Изобретение относится к медицине, а именно к травматологии и ортопедии, и предназначено для оперативного лечения 2-х фрагментарных переломов хирургической шейки плечевой кости.The invention relates to medicine, namely to traumatology and orthopedics, and is intended for surgical treatment of 2 fragmentary fractures of the surgical neck of the humerus.
Интрамедуллярный остеосинтез при переломах хирургической шейки плечевой кости имеет важное преимущество перед другими технологиями остеосинтеза. При интрамедуллярном остеосинтезе сохраняется периостальное кровоснабжение отломков, минимизируется интраоперационная кровопотеря, снижается степень травматизации мягких тканей, уменьшается длительность операции, что заметно повышает качество жизни больных уже в раннем послеоперационном периоде и приводит к улучшению анатомо-функциональных результатов лечения.Intramedullary osteosynthesis for fractures of the surgical neck of the humerus has an important advantage over other osteosynthesis technologies. В качестве прототипа нами выбран способ фиксации проксимального отломка плечевой кости при помощи двух спиц, проведенных через большой и малый бугорки плечевой кости по бокам от костно-мозгового канала (RU 2513594 С1). Эти спицы фиксируются в суставном отростке лопатки. При оперативном лечении перелома хирургической шейки плечевой кости проводят репозицию и фиксацию отломков с использованием интрамедуллярного фиксатора. При этом производят трансдельтовидный доступ, продольным разрезом длиной 2-3 см от края большого бугорка плечевой кости к центру головки плечевой кости рассекают надостную мышцу. Вводят две репонирующие спицы диаметром 3,2 мм в область большого бугорка плечевой кости по направлению к головке плечевой кости. Производят репозицию проксимального отломка плечевой кости путем наклона вниз и наружной ротации проведенных спиц, ориентируя головку плечевой кости в анатомически правильную позицию. Затем через акромиальный отросток лопатки в большой и малый бугорки плечевой кости проводят две фиксирующие спицы с учетом возможности проведения между ними интрамедуллярного стержня. После чего репонирующие спицы удаляют. Выполняют закрытую репозицию дистального отломка плечевой кости по отношению к фиксированному проксимальному отломку плечевой кости. В области головки плечевой кости на 1,5 см кзади и медиальнее межбугорковой борозды формируют трепанационное отверстие. Через него вводят в костномозговой канал гвоздь, соединяя проксимальный и дистальный отломки плечевой кости. Затем фиксирующие спицы удаляют и фиксируют гвоздь винтами в области головки, большого, малого бугорков и диафиза плечевой кости.During surgical treatment of a fracture of the surgical neck of the humerus, reposition and fixation of fragments is carried out using an intramedullary fixative. Пациент Н., 67 лет. Травма бытовая - упал дома на область правого плеча. Доставлен бригадой скорой помощи в стационар с диагнозом: Закрытый перелом хирургической шейки правой плечевой кости со смещением отломков. На 3-и сутки пациенту выполнена операция: закрытая репозиция отломков и, интрамедуллярный остеосинтез по предлагаемому способу. Все рентгенограммы плечевого сустава были выполнены в двух проекциях. На контрольных рентгенограммах через 6 недель с момента операции достигнута консолидация перелома правой плечевой кости. При осмотре через 18 недель с момента операции функция правого плечевого сустава восстановлена, объем движений полный, болевой синдром отсутствует.Patient N., 67 years old.
