Поставленная задача решается тем, что в способе ушивания перфоративного отверстия, включающем два этапа: 1) подготовительный этап, способствующий улучшению условий для сведения краев перфоративного отверстия и снижения вероятности прорезывания нити за счет уменьшения напряжения в зонах максимального сопротивления при сжатии кругового кольца взаимно противоположными силами путем произведения секторального трапециевидного иссечения серозной оболочки (при точечных перфорациях - простое линейное рассечение) стенки двенадцатиперстной кишки в поперечном направлении краниально и каудально от перфоративного отверстия, причем размер основания трапециевидных лоскутов равен диаметру перфоративного отверстия, верхушки располагаются на границе перифокального вала; 2) наложение асептического непрерывного серозно-мышечного шва, при этом первый стежок шва делают на расстоянии 0,5-1,0 см орально и краниально от перфоративного отверстия, причем первый вкол делают в поперечном направлении, на нити предварительно сформирован первый узел-«фиксатор», второй, третий и четвертый стежки окаймляют перфоративное отверстие по типу полукисета, пятый стежок параллелен первому каудально и орально от пефоративного отверстия, нить натягивают до соприкосновения противоположных краев перфоративного отверстия и формируют второй узел-«фиксатор». По указанному способу в ГК БСМП-2 города Омска произведено ушивание перфорации при прободной пилородуоденальной язве у 18 пациентов, послеоперационный период протекал гладко, осложнений не отмечено. При ФГДС-контроле отмечено рубцевание язвы по линейному типу, без деформации просвета.
