Abstract
Description
The invention relates to medicine, namely to vascular surgery, and can be used for the effective prevention of life-threatening thromboembolic complications and the treatment of varicose veins in the prehospital (outpatient) stage without the need for hospitalization of patients.
Varicothrombophlebitis is a disease associated with the presence of a thrombus in the superficial vein, which is usually accompanied by clinically determined inflammation of the vein wall around the thrombosis zone. Among all patients with varicothrombophlebitis of superficial veins, a large proportion of patients are patients with varicose veins. The treatment of patients with thrombophlebitis of the superficial veins of the lower extremities with varicose veins requires an integrated approach aimed at stopping the main symptoms of the disease and, most importantly, preventing life-threatening complications, such as pulmonary embolism.
One of the promising directions in the treatment of varicose veins of the lower extremities is the methods of thermobliterating action on the venous wall: Endovenous Laser Obliteration (EVLO) and Radio Frequency Obliteration (RFO). The indicated minimally invasive methods of thermobliteration are performed under outpatient anesthesia under foggy anesthesia, practically without interrupting the patient from work, and provide a high cosmetic effect and quality of life. Good tolerance of the procedure and low invasiveness allows us to recommend this technique for widespread use.
Currently, most European and world phlebological communities endovenous obliterative techniques are considered as the operation of choice in the treatment of varicose veins of the lower extremities. The classical methods of surgical treatment are resorted to only if there are contraindications to their implementation.
A comparative analysis of the long-term results of treatment of varicose veins with the help of endovasal technologies (EVLO and RFA) shows the advantages of EVLO. If we consider, as a result of radical treatment, not occlusion, but the absence of a visualized venous segment during ultrasound angioscanning, its complete resorption, which confirms the radical nature of the technique, then the most radical technique is EVLO.
Thus, an analysis of domestic and foreign literary sources showed that EVLO has a clear advantage over classical surgical methods for treating varicose veins of the lower extremities. At the same time, we did not find reports of its use as a method of treating ascending varicothrombophlebitis in the lower extremities.
A known method for the radical treatment of acute varicothrombophlebitis of the lower extremities using laser and plasma technologies, which consists in performing a crossectomy, direct and / or indirect thrombectomy, ligation of failed perforating veins and varicose dilated inflows of the saphenous veins, and eliminating pathological venous veins with the help of laser coagulation (EVLK) of insolvent saphenous vein trunks, which is performed under tumecent anesthesia of the EVLK zone with chilled 6-7 ° C with ozonized physiological saline at an ozone concentration of 4-5 μg / ml, continuous ultrasound navigation is carried out, then treated with a constant cure rate of 20-25 W with an argon-plasma flow of the EVLK zone of varicothrombophlebitis and surgical wounds, followed by elastic compression operated limb (RF Patent No. 2466687, IPC АВВ 17/00, publ. November 20, 2012 - prototype).
In the known method, surgery is performed under spinal anesthesia, which increases the postoperative period, requires treatment in a hospital, and may cause headaches in the patient. To enter an optical fiber (fiber), the main saphenous vein is freed from thrombotic masses, which in this case seems impractical, complicates the operation and can lead to damage to the vein. The endovasal laser coagulation (EVLK) procedure for failed trunks of saphenous veins is performed using a high-power laser of 20-25 W, which can lead to burns of paravasal tissues.
The problem to which the claimed invention is directed is to create an effective minimally invasive method for treating patients with acute ascending varicothrombophlebitis of the veins of the lower extremities, contributing to the rapid rehabilitation of patients, reducing the time of temporary disability and accelerating their social and labor rehabilitation.
The problem is solved in that the method of treating acute ascending varicothrombophlebitis of the veins of the lower extremities by endovenous laser obliteration, according to the claimed invention, consists in pre-operatively preparing, performing a puncture at a distance of 0.5 cm to 1 cm above the thrombus head, through the installed sheath is inserted over the thrombus by a fiber with a conical end face, under ultrasonic control they perform tumecent anesthesia and perform endovenous laser obliteration of the trunk into at a rate of 70 to 100 J per 1 cm of vein, the wavelength of the laser radiation was chosen equal to 1460 nm, followed by elastic compression of the operated lower limb. Tumescent anesthesia is performed with 0.025% r-rum lidocaine.
The technical result, the achievement of which is ensured by the implementation of the entire claimed combination of essential features, is to simplify the treatment of acute ascending varicothrombophlebitis of the veins of the lower extremities while reducing the degree of tissue injury.
A significant difference of the proposed method from the prototype is that endovenous laser obliteration is the main treatment method, while in the prototype this procedure is considered only as a stage of an integrated approach to treatment. The proposed technique is innovative and has not previously been used to treat acute ascending varicothrombophlebitis of the veins of the lower extremities.
In the known method of treatment, compulsory ligation of the estuarine inflows (crossectomy) is performed, in the claimed method this procedure is not performed due to its inappropriateness, which simplifies the treatment of acute ascending varicothrombophlebitis. In addition, unlike the prototype in the claimed method of treatment, surgical intervention is performed under local (tumescent) anesthesia, which minimizes pain, also allows to simplify treatment and conduct it on an outpatient basis, significantly reducing the postoperative period.
In the inventive method, endovenous laser obliteration of the vein trunk is carried out by a laser with a laser wavelength of 1460 nm at a rate of 70 to 100 J per 1 cm of vein, which minimizes injury to the paravasal connective tissue of the intraorgan blood vessels.
Thus, the technical results are a simplification of the method of treatment of acute ascending varicothrombophlebitis of the veins of the lower extremities and a decrease in the degree of tissue injury, are interconnected and are due to the same set of essential features.
The inventive method is implemented as follows.
When conducting a method of treating acute ascending varicothrombophlebitis of the veins of the lower extremities, preoperative preparation is preliminarily carried out, which includes at least collecting complaints and anamnesis, examining a surgeon, a clinical blood test, a biochemical blood test, a blood group and a Rh factor, a blood test for hepatitis markers "B" and "C", HIV, syphilis, ECG, color duplex angioscanning (CDS) of the veins of the lower extremities with compression samples. After appropriate preoperative preparation, the patient is punctured at a distance of 0.5 cm to 1 cm above the thrombus head, depending on the level of localization of the thrombus head, then through the installed introducer sheath 6 F or 7 F (catheter size 100-110 cm) a light guide with a conical end face is introduced (with a ring diagram) to the level of the ostial valve. Then, under ultrasound guidance, a tumecent anesthesia is performed with a 0.025% lidocaine solution. The light guide is brought to the anastomosis (safeno-femoral or safeno-popliteal, depending on the pool of the affected veins). Moreover, in contrast to the prototype, where the fiber is inserted into the trunk saphenous vein freed from thrombotic masses, in the inventive method, the fiber is inserted over the thrombus, thereby stopping its distribution.
Endovenous laser obliteration of the vein trunk is performed from 70 to 100 J per 1 cm of vein, the wavelength of the laser radiation is chosen equal to 1460 nm (water-absorbing spectrum), which excludes burns of paravasal connective tissues of intraorgan blood vessels. This procedure is performed using the LAMI Helios-3 robotic laser complex. After the endovenous laser obliteration procedure is performed, a light guide is removed at a speed of 0.7 cm per second, an aseptic dressing is applied and compression hosiery is put on with a pressure of 23-25 mm Hg. Art. (2nd class of compression). The patient activates immediately after surgery and can be discharged.
