Известен способ удлиняющей тенотомии, заключающийся в проведении продольного разреза в сагиттальной плоскости через всю толщину сухожилия, выполнении поперечных разрезов, идущих из верхнего и нижнего концов продольного разреза в противоположных направлениях, рассекая лишь половину поперечника сухожилия, смещении концов сухожилия относительно друг друга и сшивании их при среднефизиологическом натяжении (см. Лубегина З.П. Дегенеративные изменения в концах сухожилия снижают их механическую прочность, увеличивая риск возникновения несостоятельности сухожильного шва. Высокий риск прорезывания сухожильных швов исключает начало функционального лечения в послеоперационном периоде. Кроме того, при сшивании концов рассеченного сухожилия образуется значительное утолщение этого участка, что приводит к образованию обширных и плотных рубцовых сращений, затрудняющих нормальную функцию конечности.The known method has significant disadvantages. Способ удлиняющей тенотомии выполняется следующим образом. Производят продольный разрез кожи длиной до 3-4 см в проекции укороченного сухожилия. Выделяют сухожилие из синовиальных оболочек. Выполняют проксимальные и дистальные поперечные разрезы 1 и 2, а между ними - промежуточные поперечные разрезы 3 и 4 в сагиттальной плоскости через всю толщину сухожилия до его середины поочередно к наружному и внутреннему краям на одинаковом расстоянии друг от друга. Производят косопоперечные разрезы 5 и 6 под углом 135°, равные по длине поперечным, идущие из вершины проксимального и дистального поперечных разрезов 1 и 2 во встречном направлении. Выполняют по два косопоперечных разреза 7 и 8 под углом 135° во взаимно противоположных направлениях из вершины промежуточных поперечных разрезов 3 и 4, равные им по длине (фиг.1). Натягивают (напрягают) сухожилие и производят его удлинение за счет раскрытия поперечных разрезов 1, 2, 3, 4 и косопоперечных разрезов 5, 6, 7, 8 с образованием диастазов 9, 10, 11, 12 (фиг.2). Клинический пример. Больной З., 12 лет, поступил в отделение ортопедии детей ФГУ «ННИИТО Росмедтехнологий» с диагнозом: эквинополая деформация правой стопы. В плановом порядке выполнено удлинение ахиллова сухожилия справа по предложенной методике. Больной осмотрен по снятию гипсовой повязки через 1 месяц после операции. Стопа в правильном положении. Пальпаторно определяется анатомическая целостность ахиллова сухожилия на всем протяжении. Пассивные и активные движения в голеностопном суставе в полном объеме. Удлинение сухожилия осуществляется за счет его рассечения в поперечном и косопоперечном направлениях с сохранением непрерывной целостности сухожилия, что значительно снижает травматичность операции. Несмотря на то что при выполнении поперечных разрезов сухожилие пересекается только на 1/2 его диаметра, дополнительные разрезы в косопоперечном направлении компенсируют этот недостаток и усиливают степень раскрытия разрезов, значительно увеличивая величину удлинения сухожилия. Необходимая величина удлинения легко дозируется полнотой раскрытия поперечных и косопоперечных разрезов при натяжении сухожилия. Удлинение достигается за счет небольшого количества разрезов и не требует широкого обнажения сухожилия, что значительно снижает травматичность процедуры. Отсутствие необходимости выделения сухожилия на большом протяжении дает возможность сохранить сосуды его брыжейки, что создает оптимальные условия для последующей регенерации. Сочетание поперечных разрезов с разрезами, идущих под углом в косопоперечных направлениях, позволяет эффективно удлинять сухожилия среднего и крупного диаметра. При достижении эффекта удлинения не происходит утолщения, деформации сухожилия, что снижает риск образования обширных и плотных рубцовых сращений, улучшая функциональный результат лечения.
