Переломы и вывихи костей стопы и таранной кости - редкая и сложная патология. Являясь следствием высокоэнергетических воздействий они часто встречаются при политравме. Из-за уникальной анатомии, особенностей кровоснабжения и многочисленных сложных сочленений лечение повреждений таранной кости костей стопы представляет значительные трудности. Их течение часто сопровождаются развитием инфекционных осложнений, дегенеративного артроза, деформаций среднего и заднего отдела стопы, аваскулярного некроза, неудовлетворительными исходами лечения и высоким уровнем инвалидности.Fractures and dislocations of the bones of the foot and talus are rare and complex pathologies. Известен способ лечения повреждений и заболеваний таранной кости (а.с. СССР №1146024), включающий резекцию патологических измененных тканей таранной кости, формирование паза по оси нагрузки в таранной кости, помещение аутотрансплантата в сформированный паз. Данный способ, в виду травматичности мягких и костных тканей, а также предполагаемого широкого доступа, ухудшает выскуляризацию вышеуказанных тканей, тем самым, провоцирует развитие асептического некроза таранной кости, а также, может привести к вальгусному положению стопы. Вместе с этим, данная операция травматична из-за необходимости взятия трансплантата из малоберцовой кости неповрежденной голени, не исключено развитие неврологических расстройств и переломов ослабленного участка кости неповрежденной конечности. Вышеуказанный костнопластический способ является достаточно травматичным, не позволяет производить дополнительную компрессию, что может привести к несостоятельности артродеза.A known method of treating injuries and diseases of the talus (USSR AS No. 1146024), including resection of pathological altered tissues of the talus, the formation of a groove along the load axis in the talus, placing an autograft in the formed groove. Наиболее близким по достигаемому техническому результату (прототип) является способ малоинвазивного артродеза подтаранного сустава (Патент РФ №RU2549297C1), который заключается в том, что операцию проводят путем прокола кожи по задней поверхности голени проксимальнее пяточного бугра на 1-1,5 см по наружному краю ахиллова сухожилия. Вводят в полость подтаранного сустава бор и обрабатывают суставную поверхность заднего отдела пяточно-таранного сочленения. Для обработки суставных поверхностей переднего отдела тарано-пяточного сустава выполняют дополнительный доступ путем разреза кожи, не превышающий 0,5-0,8 см, по тыльно-наружному краю стопы в проекции тарзального синуса. Формируют полость между пяточной и таранной костями с последующей установкой в нее аутотрансплантата из наружного мыщелка бедренной кости. В асептических условиях после двукратной обработки операционного поля растворами антисептиков под наркозом, либо одним из видов региональных анестезий на ортопедическом операционном столе производится линейный разрез до 4 см по переднемедиальной поверхности тыла стопы в проекции таранно-ладьевидного сустава. Тупо и остро с помощью распатора выделяется таранно-ладьевидный сустав, отодвигаются мягкие ткани. Под контролем электронно-оптического преобразователя (ЭОП) устанавливается остеотом с двумя клинками перпендикулярно суставу. Производится остеотомия суставных поверхностей под контролем ЭОП. Появившиеся отломки удаляются из раны, образованные остеотомированные поверхности тщательно обрабатываются костной ложкой. Зона сустава обрабатывается антисептическими растворами. После, через тело ладьевидной кости, вне ладьевидно-клиновидного сустава, проводятся 2 спицы Киршнера, под диаметр винтов 4,0 мм, спереди назад параллельно друг другу. По спицам с помощью канюлированного сверла рассверливаются каналы под винты 4,0 мм. По спице устанавливаются канюлированные винты 4,0 мм с шайбами с дальнейшей компрессией в зоне таранно-ладьевидного сустава. Channels for 4.0 mm screws are drilled along the needles using a cannulated drill. Cannulated screws 4.0 mm with washers are installed along the spoke with further compression in the area of the talo-navicular joint. The surgical wound is treated with antiseptic solutions with layer-by-layer suturing.

Больной В. 35 лет., поступил в клинику с болями в области стопы (факт травмы отрицает) с диагнозом: асептический некроз ладьевидной кости правой стопы. При поступлении выполнены рентгенограммы правой стопы и голеностопного сустава (Фиг. 1). Спустя 3 дня в плановом порядке через минидоступ к таранно-ладьевидному суставу с установкой остеотома с двумя клинками (Фиг. 2), выполнена остеотомия суставных поверхностей таранно-ладьевидного сустава (Фиг. 3). После через зону остеотомии проведены 2 спицы киршнера (Фиг. 3), по которым, посте рассверливания каналов канюлированным сверлом, установлены канюлированные винты 4,0 мм с шайбами с дальнейшей компрессией. На контрольной рентгенограмме правой стопы определялась надежная фиксация в зоне остеотомии винтами с каналом (Фиг. 5). Пациент передвигается с дополнительной опорой, без внешней иммобилизации, с нагрузкой 1/7 веса тела на правую нижнюю конечность.Patient V., 35 years old., Was admitted to the clinic with pain in the foot (denies the fact of injury) with a diagnosis of aseptic necrosis of the scaphoid bone of the right foot. Данный способ технически простой, не требует дорогостоящего оборудования, позволяет избежать прогрессирующего аваскулярного некроза и стойкого болевого синдрома и может быть использован в любом травматологическом отделении.Thus, the claimed method has shown high efficiency in the treatment of chronic intra-articular injuries and arthrosis of the talus and foot bones.
