Abstract
Изобретение относитс к медицине, конкретнее к детской хирургии и урологии. Цель - профилактика вывиха интрамураль- ного отдела мочеточника при оперативном лечении пузырно-мочеточникового рефлюк- са. Вскрывают мочевой пузырь, локализуют и инкубируют устье, фиксируют к детрузору стенку мочеточника. Выдел ют мочеточник окаймл ющим разрезом вокруг усть радиусом около 0,5 см слизистой мочевого пузыр . Мобилизуют внутрипузырный отдел мочеточника (I). Отступив на 3-3,5 см выше усть , фиксируют его стенку по задней полуокружности узловыми нерассасывающимис швами к детрузору с внутренней стороны у места вхождени мочеточника в мочевой пузырь (II). Затем укладывают свободную внутрипузырную часть мочеточника в его ложе и фиксируют устье к слизистой мочевого пузыр (III) на прежнем месте 3-4 кетгутовы- ми швами. Дефект слизистой над внутриру- зырным отделом мочеточника ушивают узловыми кетгутовыми швами (IV). 1 ил. tThe invention relates to medicine, more specifically to pediatric surgery and urology. Отступив на 3-3,5 см выше усть последнего дл устранени из- быточной подвижности мочеточника и стабилизации его внутрипузырного отдела, фиксируют его стенку по полуокружности узловыми швами из нерассасывающегос материала к детрузору с внутренней сторо- ны у места вхождени мочеточника в мочевой пузырь (II), устран недостаточность фиксирующего внутрипузырный отдел мочеточника в детрузоре механизма. Затем ук- ладывают свободную внутрипузырную часть мочеточника в его ложе и фиксируют устье к слизистой мочевого пузыр (III) на прежнем месте 3-4 кетгутовыми швами. Анамнез болезни больной К: за 4 года до поступлени на лечение предлагаемым способом была вы влена лейко- цитури . проводилось лечение по поводу пиелонефрита; год спуст при рентгеноуро- логическом обследовании диагностирован левосторонний пузырно-мочеточниковый рефлюкс, проводилось лечение парафиновыми аппликаци ми; еще через 3 года выставлен диагноз: левосторонний пузырно-мочеточниковый рефлюкс III степени , гипертоничный гиперрефлекторный мочевой пузырь, хронический пиелонефрит. Результаты обследовани : анализы крови и мочи без патологии, урофлоуметри - норм, скорость мочи, ретроградна цистометри - гипертоничный гиперрефлекторный мочевой пузырь. Профилометри уретеровези- кального соусть слева: при наполненном мочевом пузыре длина внутрипузырного отдела мочеточника 2,3 см, градиент давлени в нем 64 см Н20; при опорожненном мочевом пузыре: длина внутрипузырного отдела мочеточника 1,3 см, градиент давлени 6 см Н20. Под интубационным наркозом, после мобилизации передней стенки мочевого пузыр продольным разрезом вскрыт мочевой пузырь, найдено устье мочеточника, последний интубировали дренажными трубками с фиксацией их к кра м мочеточника кетгутовыми лигатурами (держалки ). Отступив от устьев на рассто ние 0,5 см окаймл ющим разрезом слизистой мочевого пузыр вокруг усть выделени и мобилизовали внутрипузырный отдел мочеточника на рассто нии около 4-5 см до его свободный подвижности в пределах пузыр- но-мочеточникового соусть , подт гива за держалки. Отступив на 3-3,5 см выше усть мочеточника дл устранени избыточной подвижности его и стабилизации внутрипузырного отдела фиксировали стенку мочеточника по полуокружности узловыми швами (капрон) к детрузору с внутренней стороны у места вхождени мочеточника в мочевой пузырь, устранив недостаточность механизма, фиксирующего внутрипузырный отдел мочеточника в детрузоре. Свободную часть внутрипузырного отдела мочеточника уложили в его прежнее ложе, устье фиксировали к слизистой мочевого пузыр на прежнее место 3-4 кетгутовыми швами. Дефект слизистой над внутрипузыр- ным отделом мочеточника ушили 2-3 узловыми кетгутовыми швами. Мочеточниковый катетер вывели через отдельное контрапер- туторное отверстие. Двухр дным кетгуто- вым швом ушили переднюю стенку мочевого пузыр над уретральным катетером . Послеоперационное течение гладкое, дизури не беспокоила, обострени пиелонефрита не было. Мочеточкиковый катетер удален на 5-е сутки, цистостомическа труб- ка удалена на 9-е сутки, после чего восстановилосьсамосто тельное мочеиспускание. Заживление раны первичным нат жением. Defect of the mucous membrane over the intravesical ureter department was stitched with 2-3 nodal catgut sutures. The ureteral catheter is pulled out through a separate contra-coupling hole. With a double-stranded catgut stitch, the front wall of the bladder was stitched over the urethral catheter. The postoperative course is smooth, dizuri was not disturbed, pyelonephritis was not exacerbated. The ureteric catheter was removed on the 5th day, the cystostomy tube was removed on the 9th day, after which the urinalis was restored. Wound healing by primary tension. Discharged home on the 12th day after the operation in a satisfactory condition.
Предложенный способ антирефлюкс- ной операции применен у 14 детей на 15 мочеточниках (в одном случае одномоментно с обеих сторон).
