Предлагаемое изобретение относится к медицине, а именно к травматологии и ортопедии, и может применяться для остеосинтеза переломов хирургической шейки плеча типа А, В, С в соответствии с универсальной классификацией переломов AO/ASIF [Мюллер М.Е. /Руководство по внутреннему остеосинтезу //М.Е. Мюллер, М. Альговер, Р. Шнайдер, Х. Виллингер. - Швейцария, Spinger-Verlag. - С.650].The present invention relates to medicine, namely to traumatology and orthopedics, and can be used for osteosynthesis of fractures of the surgical neck of the shoulder of type A, B, C in accordance with the universal classification of fractures AO/ASIF [Müller M.E. /Guide to internal osteosynthesis //M.E. В последние десятилетия в мире наблюдается рост переломов хирургической шейки плеча, особенно среди лиц старших возрастных групп, в связи с увеличением продолжительности жизни на фоне развития инволюционного остеопороза костей. У пациентов среднего возраста при отсутствии остеопороза при переломах шейки плеча со смещением отломков операцией выбора является открытая репозиция с применением различных металлофиксаторов, включая эндопротезирование плечевого сустава. Однако у пожилых людей применение остеосинтеза может быть ограничено плохим качеством кости и, как следствие, вероятностью замедленного сращения. Применение металлических конструкций на фоне остеопороза не обеспечивает даже первичной стабильности костных фрагментов и, следовательно, отсутствуют условия для эффективной консолидации. Кроме того увеличивается риск послеоперационной миграции металлоконструкции и необходимость проведения повторного оперативного вмешательства [Шагдуров В.А. Руководство по внутреннему остеосинтезу. М.: Ad Marginem; 1996. 780 с.]. После репозиции и фиксации пластиной и винтами, введенными, в том числе, в головку плеча, на рану послойно накладывают швы. После операции руку помещают в позицию отведения Время послеоперационной реабилитации 6-8 недель. Известно изобретение для остеосинтеза переломов и переломо-вывихов шейки плеча (Патент RU №71874, A61B 17/56, опубл. 27.03.2008), отличающееся тем, что применяют U-образную шпильку длинной 10 см, с расстоянием между сторонами 1 см, вогнутой по всей длине по радиусу 9 см и имеющей прямоугольное сечение 0,2-0,3 см. Внутренний остеосинтез U-образной шпилькой не требует обширного оперативного доступа и обнажения кости на большом протяжении. Это уменьшает опасность развития инфекционных осложнений по сравнению с остеосинтезом накостными пластинами. U-образная шпилька, введенная интрамедуллярно, ввиду небольшого сечения, не приводит к нарушению кровоснабжения, не вызывает участков некроза кости. Прототипом нашего способа является патент РФ №2749296 (A61B 17/56, A61B 17/58, опубл. 27.03.2011)), представляющий собой способ оперативного лечения перелома хирургической шейки плеча, заключающийся в том, что проводят репозицию отломков плечевой кости, накладывают Т-образную пластину, фиксируют дистальный отломок винтами через нижние отверстия пластины, формируют два канала в головке плечевой кости по передней и задней поверхности, проводят через сформированные в головке отверстия и отверстия в Т-образном выступе пластины серкляжных петель-стяжек, которые экстракортикально соединяют верхние отверстия пластины с головкой плечевой кости. Недостатком данного способа является незначительная прочность фиксации проксимального фрагмента перелома с помощью двух серкляжных петель. Предложенный нами способ остеосинтеза осуществляется следующим образом. После фиксации дистального отдела Т-образной пластины кортикальными винтами к диафизу плечевой кости, через отверстие Т-образного фрагмента пластины, расположенного ближе к передней поверхности головки плечевой кости, проводят губчатый винт, а через второе отверстие - серкляжную петлю, концы которой проводят через канал, сформированный по передней поверхности головки плеча, и из встречных концов проволочных петель формируют окончательный узел. Способ обеспечивает усиление фиксации проксимального фрагмента с помощью дополнительного губчатого винта, устанавливаемого вместо серкляжной петли, при этом сохраняется возможность минимизации интраоперационного повреждения костной ткани в зоне перелома и возможность применять остеосинтез даже при остеопоротическом процессе в головке плечевой кости, где есть риск миграции металлофиксатора.The proposed method of osteosynthesis is carried out as follows. The method provides enhanced fixation of the proximal fragment with the help of an additional spongy screw installed instead of the cerclage loop, while maintaining the possibility of minimizing intraoperative damage to the bone tissue in the fracture zone and the possibility of applying osteosynthesis even in the osteoporotic process in the head of the humerus, where there is a risk of metal fixator migration.
Остеосинтез по предложенному нами способу осуществляется следующим образом. Наилучший доступ к проксимальной части плечевой кости - через разрез вдоль бороздки между M.pectoralis и М.deltoideus. Расположение металлоконструкции представлено на фиг. 1. После ревизии перелома и открытой репозиции отломков Т-образную пластину (1) фиксируют к диафизу плеча (2) кортикальными винтами (3). Далее устанавливают один губчатый винт (4) в проксимальное отверстие, расположенное ближе к передней поверхности головки плеча. Затем сверлом формируют внутрикостный канал (5) по задней поверхности головки плечевой кости в направлении сверху вниз, через который проводят проволочный серкляж (6), концы которого протягивают в свободное отверстие Т-образного фрагмента пластины и формируют окончательный узел (7). Таким образом, фиксация проксимального отломка проводится с помощью проволочной петли-стяжки, проходящей экстракортикально над линией перелома (8) и губчатого винта. Данный способ позволяет минимизировать резорбцию костной ткани в периимплантационной зоне и позволяет применять данный способ даже на фоне остеопороза, когда другие металлофиксаторы не способны закрепиться в головке плечевой кости.Osteosynthesis according to our proposed method is carried out as follows. Thus, fixation of the proximal fragment is carried out using a wire loop-tie, passing extracortically above the fracture line (8) and a sponge screw. This method makes it possible to minimize bone tissue resorption in the peri-implantation area and allows this method to be used even against the background of osteoporosis, when other metal fixators are not able to gain a foothold in the head of the humerus.
