Abstract
Description
Изобретение относится к медицине, а именно к детской травматологии и ортопедии, и предназначено для лечения рассекающего остеохондрита мыщелков бедренной кости у детей и подростков.The invention relates to medicine, namely to pediatric traumatology and orthopedics, and is intended for the treatment of dissecting osteochondritis of the femoral condyles in children and adolescents.
Известен способ эндоскопически-ассистированного нанесения остеоперфорации, трансхондрально до зоны удовлетворительной васкуляризации (Герасименко М.А., Белецкий А.В., Жук Е.В. Болезнь Кенига коленного сустава у детей: клиника, диагностика и лечение. БМЖ, 2007, 2 (20)). Однако данная методика обладает рядом недостатков, а именно не определено точное количество остеоперфорации, необходимое для активации регенерации; понятие зона удовлетворительной васкуляризации понимается специалистами по-разному, а следовательно, нет гарантий, что использование способа каждым из специалистов приведет к желаемому результату - улучшению регенерации тканей. Известен способ лечения болезни Кенига у детей и подростков (патент RU №2360617С1). Последний является наиболее близким к предлагаемому нами способу, мы приняли его за прототип. A known method of treating Koenig's disease in children and adolescents (patent RU No. 2360617C1). The latter is the closest to our proposed method, we took it as a prototype. However, it has the following disadvantages:

Для достижения медико-технического результата предлагаемого способа выполняют артроскопию коленного сустава с помощью 2-х стандартных доступов (антеро-медиального и антеро-латерального), производят ревизию и санацию коленного сустава. Под контролем ЭОП проводят спицу-«метку», прокол которой выполняют через метафизарную зону бедренной кости и проводят в очаг остеосклероза. Затем, ориентируясь на спицу- «метку» и под периодическим контролем ЭОП, проводят рассверливание зоны остеосклероза бедренной кости, не повреждая хрящевую поверхность.To achieve the medical and technical result of the proposed method, arthroscopy of the knee joint is performed using 2 standard approaches (antero-medial and antero-lateral), revision and rehabilitation of the knee joint are performed. Способ выполняют следующим образом:The method is as follows:
Под наркозом, в асептических условиях, после обработки операционного поля йодопироном и спиртом из 2-х стандартных доступов: антеро-медиального и антеро-латерального, выполняют артроскопию коленного сустава. Сустав промывают физиологическим раствором (0,9% NaCl). Далее под контролем ЭОП проводят спицу-«метку», прокол которой выполняют через метафизарную зону бедренной кости и проводят в очаг остеосклероза (рис.1 и 2). После установки метки осуществляют остеоперфорацию спицей, имеющей диаметр 1.5 мм. Остеоперфорацию наносят в количестве 15-20 раз. После появления ощущения сопротивления здоровых тканей продолжают остеоперфорацию до хрящевой поверхности под контролем ЭОП (рис.3). При этом осуществляют артроскопический контроль хрящевой поверхности в области остеосклероза. Сустав промывают. В месте артроскопических доступов накладывают швы и асептическую повязку. Пациент Мамаев Е.В., 7 лет, поступил в ЦИТО с диагнозом: Рассекающий остеохондрит медиального мыщелка правой бедренной кости 2-я стадия. Из анамнеза известно: боли в правом коленном суставе стали беспокоить 1 год назад. Обратился за помощью по месту жительства, выполнены рентгенограммы и компьютерная томография, поставлен диагноз: Рассекающий остеохондрит медиального мыщелка правой бедренной кости 2-й стадии. Проводилось физиотерапевтическое лечение (электрофорез, ультразвук), ЛФК. С кратковременным положительным эффектом. Боли усиливались при физической нагрузке. При осмотре коленный сустав обычной формы и размеров. Ограничено максимальное сгибание в суставе. Пальпация умеренно болезненна в проекции медиального мыщелка бедренной кости. Рентгенологически в области проекции медиального мыщелка бедренной кости определяется бесструктурная гомогенная тень, отграниченная от нормальной костной ткани зоной просветления. Выполнена артроскопия: при ревизии суставной хрящ медиального мыщелка над зоной асептического некроза размягчен, целостность его сохранена. Под контролем ЭОП произведена постановка меток в область патологического очага внесуставно, спицей, имеющей диаметр 1.5 мм, нанесено 18 внесуставных остеоперфораций. После появления ощущения сопротивления здоровых тканей продолжают остеоперфорацию до хрящевой поверхности под контролем ЭОП. Артроскопический контроль показал, что хрящевая поверхность не повреждена. Наложены швы на место введения артроскопа. Со 2-х суток начато физиотерапевтическое лечение. Выписана домой на 5-е сутки на амбулаторное лечение. Нагрузка на конечность разрешена через 3 месяца после операции. On examination, the knee joint is of normal shape and size. Limited maximum flexion in the joint. Palpation is moderately painful in the projection of the medial condyle of the femur. Radiologically, in the area of the projection of the medial condyle of the femur, a structureless homogeneous shadow is defined, delimited from normal bone tissue by the enlightenment zone. Arthroscopy was performed: during revision of the articular cartilage of the medial condyle over the area of aseptic necrosis is softened, its integrity is preserved. Under the control of the image intensifier, marking was made in the region of the pathological lesion extra-articularly, with a spoke having a diameter of 1.5 mm, 18 extra-articular osteoperforations were applied. After the sensation of resistance of healthy tissues appears, osteoperforation is continued to the cartilaginous surface under the control of the image intensifier. Arthroscopic examination showed that the cartilaginous surface was not damaged. Sutures were placed at the site of arthroscope insertion. From the 2nd day, physiotherapy treatment has been started. Discharged home on the 5th day for outpatient treatment. Limb load allowed 3 months after surgery. A total of 36 patients were operated on.
