The esophagus in the sutures are not seized. To form the cuff, it is usually sufficient to apply 4-5 stitches. The laparotomic wound is sutured. After the operation is completed, the anesthetist removes the probe from the esophagus.
Клинический пример. Больной Х., 44 лет (история болезни N 28408/95), поступил в клинику с жалобами на боли в эпигастрии, звучную отрыжку, мучительную изжогу. Рентгенологически выявлено смещение кардии в заднее средостение, желудочно-пищеводный рефлюкс. По данным фиброэндоскопии - недостаточность кардии, рефлюкс-эзофагит тяжелой степени. По результатам pH-метрии в абдоминальном отделе пищевода в горизонтальном положении закисление - pH 3,1. Эзофагоманометрия: градиент пищеводно-желудочного перехода снижен до 60 мм вод. ст. Произведено хирургическое вмешательство. Под эндотрахеальным наркозом выполнена верхняя срединная лапаротомия. В желудок введен зонд. Мобилизован и взят на держалку абдоминальный отдел пищевода. Не проводя мобилизации фундального отдела желудка, задняя стенка дна желудка проведена за пищевод и фиксирована к медиальной поверхности пищевода одним атравматическим серомускулярным швом в точке на 2 см выше анатомической кардии. Затем выше точки фиксации на протяжении 4 см передняя стенка дна желудка подшита к проведенной позади пищевода задней стенке дна желудка 4-мя узловыми швами, при этом в шов захвачена правая ножка диафрагмы. После завершения операции зонд из пищевода удален. Clinical example. Послеоперационное течение гладкое, выписан на 8-е сутки после операции. Обследован через 6 месяцев - жалоб не предъявляет. По данным рентгеноскопии и фиброэндоскопии отклонений от нормы нет, pH-метрия абдоминального отдела пищевода выявила нормоцидное состояние (pH 7,0), эзофагоманометрия: градиент пищеводно-желудочного перехода восстановлен (180,0 мм вод.ст.). Предложенный способ хирургической коррекции применен в клинике у 14 пациентов с рефлюкс-эзофагитом. Каких-либо осложнений, связанных с применением способа, не отмечено. Оценка результатов лечения в сроки до 2-х лет после операции проводилась по данным клинического и инструментального (рентгеноскопия, pH-метрия, эзофагоманометрия, фиброэндоскопия) обследования в сравнении с 46 больными, оперированными по традиционной методике (фундопликация Nissen). В I группе осложнений не наблюдалось, что наглядно доказывает преимущества предлагаемого способа хирургической коррекции РЭ в сравнении с фундопликацией Nissen. Данные, приведенные в таблице, с высокой степенью достоверности (p<0,05) доказывают, что антирефлюксный эффект предложенной фундопликации выше традиционно применяемой фундопликации Nissen в лечении рефлюкс-эзофагита. Таким образом, результаты клинических исследований показывают, что предложенная фундопликация повышает надежность фиксации желудочно-пищеводной манжеты, тем самым обеспечивая выраженный антирефлюксный эффект.
