Abstract
Description
The invention relates to medicine, namely to the surgical treatment of injuries and diseases of the spine and can be used to prevent intraoperative blood loss.
For surgical interventions in the posterior spine, especially transpedicular fixation with submersible structures, it is necessary to form operational access to the arches of the vertebrae.
Options for tissue separation in surgery are divided into two groups: first, separation is achieved by using a scalpel or other cutting tool; the second - tissue separation is carried out using blunt instruments, for example, an elevator.
Known is the formation of access to the posterior spine using layer-by-layer dissection of tissues - skin, fascia and muscles with a scalpel (Movshovich I. A. Operative orthopedics, Moscow, 1983, 413 pp.). The disadvantage of this method is blood loss, which inevitably occurs when injuring arterial and venous vessels , the size of which can reach, on average, up to 150 ml (Berdyugin K.A., Berdyugina O.V., Kutepov S.M. Errors and complications of spinal osteosynthesis / edited by Belokrylov N.M. - Yekaterinburg: Publishing house of GBOU VPO UGMA of the Ministry of Health and Social Development of Russia, 2011 .-- 326 s). In addition, when using standard methods of stopping blood, for example, with tamponade, an increase in the size of the surgical field is often required.
769-771.). https://sinref.ru/000_uchebniki/03200medecina_2/308/050.htmThere is a method of separating soft tissues in the region of the posterior spine using an elevator (B. Boychev, V. Comforti, K. Chokanov Operative orthopedics and traumatology, State Publishing House "Medicine and Physical Education" Sofia "1961, S. 769-771.). https://sinref.ru/000_uchebniki/03200medecina_2/308/050.htm
However, bleeding is inevitable with access, which can reduce the visibility of the area of operation, therefore adequate hemostasis is required.
A technique is known for posterior access to the thoracic and lumbar spine, (https://meduniver.com/Medical/travmi/zadnii_dostup_k_pozvonoc hniku.html), including a midline skin incision, dissection of the subcutaneous layer to the fascia and insertion of a retractor. With access, severe bleeding from the external superior venous plexus of the vertebrae is possible, stopped by tamponade, for example, during operations for scoliosis, subperiosteal discharge is carried out to the end of the transverse processes, and the liberated sides are tamped with gauze tuffers. However, with tamponade with gauze tuffers, bleeding does not stop completely, while the number of tupers must be increased, which accordingly leads to a decrease in the area of the surgical field, and this will require an additional increase in the incision.
Известен способ передней декомпрессии спинного мозга при переломах грудных и поясничных позвонков, включающий задний оперативный доступ к грудному отделу позвоночника: разрез кожи, рассечение фасций, резекцию дужки ножки дуги и межпозвонкового сустава (Aebi М, Etter С, Kehl Т, et al. Stabilization of the lower thoracic and lumbar spine with the internal spinal skeletal fixation system. Indications, techniques, and first results of treatment. // Spine. - 1987. - V. 12. - P. 544-551). There is a method of anterior decompression of the spinal cord for fractures of the thoracic and lumbar vertebrae, including rear operative access to the thoracic spine: skin incision, fascia dissection, resection of the arch of the pedicle of the arch and intervertebral joint (Aebi M, Etter C, Kehl T, et al. Stabilization of the lower thoracic and lumbar spine with the internal spinal skeletal fixation system. Indications, techniques, and first results of treatment. // Spine. - 1987. - V. 12. - P. 544-551). The surgical field, in comparison with similar accesses, is sufficient in size for effective surgical intervention, in particular, open transpedicular fixation of the spine with an immersion structure. To reduce bleeding, subcutaneous vessels are coagulated and plugging of the surgical field is performed, however, intraoperative bleeding is possible and it does not stop completely, its consequence is a decrease in the visualization of the surgical field, which affects the effectiveness of surgical intervention.
The technical task of the invention is the reduction of intraoperative blood loss during surgical access to the posterior support complex of the spine, while maintaining access dimensions and improving visualization of the surgical field.
To achieve a result after a midline skin incision above the spinous processes of the vertebrae and dissection of the fascia of the back during tissue separation, the surgical wound is tamped with gauze soaked in 2% hydrogen peroxide, while the tissue is advanced between the points of attachment of the longest back muscle to the vertebrae, first on one side, then on the other hand, after five minutes, a tight swab wipes are removed and the necessary surgical intervention is performed.
The use of gauze napkins moistened with 2% hydrogen peroxide for tamponade of the operative field, promoting the napkin when separating tissues between the points of attachment of the longest muscle of the back to the vertebrae, first on the one hand, then on the other allows hemostasis of the surgical wound, which reduces intraoperative bleeding, and, accordingly, improve visualization of the surgical field without losing its size.
The method is as follows.
Surgery is performed under endotracheal anesthesia in the position of the patient on the abdomen. The skin incision is made in the midline over the spinous processes for 1-2 vertebrae above and below the damaged. Next, the fascia of the back is dissected with an electric knife. After that, they take a gauze napkin, abundantly soaked with hydrogen peroxide 2%, which is carefully squeezed and with the help of a wide elevator or other tool begin its advancement between the points of attachment of the longest back muscle to the vertebrae. At the end of tissue separation, a tight wound tamponade is made with a 2% hydrogen peroxide napkin. This allows for the implementation of chemical and physical hemostasis in the on-line access. Then similarly carry out the separation of tissues and the introduction of napkins on the other side of the surgical field. Five minutes later, wipes are removed from the wound. In the place of their adherence to the soft tissues, effective hemostasis is formed.
1-5.The method is illustrated by the photos shown in FIG. 1-5.

After access is performed, transpedicular fixation of the spine or other prescribed surgical intervention is performed according to the standard scheme.
The proposed method of operative access to the posterior support complex of the spine was used in surgical interventions in 1200 patients with fractures and diseases of the spine, while intraoperative blood loss was less than 50 ml.
Thus, the application of the proposed method allows to practically eliminate intraoperative blood loss, the visualization of the surgical field is improved, its size is preserved, while the efficiency of the surgical intervention is increased without increasing the duration of the operation.
