Мыщелки бедренной и большеберцовой костей являются второй по распространенности локализацией асептического некроза после головки бедренной кости. Лечение данного заболевания направлено на остановку дальнейшего прогрессирования или отсрочку развития поздних стадий артроза коленного сустава. Технический результат заявленного изобретения достигается за счет того, что в предлагаемом способе лечения остеонекроза, включающем субхондральную декомпрессионную перфорацию суставных концов бедренной и большеберцовой кости, туннелизацию (перфорацию) мыщелков выполняют через кожный прокол в области наружной поверхности кости спицей Киршнера под артроскопическим и рентгенологическим контролем, при этом каналы расходятся веерообразно к противоположному кортикальному слою в трёх и более направлениях в одной плоскости, параллельно суставной щели коленного сустава, причем во встречном направлении с внутренней поверхности кости через кожный прокол осуществляется подобная же веерообразная туннелизация мыщелков. Способ осуществляется следующим образом.The method is carried out as follows.
Оперативное лечение проводят под эпидуральной анестезией. После обработки нижней конечности антисептиком выполняют диагностическую артроскопию коленного сустава. При необходимости проводится коррекция внутрисуставной патологии, однако преследуется органосберегающая тактика. После настройки электронно-оптического преобразователя, в области вмешательства спицей Киршнера выполняется кожный прокол с латеральной стороны метадиафиза бедренной кости. Точка проведения спицы определяется рентгенологическим и визуальным контролем. При рентгеноскопии на боковой проекции приблизительно на 10-15 мм от границы очага остеонекроза (ближней к суставной щели), параллельно суставной щели коленного сустава проводится линия (АВ) через мыщелок бедренной и/или большеберцовой кости (Фиг.3а). Точка С, которая делит линию АВ пополам (Фиг.3а) и является местом введения спицы. Больная Д., 45 лет. Диагноз: Остеонекроз мыщелков левой бедренной и большеберцовой костей 2 ст. по ARCO. Лечилась консервативно, принимая анальгетики и хондропротекторы. Эффект лечения был выражен слабо. На момент обращения жалобы на боль в левом коленном суставе, ВАШ 6-7. При пальпации по внутренней поверхности левого коленного сустава отмечала болезненность. На МРТ-снимках (Фиг.1). остеонекроз мыщелков бедренной и большеберцовой кости. После обработки операционного поля антисептиком была выполнена диагностическая артроскопия (Фиг.2б). Определялись интактный суставной хрящ и внутрисуставные структуры (Фиг.2а). После настройки электронно-оптического преобразователя на область манипуляции, спицей Киршнера диаметром 2.0 мм выполнен кожный прокол по наружней поверхности бедренной кости. Через намеченную точку веерообразно параллельно суставной щели проведено три канала к противоположному кортикальному слою (Фиг.3б). Аналогичные каналы выполнены из срединной точки медиального мыщелка бедренной кости по направлению к наружному мыщелку бедренной кости. Patient D., 45 years old. На второй день после операции пациентка приступила к ходьбе при помощи костылей без нагрузки на оперированную конечность. Через 1,5 недели после операции она отметила уменьшение интенсивности болевого синдрома при движении и увеличение амплитуды движений в левом коленном суставе (70-180°). При клиническом осмотре пациентки через три месяца после выписки из клиники интенсивность болевого синдрома значительно уменьшилась. Увеличилась функция сустава: безболезненное полное разгибание и сгибание до 50°. Дистанция стабильной безболезненной ходьбы до 2 км. Удовлетворена лечением.
