Хирургическое лечение хронического панкреатита и его осложнений, опухолей, кист и свищей тела поджелудочной железы демонстрируются достижениями современной панкреатологии, но его отдаленные результаты, особенно мини-инвазивные оперативные вмешательства, не удовлетворяют хирургов панкреатологов из-за ее нерадикальности. Способ осуществляется следующим образом. Проводят срединную лапаротомию, ревизию органов брюшной полости оценивая в первую очередь техническую возможность выполнения операции. Вскрывают желудочно-ободочную связку. Поджелудочную железу выделяют и берут за верхние и нижние края, подтягивают к верхним левым мезентеральным сосудам, после чего осуществляют мобилизацию задней поверхности с подлежащей селезеночной артерией и веной, затем проводят удаление патологического очага путем резекции железы в пределах здоровых тканей и ее разделе на две части. Далее приступают к обработке обеих культей поджелудочной железы. Проводят тщательный гемостаз, ревизию проксимальной и дистальной культи железы на предмет проходимости протоковой системы. Из проксимальной культи поджелудочной железы выделяют панкреатический проток, прошивают и перевязывают отдельным швом, культю ушивают «U» образными монолитными швами 3/0-4/0, которыми сопоставляется передняя и задняя поверхности культи железы. В дистальную часть протока вводят внутренний «каркасный» стент, фиксируя хромированным кетгутовым узловым швом (фиг. 1 поз. 1). Затем, к культям поджелудочной железы подводят в виде полукруга петлю тонкой кишки для перитонизации закрытой проксимальной культи (фиг. 1 поз. 2) и формирования панкреатикоэнтероанастомоза с дистальной культей путем предварительного рассечения против брыжеечного участка серозно-мышечного слоя тонкой кишки длиной, соответствующей культе поджелудочной железы. В центре, точно против панкреатического протока, открывают маленьким разрезом обнаженную слизистую оболочку, куда вставляем «каркасный» стент. Панкреатический проток несколькими швами из хромированного кетгута подшивают к краям слизистой кишки, и таким образом, формируют панкреатикоэнтероанастомоз в виде duck ad mucosae (фиг. 1 поз. 3) с дистальной культей поджелудочной железы. В этом положении стенку тонкой кишки подшивают к капсуле поджелудочной железы серозно-мышечными швами на всем протяжении дистальной культи. Пример 1. Больной К., 47 лет поступил в отделение гепатохирургия РКБ с жалобами на интенсивные боли в эпигастрии опоясывающего характера, усиливающиеся после приема пищи, сопровождающиеся тошнотой, рвотой, потерей массы тела. Из анамнеза выяснено, что часто злоупотреблял алкоголю, неоднократно лечился в хирургических отделениях с приступами обострения панкреатита. Получал консервативное лечение, которое давало временный эффект. При поступлении по данным УЗИ выявлено увеличение размеров тела и головки поджелудочной железы, расширение вирсунгова протока, холедох не расширен. На компьютерной томографии выявлено увеличение тела железы до 33 мм, расширение вирсунгова протока в каудальной части железы. В паренхиме железы большое количество кальцинатов, особенно в области тела. Выставлен диагноз хронический калькулезный панкреатит с частыми рецидивами. После подготовки проведена операция - лапаротомия, резекция тела и перешейка поджелудочной железы в соответствии с вышеописанным способом. Длительность операции составила 170 минут. На третьи сутки после операции нормализовалась температура тела, улучшился аппетит. На 7 сутки произведено ультразвуковое исследование, в брюшной полости патологических жидкостных образований не выявлено, дренажная трубка в проекции панкреатикоэнтероанастомоза. Послеоперационный период протекал без осложнений. Признаков несостоятельности анастомоза не было. По дренажу брюшной полости в области панкреатикоэнтероанастомоза поступало геморрагическое отделяемое в небольшом количестве. Рана зажила первичным натяжением. Дренажная трубка в области панкреатикоэнтероанастомоза удалена на 8-е сутки после операции, подтекания по каналу не отмечено. Больного в удовлетворительном состоянии на 11 сутки выписали домой на амбулаторное лечение. Больной был повторно обследован через 3 месяца после выписки: отмечает полное купирование болевого синдрома, восстановление нормального ритма приема пищи, нормализацию массы тела. Признаков сахарного диабета и внешнесекреторной недостаточности поджелудочной железы не выявлено.Example 1. Patient K., 47 years old, was admitted to the hepatosurgery department of the RCH with complaints of intense pain in the epigastric