Известен способ удлиняющей тенотомии ахиллова сухожилия, заключающийся в проведении продольного разреза в сагиттальной плоскости через всю толщину сухожилия, выполнении поперечных разрезов, идущих из верхнего и нижнего концов продольного разреза в противоположных направлениях с рассечением лишь половины поперечника сухожилия, смещении концов сухожилия относительно друг друга и сшивании их при средне-физиологическом натяжении (см. Лубегина З.П. Предлагаемый способ осуществляется следующим образом. Острым концом скальпеля выполняют прокол кожи 1 на 1 см выше места прикрепления ахиллова сухожилия 2 к пяточной кости 3. Погружают конец скальпеля до ахиллова сухожилия 2. Определяют концом скальпеля границы наружного и внутреннего края ахиллова сухожилия 2. Располагают острый конец скальпеля у внутреннего края ахиллова сухожилия 2 на вентральной его поверхности 4. Выполняют повторяющиеся движения скальпеля от внутреннего края к наружному краю сухожилия сверху вниз в горизонтальной плоскости, постепенно пересекая ахиллово сухожилие 2 поперечно в вентро-дорзальном направлении с сохранением неповрежденной части 5 менее чем 1/4 его диаметра и не доходя до задних отделов сухожильного влагалища 6 и брыжеечных сосудов 7 (фиг. 1). Выводят стопу в положение возможной тыльной флексии за счет разгибания в голеностопном суставе и надрывают ахиллово сухожилие 2 в неповрежденной его части 5 с расхождением концов сухожилия без повреждения задних отделов сухожильного влагалища 6 и брыжеечных сосудов 7 (фиг. 2). Клинический пример. Больной С., 3 месяцев, поступил в отделение ортопедии детей ФГБУ «ННИИТО» с диагнозом: врожденная правосторонняя косолапость 3 степени на этапе лечения. В плановом порядке выполнено оперативное лечение по предложенной методике. Больной осмотрен через 6 месяцев. Стопа в правильном положении. Осевая опора возможна на все отделы стопы. Пальпаторно определяется восстановление анатомической целостности ахиллова сухожилия на всем протяжении. Пассивные и активные движения в голеностопном суставе в полном объеме. Сила подошвенной флексии расценена как хорошая. Ребенок пользуется ортопедической обувью. Восстановительное лечение проводится курсами раз в 3 месяца.Clinical example. Частичное поперечное пересечение ахиллова сухожилия с сохранением неповрежденной части менее чем 1/4 его диаметра, которую надрывают при устранении эквинусного положения стопы, исключает повреждение сосудов брыжейки ахиллова сухожилия, сохраняя нормальное кровоснабжения частей сухожилия. В результате чего регенерация сухожилия происходит в благоприятных условиях за счет замещения диастаза между концами сухожилия волокнистой тканью, по своим механическим свойствам, внешнему виду и функции ничем не отличающейся от здорового сухожилия. После надрыва непересеченной части сухожилия расхождение его концов происходит дозированно на величину, необходимую для устранения деформации. Удержание концов от чрезмерного расхождения происходит за счет эластичных свойств неповрежденных задних отделов сухожильного влагалища, брыжейки сухожилия. Эластичные свойства неповрежденной части задних отделов сухожильного влагалища, брыжейки сухожилия частично воссоздают функциональное напряжение мышцы и предупреждают ее дегенеративное перерождение, что позволяет значительно улучшить функциональный результат лечения. Пересечение ахиллова сухожилия в вентро-дорзальном направлении с сохранением неповрежденной части менее чем 1/4 его диаметра осуществляется за счет повторяющихся движений скальпеля от внутреннего края к наружному краю сухожилия и сверху вниз в горизонтальной плоскости. Это исключает возникновение повреждения заднего большеберцового сосудисто-нервного пучка и предупреждает развитие таких грозных осложнений, как кровотечение, образование ложных аневризм, денервации стопы. Подкожный разрыв ахиллова сухожилия происходит в месте его неполного пересечения без приложения значительных усилий. Незначительный объем рассекаемых структур при коррекции деформации исключает формирование обширных и грубых рубцов, снижает риск возникновения инфекционных осложнений в послеоперационном периоде и возврата деформации в отдаленные сроки. Способ прост по технике исполнения, позволяет получить стойкую коррекцию деформации и улучшить функциональный результат оперативного лечения.The method of subcutaneous transverse tenotomy of the Achilles tendon in children is less traumatic and can effectively eliminate the equinus deformity of the feet. The elastic properties of the undamaged part of the posterior tendon vagina, the tendon mesentery partially recreate the functional tension of the muscle and prevent its degenerative degeneration, which can significantly improve the functional result of the treatment. The intersection of the Achilles tendon in the ventro-dorsal direction with the preservation of the intact part of less than 1/4 of its diameter is due to repeated movements of the scalpel from the inner edge to the outer edge of the tendon and from top to bottom in the horizontal plane. This eliminates the occurrence of damage to the posterior tibial neurovascular bundle and prevents the development of such formidable complications as bleeding, the formation of false aneurysms, and denervation of the foot. Subcutaneous rupture of the Achilles tendon occurs at the site of its incomplete intersection without significant effort. An insignificant volume of dissected structures during the correction of deformation excludes the formation of extensive and rough scars, reduces the risk of infectious complications in the postoperative period and the return of deformation in the long term. The method is simple in execution technique, allows to obtain a stable correction of deformation and improve the functional result of surgical treatment.
