Abstract
Description
Острый варикотромбофлебит (ОВТФ), развивающийся у 30% больных варикозной болезнью нижних конечностей (ВБНК), является одной из актуальных проблем общехирургической практики. Лечение этой группы больных является непростой социальной задачей, учитывая высокую распространенность фоновой нозологии среди взрослого населения и опасность тромбоэмболических осложнений. Как известно, при выраженном паравазальном воспалении радикальные интервенции на основе классической флебэктомии травматичны, повышают риск развития раневых осложнений, не всегда приемлемы с эстетических позиций. С другой стороны, рутинная консервативная терапия также не в состоянии быстро купировать локальный тромбовоспалительный процесс, затягивая его на длительный срок. Все эти моменты диктуют необходимость разработки более совершенных лечебных методик, в которых эффективные приемы традиционной хирургии рационально сочетались бы с принципиально иными энергиями, к числу которых относятся плазменная технология, эндовазальная лазерная коагуляция (ЭВЛК).Acute varicothrombophlebitis (AFVF), which develops in 30% of patients with varicose veins of the lower extremities (VBNK), is one of the urgent problems of general surgical practice. Treatment of this group of patients is a difficult social task, given the high prevalence of background nosology among the adult population and the risk of thromboembolic complications. As you know, with severe paravasal inflammation, radical interventions based on classical phlebectomy are traumatic, increase the risk of wound complications, and are not always acceptable from an aesthetic point of view. On the other hand, routine conservative therapy is also not able to quickly stop the local thrombus-inflammatory process, delaying it for a long time. All these points dictate the need to develop more advanced therapeutic techniques in which the effective methods of traditional surgery would be rationally combined with fundamentally different energies, which include plasma technology, endovasal laser coagulation (EVLK).
Разработанный нами способ радикального лечения ОВТФ осуществляется следующим образом. Под спинномозговой анестезией в качестве первого этапа комбинированной флебэктомии (КФ) выполняют перевязку несостоятельных перфорантных вен и варикозно расширенных притоков БПВ (МПВ) на стопе и голени из небольших эпифасциальных доступов, локальную тромбэктомию по методике K.Sigg (1976), а также кроссэктомию с лигированием всех приустьевых притоков БПВ (МПВ). При наличии флотирующего тромба с пролабированием его верхушки из БПВ (МПВ) в глубокие вены нижней конечности кроссэктомию сочетаем с удалением флотирующей «головки» тромба из сафено-феморального (сафено-поплитеального) соустья. Культя вены не более 0,5-0,6 см. Дистальный конец ствола БПВ (МПВ) раскрыт на зажимах, в просвете его определяются мягкие тромботические массы, которые затем удаляются на максимально возможном протяжении с помощью электроотсоса или баллонного ангиокатетера Фогарти. После этого мы приступаем ко второму этапу КФ - эндовазальной лазерной коагуляции (ЭВЛК) ствола БПВ (МПВ), которая состроит из нескольких последовательно выполняемых:Our developed method for the radical treatment of AFP is as follows. Положение рабочего органа в дистальном сегменте БПВ (МПВ) контролируется пальпаторно, визуально (по свечению пилотного лазера), а также методом интраоперационной ультразвуковой навигации.1. 2. Инфильтрационная тумесцентная анестезия. Ее цель - защита паравазальных тканей и анатомических структур (лимфатические коллекторы, нервы), расположенных в проекции коагулируемого ствола БПВ (МПВ), от повреждающего термического воздействия. Мы предлагаем для этого охлажденный озонированный физиологический раствор (температура 6-7°С, концентрация озона 4-5 мкг/мл, источник - установка "Медозон"). Инфильтрация раствора осуществляется под непрерывной ультразвуковой навигацией строго в фасциальный футляр вены (дополнительная компрессия и спазм), а также над ними в подкожную клетчатку над поверхностной фасцией (дополнительная защита кожи). Общий объем раствора не превышает 500 мл: примерно 350 мл рассчитывается на бедро и 150 мл на голень.2. Infiltration tumescent anesthesia. 3. Лазерная коагуляция несостоятельных стволов БПВ (МПВ). Источник - импульсно-периодический аппарат «Лазермед-1-25» (длина волны 1,06 мкм, ООО «РИК», г.Тула), режим воздействия только постоянный. Мощность излучения изменяется е учетом диаметра ствола БПВ (МПВ) в диапазоне от 20 до 25 Вт. Световод извлекается из подкожной вены с шагом 2-4 мм/сек.3. Перед наложением швов все операционные раны, зону ЭВЛК, а также область варикотромбофлебита последовательно обрабатывают аргоно-плазменным потоком в терапевтическом режиме (источник - установка «Скальпель Плазменный - ЦПТ»). Облучение расфокусированной плазменной струей проводится ежедневно, из расчета 10 секунд на фокусное поле площадью 1 см2, расстояние от сопла манипулятора до поверхности 3-4 см, температура охлажденного ионизированного потока в зоне контакта 37-38°С, продолжительность процедур от 4 до 7 мин. Плазменная стимуляция всех зон хирургической интервенции продолжается в течение первой недели послеоперационного периода ежедневно. По завершении КФ осуществляется пластическая компрессия оперированной конечности с помощью лечебного трикотажа II класса. Последняя носит постоянный характер и усиливается дополнительной компрессией БПВ (МПВ) с помощью латексных валиков, устанавливаемых по ходу коагулированного сосуда. Проводится противовоспалительная, флеботропная и симптоматическая терапия в сочетании с аппликацией гепаринсодержащих мазей. The latter is permanent and is enhanced by additional compression of the BPV (MPV) using latex rollers installed along the coagulated vessel. Anti-inflammatory, phlebotropic and symptomatic therapy is carried out in combination with the application of heparin-containing ointments. Dynamic ultrasound control of the EVLK area was carried out on the 2nd, 7th and 14th day of the postoperative period.
Вышеизложенная методика радикального лечения ОВТФ с использованием "высоких" энергий применен у 4 пациентов с давностью патологического процесса от 2 до 7 суток. Каких-либо особенностей течения послеоперационного периода не отмечено. Выраженность болевого синдрома отличалась от таковой у лиц, оперированных по общепринятой методике (классическая КФ) в сторону значительного уменьшения. Ни в одном случае не отмечено развития парестезии, воспалительно-гнойных осложнений и тромбоза магистральных вен. Локальная тромбэктомия в сочетании с регулярной плазменной обработкой в режиме стимуляции заметно ускоряла разрешение ОВТФ (перифлебита, болей).The above method of radical treatment of AFP using "high" energies was used in 4 patients with a prescription of the pathological process from 2 to 7 days. Классические радикальные вмешательства по типу КФ обычно включают кроссэктомию, механическую экстракцию пораженных несостоятельных стволов БПВ (МПВ), а также их притоков на голени и бедре с использованием традиционных оперативных приемов (операции Бэбкокка, Нарата), эпифасциальную перевязку несостоятельных перфорантных вен (НПВ) на голени (операция Кокетта), а также множественную локальную тромбэктомию (флебоцентез) из подкожных притоков БПВ (МПВ) небольшого калибра но методике K.Sigg (1979).Classical CF-type radical interventions usually include crossectomy, mechanical extraction of the affected defective BPV (MPV) trunks, as well as their tributaries on the lower leg and thigh using traditional surgical techniques (Babcock, Narat operation), epiphasic ligation of the incompetent perforating veins (IVC) on the lower leg (Coquette operation), as well as multiple local thrombectomy (phlebocentesis) from the subcutaneous inflows of the small intestine lesions (MPV) using the K.Sigg method (1979).
