Однако эти операции в основном применяются для лечения панкреатита с преимущественным поражением головки поджелудочной железы, тогда как в клинике часто встречаются пациенты с субтотальным и тотальным вовлечением паренхимы железы в фиброзно-дегенеративный процесс. В качестве прототипа авторы изобретения предлагают способ резекции поджелудочной железы, который предложил Ч.Фрей [Frey C.F., Smith G.J. Description and rationale of a new operation for chronic pancreatitis. Pancreas, 1987; 2: 701-706.]. Данная методика подразумевает иссечение центрального участка головки поджелудочной железы с сохранением узкой полоски паренхимы вдоль двенадцатиперстной кишки с иссечением санториниева протока и передней стенки вирсунгова протока. Далее между полостью, сформированной после удаления патологических тканей головки поджелудочной железы, и просветом отключенной по Ру петли тонкой кишки формируется панкреатоеюноанастомоз с использованием двухрядного шва. При необходимости дополнительного дренирования протоковой системы в анастомоз также включается продольно вскрытый вирсунгов проток по типу продольной панкреатоеюностомии по Partington-Rochelle.As a prototype, the inventors propose a pancreatic resection method, which was proposed by C. Сущность изобретения заключается в иссечении патологически измененных тканей тела поджелудочной железы с полным удалением вирсунгова протока в головке и теле железы, применении однорядного шва для формирования панкреатоеюноанастомоза, использовании гемостатического препарата «ТахоКомб» для дополнительной герметизации анастомоза.The essence of the invention lies in the excision of pathologically altered body tissues of the pancreas with the complete removal of the Wirsung duct in the head and body of the gland, the use of a single-row suture to form pancreatojejunoanastomosis, and the use of the TachoComb hemostatic preparation for additional sealing of the anastomosis.
Резекция осуществляется следующим образом. Производится доступ в сальниковую сумку путем пересечения желудочно-ободочной связки, обнажается передняя поверхность поджелудочной железы. По внутреннему контуру двенадцатиперстной кишки накладывается 5-7 швов для перевязки ветвей поджелудочно-двенадцатиперстных артерий, питающих головку поджелудочной железы. Затем при помощи электрокоагулятора производится поэтапное кратерообразное иссечение центральной части головки поджелудочной железы с санториниевым и вирсунговым протоками (фиг.1). В просвет вирсунгова протока в направлении хвоста железы вводится тонкий зонд, служащий ориентиром для дальнейшего хода резекции (фиг.2). После этого производится иссечение измененных тканей тела железы в виде желоба до уровня перехода тела железы в хвост вместе с вирсунговым протоком. В результате в ткани железы формируется открытая кпереди желобовидная полость, в которую непосредственно открываются протоки II и III порядка (фиг.3). Выполняется гемостаз с применением электрокоагуляции или путем прошивания отдельных сосудов тонкими атравматическими нитями (5/0). Затем формируется отключенная по Ру петля тощей кишки. Отключенная часть кишки вскрывается продольно, и между ней и сформированной в поджелудочной железе полостью накладывается анастомоз однорядным непрерывным швом (атравматический шовный материал 4/0 - 5/0)(фиг.4). С целью дополнительной герметизации анастомоза на область наложенного шва производится аппликация пластин препарата «ТахоКомб» (фиг.5). Дренирование зоны анастомоза осуществляется при помощи микроеюностомы, накладываемой на анастомозированной петле тощей кишки в 10-15 см дистальнее анастомоза.Resection is as follows. After this, the altered tissues of the glandular body are excised in the form of a gutter to the level of transition of the glandular body to the tail along with the Wirsung duct. As a result, in the gland tissue an anterior groove-shaped cavity is formed, into which ducts of the II and III order directly open (Fig. 3). Hemostasis is performed using electrocoagulation or by flashing individual vessels with thin atraumatic threads (5/0). Then, a loop of the jejunum disconnected along Ru is formed. The disconnected part of the intestine is opened longitudinally, and between it and the cavity formed in the pancreas an anastomosis is applied with a single-row continuous suture (atraumatic suture material 4/0 - 5/0) (Fig. 4). In order to additionally seal the anastomosis on the suture area, the application of the TahoKomb preparation plates is performed (Fig. 5). Drainage of the anastomotic zone is carried out using microjunostomy, superimposed on the anastomosed loop of the jejunum 10-15 cm distal to the anastomosis.

Применение описанного способа резекции поджелудочной железы при хроническом панкреатите осуществлено на базе хирургического отделения ГУЗ «Областная клиническая больница» города Твери.The application of the described method for pancreatic resection in chronic pancreatitis was carried out on the basis of the surgical department of the State Clinical Hospital “Regional Clinical Hospital” of the city of Tver.

Больной В., 27 лет, поступил в хирургическое отделение ГУЗ «Областная клиническая больница» с жалобами на интенсивные боли в эпигастрии опоясывающего характера, усиливающиеся после приема пищи, сопровождающиеся тошнотой, рвотой, потерей массы тела. В анамнезе злоупотребление алкоголем. Произведено обследование: в ходе ультразвукового исследования выявлено увеличение размеров головки и тела поджелудочной железы, вирсунгов проток не расширен. При компьютерной томографии выявлено увеличение головки поджелудочной железы до 35 мм, тела до 23 мм, расширения вирсунгова протока, общего желчного протока, воротной вены не выявлено. В паренхиме железы большое количество кальцинатов. После предоперационной подготовки больному выполнена резекция головки и тела поджелудочной железы в соответствии с вышеописанным способом. Длительность операции составила 160 минут. Послеоперационный период протекал без осложнений. Признаков несостоятельности анастомоза не было. По дренажу зоны панкреатоеюноанастомоза поступал прозрачный панкреатический сок, без примеси крови. Рана зажила первичным натяжением. Дренаж зоны панкреатоеюноанастомоза удален на 14-е сутки после операции, подтекания секрета по каналу не отмечено. Больной выписан на долечивание в амбулаторных условиях в удовлетворительном состоянии. Пациент был повторно обследован через 3 месяца после выписки: отмечает полное купирование болевого синдрома, восстановление нормального ритма приема пищи, нормализацию массы тела. Transparent pancreatic juice, without blood impurity, was supplied through the drainage of the pancreatojejunoanastomosis zone. The wound healed by primary intention. The drainage of the pancreatojejunoanastomosis zone was removed on the 14th day after the operation, no leakage of secretion along the channel was noted. The patient was discharged for aftercare in an outpatient setting in satisfactory condition. The patient was re-examined 3 months after discharge: noted complete relief of pain, restoration of a normal rhythm of food intake, normalization of body weight. Signs of diabetes mellitus and exocrine pancreatic insufficiency have not been identified.