herpes zoster, aggravated after eating, accompanied by nausea, vomiting, loss of body weight. From the anamnesis it was found that he often abused alcohol, was repeatedly treated in surgical departments with bouts of exacerbation of pancreatitis. He received conservative treatment, which gave a temporary effect. Upon admission according to ultrasound, an increase in the size of the body and head of the pancreas was revealed, expansion of the Wirsung duct, choledochus not expanded. Computed tomography revealed an increase in the body of the gland up to 33 mm, an expansion of the Wirsung duct in the caudal part of the gland. In the parenchyma of the gland, a large amount of calcifications, especially in the area of the body. Diagnosed with chronic calculous pancreatitis with frequent relapses. After preparation, an operation was performed - laparotomy, resection of the body and the isthmus of the pancreas in accordance with the above method. The duration of the operation was 170 minutes. On the third day after the operation, body temperature returned to normal and appetite improved. On the 7th day an ultrasound scan was performed, no abnormal fluid formations were found in the abdominal cavity, a drainage tube in the projection of pancreaticenteroanastomosis. The postoperative period was uneventful. There were no signs of failure of the anastomosis. A small amount of hemorrhagic discharge was received by abdominal drainage in the area of pancreaticenteroanastomosis. The wound healed by primary intention. The drainage tube in the area of pancreaticenteroanastomosis was removed on the 8th day after the operation, leakage along the channel was not noted. The patient in satisfactory condition on day 11 was discharged home for outpatient treatment. The patient was re-examined 3 months after discharge: notes complete relief of pain, restoration of a normal rhythm of food intake, normalization of body weight. Signs of diabetes mellitus and exocrine pancreatic insufficiency have not been identified.

Больной X., 55 лет, поступил в отделение гепатохирургия с диагнозом: хронический калькулезный панкреатит с преимущественным поражением тела и перешейка поджелудочной железы. По заявляемому способу выполнили операцию: лапаротомию, центральную резекцию тела поджелудочной железы, гемостаз и ревизию проходимости протоковой системы железы, полным ушиванием «U» образным швом проксимальной культи и ее перитонизацию стенкой тонкой кишки, а в дистальную часть протока введен «каркасный» стент, фиксирован узловым швом к резекционному краю с последующим формированием панкреатикоэнтероанастомоза в виде duck ad mucosae и межкишечного соустья по Брауну. Послеоперационный период протекал гладко. При контрольных обследованиях показатели лабораторных и инструментальных данных без особых нарушений. Больной был выписан в удовлетворительном состоянии на 15 сутки после операции. При контрольном осмотре через 6 месяцев состояние удовлетворительное. На УЗИ: культя поджелудочной железы средних размеров, панкреатический проток не расширен, кистозной трансформации ткани железы не выявлено. Больной набрал вес, диспепсических и эндокринных нарушений нет, болевой синдром отсутствует, трудоспособность восстановлена.Patient X., 55 years old, was admitted to the department of hepatosurgery with a diagnosis of chronic calculous pancreatitis with a primary lesion of the body and the isthmus of the pancreas. The patient was discharged in satisfactory condition 15 days after surgery. At the control examination after 6 months, the condition is satisfactory. Ultrasound: a pancreatic stump of medium size, the pancreatic duct is not enlarged, cystic transformation of the gland tissue was not detected. The patient gained weight, there are no dyspeptic and endocrine disorders, there is no pain, the ability to work is restored.
Таким образом, предлагаемый способ центральной резекции поджелудочной железы применен у 12 больных, оперированных по поводу хронического панкреатита (3), опухоли (2), кист (4) и свищей железы (3). Послеоперационных осложнений и летальных исходов не наблюдалось. В техническом плане менее травматичен, более прост в выполнении, что сокращает длительность времени операций. Исключает заброс химуса в протоковую систему железы и снижает риск развития панкреатита. Сроки госпитализации больных в основной группе были значительно меньше, чем в контрольной. Способ по изобретению был использован у 12 пациентов, ни в одном из наблюдений не было отмечено рецидива заболевания.
